Abstract:Elderly patients suffering from osteoporotic fractures are more susceptible to delayed union or nonunion, and their bodies then are in a state of low-grade chronic inflammation with decreased antioxidant capacity. Tanshinone IIA is widely used in treating cardiovascular and cerebrovascular diseases in China and has anti-inflammatory and antioxidant effects. We aimed to observe the antioxidant effects of Tanshinone IIA on mesenchymal stem cells (MSCs), which play important roles in bone repair, and the effects of local application of Tanshinone IIA using an injectable biodegradable hydrogel on osteoporotic fracture healing.

What problem does this paper attempt to address?
This paper mainly discusses the problems during the fracture healing process in elderly patients with osteoporotic fractures. In these cases, fracture healing may be delayed or non-union, and the body is in a state of chronic low-grade inflammation with decreased antioxidant capacity. Tanshinone IIA, mentioned in the study, is commonly used in China for the treatment of cardiovascular and cerebrovascular diseases and has anti-inflammatory and antioxidant properties. The experiment observed the effects of Tanshinone IIA on mesenchymal stem cells (MSCs), which play a crucial role in bone repair, through in vitro and in vivo experiments. The study found that Tanshinone IIA pretreatment promoted the expression of nuclear factor erythroid 2-related factor 2 (Nrf2) and antioxidant enzymes, reduced hydrogen peroxide (H2O2)-induced ROS accumulation, thus protecting MSCs from oxidative stress-induced apoptosis and reducing osteogenic differentiation. In the osteoporotic fracture model of ovariectomized (OVX) mice, local application of Tanshinone IIA through injectable biodegradable hydrogel accelerated fracture healing, increased bone density, improved mechanical properties, reduced cell apoptosis, and increased Nrf2 expression. In conclusion, the paper aims to address how to use Tanshinone IIA to promote the healing of osteoporotic fractures and reduce associated complications by protecting MSCs through antioxidant mechanisms, thereby improving fracture repair outcomes.
