Cold knife conization (CKC) is a common treatment for HSIL, but positive surgical margins increase the risk of residual lesions and recurrence. Therefore, accurately predicting the risk of positive surgical margins is crucial for improving treatment outcomes and reducing recurrence. ### Methods: - **Data Collection**: Retrospective analysis of clinical data from 3,343 HSIL patients who underwent CKC at the First Affiliated Hospital of Yangtze University from January 2012 to December 2022. - **Statistical Analysis**: Use univariate and multivariate analyses to identify independent risk factors associated with positive surgical margins and construct prediction models. - **Model Evaluation**: Construct and evaluate models using various machine learning methods, including logistic regression, support vector machine, K-nearest neighbors, decision tree, random forest, XGBoost, and naive Bayes. ### Results: - **Risk Factors**: Independent risk factors include gland involvement, transformation zone type III, HPV16/18 infection, multiple high-risk HPV infections, TCT ≥ ASC-H, and lesions covering ≥3 quadrants. - **Model Performance**: The logistic regression model showed the best predictive performance with an accuracy of 74.7%, sensitivity of 76.7%, specificity of 74.4%, and an AUC of 0.826. ### Conclusion: - **Independent Risk Factors**: HPV16/18 infection, multiple high-risk HPV infections, gland involvement, extensive lesion coverage, higher TCT grade, and transformation zone type III are independent risk factors for positive surgical margins. - **Prediction Model**: The logistic regression model provides a robust and clinically valuable tool for predicting the risk of positive surgical margins, guiding clinical decision-making and patient management. With these research findings, clinicians can identify high-risk patients preoperatively, allowing for more individualized treatment and management strategies, improving treatment outcomes, and reducing the risk of recurrence.
