Abstract:Objective To identify associations between anthropometric indicators (height, weight and body mass index (BMI)) and both refraction and ocular biometrics in Chinese schoolchildren in Tianjin, China. Design Cross-sectional study. Participants A total of 482 (86.07%) students (6–15 years old) with no history of ocular or systemic pathologies were enrolled in this study. Methodology Height and weight were measured using standardised protocols. Ocular biometrics (axial length (AL), vitreous chamber depth (VCD) and corneal curvature (CC)) were measured by a low-coherence optical reflectometry device. Cycloplegic refraction was measured using autorefraction. The AL/CC ratio and spherical equivalent refraction (SER) were calculated. Myopia was defined as SER ≤−0.50 dioptres (D). Multiple linear regression analysis was performed to explore the associations between anthropometric indicators (height, weight and BMI) and both refraction and ocular biometrics. Results The overall prevalence of myopia was 71.16%. Overall, only height was associated with ALs, VCDs, AL/CC ratios and refractions after controlling for age, gender, parental myopia, family income, reading and writing distance and time spent outdoors. Furthermore, age-specific results demonstrated that height and weight were independently associated with refraction in participants aged 6–8 years and 9–11 years participants. Higher heights in schoolchildren were associated with longer ALs (regression coefficient b =+0.25 for each 10 cm difference in height, p<0.01), deeper VCDs ( b =+0.23, p<0.01), higher AL/CC ratios ( b =+0.04, p<0.01) and more negative refractions ( b =−0.48, p<0.01). Heavier weights were also associated with longer ALs (+0.29 mm, p<0.01), deeper VCDs (+0.29 mm, p<0.01), higher AL/CC ratios (+0.04, p<0.01) and more negative refractions (−0.48 D, p<0.01). Conclusions Height and weight remained independently related to refraction and various ocular biometrics during the early adolescent growth period after adequately controlling for covariates, which could support the idea that a shared mechanism may regulate the coordinated growth of body and eye size in children.

Abstract:Purpose. To investigate the refraction and ocular biometry characteristics and to examine the prevalence of refractive errors in preschool children aged 3 to 6 years in Shanghai, China. Methods. A school-based cross-sectional study was conducted in Jiading and Xuhui District, Shanghai, in 2013. We randomly selected 7 kindergartens in Jiading District and 10 kindergartens in Xuhui District, with a probability proportionate to size. The children underwent comprehensive eye examinations, including cycloplegic refraction and biometric measurements. Myopia, hyperopia, astigmatism were defined as spherical equivalent (SE) <= -0.50 D, SE >= + 2.00 D, and cylindrical diopters <= -1.00 D. Results. The mean SE for 3-to 6-year-old children was + 1.20D (standard deviation [SD] 1.05), and the mean axial length (AL) was 22.29mm (SD 0.73). The overall prevalence of myopia and astigmatism was 3.7% and 18.3%, respectively. No difference in prevalence of astigmatism was found across age groups. There was a statistically significant association between lower cylindrical diopters and higher spherical diopters (Spearman's correlation: -0.21, P < 0.001). Conclusion. Chinese children aged 3 to 6 years in the Shanghai area were mostly mildly hyperopic, with a low prevalence of myopia. Refractive astigmatism for children may be relatively stable throughout the preschool stage. Astigmatism was significantly associated with refractive error.
Distribution of Ocular Biometry in 7- and 14-Year-old Chinese Children.

Abstract:PURPOSE:To describe distributions of ocular biometry and their associations with refraction in 7- and 14-year-old children in urban areas of Anyang, central China.METHODS:A total of 2271 grade 1 students aged 7.1 ± 0.4 years and 1786 grade 8 students aged 13.7 ± 0.5 years were measured with ocular biometry and cycloplegic refraction. A parental myopia questionnaire was administered to parents.RESULTS:Mean axial length, anterior chamber depth, lens thickness, central corneal thickness, corneal diameter, corneal radius of curvature, axial length/corneal radius of curvature ratio, and spherical equivalent refraction were 22.72 ± 0.76 mm, 2.89 ± 0.24 mm, 3.61 ± 0.19 mm, 540.5 ± 31 μm, 12.06 ± 0.44 mm, 7.80 ± 0.25 mm, 2.91 ± 0.08, and +0.95 ± 1.05 diopters (D), respectively, in 7-year-old children. They were 24.39 ± 1.13 mm, 3.42 ± 0.41 mm, 3.18 ± 0.24 mm, 548.9 ± 33 μm, 12.03 ± 0.43 mm, 7.80 ± 0.26 mm, 3.13 ± 0.14, and -2.06 ± 2.20 D, respectively, in 14-year-old children. Compared with 7-year-old children, the older group had significantly more myopia (-3.0 D), longer axial length (1.7 mm), deeper anterior chamber depth (0.3 mm), thinner lens thickness (-0.2 mm), thicker central corneal thickness (10 μm), and greater axial length/corneal radius of curvature ratio (0.22) (all p < 0.001), as well as smaller corneal diameter (-0.03 mm, p = 0.02) and similar corneal radius of curvature. Sex differences were similar in both age groups, with boys having longer axial length (0.5 mm), deeper anterior chamber depth (0.1 mm), shorter lens thickness (0.03 mm), greater central corneal thickness (5 μm), greater corneal diameter (0.15 mm), and greater corneal radius of curvature (0.14 mm) than girls (all p < 0.01). The most important variables related to spherical equivalent refraction were vitreous length, corneal radius of curvature, and lens thickness.CONCLUSIONS:The 14-year-old group had larger parameter dimensions than the 7-year-old group except for corneal radius of curvature (unchanged) and lens thickness and corneal diameter (both smaller). Boys had large parameter dimensions than girls except for lens thickness (smaller). Axial length, corneal radius of curvature, and lens thickness were the most important determinants of refraction.
Relationship of lifestyle and body stature growth with the development of myopia and axial length elongation in Taiwanese elementary school children

Abstract:Context: The development of myopia and growth of the eye, occur at a time when body stature is increasing. Aims: To investigate the relationship of lifestyle and body growth with axial elongation and myopia development among schoolchildren aged 7 to 9 years. Settings and design: Prospective study. Materials and methods: Children in elementary schools without serious eye disorders were invited to participate. We measured cycloplegic refraction, corneal curvature, intraocular pressure, axial length, body height, and weight. Questionnaires about the children's daily lifestyles, family members' myopia and parents' socio-demographic status were completed. The children were followed up every 6 months in a 3-year period. Statistical analysis used: Bivariate correlations, simple and multiple regression. Results: Eighty-eight children participated in this study. Forty-eight were myopic at the beginning of the study, and their myopia correlated with longer axial length and parental myopia (P = 0.015, 0.012). Sixty-five children (74%) completed the study, and the rates of change per year were -0.43 ± 0.58 (mean + standard deviation) diopters in spherical equivalence, 0.32 ± 0.25 mm in axial length (AL), 5.73 ± 2.71 cm in body height, and 3.84 ± 2.23 kg in weight. The axial length change was positively correlated with the height change (P < 0.001). The myopia shift was correlated to axial length change (P = 0.000) but not correlated to height change. Using multiple regression test, near work was the only significant risk factor for myopia progression (P = 0.022). Conclusions: Our study showed that body height increment was correlated to axial length elongation but not to myopia shift in children aged 7-9 years. Genetic factors such as parental myopia and body height had a possible influence on myopia development, and the environment factor as near work intensity was related to myopia progression.

Abstract:Objectives Emmetropia depends on the precise coordination of ocular biometry, including axial length (AL), corneal curvature, lens thickness and anterior chamber depth (ACD). Disruption of this coordination leads to refractive errors such as myopia. This article aimed to determine the factors affecting ocular biometry and myopia development in young children. Design A cross-sectional study. Setting This study was conducted in a primary school in the Yanqing district of Beijing, China. Participants 792 students in grades 1–3 without hyperopia (>+2.00 D), strabismus, or amblyopia were selected. Exclusions: students had conditions affecting best corrected visual acuity and whose guardians refused to provide informed consent. Ocular biometric measurements and non-cycloplegia autorefraction were performed. The questionnaire addressed factors such as perinatal factors and environmental factors. Interventions None. Primary and secondary outcomes Ocular biometry and myopia. Results According to the multivariate logistic regression analysis, electronic screen use >2 hours/day (OR=2.175, p=0.013), paternal myopia (OR=1.761, p=0.002), maternal myopia (OR=1.718, p=0.005), taller height (OR=1.071, p<0.001), maternal education (OR=0.631, p=0.012) and maternal gestational hypertension (OR=0.330, p=0.042) were associated with myopia. AL was affected by female sex (OR=0.295, p<0.001), older age (OR=1.272, p=0.002) and taller height (OR=1.045, p<0.001). Female sex (OR=0.509, p<0.001), taller height (OR=1.046, p 2 hours each day (OR=3.596, p 2 hours each day (OR=0.431, p=0.001) influenced ACD incidence. Central corneal thickness (CCT) was associated with older age (OR=1.113, p=0.008), paternal education (OR=1.474, p=0.007), premature birth (OR=0.494, p=0.031), history of blue light therapy in infancy (OR=0.636, p=0.041) and history of incubator therapy in infancy (OR=0.263, p=0.009). Only sex influenced corneal curvature. Conclusions The factors associated with myopia were partly related to ACD and AL, and perinatal factors were associated with myopia and CCT. Abstract:BackgroundMyopia subjects usually have long axial lengths and are predisposed to deep anterior chamber depths and open angles. Jiangning eye study is a randomized, population based study in Shanghai to determine whether myopia has an effect on the prevalence of occludable angles.MethodsA total of 2478 residents aged 50 years and over were randomly selected, and 2044 (82.5%) individuals of them participated in ophthalmic examinations between November 2012 and February 2013. Eye examinations included autorefraction, noncontact tonometry, measurement of axial length, anterior chamber depth, lens thickness and spectral domain optical coherence tomography.ResultsThe crude prevalences of iris trabecular meshwork contact in Jiangning Chinese individuals with myopia, emmetropia, and hyperopia are 1.25%, 6.44%, and 7.43%, respectively. There is also iris trabecular meshwork contact and shallow anterior chamber depth in myopia subjects even with long axial lengths.ConclusionsThe prevalence of iris trabecular meshwork contact in myopia subjects is lower than their emmetropia or hyperopia counterparts. Axial length is an important factor associate with occludable angle.
Macular choroidal thickness in Chinese preschool children: decrease with axial length but no evident change with age

Abstract:Aim: To explore the distribution pattern of macular choroidal thickness (ChT) and its association with age as well as refractive status in Chinese preschoolers. Methods: School-based, cross-sectional study. A total of 550 healthy preschool children aged 3 to 6 years old from 6 kindergartens were enrolled. Comprehensive ocular examinations, including measurement of visual acuity, axial length, intraocular pressure and slit-lamp examination before cycloplegia, as well as refraction measurement and swept-source optical coherence tomography (SS-OCT) examination after cycloplegia, were performed. The macular ChT in each sector of the ETDRS grid was measured by the built-in software of SS-OCT. Results: The mean central ChT of the participants was 312±59 µm. The mean axial length and spherical equivalent refraction were 22.36±0.72 mm and 1.51±0.83 D, respectively. Axial length increased with age (P<0.001), while the spherical equivalent refraction was similar among different age groups. Similarly, no significant difference was observed in ChT in all sectors among different age groups (all P>0.05). The central ChT of 3-4, 5 and 6 years old children was 314±59 µm, 312±60 µm and 312±59 µm, respectively (P=0.920). No difference was observed in ChT in most of the sectors between genders. No statistical significant difference was observed among different refractive groups (all P>0.05), though the ChT of each sector seemed to be smaller in myopic children. Axial length and weight were the independent factors of central ChT. Children with longer axial length (β=-21.184, P<0.001) and smaller weight (β=1.502, P=0.041) tended to have thinner choroid. Conclusion: In preschool children, the ChT remains relatively stable with age, while a negative association between ChT and axial length existed. This will be helpful to elucidate the characteristics of ChT during the early refractive development.
Association of sleep disorders with subfoveal choroidal thickness in preschool children

Abstract:Abstract Objective To explore the association between sleep disorders and subfoveal choroidal thickness (SFCT) in preschool children. Methods In this population-based cross-sectional study, children aged 60–72 months were measured for SFCT using spectral-domain optical coherence tomography (SD-OCT) and for sleep disorders using the Chinese version of Children’s Sleep Habits Questionnaire (CSHQ). Multiple linear regression analyses were performed to assess the association between sleep disorders and SFCT. Results A total of 1337 children (mean (SD) age: 66.88 (3.41) months) were included in the analyses. In multivariable linear analysis, a higher total CSHQ score (indicating higher likelihood of sleep disorders) was associated with a thinner subfoveal choroid (beta, −0.070; 95% CI, −0.141 to −0.001; P = 0.046). When each of eight CSHQ subscale scores was analysed by the multivariable model, only the Daytime Sleepiness subscale score was negatively associated with the SFCT (beta, −0.115; 95% CI, −0.183 to −0.046; P = 0.001). The children with clinically significant daytime sleepiness ( n = 364, 27.2%) had significantly thinner subfoveal choroid than other children (295.47 vs. 308.52 μm, P = 0.007). Conclusions Only daytime sleepiness was significantly associated with SFCT in preschool children in this study. The potential relationship between sleep disorders during childhood and children’s ocular development needs further research.

Age-specific distribution of oculometric parameters and myopia in children aged 8-12y

Abstract:Aim: To describe the distribution of ocular biometrics and to evaluate its associations with refractive error and to assess the contribution from ocular parameters to refractive error among Chinese myopic children. Methods: This cross-sectional study evaluated subjects aged 8-12y. Keratometry, ocular biometry, and cycloplegic autorefraction were performed on each subject. Spherical equivalent refraction (SER) and ocular biometrics were assessed as a function of age and gender. The Pearson correlation analysis between SER and ocular biometrics was carried out. Multiple linear regression was performed to analyze the association between SER and ocular parameters. Results: A total of 689 out of 735 participants (321 boys, 48.1%) were analyzed, with a mean SER of -2.98±1.47 diopter (D). Axial length (AL), anterior chamber depth (ACD), corneal radius of curvature (CR), horizontal visible iris diameter (HVID), central corneal thickness (CCT) and lens power (LP) showed normal distribution. The AL, AL/CR ratio, ACD and CR increased from 8 to 12y of age, while SER and LP decreased, HVID and CCT remained stable. There was no difference in gender. SER decreased by 0.929 D for every 1 mm increase in AL and decreased by 1.144 D for every 0.1 increase in AL/CR ratio. The Pearson correlation coefficient between SER and AL was -0.538 (P<0.01) and -0.747 (P<0.01) between SER and AL/CR ratio. For the SER variance, AL explained 29.0%, AL/CR ratio explained 55.7%, while AL, CR, ACD and LP explained 99.3% after adjusting for age and gender. Conclusion: The AL, CR, ACD and LP are the most important determinants of myopic refractive error during myopia progression.
The impact of parental myopia and high myopia on the hyperopia reserve of preschool children

Abstract:Introduction: The aim of this study was to explore the association between parental myopia and high myopia with children’s refraction and ocular biometry in large-scale Chinese preschool children from the Beijing Hyperopia Reserve Study. Subjects/Methods: This cross-sectional kindergarten-based study enrolled children aged 3–6 years. Cycloplegic refraction, axial length (AL), and corneal radius (CR) were measured for all children. Parents were asked to complete a questionnaire about refractive status (no myopia, mild myopia -6D). Results: The study enrolled 2053 children (1069 boys and 984 girls), with a mean age of 4.26±0.96 years and mean spherical equivalent refraction (SER) of 1.11±0.97 diopter. Of the children, 90.7% had at least one myopic parent, and 511 children (24.9%) had at least one highly myopic parent. SER decreased significantly with increasing severity of parental myopia (P<0.001)). Preschool children’s myopia was independently associated with parental myopia (OR, 10.4 and 11.5 for one and two highly myopic parent(s)). Age (OR=1.1), gender (OR=1.7; girls as references), near work time (OR=1.2), and both maternal (OR, 1.4 and 2.0 for moderate and high myopia) and paternal myopia (OR, 1.6 and 1.9 for moderate and high myopia) were independent risk factors for lacking hyperopia reserve. Conclusion: Severe parental myopia was associated with a lower SER, longer AL, and higher AL/CR ratio in preschool children. Parental myopia and near work may predispose children to faster elimination of hyperopia reserves before exposure to higher educational stress.

Distribution of Refraction and Ocular Biometric Parameters in a Population of Junior Middle School Children in Anyang of Henan Province

Abstract:OBJECTIVE:To investigate the distribution of spherical equivalent and ocular biometric parameters in a population of grade 7 children in central China.METHODS:2 363 grade 7 students of junior high schools were randomly sampled. The students have been examined at baseline and followed up annually. Detailed questionnaires and most of the ocular examinations were performed. Spherical equivalent (Seq) was calculated as sphere +1/2 cylinder from cycloplegic autorefraction. Myopia was defined as spherical equivalent ≤ -0.5D, hyperopia as spherical equivalent ≥ +0.5D and emmetropia between -0.5D and +0.5D. The Lensar LS900 was used to measure corneal curvatures, axial length and anterior chamber depth. Right eyes results were analyzed.RESULTS:Among 2 267 children who have the baseline examination, 1 839 children participated the follow up examination(response rate, 81.1%), with an mean age of 14.7 years (range, 12.9-17.6) and male ratio of 48.4%. The mean spherical equivalent refraction was (-2.62 ± 2.19)D. The prevalence rates of myopia, high myopia, emmetropia and hyperopia were 82.7%, 7.1%, 9.8% and 7.5%. The cumulative incidence rates of myopia and high myopia were 47.1% and 4.5% respectively. Axial length, anterior chamber depth, and corneal curvature were normally distributed. The mean axial length, anterior chamber depth, and corneal curvature were (24.8 ± 1.1)mm, (3.18 ± 0.25)mm, (42.8 ± 1.4)D and (43.9 ± 1.6)D respectively. Axial length was longer, anterior chamber depth was deeper, and corneal were flatter in the male(P = 0.000). Axial length and anterior chamber depth correlated negatively with refraction.CONCLUSIONS:A moderate myopic distribution of refraction was present in the grade 9 students in central China. The prevalence rates of myopia and high myopia were relatively high in this 14-year-old population compared to other countries. It is necessary to strengthen the prevention of juvenile myopia.
Investigation of the Associated Factors Leading to Myopia in Five Primary School Students in Hangzhou

Abstract:Objective To investigate the associated factors leading to myopia in primary school students in a relatively developed region in Hangzhou.Methods This was a cross-sectional study.By cluster sampling,primary students from 5 schools (grades 1-6) in Zhaohui District,Hangzhou,were recruited for this investigation.The right eye was examined and data were collected on spherical refraction,cylinder refraction,axial length and corneal curvature.Information was also obtained on family history,time spent on near reading after school and on weekends,and time spent on outdoor activities both after school and on weekends.A one-way ANOVA was used for analysis.Results There were significant differences in gender,family history,time spent on near reading and outdoor activity between myopic and non-myopic groups.Females were more likely to be myopic than males (63.7％ vs.67.4％).The students with myopic parents (F=10.559,P＜0.05) or those who spent more time on near reading on weekends (F=7.555,P＜0.05) and after school (F=2.492,P＜0.05) were more likely to be myopic.On the other hand,outdoor activities both after school (F=11.351,P＜0.05) and during weekends (F=6.387,P＜0.05) reduced the prevalence of myopia.Conclusion Myopia in primary school students is associated with gender,family history,near reading and outdoor activity.So both genetic and environmental factors contribute to the development of myopia.The effect of environmental factors,especially near reading,is time-dependent,and ultraviolet exposure during outdoor activity plays a significant role.
Corneal and Lenticular Biometry in Chinese Children with Myopia

Abstract:Background This study aimed to evaluate the distribution characteristics of corneal and lenticular parameters in Chinese children with myopia and explored their association with other ocular components. Methods In this cross-sectional study, all ocular biometric parameters were measured using a Zeiss IOLMaster 700 Biometry. Simulations of the corneal and lenticular curvature radii were implemented using a customised MATLAB program based on cross-sectional swept-source optical coherence tomography images obtained from the same device. The associations of the calculated and simulated refractive powers of the cornea and lens with other ocular parameters were evaluated. Results In total, 119 children with myopia were recruited. Boys had a deeper anterior chamber and longer axial length (AL) than girls, while girls had steeper anterior corneal and lenticular curvatures and greater corneal and lenticular power. Children aged 10 years and older showed a larger anterior lenticular radius of curvature (sR(al)) and less lenticular power (P-L,P-OCT) than younger participants. There was a significant positive correlation between AL and the anterior corneal radius of curvature, regardless of sex or age. The sR(al) exhibited a significant increasing trend, and P-L,P-OCT exhibited a declining trend with a longer AL only in children younger than 10 years. Conclusion AL is the most influential factor in the determination of spherical equivalent refractive error, while decreases in both corneal and crystalline lens power are significantly inversely correlated with axial elongation.
Correlation Between Increase of Axial Length and Height Growth in Chinese School-Age Children

Abstract:Purpose To identify the relationship between the increase in axial length (AL) and height in school-age children and explore the influence of refractive status on such a relationship. Methods In this 5-year cohort study, 414 Chinese children (237 boys) aged 6–9 years (mean 7.12) underwent measurements annually. AL was measured using the Lenstar; height with the children standing, without shoes; and refraction using subjective refraction without cycloplegia. Participants were divided according to the refractive status: persistent emmetropia, persistent myopia, and newly developed myopia. The measurement time points of the persistent emmetropia and persistent myopia groups were marked as T 1 , T 2 , T 3 , T 4 , and T 5 . The time of myopia onset in the newly developed myopia group was marked as t 0 ; the preceding time points were marked as t −1 , t −2 , and so on, and the succeeding as t 1 , t 2 , and so on. The association between increase in AL and height was analyzed using simple correlation analysis. Results The mean changes in AL, height, and refraction were 1.39 mm, 23.60 cm, and −1.69 D, respectively, over 5 years in all children. The increase in AL and height were positively correlated for T 1 ~T 2 , T 1 ~T 3 , T 1 ~T 4 , and T 1 ~T 5 ( r = 0.262, P < 0.001; r = 0.108, P = 0.034; r = 0.165, P = 0.001; r = 0.174, P = 0.001, respectively). The changes in AL and height in the newly developed myopia group were significantly correlated ( r = 0.289, P = 0.009) after myopia onset ( t 0 ~ t 2 ). Conclusion The increase in AL and height were positively correlated, especially in the newly developed myopia group after myopia onset. Thus, when children grow quickly, AL elongation should be monitored.

Abstract:Objective Gender differences in ocular biometric measurements of opposite-sex and same-sex twin pairs are still unclear. We aimed to investigate the difference between ocular biometric measurements in adolescent twin pairs. Materials and methods This retrospective study included a total of 64 eyes of 64 adolescents from 32 twins. The ocular biometric measurements and refractive prediction error (RE) were acquired from four groups of dizygotic (DZ) twins: boys from same-sex twin-pairs (SSM, n = 20), boys from opposite-sex twin-pairs (OSM, n = 8), girls from opposite-sex twin-pairs (OSF, n = 8), and girls from same-sex twin-pairs (SSF, n = 29). Results The mean age of the patient was 9.92 ± 2.84 (range: 6–18) years. Overall, boys had higher height, AL, WTW, but lower Ks, and Kf than girls ( p < 0.05). Specifically, SSF was found to have the lowest lens thickness (LT), anterior chamber depth (ACD), central corneal thickness (CCT), white to white (WTW), and axial length (AL) levels, while the highest keratometry readings in the flat (Kf) and steep (Ks) levels compared with OSM, OSF, and SSM adolescents ( p < 0.05). Compared with the OSF adolescents, ACD levels of the SSF adolescents were significantly lower [(2.99 ± 0.35) and (3.26 ± 0.15) mm, p = 0.033)], but Kf indicator was significantly larger [(43.93 ± 1.64) and (42.91 ± 1.75), p = 0.016)]. Conclusion Our study indicates that there was a significant difference in ocular biometric measurements between twin pairs, and sharing the uterus with a DZ twin SSF has smaller ocular indicator measurements. Our findings provide information on the eyeball and refractive development in adolescents.

Abstract:Purpose: To explore the associations between macular choroidal and retinal thickness and axial elongation in non-myopic and myopic junior students. Methods: In this school-based longitudinal observational study, axial length was measured by optical low-coherence reflectometry, and choroidal thickness and retinal thickness were measured by spectral-domain optical coherence tomography. Myopia was defined as non-cycloplegic objective spherical equivalent refraction ≤ -0.50 diopters. Structural equation modeling and multiple linear regression models were used to analyze the associations between baseline choroidal and retinal thickness with axial elongation. Results: Out of 1307 students examined at baseline in 2017, 1197 (91.58%) returned for follow-up examination in 2018, with a median age of 12.00 years (interquartile range [IQR], 1.00) and included 667 boys (55.72%). Within a 1-year period, the median axial elongation of right eyes was 230 µm (IQR, 180) in boys and 200 µm (IQR, 160) in girls (P = 0.032). The thinner temporal choroidal thickness was associated with greater 1-year axial elongation only in myopic students (β, -0.20; 95% confidence interval [CI], -0.37, -0.03), the thinner temporal retinal thickness was associated with greater 1-year axial elongation in both non-myopic (β, -2.67; 95% CI, -4.52, -0.82) and myopic (β, -0.99; 95% CI, -1.68, -0.30) students, after adjustment for sex, age, and height. Subfoveal and nasal choroidal and retinal thickness were not significantly associated with axial elongation in either non-myopic or myopic students. Conclusions: A thinner temporal choroid at age 12 years may predict greater 1-year axial elongation in myopic students, and a thinner temporal retina may predict greater 1-year axial elongation in both non-myopic and myopic students. This finding may help to identify children at risk and control axial elongation with potential preventive strategies.

Abstract:Importance: Cohort studies could not only reveal associations between change of refractive components and onset/progression of myopia, but also risk factors, which is important for understanding mechanism and providing strategies. Background: Prevalence of myopia is high in Shanghai, being reported to be 52.2% in children aged 10 years old. Design: Cohort study. Participants: A total of 1856 students from six randomly selected primary schools in Baoshan District, Shanghai. Methods: Children underwent comprehensive ocular measurement, including axial length (AL), corneal curvature radius and cycloplegic auto-refraction. Questionnaires about eye usage time were collected. Grade 1 students were followed for 4 years, and grade 2 and 3 students for 2 years. Main outcome measures: (i) Change of spherical equivalent (SE) and AL and (ii) risk factors for progression and incidence of myopia. Results: The average 2-year progress of SE was 0.91D, 0.91D and 1.11D for grade 1, 2 and 3, respectively, and the average elongation of AL was 0.70 mm, 0.64 mm and 0.71 mm, respectively. Only parental myopia, but not near work time, near work diopter time, outdoor activity time or attending tutoring classes, was associated with myopia incidence and progression in the present population. Using baseline SE could be a simple and effective indicator for myopia prediction. Conclusions and relevance: Incidence and progression of myopia is relatively high in schoolchildren in Shanghai compared with children of Western countries, East Asia and other parts of China. Effective strategies to control myopia prevalence are in urgent need.
Characteristics of axial length and interocular axial length difference in Chinese children and teenagers

Abstract:Abstract Purpose To investigate axial length and interocular axial length difference distributions and correlating factors in Chinese children and teenagers. Methods This study retrospectively reviewed the axial lengths (AL) and interocular axial length differences (ALD) of 4422 children. Sex, ocular alignment, outdoor activities, total media exposure (TV watching and electronic devices), sleep duration, parental myopic state, and parental age and their relationships with mean AL and the ratio of axial length to corneal radius curvature (AL/CR) were assessed. Factors associated with the mean AL, AL/CR, ALD, and the interocular difference in AL/CR (△AL/CR) in all participants and four age groups. Results The mean AL and ALD varied mainly by age. Children with esotropia (in preschoolers, P = 0.014, 0.029), 15–21 h of outdoor durations per week(all, P = 0.017), and > 14h total media exposure per week (all, P = 0.033) were more likely to have shorter AL or AL/CR. Esotropia(in primary school, P = 0.001), shorter weekdays sleep duration (all, P = 0.009), or long weekend sleep duration (all, P = 0.033) were positively associated with ALD or △AL/CR. Conclusion AL and ALD were mainly associated with age and esotropia. Sleep duration on weekdays or weekends might associate with the mean or interocular difference in axial length distribution.
