Abstract:A 56-year-old female presented to the dermatology clinic with pigmentation on the lips, gums and tongue for 5 years. The amout and range of lesions gradually increased. The patient denied having digestive tract symptoms. The family history was unremarkable. Physical examination showed brown to black spotted patches on the lips, gums and buccal mucosa (Fig. A, B). Light brown stripes appear on multiple nails (Fig. C). Except for the mucocutaneous lesions, no other significant signs were found. No abnormality was detected in gastrointestinal endoscopy. The feces examination did not find red blood cells. No endocrine abnormal was found. Dermoscopy demonstrated brownish uniform pigmentation on the pale red background of the lips. Brown longitudinal stripes with uniform color were visible on the nails. Histopathology of a biopsy taken from the lip showed hyperpigmentation in the basal layer of the epidermis (Fig. 1). A diagnosis of Laugier-Hunziker (LH) syndrome was made. LH syndrome is a rare clinical syndrome, often manifested as pigmentation in the lips and buccal mucosa, hands, feet and nails. The diagnosis of the disease is exclusive. It is necessary to distinguish it from other diseases with mucocutaneous pigmentation, including Peutz- Jeghers syndrome, Addison syndrome, and melanoma. Unlike Peutz- Jeghers syndrome, it often occurs in middle age and has no polyps in the gastrointestinal tract. Peutz- Jeghers syndrome often occurs at childhood and accompanied with multiple polyps in the gastrointestinal tract, although they have similar mucocutaneous appearance. Addison's disease often manifests as diffuse skin and mucous membrane pigmentation, especially in the exposed parts of the face and limbs. Abstract:The objective of this case report is to describe a patient who was referred to a specialized medical service to diagnose lesions in the oral mucosa, lips, and a generalized skin manifestation. The patient is a 69-year-old melanodermic woman with a medical history of high blood pressure and chronic kidney disease. She presented with complaints of skin lesions that initially developed on her arms, subsequently spreading to her body and mouth, accompanied by painful symptoms. Hyperpigmented patches were observed on her neck, dorsum, arms, and legs, and white striae were noted in her nails.

Abstract:Oral lichen planus (OLP) is an autoimmune condition that affects the squamous epithelium of the oral mucosa and is also recognized as a potentially malignant disorder. OLP predominantly impacts female patients and is seldom observed in adolescents and young adults. Here, we present a case of a 20-year-old patient diagnosed with OLP. During the physical examination, the most noteworthy findings included hyperpigmented lesions on the lower lip and the classic presentation of reticular OLP, marked by bilateral symmetrical striae on the buccal mucosa.

Abstract:Women are primarily affected by lichen planus, a chronic autoimmune skin and mucosal disorder in their 40s to 60s. Medication, systemic disorders, and mental stress are some of the factors that can cause it, though the precise cause is still unknown. Middle-aged females are the main victims of the disease; children are rarely affected. Oral lichen planus (OLP) is less common in atrophic and erosive forms. There is disagreement over whether OLP causes cancer; therefore physicians need to keep a close watch for any intraoral lichenoid lesions, and patients with OLP are advised to follow up regularly. This case report details a 52-year-old woman's right buccal mucosa and tongue affected by reticular lichen planus.

Abstract:A 45-year-old brown-skinned woman presented with a 5-year history of asymptomatic grayish brown lesions on the face, arms, and legs. She had no medical history of previous diseases or contact dermatitis. She revealed that she had used olive oil all over her body for the last 8 years every other day. Physical examination showed multiple, well-defined, oval-shaped, dark brown, smooth-surfaced macules with no elevated active borders (Figure 1). There were no associated lesions on the nails, scalp, or mucosae. Serologic tests for autoantibodies and hepatitis A, B, and C virus infections were non-reactive. A patch test for olive oil was also negative. A skin biopsy revealed epidermal atrophy, orthokeratosis, basal cell vacuolation, and a band-like lymphocytic infiltrate in the upper portion of the dermis with abundant colloid bodies and pigmentary incontinence in the papillary dermis (Figure 2). A diagnosis of lichen planus pigmentosus (LPP) was confirmed, and betamethasone butyrate propionate was applied for 2 months over the lesions, with a limited therapeutic effect. Clinical improvement was seen only after she discontinued the olive oil application (Figure 3).

Abstract:Oral lichen planus (OLP), a well-known entity, is often neglected; however, its assessment is considered essential for evolving future strategies for treatment. Thirty-three patients with OLP from 200 consecutive patients with oral mucosal lesions were studied according to demography. Tobacco chewing, smoking, and alcohol intake were recorded. Examination of the oral cavity, using a head light and tongue depressor, was undertaken to define the morphology of the lesion(s). Colored photographs were an essential part of the process. OLP, a frequently encountered manifestation of oral mucosal lesions, was responsible for 16.50% of the total number of lesions. There was a preponderance of lesions in women aged 40 to 59 years, in contrast to men in the same age group. The duration of the lesions was variable, and tobacco chewing/smoking and amalgam fillings might have exacerbated them. Morphology ranged from a reticular, erythematous pattern to an erosive one. The diagnosis is largely clinical, but it can be confirmed by histopathology.

Abstract:Lichen planus (LP) is a chronic autoimmune inflammatory dermatosis that is characterized by purple, pruritic plain topped papules, involving the skin, mucous membrane, nail and hair follicles. According to the site of involvement there are various forms of LP. Palmoplantar LP is one of the rare clinical variants, which is meanwhile accompanied with the oral involvement being extremely uncommon. Herewith, we report a case of a woman who has oral LP with palmoplantar involvement, treated with oral acitretin and topical corticosteroids. Now the patient is responding well to the treatment.

Abstract:OBJECTIVE:To investigate the epidemiological and clinical characteristics of a relatively large cohort of patients with oral lichen planus (OLP) from eastern China.STUDY DESIGN:A total of 518 patients with histologically confirmed OLP in a long-term follow-up period (6 months-21.5 years) were retrospectively reviewed in our clinic.RESULTS:Of the 518 patients, 353 females and 165 males were identified. The average age at diagnosis was 46.3 years (range 9-81 years) with the buccal mucosa being the most common site (87.8%). At initial presentation, white lichen and red lichen was seen in 52.3% and 47.7% patients, respectively. Of these, 5 (0.96%) patients previously diagnosed clinically and histopathologically as OLP developed oral cancer. All of them were the females with no a history of smoking or alcohol use.CONCLUSIONS:Clinical features of eastern Chinese OLP patients were elucidated. Notably, approximately 1% of OLP developed into cancer, which provides further evidence of potentially malignant nature of OLP.

Abstract:In dermatology, lichenoid describes lesions with a violaceous hue that is a clinical reflection of basal cell damage in the epithelium and dense mononuclear infiltrate in the sub‐epithelium. The violaceous color results from pigment incontinence due to basal cell damage and the Tyndall effect. Although classically described in lichen planus, a lichenoid hue is noted in the oral mucosa in several other disorders that often lead to diagnostic dilemmas. Early and accurate diagnosis is important for the appropriate management of the underlying condition and prognostication. Dermatologists play a central role in managing such patients since, apart from the oral mucosa findings, the cutaneous features also help to significantly differentiate various conditions. Mimickers of oral lichen planus include nicotine stomatitis, oral submucous fibrosis, oral lichenoid lesions, mucosal discoid lupus erythematosus, pemphigus vulgaris, paraneoplastic pemphigus, mucous membrane pemphigoid, fixed drug eruption, plasma cell cheilitis/gingivitis, and erythema multiforme. While a detailed history and clinical examination can help reach a diagnosis in most cases, histopathology, immunofluorescence, and other relevant investigations help establish the diagnosis.

Involvement of the oral mucosa without concurrent genital or skin lesions has very rarely been reported in the literature. Here we report on two Chinese women with LSA limited to the dorsum of the tongue, and describe the clinical manifestations and histopathological findings of these two patients.

Abstract:PURPOSE:To study the clinical and pathological characteristics of oral lichen planus (OLP) in a large sample. MATERIALS AND METHODS:A comprehensive analysis was conducted on 105 patients with oral lichen planus (OLP), considering various factors including sex, age, disease site, lesion type, lesion area, morphological characteristics, self-reported symptoms, and history of systemic diseases. Histopathological examination was performed for each patient, and the pathology results were analysed according to sex and age group. RESULTS:70.5% of the OLP patients were female, and OLP was most likely to occur in the cheek, followed by the tongue, lips, gums and palate. The patients with moderate pain according to the VAS score accounted for 60%. Thirty-nine percent of the OLP patients had a systemic disease, and the most common clinical type of OLP was nonerosive. Most of the pathological results showed liquefaction degeneration of basal cells and infiltration of lamina propria lymphocytes. There was no statistically significant difference in pathological manifestations between male and female patients, and there were statistically significant differences in pathological manifestations among different ages patients. CONCLUSION:This study analysed the sociodemographic data and clinical manifestations of 105 OLP patients to guide follow-up treatment planning and disease monitoring. Moreover, pathological manifestations should be analysed to avoid delayed treatment and to monitor for carcinogenesis. Furthermore, the correlation of pathological manifestations among OLP patients with different sexes and ages is conducive to further research on the specific differential manifestations and possible underlying mechanisms involved.

Abstract:A 50-year-old female presented with painful oral ulcers for five years and the painful erosion at vulval region for two years.She was diagnosed as oral lichen planus for many times and had no effect with corresponding treatment.Physical examination found the erosions in right buccal mucosa,on the upper and lower gingiva,the aditus vaginae,and the lower part of labium minus.Histopathologic examination supported the diagnosis lichen planus.This condition is a variant of erosive lichen planus.

Abstract:Oral lichen planus (OLP) is a chronic inflammatory oral mucosal disease that occurs more frequently in middle-aged and elderly female patients. Previous studies indicate that OLP is a T-cell dysfunction-induced localized autoimmune disease. Clinically, six types of OLP, namely reticular, papular, plaque-like, atrophic/erosive, ulcerative, and bullous types, can be identified. OLP more commonly affects buccal mucosa, tongue, and gingiva. It always has a bilateral and symmetric distribution of the oral lesions. Plaque-like and atrophic/erosive OLP may be misdiagnosed as oral leukoplakia and oral erythroleukoplakia, respectively. Our previous study found serum autoantibodies in 195 (60.9%) of the 320 OLP patients. Specific serum anti-nuclear, anti-smooth muscle, anti-mitochondrial, gastric parietal cell, thyroglobulin, and thyroid microsomal autoantibodies are present in 28.1%, 8.4%, 1.6%, 26.3%, 21.3%, and 24.4% of 320 OLP patients, respectively. Furthermore, we also discovered that 21.9%, 13.6%, 7.1%, 0.3%, and 14.8% of 352 OLP patients have hemoglobin, iron, vitamin B12, and folic acid deficiencies, and abnormally high serum homocysteine level, respectively. Therefore, it is very important to examine the serum autoantibody, hematinic and homocysteine levels in OLP patients before starting the treatments for OLP patients. Because OLP is an immunologically-mediated disease, corticosteroids are the drugs of choice for treatment of OLP.
[Oral Lichen Planus: a Retrospective Study of 724 Chinese Patients].

Abstract:OBJECTIVE:To retrospectively analyze oral lichen planus (OLP) patients and to summarize the clinical characteristic of OLP in Chinese patients.METHODS:A total of 724 patients with histologically and clinically confirmed OLP were collected from department of oral mucosa in School of Stomatology, Ninth People's Hospital, School of Medicine, Shanghai Jiao Tong University between 1978 and 2006.RESULTS:The OLP patients were followed up from 1 month to 259 months (mean 21 months). Of the 724 patients, 68.23% were women and 31.77% were men. The buccal mucosa was the most common site of involvement in each form, followed by the dorsal tongue, ventral tongue, lip, gingiva, palate and the floor of mouth. The reticular form of the disease was the predominant type in 53.82% of patients at initial presentation. About 48.76% of the patients had multiple oral sites of involvement. Fifteen OLP lesions developed into carcinoma, the malignant transformation rate was 2.07%.CONCLUSIONS:Because OLP has potential of malignant change, a long time and periodical follow-up is of great importance for detection of malignant lesions.
An Unusual Presentation of Lichen Planus

Abstract:Lichen planus is a chronic papulosquamous eruption of the skin, scalp, nails, and mucous membranes. "Pruritic, purple, polygonal, planar, papules, plaques" are the traditional six "P's" of lichen planus. We describe an unusual case of lichen planus presenting as cellulitis. A 64-year-old lady with a past medical history of pyoderma gangrenosum, inclusion body myositis, and chronic kidney disease presented with a two-week history of swelling, erythema, tenderness, hyperkeratotic plaques, and blisters on the medial aspect of both thighs. She had a previous history of pyoderma gangrenosum exacerbations with similar presentations; however, current lesions were different from prior presentations. We considered the differential diagnoses of bacterial cellulitis versus pyoderma gangrenosum exacerbation. Due to the difference in these lesions from previous episodes, the patient was empirically treated for bacterial cellulitis with intravenous cefepime and linezolid. The infectious diseases team was consulted and valacyclovir was added to cover for possible herpes infection, with no improvement in symptomatology. Dermatology was then consulted, and a clinical diagnosis of psoriasiform dermatitis was made. A skin biopsy was obtained and the patient was started on prednisone. There was an immediate improvement in the papules within 24 hours. The papules cleared, leaving behind violaceous flat plaques, clinically diagnosed as lichen planus. The affected area was shrinking as compared to previous examinations. The skin biopsy was reported as chronic psoriasiform dermatitis with the main differential of lichen planus. The patient was discharged home on a tapering dose of oral prednisone, topical clobetasol, and oral moxifloxacin. This case demonstrates the importance of familiarity with rare clinical subtypes as a suspicion for lichen planus. The vesiculobullous subtype of lichen planus, as seen in this patient, tends to present as blisters and cellulitis from infection of the bullae. Treatment of the infection alone is not enough and steroids are essential. This knowledge helps change management, allows for earlier improvement and better patient outcomes.
A Clinical Study of 674 Patients with Oral Lichen Planus in China

Abstract:BACKGROUND:Oral lichen planus (OLP) is a chronic inflammatory disease with different clinical presentations that can be classified as reticular, atrophic or erosive. Although OLP is a relatively common disorder, the reports comprising large numbers of OLP patients with specific character are lacking in the literature. The purpose of this paper was to describe the clinical characteristics of OLP in 674 Chinese patients.METHODS:A total of 674 charts of patients with histologically confirmed OLP were collected from Stomatological Hospital of Wuhan University between 1963 and 2003.RESULTS:Of the 674 patients, 65.9% were women and 34.1% were men. The most common clinical presentation was the reticular type (51.3%), and symptomatic OLP was noted in 67.5% of the patients, mainly in those with the erosive form. The erosive presentations showed significantly longer duration, more sites affected and a much greater old patients predominance than reticular or atrophic ones. About 90.9% of the patients had multiple oral sites of involvement and isolated lower lip lichen planus were observed in 60 cases (8.9%) and isolated gingiva lichen in only one case (0.2%). Skin involvement of lichen planus was found in 11.4% of patients. No statistically significant differences could be identified between OLP and diabetes, cardiovascular disease, smoking or alcohol use. Precipitating factors that resulted in an exacerbation of the disease were frequently noted and included foods, stress, dental cusp and poor oral hygiene. The transformation of OLP into malignancy was observed in four patients at sites previously diagnosed by clinical examination as erosive or atrophic lichen planus.CONCLUSIONS:Patients with OLP in China usually present with distinctive clinical morphology and characteristic distribution and few may display lesions with a confusing array of forms mimicking other diseases. A long time follow up is of utmost importance to detect its malignant transformation.

Abstract:Oral lichen planus (OLP) is a common oral mucosal chronic disease,the prevalence rate is 0.1％ to 4.0％ of the general population,and it occurs mostly in women aged 30 to 60.OLP pathogenesis has not yet been clear;currently it is considered as a class of T cell-mediated autoimmune diseases.It is related to genetics,drug,infection,oral irritation and mental factors.The clinical manifestations are white or gray textures,pimples,plaques,etc.,which may be accompanied by congestion,erosion,blisters and other lesions.Because 1.1％ of OLP will develop into oral squamous cell carcinoma,World Health Organization lists it as a potentially malignant disease,and clinicians should pay attention accordingly.Here we summarize the pathogenesis,diagnosis and treatment of OLP.

Abstract:A woman in her 70s presented with an enlarged and variegated plaque on her left axilla, which she had had for 4 years. The lesion started as a small eczematous, pink-to-brown plaque. As the lesion was asymptomatic and slow growing, she did not initially seek treatment. Recently, the color of the lesion had changed to an alarming dark brown color (<a class="figure-link section-jump-link" href="#dcg200012f1">Figure</a>, A). No breast mass or lymphadenopathy was observed. Her medical and family history was unremarkable. A skin biopsy specimen was obtained and submitted for further histopathologic analysis.
