Abstract:1297 Objectives The standard uptake value (SUV) of 18F-FDG PET/CT is important in the diagnosis and treatment response evaluation of lung cancer. The aim of the present study is to investigate the clinical value of SUV of 99mTc-3PRGD2 SPECT/CT in lung cancer diagnosis. Methods Twenty-four patients with pathologically approved lung cancer (21 non-small cell lung cancer and 3 small cell lung cancer),4 patients with benign diseases (3 pulmonary consolidation and 1 adenoma), and 6 healthy volunteers were enrolled in the study. All subjects were injected with 629.0-921.3 MBq of 99mTc-3PRGD2 (mean ± SD, 759.3 ± 80.1 MBq), and a SPECT/CT (Discovery NM/CT 670, GE Healthcare) was performed 30 minutes later. The images were analyzed using Q.Metrix (GE Healthcare) after OSEM reconstruction with resolution recovery technology, scatter correction and Xray CT based attenuation correction. The mean SUV (SUVmean) and the maximum SUV (SUVmax) of the superior, middle, and inferior mediastinum and the right lower lobe of the liver in healthy volunteers, as well as the corresponding values of the primary lesions in the patients, were measured with a 3 cm-diameter volume of interest (VOI). The longest diameters of the primary lesions in CT images of the patients were measured as well. Results The median age of the lung cancer patients, benign disease patients and healthy volunteers was 60.5 (43-76) years old, 62.0 (52-76) years old and 56.0 (44-64) respectively. Fourteen of the 24 lung cancer patients (58.3%), 2 of the 4 benign disease patients (50%) and 3 of the 6 healthy volunteers (50%) were male. The mean value of the SUVmean and the SUVmax of the superior, middle, and inferior mediastinum were 0.64 ± 0.27 g/ml and 1.06 ± 0.41 g/ml. The SUVmean and the SUVmax of the right lower lobe of the liver in healthy volunteers were 1.85 ± 0.72 g/ml and 2.35 ± 0.92 g/ml. The longest diameter of the primary lesion in CT images in the 24 lung cancer patients was 3.68 ± 2.02 cm; the corresponding SUVmean and SUVmax values were 1.67 ± 1.04 g/ml and 3.68 ± 2.12 g/ml. In the 4 benign lesions, the longest diameter in CT images was 2.60 ± 0.45 cm, and the SUVmean and SUVmax were 0.64 ± 0.27 g/ml and 0.96 ± 0.36 g/ml, respectively. The diagnostic sensitivity, specificity and accuracy of SUVmean and SUVmax of 99mTc-3PRGD2 in lung carcinoma verses the mean + 2SD of mediastinum were 0.67, 1.00, 0.71 and 0.67, 1.00, 0.71, respectively. Conclusions (1) The mean + 2SD of the SUV mean and SUVmax of mediastinum seems a valid threshold for 99mTc-3PRGD2 SPECT/CT in lung carcinoma diagnosis; (2) SUV of 99mTc-3PRGD2 SPECT/CT, as an indicator of lung cancer, is in good compliance and accordance with pathological diagnosis. The SUV of 99mTc-3PRGD2 SPECT/CT is a promising tool in the diagnosis and differential diagnosis of lung cancer.
