Abstract:Background/Objective: Non-invasive neuromodulation techniques, such as repetitive Transcranial Magnetic Stimulation (rTMS) and transcranial Direct Current Stimulation (tDCS), have increasingly been investigated for their potential as treatments for neurological and psychiatric disorders. Despite widespread dissemination of these techniques, the underlying therapeutic mechanisms and the ideal stimulation site for a given disorder remain unknown. Increasing evidence support the possibility of non-invasive neuromodulation affecting a brain network rather than just the local stimulation target. In this article, we present evidence in a clinical setting to support the idea that non-invasive neuromodulation changes brain networks. Method: This article addresses the idea that non-invasive neuromodulation modulates brain networks, rather than just the local stimulation target, using neuromodulation studies in tinnitus and major depression as examples. We present studies that support this hypothesis from different perspectives. Main Results/Conclusion: Studies stimulating the same brain region, such as the dorsolateral prefrontal cortex (DLPFC), have shown to be effective for several disorders and studies using different stimulation sites for the same disorder have shown similar results. These findings, as well as results from studies investigating brain network connectivity on both macro and micro levels, suggest that non-invasive neuromodulation affects a brain network rather than just the local stimulation site targeted. We propose that non-invasive neuromodulation should be approached from a network perspective and emphasize the therapeutic potential of this approach through the modulation of targeted brain networks.

Abstract:Thomas Edison, Albert Einstein, Nikola Tesla, Aldous Huxley, Sergei Rachmanicoff, Chopin, Goethe. What dothese people have in common? They all used hypnosis to explore and expand the boundaries of their intelligence. The aim of the current review is to investigate the impact of clinical hypnosis on Metacognition, Consciousness and Intelligence based on the counterpart model. Furthermore, we examine the role of virtual reality hypnosis in service of metacognition. The results showed that clinical hypnosis -with the power of attention and imagery- can effectively and rapidly reformat cognitive and metacognitive skills, upgrade intelligence and raise conscious experience into the higher energy levels. Hypnosis improves metacognition by re-organizing the survival-emotional-reward centers, smoothing the non-verbal pathways for effortless self-regulation. We concluded that the clinical hypnosis guarantees peak performance, innovation, success, and happiness in all areas of life. It can also play a significant role in education as a tool for dealing with the well-established learned weaknesses, for mental/emotional training and brain rewiring, facilitating learning, unlearning and relearning. Virtual reality hypnosis is a top promising technology assisting metacognitive training.
Brain Activity and Functional Connectivity Associated with Hypnosis

Abstract:Hypnosis has proven clinical utility, yet changes in brain activity underlying the hypnotic state have not yet been fully identified. Previous research suggests that hypnosis is associated with decreased default mode network (DMN) activity and that high hypnotizability is associated with greater functional connectivity between the executive control network (ECN) and the salience network (SN). We used functional magnetic resonance imaging to investigate activity and functional connectivity among these three networks in hypnosis. We selected 57 of 545 healthy subjects with very high or low hypnotizability using two hypnotizability scales. All subjects underwent four conditions in the scanner: rest, memory retrieval, and two different hypnosis experiences guided by standard pre-recorded instructions in counterbalanced order. Seeds for the ECN, SN, and DMN were left and right dorsolateral prefrontal cortex, dorsal anterior cingulate cortex (dACC), and posterior cingulate cortex (PCC), respectively. During hypnosis there was reduced activity in the dACC, increased functional connectivity between the dorsolateral prefrontal cortex (DLPFC;ECN) and the insula in the SN, and reduced connectivity between the ECN (DLPFC) and the DMN (PCC). These changes in neural activity underlie the focused attention, enhanced somatic and emotional control, and lack of self-consciousness that characterizes hypnosis.

Abstract:Non-Invasive Brain Stimulation (NIBS) stands as an advanced technology embraced by researchers and clinicians to influence thoughts, emotions, and behaviors. The prevalent NIBS methods include transcranial Direct Current Stimulation (tDCS) and Transcranial Magnetic Stimulation (TMS), both proficient in either exciting or depressing neural activities in specific cortical regions. Recently, NIBS has been integrated into hypnosis research with the goal of enhancing hypnotizability. Specifically, the limited existing studies have predominantly focused on the dorsolateral prefrontal cortex (DLPFC) due to its significant role in neutral hypnosis. Overall, these studies suggest the fascinating potential to alter hypnotizability and hypnotic phenomena, although the impact on responsiveness to suggestions remains modest. In contrast to psychological and pharmacological methods, NIBS enables alterations in hypnotic experiences that are independent of operators and noninvasive. This grants researchers the chance to employ a causal approach in investigating the brain-behavior relationship associated with suggestibility. The present paper evaluates existing NIBS studies in this domain, delving into the neurocognitive mechanisms at play and their potential implications for hypnosis research and practice.

Abstract:In this review, the role of hypnosis and related psychotherapeutic techniques are discussed in relation to the anxiety disorders. In particular, anxiety is addressed as a special form of mind/body problem involving reverberating interaction between mental and physical distress. The history of hypnosis as a therapeutic discipline is reviewed, after which neurobiological evidence of the effect of hypnosis on modulation of perception in the brain. Specific brain regions involved in hypnosis are reviewed, notably the dorsal anterior cingulate gyrus and the dorsolateral prefrontal cortex. The importance of hypnotizability as a trait, stable variability in hypnotic responsiveness, is discussed. Analogies between the hypnotic state and dissociative reactions to trauma are presented, and the uses of hypnosis in treating posttraumatic stress disorder, stressful situations, and phobias as well as outcome data are reviewed. Effects of hypnosis on control of somatic processes are discussed, and then effects of psychosocial support involving Supportive-Expressive Group Therapy and hypnosis on survival time for cancer patients are evaluated. The evidence indicates an important role for hypnosis in managing anxiety disorders and anxiety related to medical illness.

Abstract:Many attempts have been made to enhance hypnotizability. The most recent studies adopted the non-invasive brain stimulation to deactivate the dorsolateral prefrontal cortex (DLPFC) during hypnosis, indicating this as a promising approach. However, it is still no clear whether individual factors can predict the effects of stimulation on hypnotizability. In the present study we adopted the phenomenological consciousness inventory (PCI) to retrospectively assess the mental processes during hypnosis and to predict hypnotizability, here defined as "hypnoidal state." The aim was to investigate the possible role of the hypnotic susceptibility on the efficacy of a validated approach of hypnosis enhancement through cathodal transcranial electrical stimulation (tDCS) of the left DLPFC. Results indicated that the lower hypnoidal state at baseline predicted the greater enhancement after the active tDCS. These findings suggest the subjects with lower hypnotic responsiveness as the best candidates for the tDCS interventions of hypnosis enhancement, at least for the montage targeting the left DLPFC. Neurocognitive underpinnings and clinical implications of the results are discussed.

In the Intersubjective Space: Hypnosis Through a Neuropsychological Lens

Abstract:This article explores five interwoven principles about relationship that impact on attentional focus as it relates to the practice of clinical hypnosis. It first reviews how relationship is an irreducible feature of life that greatly predates the arrival of human beings. Second, it describes brain structures that, from an evolutionary perspective, appeared relatively recently, and the neuropsychological abilities those structures confer on human relationships. Third, it links those social brain structures to trance, an inborn response to novelty that is an important feature of our adaptive learning capacity. It further suggests that narrative is a multilevel concept that is deeply embodied and constitutes the sorbate from which hypnotic interactions can draw their rich impact. Finally, the article posits that hypnosis represents a skill set through which attuned clinicians engage in co-creative dances with clients, where the choreography of their interaction attends and responds to spontaneously arising and/or deliberately seeded opportunities for adaptive change. Reconstructed descriptions of interactions with clients are provided to illustrate the application of the principles presented.

Abstract:This comprehensive review delves into the cognitive neuroscience of hypnosis and variations in hypnotizability by examining research employing functional magnetic resonance imaging (fMRI), positron emission tomography (PET), and electroencephalography (EEG) methods. Key focus areas include functional brain imaging correlations in hypnosis, EEG band oscillations as indicators of hypnotic states, alterations in EEG functional connectivity during hypnosis and wakefulness, drawing critical conclusions, and suggesting future research directions. The reviewed functional connectivity findings support the notion that disruptions in the available integration between different components of the executive control network during hypnosis may correspond to altered subjective appraisals of the agency during the hypnotic response, as per dissociated and cold control theories of hypnosis. A promising exploration avenue involves investigating how frontal lobes' neurochemical and aperiodic components of the EEG activity at waking-rest are linked to individual differences in hypnotizability. Future studies investigating the effects of hypnosis on brain function should prioritize examining distinctive activation patterns across various neural networks.

Abstract:Milton Erickson first conceptualized the hypnotist trance (HT) as a unique psychological state developed in clinicians during hypnosis sessions. This qualitative study aimed to investigates HT through the experiences of 12 skilled Clinician Hypnosis Specialists (CHS). Data were collected via semi-structured face-to-face interviews, exploring participants' attitudes toward HT, its impact on their practice, and their strategies for developing and regulating it. Thematic analysis revealed that most CHS view HT as enhancing empathy, communication, and therapeutic effectiveness. However, challenges such as time distortion, hypnotic regression, and countertransference issues were also noted. The study highlights HT's dual nature – offering significant therapeutic benefits while presenting challenges that need careful management. These findings emphasize the importance of comprehensive HT training in hypnotherapy education and advocate for further research to explore HT across diverse contexts and expertise levels to deepen understanding of this complex phenomenon.

Abstract:The ability to flexibly modulate our behaviors in social contexts and to successfully interact with other persons is a fundamental, but pivotal, requirement for human survival. Although previous social neuroscience research with single individuals has contributed greatly to our understanding of the basic mechanisms underlying social perception and social emotions, much of the dynamic nature of interactions between persons remains beyond the reach of single-brain studies. This has led to a growing argument for a shift to the simultaneous measurement of the brain activity of two or more individuals in realistic social interactions-an approach termed "hyperscanning." Although this approach offers important promise in unlocking the brain's role in truly social situations, there are multiple procedural and theoretical questions that require review and analysis. In this paper we discuss this research trend from four aspects: hyperscanning apparatus, experimental task, quantification method, and theoretical interpretation. We also give four suggestions for future research: (a) electroen-cephalography and near-infrared spectroscopy are useful tools by which to explore the interactive brain in more ecological settings; (b) games are an appropriate method to simulate daily life interactions; (c) transfer entropy may be an important method by which to quantify directed exchange of information between brains; and (d) more explanation is needed of the results of interbrain synchronization itself.
Altered Brain Connectivity and Network Topological Organization in a Non-ordinary State of Consciousness Induced by Hypnosis

Abstract:Abstract Hypnosis has been shown to be of clinical utility; however, its underlying neural mechanisms remain unclear. This study aims to investigate altered brain dynamics during the non-ordinary state of consciousness induced by hypnosis. We studied high-density EEG in 9 healthy participants during eyes-closed wakefulness and during hypnosis, induced by a muscle relaxation and eyes fixation procedure. Using hypotheses based on internal and external awareness brain networks, we assessed region-wise brain connectivity between 6 ROI (right and left frontal, right and left parietal, upper and lower midline regions) at the scalp level and compared across conditions. Data-driven, graph-theory analyses were also carried out to characterize brain network topology in terms of brain network segregation and integration. During hypnosis, we observed (1) increased delta connectivity between left and right frontal, as well as between right frontal and parietal regions, (2) decreased connectivity for alpha (between right frontal and parietal and between upper and lower midline regions) and beta-2 bands (between upper midline and right frontal, frontal and parietal, also between upper and lower midline regions), and (3) increased network segregation (short-range connections) in delta and alpha bands, and increased integration (long-range connections) in beta-2 band. These higher network integration and segregation were measured bilaterally in frontal and right parietal electrodes, which were identified as central hub regions during hypnosis. This modified connectivity and increased network integration–segregation properties suggest a modification of the internal and external awareness brain networks that may reflect efficient cognitive-processing and lower incidences of mind-wandering during hypnosis.

Abstract:Aspects of hypnosis and its application in psychotherapy, psychosomatics and medicine are examined and contextualized in the 250-year history of hypnosis. Imagination as an essential element of hypnotic treatments appeared as early as 1784 as an argument rejecting the theory of animal magnetism of Franz Anton Mesmer. In somnambulism of German romanticism, another proto-form of hypnosis after 1800, concepts of the mind–body problem were dealt with, which still characterize the understanding of unconscious mental processes today. Hypnosis was at the beginning of psychoanalysis, but was not pursued further by Sigmund Freud from 1900 onwards. Nevertheless, there were some hypnoanalytical approaches in the 20th century, as well as attempts to integrate hypnosis into behavior therapy. Techniques of imagination and relaxation combine both; in particular findings from cognitive psychology explain processes of both hypnosis and cognitive behavioral therapy. The influence of social psychology brought a new perspective to the debate about the nature of hypnosis, which continues to this day: is hypnosis to be understood as a special state of consciousness or is it a completely normal, mundane interaction? The experiments that were carried out to support one side or the other were also dependent on the hypnotizability of the subjects involved, as the more difficult hypnotic phenomena such as paralysis, hallucinations or identity delusions can only be demonstrated by highly hypnotizable subjects. The fact that these are not mere compliance reactions has now been proven by many studies using imaging techniques. But even those who are moderately hypnotizable benefit from hypnosis rituals. Variables postulated by socio-cognitive hypnosis researchers, such as motivation and expectation, are relevant, as is a good "hypnotic rapport." Practical application of hypnotherapy today is characterized by the innovative techniques and strategies developed by Milton H. Erickson. Research into the effectiveness of hypnosis in the field of psychotherapy and psychosomatics still leaves much to be done. The situation is different in the field of medical hypnosis, where there are considerably more studies with a satisfactory design and verifiable effects. However, the impact in practical application in everyday medical practice is still low. Newer developments such as virtual reality and artificial intelligence are being looked at with critical interest.

Functional brain basis of hypnotizability

Abstract:Context: Focused hypnotic concentration is a model for brain control over sensation and behavior. Pain and anxiety can be effectively alleviated by hypnotic suggestion, which modulates activity in brain regions associated with focused attention, but the specific neural network underlying this phenomenon is not known. Objective: To investigate the brain basis of hypnotizability. Design: Cross-sectional, in vivo neuroimaging study performed from November 2005 through July 2006. Setting: Academic medical center at Stanford University School of Medicine. Patients: Twelve adults with high and 12 adults with low hypnotizability. Main outcome measures: Functional magnetic resonance imaging to measure functional connectivity networks at rest, including default-mode, salience, and executive-control networks; structural T1 magnetic resonance imaging to measure regional gray and white matter volumes; and diffusion tensor imaging to measure white matter microstructural integrity. Results: High compared with low hypnotizable individuals had greater functional connectivity between the left dorsolateral prefrontal cortex, an executive-control region of the brain, and the salience network composed of the dorsal anterior cingulate cortex, anterior insula, amygdala, and ventral striatum, involved in detecting, integrating, and filtering relevant somatic, autonomic, and emotional information using independent component analysis. Seed-based analysis confirmed elevated functional coupling between the dorsal anterior cingulate cortex and the dorsolateral prefrontal cortex in high compared with low hypnotizable individuals. These functional differences were not due to any variation in brain structure in these regions, including regional gray and white matter volumes and white matter microstructure. Conclusions: Our results provide novel evidence that altered functional connectivity in the dorsolateral prefrontal cortex and dorsal anterior cingulate cortex may underlie hypnotizability. Future studies focusing on how these functional networks change and interact during hypnosis are warranted.

Abstract:Hypnosis is a psychological intervention that is commonly used to enhance the effectiveness of therapeutic suggestions. Despite extensive fascination and study, the neural mechanisms behind hypnosis remain elusive. In the current study, we undertook a systematic exploration of these neural correlates. We first extracted well-studied neurophysiological features from EEG sensors and source-localized data using spectral analysis and two measures of functional connectivity: weighted phase lag index (wPLI) and power envelope correlation (PEC). Next, we developed classification models that predicted self-rated hypnotic experience based on the extracted feature sets. Our findings reveal that gamma power computed on sensor-level data and beta PEC computed between source-localized brain networks are the top predictors of hypnosis depth. Further, a SHapley Additive exPlanations (SHAP) analysis suggested reduced gamma power in the midline frontal area and increased beta PEC between interhemispheric Dorsal Attention Networks (DAN) contribute to the hypnotic experience. These results broaden our understanding of the neural correlates of deep hypnosis, highlighting potential targets for future research. Moreover, this study demonstrates the potential of using predictive models in understanding the neural underpinnings of self-reported hypnotic depth, offering a template for future investigations.

Abstract:The human brain is a complex network that supports mental function. The nascent field of network neuroscience applies tools from mathematics to neuroimaging data in the hope of shedding light on cognitive function. A critical question arising from these empirical studies is how to modulate a human brain network to treat cognitive deficits or enhance mental abilities. While historically a number of tools have been employed to modulate mental states (such as cognitive behavioural therapy and brain stimulation), theoretical frameworks to guide these interventions—and to optimize them for clinical use—are fundamentally lacking. One promising and as yet under-explored approach lies in a subdiscipline of engineering known as network control theory. Here, we posit that network control fundamentally relates to mind control, and that this relationship highlights important areas for future empirical research and opportunities to translate knowledge into practical domains. We clarify the conceptual intersection between neuroanatomy, cognition, and control engineering in the context of network neuroscience. Finally, we discuss the challenges, ethics, and promises of mind control.

Abstract:Understanding the neural substrate of altered conscious states is an important cultural, scientific, and clinical endeavour. Although hypnosis causes strong shifts in conscious perception and cognition, it remains largely unclear how hypnosis affects information processing in cortical networks. Here we manipulated the depth of hypnotic states to study information processing between cortical regions involved in attention and awareness. We used high-density Electroencephalography (EEG) to record resting-state cortical activity from 30 hypnosis experts during two hypnotic states with different depth. Each participant entered a light and a deep hypnotic state as well as two well-matched control states. Bridging top-down and lateralisation models of hypnosis, we found that interhemispheric frontoparietal connectivity distinguished hypnosis and control conditions, while no difference was found between the two hypnotic states. Using a graph-theoretic measure, we revealed that the amount of information passing through individual nodes (measured via betweenness centrality) is reduced during hypnosis relative to control states. Finally, we found that theta power was enhanced during hypnosis. Our result contributes to the current discussion around a role for theta power in bringing about hypnotic states, as well as other altered conscious states. Overall, our findings support the notion that altered top-down control in frontoparietal regions facilitates hypnosis by integrating information between cortical hemispheres.

Abstract:Clinical hypnosis is becoming increasingly recognized as a valuable clinical intervention for a wide range of psychological and medical problems. The current review paper seeks to determine the role of clinical hypnosis in learning difficulties, emotional or behavioral problems and physical disabilities. In addition, we co-examine the effectiveness of virtual/augmented reality hypnosis. Finally, we discuss the possible uses of clinical hypnosis within the educational settings. The results showed that hypnosis may hold the keys for new types of metacognitive strategies based on non-verbal pathways of effortless self-regulation.

Abstract:Meditation and hypnosis both aim to facilitate cognitive-emotional flexibility, i.e., the "de-automatization" of thought and behavior. However, little research or theory has addressed how internal thought patterns might change after such interventions, even though alterations in the internal flow of consciousness may precede externally observable changes in behavior. This chapter outlines three mechanisms by which meditation or hypnosis might alter or reduce automatic associations and elaborations of spontaneous thought: by an overall reduction of the chaining of thoughts into an associative stream; by de-automatizing and diversifying the content of thought chains (i.e., increasing thought flexibility or variety); and, finally, by re-automatizing chains of thought along desired or valued paths (i.e., forming new, voluntarily chosen mental habits). The authors discuss behavioral and cognitive neuroscientific evidence demonstrating the influence of hypnosis and meditation on internal cognition and highlight the putative neurobiological basis, as well as potential benefits, of these forms of de-automatization.

Coupled Temporal Fluctuation and Global Signal Synchronization of Spontaneous Brain Activity in Hypnosis for Respiration Control: An fMRI Study

Abstract:Hypnosis is a psychological technology proved to be effective in respiratory motion control, which is essential to reduce radiation dose during radiotherapy. This study explored the neural mechanisms and cognitive neuroscience of hypnosis for respiration control by functional magnetic resonance imaging with a within-subject design of 15 healthy volunteers in rest state (RS) and hypnosis state (HS). Temporal fluctuation and signal synchronization of brain activity were employed to investigate the altered physiological performance in hypnosis. The altered correlations between temporal fluctuation and signal synchronization were examined within large scale of intrinsic networks which were identified by seed-wise functional connectivity. As a result, hypnosis was observed with increased activity in the right calcarine, bilateral fusiform gyrus and left middle temporal gyrus, and with decreased activity in the left cerebellum posterior lobe (inferior semilunar lobule part). Compared to RS, enhanced positive correlations were observed between temporal fluctuation and signal synchronization in HS. Most importantly, coupled correlation was observed between temporal fluctuation and global signal synchronization within the identified intrinsic networks (R = 0.3843, p > 0.05 in RS; R = 0.6212, p < 0.005 in HS). The findings provide implications for the neural basis of hypnosis for respiratory motion control and suggest the involvement of emotional processing and regulation of perceptual consciousness in hypnosis.
