Abstract:To study the correlation between chronic inflammation and PCOS. Part 1: Thirty cases of PCOS with insulin resistance (Homa index≥2.69), thirty cases of PCOS without insulin resistance (Homa index<2.69) and thirty cases of controls were studied in this experiment. We examined serum levels of the anti-flammatory factor: interleukin1 receptor antagonist (IL1ra), and inflammatory markers: interleukin1β (IL1β), tumor necrosis factor alpha (TNFα). Part 2: We checked the expression of IL1RA and TNF gene in the granulosa cell of five PCOS patients and thirteen controls by real-time PCR. Part 3: One hundred and twenty-nine PCOS patients and eighty-eight controls were examined in this study. We analyzed the IL1RA gene polymorphism by PCR. Part 1: Serum levels of IL1ra, IL1β and TNFα were measured by ELISA (Biosource Inc). Part 2: RNA was isolated from granulosa cell and the real-time PCR reactions were carried out. Part 3: Genomic DNA was extracted from peripheral blood and PCR reactions were carried out. All of the data were analyzed by SPSS11.5. Part 1: 1. In the PCOS insulin resistance group, serum IL1ra, IL1β and TNFα concentrations were increased in comparison with the other two groups (P<0.05), moreover, PCOS without insulin resistance patients were also higher than the controls (P<0.05). IL1ra/IL1β was the opposite (P<0.05). 2. Serum level of IL1ra, IL1β and TNFα were higher in PCOS obese women (BMI ≥ 25kg/m2) than in PCOS lean women (BMI < 25kg/m2), but IL1ra/IL1β was the opposite (P<0.05). 3. Serum level of IL1ra, IL1β and TNFα were higher in PCOS women with normal BMI than in controls with normal BMI (P<0.05). Part 2: The expression of IL1RA and TNF gene in granulosa cell of PCOS patients were up-regulation 15.67 times and 4.26 times comparing with the controls (P<0.05). Part 3: 1. The IL1RA allele 5 frequencies among women with PCOS and controls were 5.4% and 0% (P<0.05). 2. We discovered BMI of the PCOS patients with polymorphic allele 5 was 26.66 ± 3.41kg/m2 higher than the PCOS patients without allele 5 (24.42 ± 3.84 kg/m2) (P<0.05). PCOS had correlation with chronic low-grade inflammation and had reference to insulin resistance and obesity. We presumed there was a balance of inflammation and anti-inflammation in PCOS patients. IL1RA polymorphic allele 5 may have correlation with the presence of PCOS, and PCOS people with IL1RA allele 5 may have a propensity for obesity.
