Abstract:Objective To explore catch-up growth of premature infants below corrected age of 12 months and to provide guidance on health care for premature infants after discharge from hospital .Methods Altogether 374 premature infants with gestational age at 28 - 36 + 6 weeks ,born from May 2012 to April 2014 in obstetrics and pediatrics department in nine grade Ⅲ and grade Ⅱ hospitals of six cities from all parts of China ,who received early comprehensive intervention and had complete follow -up data were selected as study subjects .Standard deviation score method was used to assess Z scores of weight ,length and head circumference of each gestational age .Distributions and changes of Z scores and characteristics of catch-up of premature infants were analyzed ,and differences in catch-up growth between small for gestational age (SGA ) and appropriate for gestational age (AGA ) premature infants were compared .Results At corrected age of 40 weeks ,proportion of premature infants with Z scores of weight ,length and head circumference lower than - 2 was 2 .1％ ,1 .9％ and 0 .5％ , respectively .At corrected age of 12 months ,proportion of premature infants with Z scores below - 2 for weight ,length and head circumference was 0 ,0 .3％ and 0 ,respectively .Z scores at every gestational age for weight (t value ranged 3 .549 - 8 .849 ,all P < 0 .001 at corrected age of 40 weeks to 9 months ;t = 3 .452 ,P = 0 .001 at corrected age of 12 months) ,length (t value ranged 3 .609 - 8 .640 ,all P < 0 .001 at corrected age of 40 weeks to 12 months) and head circumference (tvalue ranged 4 .386 - 6 .205 ,all P < 0 .001 at corrected age of 40 weeks to 6 months ;t = 3 .203 ,P = 0 .002 at corrected age of 9 months ;t = 3 .371 , P = 0 .001at corrected age of 12 months) of SGA premature infants were lower than those of AGA premature infants ,and differences were statistically significant .Change tendency of Z score for weight was different between AGA and SGA premature infants ,while change tendency of Z scores for length and head circumference between AGA and SGA premature infants was similar .At corrected age of 12 months ,incidence rate of underweight ,stunting ,marasmus and overweight was 0 ,0 .3％ ,0 .3％ and 5 .1％ ,respectively .Conclusion There is catch-up growth among premature infants until corrected age of 12 months .Physical grow th of AGA premature infants is better than that of SGA premature infants at every gestational age .Early comprehensive intervention can promote catch-up growth of premature infants .

Abstract:Objective: To explore birth rate, delivery mode, medical problems, requirement of respiratory support, and acute outcomes of late preterm infants in Zhejiang province in eastern China. Methods: Eleven tertiary hospitals were recruited. Clinical data of every nursery admission from January to December 2007 were collected and analyzed. Results: During the study period, 44,362 infants were born with an overall preterm birth rate of 8.9%, and late preterm birth rate of 6.2%. Late preterm infants had higher cesarean section rate than the whole population (64.9% vs. 58.2%). One-fifth of the nursery admissions were late preterm infants, of whom, 63.8% were delivered by cesarean section. Respiratory distress (42.1%) was the most common medical problem of late preterm infants. Hyperbilirubinemia (17.6%), hypoglycemia (8.7%) and sepsis (5.9%) were also common. The first three primary diagnoses of respiratory distress included pneumonia (39.5%), transient tachypnea of newborn (TTN) (22.5%) and respiratory distress syndrome (RDS) (19.0%). Compared with term infants, late preterm infants with respiratory distress needed more respiratory support with nasal continuous positive airway pressure (nCPAP) (21.4% vs. 11.6%) or with a mechanical ventilator (15.4% vs. 11.0%), and also had higher in-hospital mortality (0.8% vs. 0.4%). Conclusions: Late preterm infants are associated with very high cesarean section rate and have more medical problems and poorer short-term outcomes than term infants in China.
Different Catch-Up Growth Patterns in Very Preterm and Small for Gestational Age Infants

Abstract:Clinical Pediatrics, Ahead of Print. This study aimed to describe the growth pattern in preterm infants and identify factors influencing catch-up growth. A total of 288 preterm infants were divided into groups based on the degree of prematurity, sex, and size for gestational age. Growth in head circumference, length, weight-for-length, and weight was compared between groups at corrected age of 0, 3, 6, 9, 12, 18, and 24 months. Logistic regression analysis was conducted to determine risk factors for catch-up growth. At a corrected age of 24 months, the proportions of preterm infants with z-scores less than −2 for head circumference, length, weight-for-length, and weight were less than the expected 2.3% at 0.9%, 1.7%, 2.1%, and 1.7%, respectively. The head circumference, length, weight-for-length, and weight z-scores at corrected ages of 24 months were lower in the small for gestational age (SGA) group than in the non-SGA group (P < .05). The weight-for-length z-scores were higher in the late preterm birth infants than in the very preterm birth infants at a corrected age of 24 months (P < .05). At a corrected age 24 months, the proportion of male with weight z-scores <−2 was lower than that of female (P < .05). The differences in proportion of the z-scores (head circumference, length, weight-for-length, and weight) .05). We found that the factors influencing catch-up growth in preterm infants varied at different corrected age stages, and the impact of factors during hospitalization gradually diminished as the infants grew.

Associations Between Gestational Age and Childhood Sleep: a National Retrospective Cohort Study

Abstract:Background Both sleep quality and quantity are essential for normal brain development throughout childhood; however, the association between preterm birth and sleep problems in preschoolers is not yet clear, and the effects of gestational age across the full range from preterm to post-term have not been examined. Our study investigated the sleep outcomes of children born at very-preterm (<31 weeks), moderate-preterm (32–33 weeks), late-preterm (34–36 weeks), early-term (37–38 weeks), full-term (39–40 weeks), late-term (41 weeks) and post-term (>41 weeks). Methods A national retrospective cohort study was conducted with 114,311 children aged 3–5 years old in China. Children’s daily sleep hours and pediatric sleep disorders defined by the Children’s Sleep Habits Questionnaire (CSHQ) were reported by parents. Linear regressions and logistic regression models were applied to examine gestational age at birth with the sleep outcomes of children. Results Compared with full-term children, a significantly higher CSHQ score, and hence worse sleep, was observed in very-preterm ( β = 1.827), moderate-preterm ( β = 1.409), late-preterm ( β = 0.832), early-term ( β = 0.233) and post-term ( β = 0.831) children, all p <0.001. The association of pediatric sleep disorder (i.e. CSHQ scores>41) was also seen in very-preterm (adjusted odds ratio [AOR] = 1.287 95% confidence interval [CI] (1.157, 1.433)), moderate-preterm (AOR = 1.249 95% CI (1.110, 1.405)), late-preterm (AOR = 1.111 95% CI (1.052, 1.174)) and post-term (AOR = 1.139 95% CI (1.061, 1.222)), all p <0.001. Shorter sleep duration was also found in very-preterm ( β = −0.303), moderate-preterm ( β = −0.282), late-preterm ( β = −0.201), early-term ( β = −0.068) and post-term ( β = −0.110) compared with full-term children, all p <0.01. Preterm and post-term-born children had different sleep profiles as suggested by subscales of the CSHQ. Conclusions Every degree of premature, early-term and post-term birth, compared to full-term, has an association with sleep disorders and shortened daily sleep duration. Preterm, early-term, and post-term should therefore all be monitored with an increased threat of sleep disorder that requires long-term monitoring for adverse sleep outcomes in preschoolers.
Follow-up study on physical growth of 786 premature infants

Abstract:Objective To explore physical growth of premature infants under corrected age of 6 months and to provide guidance on health care for premature infants.Methods A total of 786 premature infants with complete follow-up data were enrolled.All premature infants were offered early comprehensive intervention and followed up until corrected age of 6months.Premature infants were divided into different groups according to gestational age (＜32、32～33+6 、34～35+6 、36～36+6 weeks) and the relationship between gestational age and birth weight (small,appropriate and large for gestational age),respectively.Linear mixed model was used to assess changes in physical growth of premature infants.Results Weight,length and head circumference among different gestational age groups (F values were 11.88,29.91 and 19.19,respectively) and different corrected age points (F values was 7678.78,11922.57 and 7913.78,respectively) were significantly different (all P＜0.001);Significant difference was also found on weight,length and head circumference among SGA,AGA and LGA infants (F values were 88.01,79.36 and 50.27,respectively,all P＜0.001) and different corrected age points (F values were 1 192.38,1 836.58 and 1 225.71,respectively,all P＜0.001).At corrected age of 6 months,more than 50％ of premature infants were between the 25th and the 75th percentices of Chinese full-term infant reference.Conclusions Physical growth of premature infants is closely related to gestational age and birth weight.Early comprehensive intervention has positive effect on promoting physical growth of premature infants.
Association of Gestational Age at Birth With Subsequent Neurodevelopment in Early Childhood: A National Retrospective Cohort Study in China

Abstract:Background: The association between preterm birth and neurodevelopmental delays have been well examined, however, reliable estimates for the full range of gestational age (GA) are limited, and few studies explored the impact of post-term birth on child development. Objective: This study aimed to examine the long-term neuropsychological outcomes of children born in a full range of GA with a national representative sample in China. Methods: In this retrospective population-based cohort study, a total of 137,530 preschoolers aged 3-5 years old (65,295/47.5% females and 72,235/52.5% males) were included in the final analysis. The Ages and Stages Questionnaires-Third Edition (ASQ-3) was completed by parents to evaluate children's neurodevelopment. The associations between GA and neurodevelopment were analyzed by a generalized additive mixed model with thin plate regression splines. Logistic regression was also conducted to examine the differences in children's development with different GAs. Results: There was a non-linear relationship between GA and children's neurodevelopmental outcomes with the highest scores at 40 weeks gestational age. The adjusted risks of GAs (very and moderately preterm, late-preterm, early-term, and post-term groups) on suspected developmental delays were observed in communication (OR were 1.83, 1.28, 1.13, and 1.21 respectively, each p < 0.05), gross motor skill (OR were 1.67, 1.38, 1.10, and 1.05 respectively, each p < 0.05), and personal social behavior (OR were 1.01, 1.36, 1.12, and 1.18 respectively, each p < 0.05). The adjusted OR of very and moderately preterm, late-preterm, and early-term were observed in fine motor skills (OR were 1.53, 1.22, and 1.09 respectively, each p < 0.05) and problem-solving (OR were 1.33, 1.12, and 1.06 respectively, each p < 0.05). Conclusion: GAs is a risk factor for neurodevelopmental delays in preschoolers after controlling for a wide range of covariates, and 40-41 weeks may be the ideal delivery GA for optimal neurodevelopmental outcomes. Close observation and monitoring should be considered for early- and post-term born children as well as pre-term children.
Early catch-up growth status and its influencing factors in small for gestational age preterm infants

Abstract:Objective To study the early physical growth pattern,catch-up growth situation,and the influencing factors of early growth in small for gestational age (SGA) preterm infants.Method Our study was a single center,retrospective study.Criteria for infant inclusion were prematurity,SGA (birth weight less than the 10th percentile of related gender and gestational week,according to Fenton curve 2013),born between January 2012 to October 2015,admitted to our neonatal intensive care unit (NICU) within 24 h after birth,hospitalization more than 7 days,and discharged with complete oral feeding.Corrected age (CA) was used to evaluate growth.According to our follow up plan,anthropometric data (weight,length,head circumference) were collected at corrected full term (40 ± 4 weeks),CA (3 ± 1.5) months and CA (6 ± 1.5) months.Catch-up growth was defined as ΔZ greater than 0.67 compared with that at birth,successful catch-up was defined as anthropometric data higher than 10th percentile in target population.The characteristics and influencing factors were compared between infants with and without catch-up growth.Result Eighty-one SGA preterm infants were involved,45 boys and 36 girls.The average gestational age was (34.6 ± 1.7) weeks,birthweightwas(1617 ± 348) g,birthlengthwas(41.0 ±3.2)cm and head circumference was (29.7 ± 2.0) cm.At corrected gestational age (40 ± 4) weeks,CA (3 ± 1.5) months and CA (6 ± 1.5) months,follow-up rate was 86.4％,66.7％ and 58.0％;catch-up growth in weight was 32.9％,55.6％ and 66.0％;successful catch-up growth in weight was 52.9％,64.8％ and 66.0％.At CA (40 ±4) weeks,there were more boys,sooner recover birth weight,and less patent ductus arteriosus (PDA) in catch-up infants (P ＜ 0.05).At CA (3 ± 1.5) months,catch-up infants had large gestational age,and they were longer at discharge,shorter hospital stay,less PDA,and greater body weight at CA 40 weeks,the difference was statistically significant (P ＜ 0.05).At CA (6 ± 1.5) months,there were difference in hospitalization days,percentile of body weight at CA 40 weeks and percentile of all three anthropometrics at CA (3 ± 1.5) months between catch-up and no catch-up growth infants (P ＜ 0.05).Multiple factor analysis showed that percentile of weight at CA 3 months was the independent risk factor of catch-up growth in weight at CA 6 months (P =0.002,OR =1.221,95％ CI 1.076 ～ 1.385).For every 5 percentile increase in body weight percentile at CA (3 ± 1.5) months of age,the likelihood of complete body weight catch-up growth at CA (6 ± 1.5) months increased 2.965 times (95％ CI 1.480 ～ 5.942).Conclusion Both weight and length of SGA preterm infants showed a trend of rapid gain between corrected gestational age (40 ± 4) weeks to CA (3 ± 1.5) months.The factors that influencing the completion of catch-up growth are different at different age.The weight,length,and head circumference percentile at CA about 3 months are good predictors of growth pattern and situation at CA 6 months for the SGA preterm infants.
A Longitudinal Study on the Catch-Up Growth of Preterm and Term Infants of Low, Appropriate, and High Birth Weight.

Abstract:The purpose of present study was to explore the trajectories of growth of preterm and term infants of low, high, and appropriate birth weight. A total of 2465 infants were recruited from 2005 to 2007. Infants' length and weight were measured at 12 points: birth, 42 days, and monthly from 3 to 12 months. The catch-up growth of preterm infants, measured by weight or length of chronological age, occurred from 10th or 12th month of their lives. After adjusted gestational age, preterm infants' weight and length were caught-up from the adjusted birthday, overweight/over-length even occurred from 10th/11th month. For low birth weight term infants, catch-up growth was not found when using single weight or length. The ponderal index catch-up growth occurred earlier than measured by single weight or length. The catch-down growth of high birth weight term infants was not found in the first year of their lives.

Abstract:Objective To study the characteristics and its risk fastors of brain development of the preterm infant early after birth in amplitude-integrated electroencephalography(aEEG). Methods The 153 preterm infants who had seen a doctor in Peking University First Hospital from April 2009 to August 2013 accepted the aEEG check at term of corrected gestational age ( ≥ 38 weeks but < 42 weeks of corrected gestational age). The risk factors of brain development, such as gestational age ( < 30, 30 ≤ - ≤ 33+6 and 34≤-≤36+6 weeks), clinical informations [relatively stable group including 104 cases without any serious complications or brain injury, the group only suffering from a serious brain injury (19 cases), and the group only suffering from severe systemic disease (30 cases)] and nutrition (good or malnutrition), were analyzed. Also the relationship between the aEEG and the cranial ultrasound detected at the same time and the Gesell Developmental Scale at six months of corrected gestational age. Theχ2 test, two independent samples t-test and Logistic regression analysis were used for statistical analysis. Results The aEEG of 52%(79/153) cases reached the level of normal full-term newborn at term of corrected gestational age, only 48% (74/153) were abnormal. The abnormal rate of aEEG results in relatively stable preterm infants decreased from 3/6 (<30 weeks) to 35%(13/37) at 34 ≤ - ≤ 36+6 weeks, but the difference was not statistically significant (χ2=1.998, P=0.353). The abnormal rate of aEEG results in the group suffering from a serious brain injury was higher than the relatively stable preterm infants [14/19 vs 44%(46/104) ,χ2=5.578, P=0.024]. In relatively stable preterm infants, there was no difference of the abnormal rate of the aEEG results between intrauterine malnutrition group and good nutrition group [46%(19/41) vs 43%(27/63),χ2=0.122, P=0.727]. Neither was between extrauterine malnutrition group and good nutrition group [52%(13/25) vs 42%(33/79),χ2=0.805, P=0.369]. Serious brain injury was independent risk factor of abnormal aEEG (OR=3.453, 95%CI: 1.177-10.132, P=0.024). The coincidence rate of aEEG and the cranial ultrasound examination or the scores of Gesell Developmental Scale was 57%(56/98) and 50%(10/20), respectively. Conclusions The brain catch-up development may appears early after birth in preterm infants, which are impaired by lower gestational age and the severe brain injury. It is more effective of aEEG for evaluating the brain development of preterm infants when combines with other methods.
Association of Gestational Age at Birth With Subsequent Suspected Developmental Coordination Disorder in Early Childhood in China

Abstract:IMPORTANCE It remains unknown whether children born at different degrees of prematurity, early term, and post term might have a higher risk of developmental coordination disorder (DCD) compared with completely full-term children (39-40 gestational weeks). OBJECTIVE To differentiate between suspected DCD in children with different gestational ages based on a national representative sample in China. DESIGN, SETTING, AND PARTICIPANTS A retrospective cohort study was conducted in China from April 1, 2018, to December 31, 2019. A total of 152 433 children aged 3 to 5 years from 2403 public kindergartens in 551 cities of China were included in the final analysis. A multilevel regression model was developed to determine the strength of association for different gestational ages associated with suspected DCD when considering kindergartens as clusters. MAIN OUTCOMES AND MEASURES Children's motor performance was assessed using the Little Developmental Coordination Disorder Questionnaire, completed by their parents. Gestational age was determined according to the mother's medical records and divided into 7 categories: completely full term (39 to 40 weeks' gestation), very preterm (< 32 weeks), moderately preterm (32-33 weeks), late preterm (34-36 weeks), early term (37-38 weeks), late term (41 weeks), and post term (>41 weeks). RESULTS A total of 152 433 children aged 3 to 5 years (mean [SD] age, 4.5 [0.8] years), including 80 370 boys (52.7%) and 72 063 girls (47.3%), were included in the study. There were 45 052 children (29.6%) aged 3 years, 59 796 (39.2%) aged 4 years, and 47 585 (31.2%) aged 5 years. Children who were born very preterm (odds ratio [OR], 1.35; 95% CI, 1.23-1.48), moderately preterm (OR, 1.18; 95% CI, 1.02-1.36), late preterm (OR, 1.24; 95% CI, 1.16-1.32), early term (OR, 1.11; 95% CI, 1.06-1.16), and post term (OR, 1.17; 95% CI, 1.07-1.27) were more likely to be classified in the suspected DCD category on the Little Developmental Coordination Disorder Questionnaire than completely full-term children after adjusting for the same characteristics. Additionally, there was no association with suspected DCD in younger (aged 3 years) early-term and postterm children by stratified analyses. CONCLUSIONS AND RELEVANCE In this cohort study, every degree of prematurity at birth, early-term birth, and postterm birth were associated with suspected DCD when compared with full-term birth. These findings have important implications for understanding motor development in children born at different gestational ages. Long-term follow-up and rehabilitation interventions should be considered for children born early and post term.

Abstract:【Objective】 To investigate the physical development status of the preterm low birth weight babies before they were 2 years old. 【Method】 The data of the weight and length for 79 preterm infants with birth weight less 2 500 g and gestational age ≤36 weeks was analyzed. 【Results】 The preterm low birth weight babies had the catch-up growth before the corrected age of 2 years old and no such shift was found between 40 weeks～1 month and 6～8 months.【Conclusion】 The intervention and instruction to the nutrition level must be reinforced according to the features of the physical development of the preterm low birth weight babies

Abstract:Objective To investigate the influence of early intervention with new follow-up pattern on the neurodevelopment of preterm infants. Methods From January 2014 to June 2015, a total of 408 preterm infants who were alive and discharged from department of Neonatology, Qinzhou Maternal and Child Health Care Hospital and regularly followed up at the outpatient service, were selected as research subjects. They were randomly divided into observation group (n=197) and control group (n=211) with the random digits table method. Infants in observation group were received a new follow-up pattern, including outpatient follow-up, onsite assessment and guidance, and telephone, WeChat or QQ, while infants in control group were received traditional follow-up pattern, including outpatient follow-up, and onsite assessment and guidance for the studies of neural development of infants. Physical growth monitoring and neurodevelopmental assessment were the main aspects of the follow-up. Early intervention mainly included infants nutrition management, feeding guidance, neuromuscular exercise training and social psychological support for family members, etc.. Neonatal behavioral neurological assessment (NBNA) score at 40 weeks of corrected age, and assessment results of Gesell Developmental Scale at 6 months and 12 months of corrected age were compared between two groups of infants by t test or chi-square test. This study was approved by the Medical Ethics Committee of Qinzhou Maternal and Child Heath Care Hospital and obtained the informed consents from infants′family members. Results ①There were no significant differences between two groups of infants at 40 weeks of corrected age in NBNA score abnormal rate and NBNA score value (P>0.05). ②Gesell Developmental Scale assessment results of infants at 6 months of corrected age showed as follows. The abnormal rates of 5 Gesell subscales including gross motor behavior, fine motor behavior, language behavior, adaptive behavior and personal social behavior of infants in observation group (11.7%, 10.2%, 4.1%, 8.6%, 6.6%) were all lower than those in control group (20.9%, 17.5%, 9.5%, 15.2%, 13.3%); the development quotient (DQ) values of 5 Gesell subscales of infants in observation group [(90.5±8.0), (90.8±8.6), (89.5±9.5), (90.0±9.6), (89.4±8.8)] were all higher than those in control group [(88.0±9.5), (88.2±8.3), (87.4±8.1), (87.9±9.1), (87.5±8.2)]; and all the differences above were statistically significant(χ2=6.253, 4.621, 4.678, 4.119, 5.016; t=2.686, 3.037, 2.375, 2.298, 2.240; all P<0.05). ③Gesell Developmental Scale assessment results of infants at 12 months of corrected age showed as follows. The abnormal rates of 5 Gesell subscales including gross motor behavior, fine motor behavior, language behavior, adaptive behavior and personal social behavior of infants in observation group (4.6%, 4.1%, 3.0%, 5.1%, 3.0%) were all lower than those in control group (10.4%, 10.0%, 8.5%, 10.4%, 8.5%); the DQ values of 5 Gesell subscales of infants in observation group [(91.3±6.8), (91.7±7.6), (89.5±9.4), (90.5±9.0), (91.3±7.7)] were all higher than those in control group [(89.5±7.9), (89.2±7.3), (87.5±8.0), (88.4±8.5), (89.0±7.1)]; and all the differences above were statistically significant (χ2=4.980, 5.356, 5.537, 4.035, 5.537; t=2.405, 3.459, 2.356, 2.432, 3.053; all P<0.05). Conclusion Early intervention with new follow-up pattern was more conduce to the early neurodevelopment of preterm infants. Influencing factors of the neurodevelopment of high-risk infants

Abstract:Background High-risk infants refer to newborns exposed to high-risk factors in the prenatal, natal or postnatal period. High-risk infants are at high risk of developmental retardation, and early identification of developmental abnormalities plays a vital role in improving high-risk infants’ quality of life. Aims To describe the neurodevelopment of high-risk infants aged less than 1 year old, and to analyse the incidences and influencing factors of neurodevelopmental abnormalities in order to provide a basis for neurodevelopment monitoring and management of high-risk infants. Methods High-risk infants born between January 2016 and December 2016 in the maternity and infant health hospitals of three districts in Shanghai were followed up. The Gesell Developmental Scale was used to assess the neurodevelopmental level at the time of recruitment (0–2 months) and at 9 months. Univariate and multivariate analyses of the influencing factors were conducted. Results 484 high-risk infants (male 51%, female 49%) with an average gestation age of 36.5±2.2 weeks were recruited. At the time of recruitment, the average age was 2.1 (0.8) months, and the developmental quotient (DQ) scores of full-term high-risk infants in motor ( t =3.542, p =0.001), cognitive ( t =3.125, p =0.002), language ( t =3.189, p =0.002) and social ( t =3.316, p =0.001) areas were higher than those of preterm infants. The incidences of developmental abnormalities of full-term high-risk infants in motor (χ 2 =9.452, p =0.002), cognitive ( χ 2 =6.258, p =0.012), language ( χ 2 =12.319, p =0.001) and social ( χ 2 =6.811, p =0.009) areas were lower than the preterm infants. At 9 months, there was no difference in the DQ scores and incidences of developmental abnormalities in four areas between full-term and preterm high-risk infants, and the incidence of developmental abnormalities was around 10%. Conclusion The incidence of neurodevelopmental abnormalities in high-risk infants aged less than 1 year old is high. Preterm birth and parental bad habits are significant factors affecting the neurodevelopment. Monitoring and early interventions help to improve high-risk infants’ neurodevelopment.
Analysis of Trend and Associated Factors of Neuropsychological Development of Infants and Toddlers Based on Longitudinal Data

Abstract:Objective: To explore the trend and associated factors of neuropsychological development of infants and toddlers in China. Methods: A longitudinal study was conducted among 619 infants and toddlers (2914 person-times) aged 0 to 36 months from different provinces or cities in China from January 2013 to December 2019. Results: The development age of each area increased with the extension of follow-up time, but this upward trend slowed down with physiological age at first measurement increasing. Among a low age group and each area, most of the development qualification rates in different follow-up periods were higher than that in the baseline (p < 0.05); however, many of them were not higher than that in the baseline among the medium or high age group (p > 0.05). For the areas of gross motor and self-care, the growth of qualification rate with the extension of follow-up was not obvious in the medium and high age group (both p trend > 0.05). Some impact factors of development in all areas were identified. Conclusions: The neuropsychological development delay of various areas of infants and toddlers, especially that of gross motor and self-care, should be paid early (within 1 years old) and constant attention. The impact of gender and maternal age on the development of young children has been further confirmed in the present study.
Study on the Development of Frontal Lobe in Preterm Infants and Its Affected Factors

Abstract:Objectives To explore the development of frontal lobe in preterm infants and its affected factors with three-dimensional(3D)ultrasonic methods.Methods 222 preterm infants without sever brain injury were recruited in the study.The volumes of frontal lobes were measured with 3D ultrasound.The infants received the general and nervous system examination as well as the volume tracked measurement at the corrected gestational age of 40 weeks,one month,three months,six months after birth.Results The volumes of frontal lobes increased with the advancing gestational age and continued to grow after birth.In preterm infants,the volume showed catch-up growth after birth.And the velocity was the most rapid at the corrected gestational age of 40 weeks and 1 month after birth and even reached or exceeded that in full-term infants.However,it gradually lagged behind the full-term infants in the subsequent time.The lower the maturity of preterm infants,the smaller the frontal lobe volume.The frontal lobe volume between all gestational age groups was comparable at the corrected gestational age of 40 weeks and the subsequent time points,and the differences among different gestational age groups were not significant(P 0.05).The frontal lobe volumes in the pre-term infants with a poorer intra-and extra-uterine nutritional status in different gestational ages were lagged behind the age-matched normal preterm infants(P 0.05=.In comparison with the normal preterms infant,the incidence of severe abnormalities of nervous system markedly increased in the preterm infants with severely lagged growth of frontal lobe volume(50%).Conclusions Frontal lobe volumes increased with the gestational ages proportionately and showed the catch-up growth after birth in a short time.The poorer intra-and extra-uterine nutritional status influenced the development of frontal lobe volume.The degree of the frontal lobe abnormal volumes was related to the neurological developmental abnormalities in preterm infants.

Abstract:Introduction Due to immature brain development, preterm infants are more likely to develop neurological developmental defects compared with full-term infants. Most preterm infants without neurodevelopmental damage can eventually reach the same scholastic level as their same-age peers; however, some show persistent impairment. Breast feeding (BF), which is an important public health measure, is of great significance for preterm infants. Various active substances in breast milk promote the development of the brain and central nervous system in premature infants. We present a protocol for a prospective longitudinal cohort study to explore the effect of in-hospital BF on brain development in preterm infants and possible influencing factors. Methods and analysis This study will enrol 247 Chinese preterm infants (gestational age: 30-34 weeks) delivered in Women's Hospital School of Medicine, Zhejiang University, and transferred to the neonatal intensive care unit. Demographic, clinical and in-hospital BF data will be collected through electronic medical records. Moreover, follow-up data will be obtained by telephone, interview or online. Measurements will be obtained using the Breastfeeding Self-Efficacy Scale-Short Form, neuroimaging with functional near-infrared spectroscopy, extrauterine growth restriction and the Ages and Stages Questionnaire. Follow-up will be performed at 3, 6 and 12 months after birth. Ethics and dissemination This study has been approved by the Women's Hospital School of Medicine Zhejiang University Medical Ethics Committee (2019-058). The study results are expected to be published in peer-reviewed journals and reported at relevant national and international conferences.

Abstract:Improved survival of preterm low birthweight (LBW) infants due to advances in neonatal care has brought issues such as postnatal development trajectories to the foreground. This study pools evidence from three cluster‐randomized experiments evaluating community‐based psychosocial stimulation programs conducted from 2014 to 2017 that included 3571 rural Chinese children aged 6–24 months (51.1% male, 96.2% Han Chinese). The risk of severe cognitive delay was found to be 26.5 percentage points higher for preterm LBW children than for their peers at age 2.5, with a prevalence rate of 48.3%. Results show that psychosocial stimulation interventions can improve child cognitive development at scale, with beneficial impacts on child cognition disproportionately larger for preterm LBW children, helping them to catch up developmentally.

Factors influencing the early neurodevelopment in premature infants

Abstract:Objective To assess early neurodevelopment of premature infants and explore its influencing factors and provide guidance on health care for premature infants.Methods A total of 859 premature infants were followed up and given early comprehensive intervention.Bayley scales of infant development (BSID-I) was used to evaluate their mental and motor development.Multivariable linear regression analysis was used to explore influencing factors of mental and motor development.Results At the corrected age of 6 months,the growth retardation rate of mental and motor development was 2.4％ and 3.4％,respectively.Premature infants with birth weight ≥2 000 g,monthly household income ≥5 000 yuan and breast-feeding within the first 6 months had better mental development (all P ＜ 0.05);and those with monthly household income ≥5 000 yuan,breast-feeding within the first 6 months and with completed early family intervention had better motor development (all P ＜ 0.05).Multivariable linear regression analysis showed that mental development was positively correlated with breast-feeding within the first 6 months (β =2.827,95％ CI:0.348-5.305),higher birth weight (β =4.030,95％ CI:1.351-6.710) and higher monthly household income (β =2.909,95％ CI:0.352-5.465);and mental development was positively correlated with completed early family intervention (β =2.331,95％ CI:0.146-4.516) and higher monthly household income (β =2.848,95％ CI:0.156-5.541).Conclusions Breast-feeding within 6 months and early family intervention could promote neurodevelopment of premature infants.Thus,more attention should be paid to premature infants with lower birth weight and lower household income.

Abstract:Background: The short-term and long-term severe complications of preterm infants have brought serious psychological and economic burdens to the society and family. Therefore, our study aimed to investigate the risk factors for the mortality and serious complications in very premature infants less than 32 weeks of gestational age (GA), so as to guide the antenatal and postnatal care of very premature. Methods: The very premature infants from 1 January 2019 to 31 December 2021 from 15 member hospitals of the Neonatal Intensive Care Unit (NICU) Multi-center Clinical Research Collaboration Group in Jiangsu Province were recruited. In accordance with the plan of the intensive care unit for unified management, recruitment of premature infants is carried out on the day of admission, and discharge or death is the outcome indicator in 1-2 months by telephone follow-up. The research content mainly includes three aspects: clinical information of mother and infant, outcomes and complications. According to the final outcomes, very premature infants were divided into two categories: survival without severe complications, survival with severe complications and death. Then, univariate and multivariate logistic regression model and receiver operating characteristic (ROC) analyses were used to analyze the independent risk factors. Results: A total of 3,200 very premature infants with GA less than 32 weeks were recruited. The median GA is 30.00 (28.57, 31.14) weeks, the average birth weight is 1,350 (1,110, 1,590) g, among whom 375 premature infants survived with severe complications, and 2,391 premature infants survived without severe complications. Then, it was found that GA at birth was a protective factor for death and severe complications, whereas severe neonatal asphyxia and persistent pulmonary hypertension of the newborn (PPHN) were independent risk factors for death and severe complications in very premature infants born at less than 32 weeks of gestation. Conclusions: The prognosis of very premature infants in NICU treatment depends not only on GA, but also on various perinatal factors and their clinical management, such as preterm asphyxia and PPHN occurrence, so the next step is necessary for multicenter continuous quality improvement to improve outcomes in very preterm infants.
