Abstract:In the long, challenging, and often botched history of drug rationing,12 Kaftrio represents the latest test case for fair and open priority setting. The story is familiar by now: a life changing treatment, at an eye popping list price, facing a backlash if the National Institute for Health and Care Excellence (NICE) says no. Even after secret price negotiations, NICE describes the cost of Kaftrio as "substantially above" its £30 000 per QALY gained threshold; yet, no one could begrudge the Cystic Fibrosis Trust's efforts to reverse the decision.3 Whose sympathy doesn't lie with the children and their caregivers who stand to benefit?
