Abstract:BACKGROUND: Weakness of trunk muscles is a common disorder in children with cerebral palsy (CP). They encounter decreased levels of endurance that can lead to diminished capacity of movement.OBJECTIVE: To evaluate the effect of core stability training on the endurance of trunk muscles and gait parameters in children with hemiplegic CP.METHODS: Thirty children with hemiplegic CP aged 10 to 12 years were randomly assigned to two groups of equal number; control group (A) and study group (B). Both groups underwent the same designed physical therapy program. Moreover, group B underwent core stability training 3 times/week for 8 weeks. Endurance time of trunk muscles and gait parameters were measured before and after the intervention using the trunk endurance tests and the Biodex gait trainer respectively.RESULTS: Both groups showed statistically significant improvements in the endurance time of trunk flexors and extensors and gait parameters after treatment but only group B showed statistically significant improvement in the endurance time of lateral trunk muscles. There were post-treatment statistically significant differences between both groups in favor of group B regarding all measured variables.CONCLUSION: Addition of core stability exercises to the treatment program can effectively improve the endurance time of trunk muscles and gait in children with hemiplegic CP.

Abstract:BACKGROUND The purpose of the study was to compare the effects of core stabilization exercise (CSE) and proprioceptive neuromuscular facilitation (PNF) exercise on core muscle activation, core muscle endurance, proprioception, and balance in 80 obese children. MATERIAL AND METHODS In this single-blind, randomized controlled study, 80 obese children aged 10-13 years were randomly divided into 3 groups. The first group received CSE (n=27) and the second group received PNF exercises (n=27) 3 days a week for 8 weeks. The third group, which was the control group (n=26), received no treatment. Participants were evaluated before treatment (BT) and after treatment (AT) and at follow-up (3 months after treatment). Core muscle activation level was evaluated by Sahrmann Core Stability test (SCST), lumbar core muscle endurance was evaluated by McGill core endurance tests, and lumbar joint position sense (JPS) was evaluated by laser cursor. The single-leg standing balance test (SLSBT) and Y balance test (YBT) were used for static and dynamic balance, respectively. RESULTS AT and at follow-up, core activation, core endurance, JPS, and static balance were significantly different between the groups (P<0.05). There was no significant difference between the groups in YBT dominant and non-dominant side mixed reach distances (P>0.05). Clinical effect sizes were higher in the CSE group for all outcome measures. CONCLUSIONS CSE and PNF exercises improve the level of core muscle activation, lumbar core muscle endurance, lumbar JPS, and balance in obese children. However, the results of this study show that CSE are more clinically effective in obese children. The effects decline in the medium term.

Abstract:Objective: To explore the effects of an eight-week core stability program on balance ability in persons with Parkinson’s disease. Design: Randomized controlled trial. Setting: A local Parkinson’s association. Subjects: A total of 44 participants with a clinical diagnosis of Parkinson’s disease were randomly assigned to an experimental ( n = 22) or control group ( n = 22). Intervention: The experimental group received 24 sessions of core training, while the control group received an intervention including active joint mobilization, muscle stretching, and motor coordination exercises. Main measures: The primary outcome measure was dynamic balance evaluated using the Mini-Balance Evaluation Systems Test. Secondary outcomes included the balance confidence assessed with the Activities-specific Balance Confidence Scale and standing balance assessed by the maximal excursion of center of pressure during the Modified Clinical Test of Sensory Interaction on Balance and the Limits of Stability test. Results: After treatment, a significant between-group improvement in dynamic balance was observed in the experimental group compared to the control group (change, 2.75 ± 1.80 vs 0.38 ± 2.15, P = 0.002). The experimental group also showed a significant improvement in confidence (change, 16.48 ± 16.21 vs 3.05 ± 13.53, P = 0.047) and maximal excursion of center of pressure in forward (change, 0.86 ± 1.89 cm vs 0.17 ± 0.26 cm, P = 0.048), left (change, 0.88 ± 2.63 cm vs 0.07 ± 0.48 cm, P = 0.010), and right (change, 1.63 ± 2.82 cm vs 0.05 ± 0.17 cm, P = 0.046) directions of limits of stability compared to the control group. Conclusion: A program based on core stability in comparison with non-specific exercise benefits dynamic balance and confidence and increases center of mass excursion in patients with Parkinson’s disease.

Abstract:Objectives: The aim of this study to compare the effectiveness of the balance training program (BTP), and the corestabilization program (CSP), on dynamic balance in healthy middle-aged individuals.Material and Methods: This study was a single-blind randomized controlled trial design. Forty-two healthy middle-aged participants were randomly assigned to the BTP group (n=21), or the CSP group (n=21). Participants in both groups received an intervention program (balance training or core stabilization) 3 times a week, for 60 minutes, over 6 weeks. The primary outcome was the dynamic balance measured by the timed up and go test. The other outcomes were: core muscle endurance, muscle strength of the lower extremities, the flexibility of the lower back and hamstring muscles, and gait variables. The measurements included: the prone bridge endurance test, 5 times sit to stand test, sit and reach test and a wireless movement monitoring inertial sensor system, respectively. All outcomes were measured at baseline, and then after 6 weeks. The data were analyzed by the Independent Sample t-test between groups, and the paired t-test within either group.Results: After 6 weeks, there was no statistically significant difference between the groups in either dynamic balance,or other variables, however, a statistically significant difference was found in core muscle endurance (p-value=0.003). In so saying, the BTP group, statistically significant improvements were found only in core muscle endurance, the flexibility of the lower back and hamstring muscles, and gait variables from pre- to post-test. Moreover, in the CSP group, there was a statistically significant difference from pre- to post-test in all measured outcomes. Conclusion: The Core stabilization program is not superior to a balance training program, for the improvement of dynamic balance. Although, after 6 weeks of training, the study found that the Core stabilization program was effective for improving dynamic balance. This finding may point out that the Core stabilization program helps improve balance in a middle-aged person
Effects of Core Muscle Acupuncture on Standing Balance Function of Children with Spastic Cerebral Palsy

Abstract:To investigate the effects of core muscle acupuncture on the standing balance function of children with spastic cerebral palsy. We select 30 cases of spastic cerebral palsy children with standing disequilibrium and divide randomly into two groups. Conventional rehabilitation training was adopted in the rehabilitation group, while core muscle acupuncture combined with the same rehabilitation in the rehabilitation group was adopted in the acupuncture group. In 6 months, the Pro-kin254 equilibrium training detector was utilized to detect the scores of the trunk control ability and those in standing position (region D) among the 88 items in the gross motor function measure (GMFM-88). The center of pressure (COP) in the anterior-posterior and left-right directions, the average speeds of movement in the left-right directions, and the ellipse area formed by the shifting of the center of gravity in the acupuncture group were superior after treatment to those before treatment (P < 0.05). The standard deviation of COP in the left-right direction, the average speed of movement in the left-right direction, and the swinging path length of the center of gravity n the rehabilitation group were all superior to those before treatment (P < 0.05). In the rehabilitation group the difference in the average speed of movement in the anterior-posterior direction in the acupuncture group before and after treatment was better than that in the rehabilitation group (P < 0.05). The GMFM-88 scores in standing position in both groups were better after treatment than those before treatment (P < 0.05); and the differences in the GMFM-88 scores in standing position of prior and post-treatment were higher in the acupuncture group than in the rehabilitation group (P < 0.05). Core muscle acupuncture could improve the standing balance function of children with spastic cerebral palsy.

Abstract:[Purpose] The purpose of this study was to determine the effects of core stabilization exercise on dynamic balance and gait function in stroke patients. [Subjects] The subjects were 16 stroke patients, who were randomly divided into two groups: a core stabilization exercise group of eight subjects and control group of eight subjects. [Methods] Subjects in both groups received general training five times per week. Subjects in the core stabilization exercise group practiced an additional core stabilization exercise program, which was performed for 30 minutes, three times per week, during a period of four weeks. All subjects were evaluated for dynamic balance (Timed Up and Go test, TUG) and gait parameters (velocity, cadence, step length, and stride length). [Results] Following intervention, the core exercise group showed a significant change in TUG, velocity, and cadence. The only significant difference observed between the core group and control group was in velocity. [Conclusion] The results of this study suggest the feasibility and suitability of core stabilization exercise for stroke patients.

Abstract:Introduction: This study aimed to investigate the impact of a 10-week Core Stability Training (CST) compared to Traditional Strength Training (TST) on the balance abilities of adolescent male basketball players. Methods: Subjects (age: 15.70 ± 0.75, height: 178.4 ± 8.31, weight: 66.55 ± 8.34) were randomly assigned to either the Core Stability Training group or the Traditional Strength Training group. Three selected balance assessment indicators included the Single-leg Standing with Eyes Closed Test, Star Excursion Balance Test, and Core Four-Direction Endurance Test. Results: 1) The scores were significantly different in both groups before and after the Single-leg Standing with Eyes Closed Test; ( p < 0.01, d = 1.692, d = 1.837); 2) In the Star Excursion Balance Test, the scores of the experimental group showed significant difference ( p < 0.05) or highly significant difference ( p < 0.01) with an average effect size of ( d = 1.727) when the left or right foot supported in the other directions before and after the training. However, there was no significant difference in scores in the c direction when the left foot supported ( p > 0.05, d = 0.954); 3) In the Core Four-Direction Endurance Test, there were no significant differences in scores for the control group before and after training ( p > 0.05, d = 0.567), while the experimental group showed significant differences in scores before and after training ( p < 0.05, d = 1.889). Discussion: Both CST and TST were effective in enhancing the balance abilities of adolescent basketball players. CST, in particular, demonstrated improvements in dynamic balance and agility across multiple planes. Basketball coaches are encouraged to consider incorporating CST training programs into their overall training plans for optimal balance enhancement.

Abstract:BACKGROUND AND OBJECTIVE: The aim of this study was to show the effects of an 8-week Neurodevelopmental Treatment based posture and balance training on postural control and balance in diparetic and hemiparetic Cerebral Palsied children (CPC).METHODS: Fifteen CPC (aged 5-15 yrs) were recruited from Denizli Yağmur Çocukları Rehabilitation Centre. Gross Motor Function Classification System, Gross Motor Function Measure, 1-Min Walking Test, Modified Timed Up and Go Test, Paediatric Balance Scale, Functional Independence Measure for Children and Seated Postural Control Measure were used for assessment before and after treatment. An 8-week NDT based posture and balance training was applied to the CPC in one session (60-min) 2 days in a week.RESULTS: After the treatment program, all participants showed statistically significant improvements in terms of gross motor function (p< 0.05). They also showed statistically significant improvements about balance abilities and independence in terms of daily living activities (p< 0.05). Seated Postural Control Measure scores increased after the treatment program (p< 0.05).CONCLUSIONS: The results of this study indicate that an 8-week Neurodevelopmental Treatment based posture and balance training is an effective approach in order to improve functional motor level and functional independency by improving postural control and balance in diparetic and hemiparetic CPC.

Abstract:Background: Visual impairments affect all motion aspects of individuals. Movement as the most important tool of physical education is an important factor to improve the health of people with disabilities. The present study aims to investigate the effect of six weeks of core stability training on postural control, risk of falling, and function of blind people. Material and methods: Thirty-two blind males were randomly divided into two groups: the experimental and the control one. In order to evaluate the postural control, the Berg balance scale, the modified stork stand balance test, and the tandem walking test were used. To evaluate the risk of falling and function, the Timed Get up & Go and Tinetti tests were employed, respectively. Data were analysed via MANOVA statistical analysis test at the significance level of 0.05. Results: Results showed that, after 6 weeks of core stability training, a significant increase was observed in postural control scores, function, and risk of falling in the experimental group compared to the control group and the pre-test condition (P < 0.05). Abstract:<h3>Background</h3><p>Upper limb paresis is a common problem in patients with stroke.</p><h3>Objectives</h3><p>To determine the effect of core stability exercises on upper limb function and trunk balance in hemiparetic patients.</p><h3>Subjects</h3><p>Thirty patients with hemiparesis, with age between 45 and 60 years, and with illness duration of more than 6 months. They were assigned into two equal groups, the control group, Group A: with 15 patients who received only conventional physical therapy program; the study group, Group B: with 15 patients who received conventional physical therapy program and additional core muscle training. Patients received 18 sessions for 6 weeks, three sessions/week.</p><h3>Methods</h3><p>The upper limb function was assessed using Wolf motor function test with subscales (function ability scale, time, and grip strength), the range of motion of shoulder flexion and abduction was measured by using goniometer, trunk balance was assessed using the trunk impairment scale with subscales (static sitting balance, dynamic sitting balance, and coordination). All measurement outcomes were assessed before and after applying the treatment program.</p><h3>Results</h3><p>There was no statistical significant difference between two groups in pretreatment assessment using wolf motor function test, trunk impairment scale, and shoulder range of motion. In post treatment assessment, there was no statistical significant difference between group A and group B in all the outcome measures, except for trunk impairment subscale (dynamic sitting balance); the statistical significant difference was in favor of group B.</p><h3>Conclusion</h3><p>Core muscle training is similar to conventional physical therapy program in improving upper limb function in hemiparetic patients, and has beneficial effect on improving trunk balance.</p>

Abstract:Abstract Injured athletes need to do appropriate exercises to restore dynamic balance capacity. This paper briefly introduced core stability and its training. Twenty athletes with knee injuries in the Third Hospital of Shanxi Province were divided into a regular training group and a core training group, for example, analysis. The knee joint function and coordination and balance ability of both groups before and after training were tested. The results suggest that core stability training is more conducive to promoting the recovery of knee function and coordination and balance ability under dynamic and static conditions.
Investigating the effects of core stability training on balance and gait in people with multiple sclerosis: A systematic review and meta-analysis

Abstract:Background and objectives: Balance disorders are a common concern in people with multiple sclerosis (PwMS). Core stability exercises are recommended as one of the treatment principles of rehabilitation in patients with balance disorders. This systematic review and meta-analysis investigate the effects of core stabilization exercises (CSE) on balance in PwMS. Methods: Online databases were searched from 1980 to December 15, 2022, including PubMed/Medline, Scopus, Web of Science, CINAHL, PEDro, Embase, and ProQuest to identify randomized controlled trials (RCTs) investigating the effects of CSE on balance in PwMS. The methodological qualities of the included studies were assessed using the modified Cochrane risk bias tool for randomized trials. Random-effect meta-analyzes were performed on the Berg balance test and Biodex balance system outcomes. Results: Seven RCTs were included in this study. A total of 379 patients (EDSS<6) were enrolled in the included studies. All researchers had applied CSE within six to ten weeks with the frequency of two to three sessions/week. According to qualitative results, core stability exercise significantly changes the most balanced outcomes. The meta-analyses illustrated that allocated participants to experimental groups had statistically significantly higher Berg balance test scores (standardized mean difference: 1.1; 95 % CI: [0.1, 2.1]) and insignificant lower Biodex score (standardized mean difference: -0.59; 95 % CI: [-1.09, -0.09]) compared to the control groups. Conclusion: Six to ten weeks of CSEs is an effective therapeutic strategy for balance improvement in PwMS that can help improve balance, especially in patients with moderate disability levels (EDSS3.5-6). Further randomized clinical trials are needed to compare the efficacy of CSEs and routine balance training in PwMS.
Comparing stabilization training with balance training in recreationally active individuals

Abstract:Aims The present study was undertaken to compare the efficacy of lumbar core stability training with that of balance training, at improving the balance performance in the recreationally active individuals. Most of the studies to date have been conducted on sporting professionals, whose activity demands are higher than the norm. The present study focuses on the recreationally active population, for which the activity demands are comparatively lesser. Methods Thirty recreationally active subjects (15 males and 15 females) were selected for this study. Subjects were randomly divided into three groups: core stability training group (CSTG), balance training group (BTG), and the control group. All subjects were assessed at baseline, and after six weeks, for balance performance, including static, dynamic and functional balance tests. Data were analyzed by paired t-test, ANOVA, Wilcoxon signed-rank test and Kruksal Wallis test. Findings Both training groups (BTG and CSTG) showed significant (P < 0.05) improvement in balance performance compared to the control group, however the improvement in the mean value for the CSTG was greater than that of the BTG. Conclusions Balance training as well as core stabilization training leads to improvement in static and dynamic balance. However none of these training leads to improvement in the functional balance performance.
Comparison of Single Session Auditory Versus Visual Feedback on Performance and Postural Balance in Hemiplegic Children With Cerebral Palsy

Abstract:Background: Cerebral palsy (CP) is a pediatric disorder characterized by a motor impairment resulting from a permanent, non-progressive lesion in the brain. Cerebral palsy is marked by movement and postural control impairments, which greatly affect body structure, function, daily activities, and participation. Objective: To compare the single-session auditory versus visual feedback on performance and postural balance in children with hemiplegic cerebral palsy. Method: The study was a crossover clinical trial involving a group of 25 patients diagnosed with CP hemiplegia, aged between 6 and 12 years, including both genders. Each patient underwent conventional balance therapy followed by either auditory feedback or visual feedback intervention. After a 48-hour wash-out period, they received conventional balance therapy again before undergoing the alternative intervention initially assigned. The Modified Ashworth scale (MAS), pediatric balance scale (PBS), timed one-leg stance, time up and go test (TUG), and center of pressure (CoP) displacements were assessed as the outcome measures before and after the interventions. Results: Based on the one-leg stand test, TUG, and CoP displacement outcome measures results, both interventions improved balance time, speed of movement, and postural stability in children with hemiplegic spastic cerebral palsy (P < 0.05). Moreover, after a single session of the intervention, the visual feedback group demonstrated a significantly greater improvement in the TUG test, one-leg stand test, and CoP displacement compared to the auditory group (P < 0.05). Conclusions: The results of the study suggest that combining auditory or visual feedback with conventional balance therapy is effective in treating children with hemiplegic spastic cerebral palsy; furthermore, the utilization of visual feedback would be more effective. Further research is needed to determine the long-term effects of visual and auditory feedback on the assessed outcome measures.

Abstract:Background: Children with ataxic cerebral palsy have unsteady movements and poor balance. Many therapeutic interventions are used to improve their upper and lower functioning. Aim: To assess the effectiveness of virtual reality (VR) on balance impairment in ataxic cerebral palsy children. Design: RCT. Population: Sixty-four children with ataxic cerebral palsy. Methods: Children were randomly assigned into two equal groups with 32 patients in each group. The control group received a specially developed physical therapy program and the VR group received VR training on a Wii balance board in addition to the control group's program. For three successive months the intervention program was implemented three times a week for both groups. Stability indices (overall, anteroposterior, and mediolateral) were measured using the Biodex balance system and Pediatric Balance Scale (PBS) at baseline and after three months of intervention for both groups. Results: At the beginning of the intervention, there were no statistically significant differences between the two groups (P>0.05). However, following three months of the intervention, there was a statistically significant decline in the stability index scores., as well as a statistically significant improvement in the score of the PBS in both groups with a notable advancement in favor of the VR group (P≤0.05). Conclusions: Adding VR training on a Wii balance board to a designed physical therapy program has a significant impact on improving balance deficits in ataxic cerebral palsy children. Clinical rehabilitation impact: VR has a therapeutic effect on improving balance that may result in better and more efficient rehabilitation program of children with ataxic cerebral palsy.

Abstract:INTRODUCTION: Overweight children are associated with low neuromuscular function and postural imbalance. Core stabilization exercises have shown success in the cerebral palsy and Down syndrome children. However, its efficacy in overweight children is not known till date. OBJECTIVES: To measure the postural stability (static and dynamic) and foot pressure distribution in overweight children. To determine the efficacy of core stability training on postural stability (static and dynamic) in overweight children. To determine the efficacy of core stability training on foot pressure distribution in overweight children. MATERIALS AND METHODS: This is parallel design, non – blinded, two – group, pretest - posttest randomized controlled trial. The study will recruit 52 (26 in each group) aged 8- 16 years old both boys and girls with BMI-for-age percentile growth between 85th to less than 95th percentile. The outcomes are foot pressure distribution, single limb stance, Y-Reach test and core strength respectively. The core stabilization exercises will be provided in the experimental group and no exercises will be given in the control group, 3 times a week for 6 weeks. PREDICTED RESULTS: The predicted results from this study will provide the first evidence of the effectiveness of core stabilization exercises in overweight children. A greater clinical understanding of core strength and balance in this age group may be helpful for incorporating activities in their daily life in order for the better overall development and growth. Kolmnogorov Smirnov test will be used for checking the normal distribution of the data. Descriptive statistics will be expressed either as mean±standard deviation or median and inter- quartile range, based on the data distribution. Paired t-tests or Wilcoxon signed- rank test will be used to conduct within group analyses and Independent t- tests or Mann- Whitney U- test will be used for between group analyses. For all the analyses the level of significance will be set at 0.05.

Abstract:Children with hearing impairment are unable to speak and may suffer from some physical problems such as weakness in postural performance ability and development. The aim of the current study was to explore the effects of proprioception versus core stability training for an 8-week period followed by a 6-month detraining protocol on the balance performance of deaf students. This is a randomized controlled trial design of three groups in blinded evaluators. The study was conducted at the school gym. A total of 30 participants, of male deaf students, were randomized into three groups: (1) one group receiving proprioception training (PT, n = 10), (2), one group receiving core stability training (CST, n = 10) for 8 weeks, and (3), and control group (CON, n = 10). The Balance Error Scoring System (BESS) test and Y-balance test assess static and dynamic balance at pre- and post-training following a 6-month detraining. Post 8 weeks of training intervention, PT and CST values showed significant improvements in both static ( p = 0.001) and dynamic ( p = 0.001) balance. Following the 6-month detraining, only the PT group maintained their improvements in both the static and dynamic balance tests ( p = 0.348) and the CST group showed decreases in the balance tests ( p = 0.034). The results indicate that 8 weeks rehabilitation program (PT and CST) is an optimum training modality to enhance balance in deaf students and PT induces more training effects than CST for maintaining training benefits following the detraining. Abstract:Purpose: The purpose of this study was to compare the clinical efficacy of a virtual reality rehabilitation-based training (VRT) with balance-specific training (BST) and conventional training (CT) on the balance and gross motor functions (GMF) of children with cerebral palsy (CwCP). Methods: This study was a double blinded, randomized controlled trial. Participants were recruited from different CP rehabilitation centers and clinics and were then randomly allocated using the block randomization method into three groups: (1) group 1 (VRT using a set of Xbox 360 games that triggered balance), (2) group 2 (BST applying a protocol of 13 exercises to enhance balance in different conditions), and (3) control group 3 (CT using traditional physiotherapy techniques). All groups received 18 sessions over six weeks, three sessions per week, each lasting 60 minutes. Participants were assessed at three timepoints (baseline, post-treatment, and follow-up) using the Pediatric Balance Scale (PBS), the Gross Motor Function Measure (GMFM D & E), the Five Times Sit-To-Stand Test, and upper and lower segments' center of mass (COM) displacement (UCOM and LCOM). Results: A total of 46 CwCP participated in this study. The repeated measures ANOVA revealed a statistically significant difference between groups in the dependent variables, except for the GMFM (D & E) and the PBS (p < 0.05 and partial η2 = 0.473). The post-hoc test showed a statistically significant difference in favor of the VRT group compared to other groups in terms of right UCOM (p < 0.05) with a large effect size of the time*group interaction (partial η2 = 0.87). Moreover, there was a statistically significant effect of time (i.e., baseline to post-treatment and baseline to follow-up) with F (18, 23) = 59.954, p < 0.05, Wilks' lambda = 0.021, partial η2 = 0.979. Conclusion: The findings revealed that VRT was not superior to BST in the rehabilitation of balance and GMF in CwCP aged four to 12 years. However, when compared to CT, better results were reported. Furthermore, it appears that customized programs lead to greater improvements in balance than commercial programs. Future studies are needed to assess the physiological effects of the three types of rehabilitation interventions using more advanced measurement tools, such as functional magnetic resonance imaging, following VRT protocols.
