Abstract:Mycoplasma pneumoniae is a leading cause of community-acquired pneumonia among school-aged children and young adults. Infections occur throughout the year but tend to surge during winter months across Europe. A characteristic epidemic cycle, where a substantial surge in the number of infections occurs, is seen approximately every 1–4 years and hypothesized to be driven by changes in immunity and a shift in circulating variants. Once thought to be an organism of low virulence, it has now been found to possess several virulence factors, including toxin production, biofilm formation and evasion of antibody-mediated immunity. The lack of a cell wall and reduced metabolic pathways limit the options for antibiotic treatment. Acquired macrolide resistance is a growing concern, with >80% of cases in China being macrolide-resistant. Although efforts have been made to develop a vaccine, there are still substantial hurdles to overcome in relation to vaccine-enhanced disease, which results from an inappropriate immune response among vaccinated individuals.

Abstract:Mycoplasma pneumoniae is an important cause of respiratory tract infections in children as well as adults that can range in severity from mild to life-threatening. Over the past several years there has been much new information published concerning infections caused by this organism. New molecular-based tests for M. pneumoniae detection are now commercially available in the United States, and advances in molecular typing systems have enhanced understanding of the epidemiology of infections. More strains have had their entire genome sequences published, providing additional insights into pathogenic mechanisms. Clinically significant acquired macrolide resistance has emerged worldwide and is now complicating treatment. In vitro susceptibility testing methods have been standardized, and several new drugs that may be effective against this organism are undergoing development. This review focuses on the many new developments that have occurred over the past several years that enhance our understanding of this microbe, which is among the smallest bacterial pathogens but one of great clinical importance.

Abstract:The review discusses the recurrence of Mycoplasma pneumoniae (M. pneumoniae), a bacterium causing atypical pneumonia, primarily affecting Europe and Asia due to climate change, immunity decline, antibiotic resistance, and genetic heterogeneity. The COVID-19 pandemic initially reduced M. pneumoniae cases due to preventative measures, but its reemergence suggests different transmission dynamics and exacerbates clinical severity with co-infections with other viruses. The pathogenicity of M. pneumoniae is attributed to its intracellular changes, toxin release, and adhesion processes, which can result in a variety of symptoms and problems. Antibiotics and immunomodulators are used in treatment, and attempts are being made to create vaccines. Effective management of its reappearance necessitates surveillance and preventative measures, especially in the context of co-infections and potential outbreaks. M. pneumoniae's resurgence highlights its reliance on a polarized cytoskeletal architecture for host cell attachment and pathogenicity through cytoadherence and cytotoxic agent synthesis. M. pneumoniae has returned even though the COVID-19 pandemic originally reduced incidence; this might be because of things like declining immunity and particular pathogenic characteristics. Meteorological factors like temperature and humidity, along with air quality, including pollutants like PM2.5 and NO2, increase susceptibility to environmental hazards. During the pandemic, non-pharmaceutical measures decreased transmission but did not eradicate the infection. Epidemics typically occur three to five years apart, emphasizing the need for ongoing study and observation. Antimicrobial resistance is a serious issue, necessitating caution and alternative therapies, especially in macrolides. COVID-19 pandemic lessons, such as mask use and hand hygiene, may help limit M. pneumoniae transmission.

Abstract:ABSTRACT To study the complete natural process of a Mycoplasma pneumoniae outbreak in a semiclosed room such as a primary school room, we investigated two separate M. pneumoniae outbreaks involving 81 students in total in two primary schools in Hangzhou, China. M. pneumoniae isolates from pharyngeal swabs were detected by fluorescence quantitative real-time PCR (RT-PCR) and culture. The class in school M had 39 students, with 12 (30.8%) with positive M. pneumoniae detection results. The class from school J had 42 students, with 13 (31.0%) positive. The strains from two classes were confirmed to represent two clones (3/4/5/7/2 and 5/4/5/7/2) and to be macrolide resistant (A2063G) according to P1 and multilocus variable-number tandem-repeat analysis (MLVA) genotyping, determination of MIC of antibiotics, and sequencing. Students with M. pneumoniae isolates detected were divided into three groups: those carrying the isolates, those with upper respiratory tract infection (URI), and those with pneumonia. Longitudinal sampling performed using pharyngeal swabs showed that the persistence of M. pneumoniae was longest in the group of students with pneumonia. M. pneumoniae causes pneumonia outbreaks in schools, and the incidence of pneumonia has a higher rate than that of URI. The persistence of M. pneumoniae , with a median duration of 79.50 days in the group of students with pneumonia, differs from that of the infection state.

Abstract:Background Mycoplasma pnuemoniae is an atypical fastidious bacteria known to cause respiratory infections of both upper and lower tract, with a wide range of clinical presentations. M. pnuemoniae is one of the major causes of community acquired pneumonias worldwide. Almost all of the M pnuemoniae outbreak incidences have been reported in closed- community or congregate settings, such as schools, universities and colleges, and hospitals and long-term care facilities. Since the clinical symptoms of M pneumoniae overlap with other pneumonia causing pathogens, detection via multiplex PCR remains paramount for accurate and timely diagnosis to curb the infection spread. In the present study we demonstrate an unexpected increase in the detection of M. pneumoniae in the outpatient population, during 2023, as compared to the previous two years. In addition, a detailed analysis of the ICD10 diagnostic codes associated with the outbreak is also presented.MethodsRespiratory samples (oropharyngeal, nasopharyngeal, sputum and EBL) summited to the HealthTrackRX laboratories were tested for the presence of M. pneumoniae and other respiratory bacterial and viral pathogens using the nanofluidic OpenArray qPCR platform (ThermoFisher, California, USA). Patient data was analyzed on the basis of age, gender and geographical location. Data was also classified in the basis of ICD10 diagnostic codes associated with the patient samples. All statistical analysis was performed using R version 3.6.0 (R Foundation for Statistical Computing).ResultsA total of 541,267 samples were submitted for testing at the HealthTrackRX facilities between January 1, 2021 and December 31, 2023. Only 13 samples tested positive for M. pneumoniae between 2021 (0.004%, n =3/61,498) and 2022 (0.006%, n = 10/165,423). The positivity rate of M. pneumoniae in 2023 increased to 0.19% (n = 621/314,346). The highest proportion of M. pneumoniae cases were detected from Georgia (26.8%), followed by Ohio (21.7%), Texas (8.5%) and California (6.4%). Most of the outbreak appears to be concentrated in large urban settings. Median age of the patients testing positive for M. pneumoniae was 10 (interquartile range [IQR] 8-18) years. Gender classification of the positive samples showed an almost equal distribution between females (49.8%) and males (50.1%).Co-infecting pathogens detected in samples positive for M. pneumoniae were similar in proportion to the M. pneumoniae negative population. The co-detected pathogens at a statistically significant difference were lower than the negative population. Survey of the ICD-10 codes submitted in conjunction with the samples suggest that the current outbreak is mostly associated with upper respiratory tract infections.ConclusionsThe present study shows an unprecedented increase in the detection of M. pneumoniae in the outpatient population in the United States during 2023. Our analysis suggests that this outbreak was not associated with any other bacterial or viral respiratory pathogen but appears to be case of "immunity debt" caused by the COVID19 pandemic. The outbreak of this atypical pathogen was concentrated in the pediatric population in large urban areas. Our study highlights the importance of performing continuous surveillance of respiratory pathogens, especially in the altered epidemiological landscape of the post COVID world.

Abstract:Mycoplasma pneumoniae( MP) is a frequent cause of community-acquired bacterial respiratory infections in children and adult. There are three kinds of diagnosis methods for MP infection: culture,serology and PCR assay. At the present,serology and PCR assay are the two main laboratory tests for MP infection. Tetracyclines,fluoroquinolones and macrolides are three main drugs for MP treatment in clinics. In recent years,it has been reported that some MP strains are acquired resistance to these antimicrobials.The strains with resistance to macrolides and fluoroquinolones have been reported in vitro. Macrolides-resistant isolates are significantly increasing in clinics. However,no clinical isolates resistant to fluoroquinolones have been reported in the literature.

Abstract:Objectives: Mycoplasma pneumoniae (M. pneumoniae) continues to pose a significant disease burden on global public health as a respiratory pathogen. The antimicrobial resistance among M. pneumoniae strains has complicated the outbreak control efforts, emphasizing the need for robust surveillance systems and effective antimicrobial stewardship programs. Design: This review comprehensively investigates studies stemming from previous outbreaks to emphasize the multifaceted nature of M. pneumoniae infections, encompassing epidemiological dynamics, diagnostic innovations, antibiotic resistance, and therapeutic challenges. Results: We explored the spectrum of clinical manifestations associated with M. pneumoniae infections, emphasizing the continuum of disease severity and the challenges in gradating it accurately. Artificial intelligence and machine learning have emerged as promising tools in M. pneumoniae diagnostics, offering enhanced accuracy and efficiency in identifying infections. However, their integration into clinical practice presents hurdles that need to be addressed. Further, we elucidate the pivotal role of pharmacological interventions in controlling and treating M. pneumoniae infections as the efficacy of existing therapies is jeopardized by evolving resistance mechanisms. Conclusion: Lessons learned from previous outbreaks underscore the importance of adaptive treatment strategies and proactive management approaches. Addressing these complexities demands a holistic approach integrating advanced technologies, genomic surveillance, and adaptive clinical strategies to effectively combat this pathogen.

Abstract:The objective of this paper is to explore the characteristics of Mycoplasma pneumoniae (MP) epidemics in Beijing, China. Patients with acute respiratory tract infection (ARTI) were enrolled from 35 sentinel hospitals in Beijing, 2015–2020. Their medical records were reviewed and respiratory specimens were collected for assay for nucleic acids of 24 respiratory pathogens, including MP. The genotypes of MP were analysed using a real-time PCR method. The domain V of 23s rRNA gene was sequenced to identify macrolide-resistant mutations. A total of 41,677 specimens of ARTI patients were included, with an MP positive rate of 6.16%. MP prevalence mainly occurred between August and January, and peaked in October. The increase in the MP detection rate was coincident with the elevation of the reported number of patients with pneumonia in the 35 sentinel hospitals. One or more respiratory pathogens were co-detected in 27.1% of the MP-positive patients. Type 1 MP remained predominant, and the macrolide-resistant rate of MP had exceeded over 90%. A2063G mutation accounted for 99.0% of macrolide-resistant MP infections. MP epidemic in Beijing mainly occurred between August and January with a remarkable high macrolide-resistant rate. MP is one of the important contributors to the pneumonia epidemic in autumn and winter in Beijing.

Abstract:We report a surge of patients, especially children and adolescents, with respiratory disease caused by Mycoplasma pneumoniae in Denmark since October 2023. While the surge has reached an epidemic level, no impact on hospital capacity has been observed; only 14% (446/3,195) of cases, primarily adults, required hospitalisation. Macrolide resistance was detected in less than 2% of samples tested. Timely monitoring of hospitalisations linked to M. pneumoniae infections has been established to inform the healthcare system, decisionmakers and the public.

Abstract:Abstract Mycoplasma pneumoniae ( M. pneumoniae ) is a cell wall‐less respiratory pathogen causing community‐acquired pneumonia and extrapulmonary manifestations. It is transmitted through close contact and shows periodic regional outbreaks. The coronavirus disease (COVID‐19) pandemic interfered with the global spread of M. pneumoniae . A large‐scale post‐COVID outbreak is currently ongoing in China. To help physicians better understand and manage this epidemic, we provide this review summarizing current knowledge on the pathogenesis, epidemic characteristics, macrolide resistance, diagnostic methods, and clinical treatment strategies for this pathogen.

Abstract:Mycoplasma pneumoniae , an obligate parasitic pathogen without cell wall, can cause severe upper and lower respiratory tract symptoms. It is the pathogen of human bronchitis and walking pneumonia , and named community-acquired pneumonia. In addition to severe respiratory symptoms, there are clinical extrapulmonary manifestations in the skin, brain, kidney, musculoskeletal, digestive system, and even blood system after M . pneumoniae infection. Hereby, we comprehensively summarized and reviewed the intrapulmonary and extrapulmonary pathogenesis of M. pneumoniae infection. The pathogenesis of related respiratory symptoms caused by M. pneumoniae is mainly adhesion damage, direct damage including nutrient predation, invasion and toxin, cytokine induced inflammation damage and immune evasion effect. The pathogenesis of extrapulmonary manifestations includes direct damage mediated by invasion and inflammatory factors, indirect damage caused by host immune response, and vascular occlusion. The intrapulmonary and extrapulmonary pathogenic mechanisms of M. pneumoniae infection are independent and interrelated, and have certain commonalities. In fact, the pathogenic mechanisms of M. pneumoniae are complicated, and the specific content is still not completely clear, further researches are necessary for determining the detailed pathogenesis of M. pneumoniae . This review can provide certain guidance for the effective prevention and treatment of M. pneumoniae infection.

Abstract:Abstract Objective Mycoplasma pneumoniae pneumonia (MPP) accounts for a major part of community-acquired pneumonia in children, and we performed this study to investigate the epidemiological and genetic characteristics of MPP after the outbreak of COVID-19. Methods A total of 15,538 throat swab samples were collected from inpatients with respiratory tract infections from January 2021 to December 2021. All specimens were detected by real-time reverse transcriptase polymerase chain reaction (RT-PCR). The P1 gene of Mycoplasma pneumoniae (MP) in positive samples was amplified and sequenced. Results From January 2021 to December 2021, a total of 15,538 children with acute respiratory tract infection were tested by real-time RT-PCR in our study, including 9,056 boys and 6,482 girls. Overall, 469 (3.0%, 469/15,538) tested positive for MP, with 266 (2.9%, 266/9,056) males and 203 (3.1%, 203/6,482) females (p = 0.48). The positive rates of MP infection in 7 years old groups were 1.31% (85/6,474), 1.87% (64/3,423), 3.65% (95/2,601), 8.02% (127/1,583), and 6.72% (98/1,458), respectively. The homology analysis of the P1 gene of 24 MP positive samples showed that the nucleotide sequence consistency was up to 91.2 to 99.3%. Conclusion After the outbreak of COVID-19, positive detection rate was the highest from 5 to 7 years old among children with MPP, and the genotype of MP in Hangzhou, China area was focused on P1 subtype, type I.

There is a much higher incidence of MP infection in older children than younger children. The latter,although with a lower infection rate,usually show a more severe clinical picture,and is prone to have tachypnea and retractions. High fever and irritative cough are characteristics of MP infections in older children. Serological method is the standard one for diagnosing MP infection,while PCR is more suitable for early diagnosis of MP infection in younger and immunocompromised patients. Macrolide is still the drug of choice for treatment of childhood MP pneumonia,but how to manage the refractory MP pneumonia has become a big challenge to clinical researchers.
