Abstract:BACKGROUND:Facial lifting and volume restoration are the major strategies of facial rejuvenation, but the aging of facial soft tissues has not been sufficiently explored.OBJECTIVE:This study aimed to reveal the age-related changes in full facial soft tissue of Han Chinese by using computed tomography.MATERIALS AND METHODS:The measurements were performed on head computed tomographic images of 200 Han Chinese subjects (100 men and 100 women). The thickness of facial soft tissue was measured at 20 anthropological landmarks of the skull.RESULTS:The thickness at rhinion was increased, whereas the thickness at midphiltrum, supradentale, and infradentale was decreased, indicating the significant differences in both sexes. Besides, the thickness at glabella, nasion, supramentale, and menton was decreased significantly in women. The thickness at supraorbital and lateral orbit was increased with aging in either sex. The thickness at frontal eminence, infraorbital, supraglenoid, and gonion showed significant age-related increases in women, and the thickness at zygomatic arch, supra M2, and infra M2 was significantly decreased in men.CONCLUSION:These results suggest that soft tissues in different areas of the face are selectively thickened or thinned with aging, thus providing a reference for rejuvenation procedures.
Aging of the Midface Bony Elements: A Three-Dimensional Computed Tomographic Study

Abstract:BACKGROUND: The face loses volume as the soft-tissue structures age. In this study, the authors demonstrate how specific bony aspects of the face change with age in both men and women and what impact this may have on the techniques used in facial cosmetic surgery.METHODS: Facial bone computed tomographic scans were obtained from 60 Caucasian patients (30 women and 30 men). The authors' study population consisted of 10 male and 10 female subjects in each of three age categories. Each computed tomographic scan underwent three-dimensional reconstruction with volume rendering, and the following measurements were obtained: glabellar angle (maximal prominence of glabella to nasofrontal suture), pyriform angle (nasal bone to lateral inferior pyriform aperture), and maxillary angle (superior to inferior maxilla at the articulation of the inferior maxillary wing and alveolar arch). The pyriform aperture area was also obtained. The t test was used to identify any trends between age groups.RESULTS: The glabellar and maxillary angle in both the male and female subjects showed a significant decrease with increasing age. The pyriform angle did not show a significant change between age groups for either sex. There was a significant increase in pyriform aperture area from the young to the middle age group for both sexes.CONCLUSIONS: These results suggest that the bony elements of the midface change dramatically with age and, coupled with soft-tissue changes, lead to the appearance of the aged face.

Abstract:Background: Aging leads to a panoply of changes of facial morphology. The present study was conducted to analyze modifications of the facial skeleton with aging, using high-resolution imaging and comparing the same individuals at two time points. Methods: The electronic medical record system was reviewed since its inception in 2001 for patients for whom two computed tomographic scans of the midface were obtained at least 9 years apart. The computed tomographic scans were converted into three-dimensional craniofacial models for each patient, using the initial and the follow-up computed tomographic scan data. The models were used to highlight areas of bone growth and bone resorption using a color scale and to perform a cephalometric analysis. Results: Seven patients with a mean age of 61 years and computed tomographic scans on average 10.3 years apart were included. Bone resorption was consistently present (100 percent) at the pyriform aperture and the anterior wall of the maxilla. Resorption was noted at the superocentral (71 percent), inferolateral (57 percent), and superomedial (57 percent) aspects of the orbital rim. Resorption occurred earlier at the inferolateral orbital rim followed by the superomedial orbital rim in later decades of life. Paired-analysis of change in the orbital rim height and width demonstrated a mean decrease over time but was not significant. Conclusion: Bone remodeling in the same individual, over a period of 10 years, was characterized by resorption at the pyriform aperture; anterior wall of the maxilla; and superocentral, superomedial, and inferolateral aspects of the orbital rims.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Previous studies on alterations in superficial fat compartments with age yielded contradictory results; the superficial fat tissue volume or thickness was reported to increase, decrease, or remain unchanged.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Aims</h3><p>To investigate volumetric and dimensional changes in superficial midfacial fat tissue retrospectively in patients who underwent a repeated computed tomography (CT).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>A total of 262 patients (mean age, 46 years; median elapsed time between two CT imagings, 56 months) were included. Axial and sagittal planes were used for measurements of the medial part of the superficial cheek fat (SCF), which is divided into three compartments. The thickness, width, and height of each compartment were measured, and volumes were calculated for both hemifaces. Additionally, results were compared between males and females, and between three age groups: <45, 45–54, and ≥55 years.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>The total volume of SCF and inferior compartment were significantly increased compared to the first CT findings (p<0.05). Upper and middle compartment volumes were found to decrease with age (p<0.05). Thickness was decreased in the upper and middle compartments but not in the lower compartment. The width of the lower and middle compartments increased (p<0.05), while the upper compartment did not show a significant change with age (p>0.05). The height of the superficial fat layer was significantly increased at follow-up (p<0.05). The results were similar for both sexes and all age groups (p>0.05).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>SCF undergoes significant volume and dimensional changes with aging. Therefore, superficial fat tissue should be considered for enhancement of cosmetic treatments.</p></section>

Abstract:Abstract Background: The midfacial fat is highly compartmentalized, and the deep fat compartments provide structural support for the midface. Understanding the age-related changes in the midfacial deep fat compartments is important for midfacial rejuvenation physiologically. However, previous studies showed conflicted results of these changes. Objectives: This study aims to quantify the age-related changes of deep fat compartments in the midface using ultrasound. Methods: A cross-sectional study was conducted to evaluate subjects with age more than 25 years old and the subjects were divided into four age groups equally (25–34, 35–44, 45–54, and ≥55). The thickness of the suborbicularis oculi fat compartment (SOOF) and deep medial cheek fat compartment (DMCF) was measured using a portable ultrasound device. Mean thickness differences among different age groups were analyzed by one-way analysis of variance. Multiple linear regression adjusted for body mass index (BMI) and sex was also conducted. Results: A total of 60 subjects were enrolled. The thickness of SOOF and DMCF tends to decrease with age. The mean decrease of SOOF and DMCF from the youngest group to the oldest group was 0.63 mm (36%) and 0.70 mm (25.83%), respectively. The relationship between aging and thickness of SOOF and DMCF remains significant after adjusting for BMI and sex (both P < 0.001). Conclusion: The thickness of deep fat compartments decreases with the natural aging process. The study may offer clinical implications for targeted volume rejuvenation to achieve a more youthful and natural face.

Abstract:Background: Morphologic and anatomical changes of the aging nose affect the surgical strategy of rhinoplasty in older patients, but age-related changes in noses have yet to be fully characterized. This study aimed to determine the evolution of nasal soft tissue and bone in aging noses using computed tomography. Methods: Computed tomographic images of 200 Asian patients were included. The study population was divided into two groups according to age and sex: a younger group (20 to 35 years of age) and an older group (65 to 80 years of age). Measurements of nasal soft tissue and bone on computed tomography were performed using Osirix 9.0 software. Results: With regard to soft-tissue changes, the nasal skin and soft-tissue envelope thickness at rhinion, supratip, and nasal tip increased significantly with aging in both sexes. Increase of nasal depth and decrease of nasolabial angle were significant in both sexes. Nasal length and nasofrontal angle showed significant age-related increases in men; decrease of nasal tip angle and increase of pyramidal angle were significant in women. With regard to nasal bone changes, the bone length decreased significantly in women and the bone thickness decreased significantly in both sexes. Nasofrontal angle in men and pyramidal angle in women increased significantly. Conclusion: These results suggest that the aging process leads to drooping of the nasal tip, thickened nasal skin and soft tissue, and resorption of the nasal bone, providing a reference for rhinoplasty in older patients.

Abstract:BACKGROUND: Facial aging is a dynamic process involving the aging of soft-tissue and bony structures. In this study, the authors demonstrate how the facial skeleton changes with age in both male and female subjects and what impact these structural changes may have on overall facial aesthetics.METHODS: Facial bone computed tomographic scans were obtained from 60 female and 60 male Caucasian subjects. Twenty male and 20 female subjects were placed in three age categories (20 to 40 years, 41 to 64 years, and 65 years and older). Each computed tomographic scan underwent three-dimensional reconstruction with volume rendering. Edentulous patients were excluded. The following measurements were obtained: upper face (orbital aperture area, orbital aperture width, and curvilinear analysis of the superior and inferior orbital rims), midface (glabellar angle, pyriform angle, maxillary angle, and pyriform aperture area), and lower face (bigonial width, ramus breadth, ramus height, mandibular body height, mandibular body length, and mandibular angle).RESULTS: The orbital aperture width and orbital aperture area increased significantly with age for both sexes. There was a significant increase in orbital aperture size (increase in height of the superomedial and inferolateral orbital rim) in both sexes. The glabellar and maxillary angles decreased significantly with age for both sexes, whereas the pyriform aperture area significantly increased for both sexes with age. Mandibular length and height both decreased significantly for each sex. The mandibular angle significantly increased with age for both sexes.CONCLUSIONS: These results suggest that the skeletal morphology of the face changes with age. This change in skeletal morphology may contribute to the appearance of the aging face.

Abstract:Supplemental Digital Content is available in the text Abstract In this study, data related to the total soft tissue thickness and fat layer thickness of 41 anatomical landmarks were extracted from the craniofacial computerized tomography data of 280 Chinese individuals (160 males and 120 females). The measurements were assessed according to the following factors: a. sex, b. age, and c. sex × age. Descriptive statistics and a differential analysis were carried out in each group to analyze both the total soft tissue thickness and fat layer thickness. The results showed the following. 1. The results showed that the greater the total thickness of the soft tissue, the thicker the fat layer. 2. The thicknesses of the head and face soft tissues are strongly affected by sex. The total thickness of all landmark points in the men, except for the zygomatic points, was on average greater than that in the women. In contrast to the total thickness, the fat layer, except for the point of rhinion, in the women was larger than that in the men. 3. In the comparison of the 4 age groups, most feature points did not show an evident increasing or decreasing trend with age in the total thickness of the soft tissue. However, regarding the thickness of fat, the thickness at the other points, except for the feature infraorbital fossa point, decreased with age. 4. In the analysis of the sex × age group, no statistically significant differences were found at any landmark points. This paper is significant for facial reconstruction and cosmetic surgery in the Chinese population.

Abstract:PURPOSE:Age-related change in ocular appearance occurs throughout adulthood, and involutional alterations in periocular tissues are common in middle-aged or older adults. This study sought to investigate significant differences in periocular anthropometry between young and middle-aged adults of Chinese Han ethnic group.METHODS:A total of 589 Chinese Han adults were recruited for cross-sectional investigation: 309 young adults (20 to 30 years), and 280 middle adulthood (50 to 70 years). Standardized, frontal view photographs were taken from each subject for measurements of 17 ocular anthropometric parameters (11 linear, three angular and three indicial parameters) detailing dimensions of palpebral fissures, and eyebrows. Eyelid crease morphologies were also evaluated.FINDINGS:Distribution of eyelid crease subtypes were different between the two age-groups, and an eyelid subtype (Type VI Closed Crescent) was uniquely found in middle-aged adults. Significant differences were found in majority of the ocular parameters. For most of the fissure-related measurements, values were greater in young adults. For some distance-dimensions between eyes and eyebrows, values were greater in the middle-aged adults. Detailed evaluation of periocular parameters of different age was individually performed for two subjects, which embodied findings from the cross-sectional analysis.CONCLUSIONS:By comparison of anthropometries between age-groups, we found alterations in periocular and eyelid morphologies under effect of aging, with association of laxity and loss of elasticity in periocular tissues. This study was the first to provide normative periocular dimensions of early and middle adulthoods in Chinese Han, which is useful for considerations in esthetic surgical decision-making for this population.
Comparison of 3D Facial Soft-Tissue Changes in Non-Extraction Adult Patients

Abstract:Objective To evaluate the 3-dimensional characteristics of facial appearance changes in non-extraction female adult patients.Methods 39 non-extraction subjects from 2011 to 2013 were selected.They all took 3D facial scan images and lateral cephalographs at initial (T0) and after treatment (T1).The superimpositions of 3D images were analyzed by color mapings in a same coordinate system.Results There were no statistical differences in lip changes after treatment.There exhibited soft-tissue drape by 1.5 mm significantly at subzygomatic and mandibular angle regions,and by 1.0 mm at cheek and temporal regions.Soft tissues showed no significant differences at suborbital,paranasal and zygomatic arch regions.Conclusions 3D facial scan is recommended for analysis of soft tissue changes.Soft tissue changes existed in various regions after non-extraction treatment.

Abstract:The periorbita, cheeks, and midface are an area of complex facial form and function. There is a particular pleasing visual harmony of these regions in the youthful face. With time, the robust glow and fullness of youth fades and diminishes. The skin is the first component to show the passage of time as it dulls, wrinkles, and blemishes. This is followed by a slow loosening and sagging of soft tissues as they descend from their once-fixed points of the skeletal foundation. The smooth transitions between regions are separated and hollows and ridges appear. These processes continue throughout life and eventually are accompanied by the visible consequences of bone resorption. The faces age, and as the countenance of the human spring turns through summer into autumn, function follows form, and symptoms accompany signs. A thorough understanding of both youthful and elder anatomy as well as the stigmata of aging is prerequisite to perform rejuvenating procedures. This article discusses these changes and how the older anatomy becomes the new norm.

Abstract:Purpose: To review and summarize studies on the anatomy and involutional changes of the midface. Methods: A PubMed search was performed searching for studies on the anatomy and involutional changes concerning the midface. Results: The anatomy of the midface is complex. Studies of involutional change vary in scientific quality and have conflicting results. However, it appears that among the more common changes, there is a decrease in the maxillary and pyriform angle, with changes to the orbital floor position. Further, there appears to be an inferior migration of the fat compartments of the midface during aging, exacerbating the hollow of the palpebromalar groove and causing a deepening of the nasojugal groove. Changes to the volume of the buccal extension of the buccal fat pad exacerbate these changes and contribute to the gestalt changes associated with facial aging. Here, we review the major characteristics of soft tissue and bony changes on the midface, with special reference to their anatomic relationships. Conclusions: The major findings characterizing midface aging are related largely to the soft tissue. However, more robust studies are required to quantify these changes and to appraise their impact on the overall manifestation of aging.

Esthetic Evaluation of Facial Cheek Volume: A Study Using 3D Stereophotogrammetry.

Abstract:OBJECTIVES:To investigate the influence of cheek volume on facial esthetics judged by orthodontists and non-specialists.MATERIALS AND METHODS:A 25-year-old female's natural and smiling face was captured by 3D stereophotogrammetry. Cheek volume of the 3D image was altered to different degrees three-dimensionally. For the natural and smiling face, seven groups of facial images were created: decreased grade I/II/III, increased grade I/II/III, and the original one. Thirty orthodontists and 30 nonspecialists were invited to perform esthetic evaluation of the original and transformed images using a questionnaire. Data were calculated with one-way analysis of variance (least significant difference test) and independent samples t test.RESULTS:Compared to nonspecialists, orthodontists gave lower esthetic scores to the decreased grade III facial images (maximum deformation degree: 7.500 mm and 7.327 mm in natural and smiling face-oriented image groups, respectively). The decreased grade III facial images also received the highest age ranks. However, the increased grade III facial images received the lowest scores and highest age ranks from nonspecialists (maximum deformation degree: 6.994 mm and 5.300 mm in natural and smiling face-oriented image groups, respectively) ( P < .01).CONCLUSIONS:Orthodontists and nonspecialists showed different esthetic evaluation of varied cheek volume. The influence of cheek volume in orthodontic diagnostic analysis needs further consideration.
Imaging Analysis of Abnormal Soft Tissue in Cheeks

Abstract:Objective To improve the recognition of abnormal image of soft tissue in cheeks. Methods Images of 47 cases with abnormal soft tissue in cheeks were retrospectively studied. Results (1) 15 cases were originated from soft tissue of the cheeks in which 8 cases were benign and 7 cases were malignant.6 cases were invaded by maxillofacial affection nearby,in which 1 case was benign and 5 cases were malignant.26 cases were involved by lesions of jaw bones,including benign tumor of jaw bones in 9, malignant tumor of jaw bones in 14 and chronic osteomyelitis of jaw bones in 3 and all had bony destruction; (2) According to originated position,21 cases were not originated from jaw bones,in which there were 5 cases (23.8%) with destruction of jaw bones;(3)In the 26 cases with abnormal soft tissue of cheeks originated from jaw bones, 9 were benign tumors in which all the soft tissue masses were mainly localized inside of the jaw bones, while 14 cases were malignant tumors in which all the soft-tissue masses were localized both inside and outside of the jaw bones. There was remarkable difference between the two groups(P=0.000).All 9 cases of benign tumors appeared as dilated destruction of jaw bones, while in the 14 cases of malignant tumors, 12 cases showed osteolytic bone destruction and the other 2 cases showed ossific bone destruction,there was significant difference between the two groups(P=0.000).Conclusion CT and MRI are of significance in judging original position, extent and differential diagnosis between benign and malignant lesions of abnormal soft tissue in cheeks.

Abstract:BACKGROUND: Controversy exists as to whether the changes of midface aging (elongation of the lower eyelid caused by infraorbital hollowing, flattening of the malar eminence, and increased prominence of the nasolabial fold) are attributable to gravitational migration of the check fat or to fat atrophy.METHODS: The anatomical explanation of the gravitational migration of the malar fat pad is based on previously reported magnetic resonance imaging studies of the midface cheek fat in young and older subjects and histologic studies of the superficial fascia in face-lift patients.RESULTS: Clinical and laboratory observation of the midface malar fat pad suggests that, during repeated movements of animation, levator muscle contraction and shortening produces tissue expansion pressures within the overlying cheek fat pad that cause the acutely prominent nasolabial fold of animation.CONCLUSIONS: Over time, chronic recurrent tissue expansion of the skin of the lower anterior cheek combined with stretching of the supporting intrafat pad fascial septa results in downward migration of the malar fat pad, causing infraorbital flattening and permanent prominence of the nasolabial fold. Although not the primary cause of facial aging changes, fat atrophy may contribute secondarily in those individuals whose faces become thinner as they age.

Abstract:Background: Eyelid morphology is highly susceptible to aging. Previous studies have described the process of eyelid aging in Caucasians; however, anthropometric data describing aging in Chinese eyelids are lacking. Therefore, this study aimed to quantitatively analyze the effect of aging on the eyelids of Chinese women through a three-dimensional (3D) anthropometry. Methods: In this prospective, cross-sectional study, 3D photos were captured from 188 healthy Chinese Han women, who were categorized into four age groups. Anthropometric landmarks were identified for the measurement of eyelid parameters, and a unified coordinate system was built into each subject. Linear and angular measurements were computed from the coordinates and were compared between the groups. Results: An age-related decrease was observed in the ocular width, outer canthal width, palpebral fissure height, and multiple angular measurements. This indicated upper eyelid ptosis, which reduced the lateral and superior visual field. Lateral shift of the upper eyelid arc was not observed. The lower eyelid underwent slight elevation with increasing age. Aging rendered the periorbital region esthetically less pleasing, as shown by changes in the multiple proportion indices computed. Furthermore, an intergroup comparison indicated that eyelid aging was accelerated after the age of 40 years among Chinese women. Conclusion: This study used 3D photography to quantitatively analyze how eyelids in Chinese women changed with age. The anthropometric data collected could help with antiaging cosmetic surgery planning and postoperative assessment.
