Abstract:Photodynamic therapy (PDT) as an emerging therapeutic method has drawn many attentions in the treatment field for cancer. Photosensitizer, which can convert photon energy into cytotoxic species under light irradiation, is the core component in PDT. The design of photosensitizers still faces problems of light absorption, targeting, penetration and oxygen dependence. With the rapid progress of material science, various photosensitizers have been developed to produce cytotoxic species for treatment of tumor with high selectivity, safety, and noninvasiveness. Besides, the applications of photosensitizers have been expanded to diverse cancer treatments such as drug release, optogenetics and immune checkpoint blockade. In this review, we summarize the recent advances of photosensitizers in various therapeutic methods for cancer. Prevailing challenges and further prospects associated with photosensitizers are also discussed.

Abstract:Photodynamic therapy (PDT) has been developed as a potential cancer treatment approach owing to its non-invasiveness, spatiotemporal control and limited side effects. Currently, great efforts have been made to improve PDT effect in safety and efficiency. In this review, we highlight recent advances in innovative strategies for enhanced PDT, including 1) the development of novel radicals, (2) design of activatable photosensitizers based on TME and light, and (3) photocatalytic NADH oxidation to damage mitochondrial electron transport chain. Additionally, the new mechanisms for PDT are also presented as an inspiration for the design of novel PSs. Finally, we discuss current challenges and future perspectives in the clinical practice of these innovative strategies. It is hoped that this review will provide a new angle for understanding the relationship between the intratumoural redox environment and PDT mechanisms, and new ideas for the future development of smart PDT systems.

Abstract:Natural photosensitizers (PSs) and their derivatives have drawn ever‐increasing attention in photodynamic therapy (PDT) for their wild range of sources, desirable biocompatibility, and good photosensitivity. Nevertheless, many factors such as poor solubility, high body clearance rate, limited tumor targeting ability, and short excitation wavelengths severely hinder their applications in efficient PDT. In recent years, fabricating nanostructures by utilizing molecular assembly technique is proposed to solve these problems. This technique is easy to put into effect, and the assembled nanostructures could improve the physical properties of the PSs so as to meet the requirement of PDT. In this concept, we focus on the construction of natural PSs and their derivatives nanostructures through molecular assembly technique to enhance PDT efficacy (Figure 1). Furthermore, current challenges and future perspectives of natural PSs and their derivatives for efficient PDT are discussed.

Abstract:Photodynamic therapy (PDT) is a rapidly growing discipline that is expected to become an encouraging noninvasive therapeutic strategy for cancer treatment. In the PDT process, an efficient intersystem crossing (ISC) process for photosensitizers from the singlet excited state (S1) to the triplet excited state (T1) is critical for the formation of cytotoxic reactive oxygen species and improvement of PDT performance. Thermally activated delayed fluorescence (TADF) molecules featuring an extremely small singlet–triplet energy gap and an efficient ISC process represent an enormous breakthrough for the PDT process. Consequently, the development of advanced TADF photosensitizers has become increasingly crucial and pressing. The most recent developments in TADF photosensitizers aimed at enhancing PDT efficiency for bio‐applications are presented in this review. TADF photosensitizers with water dispersibility, targeting ability, activatable ability, and two‐photon excitation properties are highlighted. Furthermore, the future challenges and perspectives of TADF photosensitizers in PDT are proposed.

Abstract:Photodynamic therapy (PDT) has garnered an increasing interest by both researchers and clinicians as a non-invasive therapy that allows a selective treatment towards tumors temporally and spatially, which is a relatively safe strategy without obvious systemic side effects and drug resistance. However, the lack of O2 content in solid tumors is one of the most important factors limiting the effectiveness of PDT, as the hypoxic tumor regions not only induce tumor development and metastasis but also reduce the responsiveness of tumor tissues to PDT along with other treatments like chemotherapy and radiotherapy. Therefore, dealing with hypoxia is desirable and has become a hot research topic for both academic and clinical fields. This review summarizes the latest strategies and applications of reconstructing the tumor microenvironment and developing novel anti-hypoxia photosensitizers (PSs). It also summarizes the synergistic strategies that combine PDT with other treatment approaches to achieve complementary effects, significantly improving the treatment accuracy and efficacy. By evaluating several examples, this review concludes that the innovation of "theranostics" PS is the core impetus for the efficacy of PDT. In addition, it highlights that the structural designing of PS can effectively regulate the energy release process of the excited state and achieve imaging and PDT simultaneously.

Abstract:Many sensitizers have not only photodynamic effects, but also sonodynamic effects. Therefore, the combination of sonodynamic therapy (SDT) and photodynamic therapy (PDT) using sensitizers for sono-photodynamic therapy (SPDT) provides alternative opportunities for clinical cancer therapy. Although significant advances have been made in synthesizing new sensitizers for SPDT, few of them are successfully applied in clinical settings. The anti-tumor effects of the sensitizers are restricted by the lack of tumor-targeting specificity, incapability in deep intratumoral delivery, and the deteriorating tumor microenvironment. The application of nanotechnology-based drug delivery systems (NDDSs) can solve the above shortcomings, thereby improving the SPDT efficacy. This review summarizes various sensitizers as sono/photosensitizers that can be further used in SPDT, and describes different strategies for enhancing tumor treatment by NDDSs, such as overcoming biological barriers, improving tumor-targeted delivery and intratumoral delivery, providing stimuli-responsive controlled-release characteristics, stimulating anti-tumor immunity, increasing oxygen supply, employing different therapeutic modalities, and combining diagnosis and treatment. The challenges and prospects for further development of intelligent sensitizers and translational NDDSs for SPDT are also discussed.

Abstract:Photodynamic therapy is a noninvasive cancer treatment that utilizes photosensitizers to generate reactive oxygen species upon light exposure, leading to tumor cell apoptosis. Although photosensitizers have shown efficacy in clinical practice, they are associated with certain disadvantages, such as a certain degree of toxicity and limited availability. Recent studies have shown that natural product photosensitizers offer promising options due to their low toxicity and potential therapeutic effects. In this review, we provide a summary and evaluation of the current clinical photosensitizers that are commonly used and delve into the anticancer potential of natural product photosensitizers like psoralens, quinonoids, chlorophyll derivatives, curcumin, chrysophanol, doxorubicin, tetracyclines, Leguminosae extracts, and Lonicera japonica extract. The emphasis is on their phototoxicity, pharmacological benefits, and effectiveness against different types of diseases. Novel and more effective natural product photosensitizers for future clinical application are yet to be explored in further research. In conclusion, natural product photosensitizers have potential in photodynamic therapy and represent a promising area of research for cancer treatment. Abstract:Zinc oxide nanoparticles (ZnO NPs) are important semiconductor materials with interesting photo-responsive properties. During the past, ZnO-based NPs have received considerable attention for photodynamic therapy (PDT) due to their biocompatibility and excellent potential of generating tumor-killing reactive oxygen species (ROS) through gentle photodynamic activation. This article provides a comprehensive review of the recent developments and improvements in optical properties of ZnO NPs as photosensitizers for PDT. The optical properties of ZnO-based photosensitizers are significantly dependent on their charge separation, absorption potential, band gap engineering, and surface area, which can be adjusted/tuned by doping, compositing, and morphology control. Here, we first summarize the recent progress in the charge separation capability, absorption potential, band gap engineering, and surface area of nanosized ZnO-based photosensitizers. Then, morphology control that is closely related to their synthesis method is discussed. Following on, the state-of-art for the ZnO-based NPs in the treatment of hypoxic tumors is comprehensively reviewed. Finally, we provide some outlooks on common targeted therapy methods for more effective tumor killing, including the attachment of small molecules, antibodies, ligands molecules, and receptors to NPs which further improve their selective distribution and targeting, hence improving the therapeutic effectiveness. The current review may provide useful guidance for the researchers who are interested in this promising dynamic cancer treatment technology.

Abstract:Photodynamic therapy (PDT) utilizes light-activated photosensitizers (PSs) to generate toxic species for therapeutics. It has become an emerging solution for cancer treatment because of its specific spatiotemporal selectivity and minimal invasiveness. Noble metal (Ru, Ir and Pt) complexes are of increasing interest as photosensitizers for their excellent photophysical, photochemical, and photobiological properties. In this review, we highlight recent advancements in the development of noble metal complex photosensitizers for PDT during the last 5 years. We will summarize the design strategies of noble metal complexes for efficient and precise PDT, including increasing the light penetration depth, reducing the oxygen-dependent nature and improving target ability. Finally, we summarize recent efforts for the development of noble-based PSs and discuss the limitations of such PSs in clinical application and future perspectives in this field, such as the combination of PDT with other treatment modalities.

Abstract:Photodynamic therapy (PDT) is a non-invasive approach for tumor elimination that is attracting more and more attention due to the advantages of minimal side effects and high precision. In typical PDT, reactive oxygen species (ROS) generated from photosensitizers play the pivotal role, determining the efficiency of PDT. However, applications of traditional PDT were usually limited by the aggregation-caused quenching (ACQ) effect of the photosensitizers employed. Fortunately, photosensitizers with aggregation-induced emission (AIE-active photosensitizers) have been developed with biocompatibility, effective ROS generation, and superior absorption, bringing about great interest for applications in oncotherapy. In this review, we review the development of AIE-active photosensitizers and describe molecule and aggregation strategies for manipulating photosensitization. For the molecule strategy, we describe the approaches utilized for tuning ROS generation by attaching heavy atoms, constructing a donor-acceptor effect, introducing ionization, and modifying with activatable moieties. The aggregation strategy to boost ROS generation is reviewed for the first time, including consideration of the aggregation of photosensitizers, polymerization, and aggregation microenvironment manipulation. Moreover, based on AIE-active photosensitizers, the cutting-edge applications of PDT with NIR irradiated therapy, activatable therapy, hypoxic therapy, and synergistic treatment are also outlined.

Photodiagnosis and Photodynamic Therapy

Abstract:Photodynamic therapy (PDT) is an established clinical treatment technology with which utilizes excitation light of a specific wavelength to excite photosensitizers (PSs) for generating reactive oxygen species (ROS) which lead to the cancer cell death. Over the past decades of PDT research, apparent progress have been made in the development of PSs. However, many inherent characteristics of traditional PSs have caused many various problems in PDT, such as low treatment efficiency at aggregation state and shallow treatment depth. In allusion to these problems, aggregation-induced emission (AIE)-based PSs have been reported in recent years. Here, the article reviews the design strategy and the biomedical applications of AIE PSs in detail, which begins with a summary of traditional PSs for a comparison between traditional PSs and AIE PSs. Subsequently, the different functional AIE PSs in photodynamic cancer cells ablation and image-guided therapy are discussed in detail taking controllable excitation wavelength, stimulus response and PDT/photothermal therapy synergistic effect as examples. These studies have demonstrated the great potential of AIE PSs as effective theranostic agents in recent years. And the review provides references for the development of new PSs and hopefully spur more research interest in AIE PSs for future research.

Abstract:Conventional photodynamic therapy (PDT) approaches face challenges including limited light penetration, low uptake of photosensitizers by tumors, and lack of oxygen in tumor microenvironments. One promising solution is to internally generate light, photosensitizers, and oxygen. This can be accomplished through endogenous production, such as using bioluminescence as an endogenous light source, synthesizing genetically encodable photosensitizers in situ, and modifying cells genetically to express catalase enzymes. Furthermore, these strategies have been reinforced by the recent rapid advancements in synthetic biology. In this review, we summarize and discuss the approaches to overcome PDT obstacles by means of endogenous production of excitation light, photosensitizers, and oxygen. We envision that as synthetic biology advances, genetically engineered cells could act as precise and targeted "living factories" to produce PDT components, leading to enhanced performance of PDT.

Abstract:Photodynamic therapy (PDT) is a non-invasive treatment modality against a range of cancers and nonmalignant diseases, however one must be aware of the risk of causing phototoxic reactions after treatment. We herein report a bioinspired design of next-generation photosensitizers (PSs) that not only effectively produce ROS but undergo fast metabolism after treatment to overcome undesirable side effects. We constructed a series of β-pyrrolic ring-opening seco-chlorins, termed beidaphyrin (BP), beidapholactone (BPL), and their zinc(II) derivatives (ZnBP and ZnBPL), featuring intense near-infrared absorption and effective O2 photosensitization. Irradiation of ZnBPL led to a non-cytotoxic, metabolizable beidaphodiacetamide (ZnBPD) via in situ generated O2.- but not 1 O2 , as revealed by mechanistic studies including time-resolved absorption, kinetics, and isotope labeling. Furthermore, water-soluble ZnBPL showed an effective therapeutic outcome, fast metabolism, and negligible phototoxic reactions.

Abstract:In recent years, the use of organic small-molecule photosensitizers, which make up the majority of photosensitizers, has significantly enhanced the therapeutic outcomes of various diseases including cancer and antibacterial treatment because of their benefits of biocompatibility, minimal invasiveness, minimal toxicity, non-drug-resistance, and facile molecular tailoring. However, many organic small-molecule photosensitizers suffer from poor photostability, targeting, and controllability, which limit the bench-to-bedside transformation. Self-assembly strategies could effectively handle these problems and difficulties. This review provides an overview of the self-assembly strategies of organic small-molecule photosensitizers for photodynamic therapy in recent years, including molecular design and modification, stimuli-responsive, photosensitizer-drug conjugate, metal coordination, and others. The article also discussed the prospects and challenges of organic small-molecule photosensitizers in clinical applications.

Abstract:There is no doubt that phototherapy has become one of the most popular frontiers for both research and treatment. In practice, the selection of photosensitizers (Ps) or photothermal agents (PTAs) is significant to the efficacy of phototherapy. With the increased demand for precision medicine, the development of new generations of reagents is more and more urgent, especially for the clinical applications. Therefore, we review a series of Ps and PTAs that are used successfully in the clinic. At the molecular level, we have outlined the designing mechanism, structure–activity relationships, and working strategies for clinical applications, involving: (1) Rational molecular modification to improve precision : introducing long alkyl chains and aromatic groups into the molecular structure to regulate lipophilicity and to enhance targeting; introducing different types and positions of substituents, utilizing the heavy atom, altering the conjugation structure, and improving purity to modulate both the light absorption of Ps and the penetration depth of light; (2) Adjusting the drug-light interval in PDT to improve the delivery : a sufficient drug-light interval ensures the effective transportation of Ps into tumor cells, enabling the exertion of phototoxicity; (3) Reasonable laser dose to improve molecular efficiency : using a fractionated and low-dose laser irradiation can control the appropriate rate of oxygen consumption, so as to persistently generate reactive oxygen species (ROS); (4) Synergistic effect of phototherapy with other treatment modalities to overcome limitations : the combination of phototherapy with other treatment modalities can compensate for the shortcomings of phototherapy and enable its wider application in clinics. Efforts have always been made in phototherapy to enhance tumor targeting and reduce the side effects. We expect these strategies can provide valuable insights for the development of novel, highly selective, and rapidly metabolized Ps or PTAs, thereby promoting the development of the next generation of phototherapy for clinical applications.

Abstract:The integration of diagnosis and therapy based on natural products has been receiving considerable attention in recent years because nature can contribute many fantastic functional molecules with good biocompatibility and low toxicity. Diagnostic and therapeutic agents combined with the technique of photodynamic therapy (PDT) and sonodynamic therapy (SDT) have been extensively developed thanks to the advantages of PDT and SDT, such as good selectivity, low toxicity, and noninvasive treatment for cancers and other diseases compared with traditional treatments. In this review, we summarize the recent advances in theranostic agents for natural products categorized as porphyrins, perylenequinone, curcumin, and others. Some representative examples of disease diagnosis in fluorescence/photoacoustic imaging and disease treatment in PDT/SDT were introduced. Potential limitations and future perspectives of these natural products for theranostic agents were also discussed.

It is believed that sonodynamic therapy is advantageous over conventional photodynamic therapy due to its better penetration abilities. However, the efficacy of sonodynamic therapy is limited by the poor reactive oxygen species generation abilities of current sonosensitizers. To fulfill the unreleased potential of sonodynamic therapy, it is reasonable to optimize the properties of the sonosensitizers according to the basic mechanism of sonodynamic therapy. Here, in this review, the most recent research progress relating to the mechanism of sonodynamic therapy will be emphasized and a series of possible principles for the design of effective sonosensitizers will be proposed. Further challenges relating to the clinical translation of sonodynamic therapy will also be discussed to give a clear picture of the problems that must be overcome.
