Abstract:Nonalcoholic fatty liver disease (NAFLD) is a complex, multifactorial, and the most common form of liver disease, with unclear pathogenesis. It is closely related to metabolic syndrome. [ 1 ] NAFLD is closely associated with metabolic diseases and alterations in hepatic lipid metabolism led to the formation of atherosclerotic plaques. Thus, NAFLD shares multiple risk factors with atherosclerosis (AS). [ 2 ] Although the mechanism underlying the development of both diseases involves a complex interaction between genetic and environmental factors, the specific mechanisms common to both NAFLD and AS are not known and may be related to insulin resistance (IR), inflammation, oxidative stress, and lipid metabolism disorders. [ 3 ] In a cross-sectional study, NAFLD was found to be strongly associated with one of the most common cancers, bladder cancer, especially in the elderly, and the prevalence of NAFLD and IR was higher in patients with bladder cancer, demonstrating that NAFLD and IR may be cofactors affecting cancer. [ 4 ] And in another study, it was shown that there is a significant relationship between AS and cancer. Therefore, AS and NAFLD may be somewhat related. [ 5 ] Clinical analysis revealed a significantly higher prevalence of AS, a greater likelihood of unstable plaque formation, and increased dyslipidemia in patients diagnosed with NAFLD. [ 6 ] In patients with NAFLD, the increased hepatic production of pro-inflammatory cytokines may not only promote the progression of liver injury but also induce pro-atherogenic effects. [ 7 ] NAFLD can develop during the development of AS and accelerate it, possibly via, oxidative stress and inflammatory stimulation. [ 8 ] Oxidative stress promotes the release of hepatic inflammatory factors, which can cause damage to the aortic intima, inducing AS. The release of oxygen free radicals in response to oxidative stress also contributes to the formation of atherosclerotic plaques. [ 9 ] In patients with NAFLD, the synthetic function of the liver decreases, and the synthesis and release of anti-atherosclerotic factors by the liver decreases, thus causing an increase in oxidative damage to the intima of the arteries and a decrease in the repair capacity, which may also promote the development of AS. [ 10 ] AS and NAFLD are diseases that span public epidemiology, underlying mechanisms, and clinical syndromes with multiple manifestations involving every organ of the body. When considered individually, the pathology appears to be definitive and indisputable, but when considered together, areas of uncertainty arise. There are many interrelationships between the 2 conditions, shared epidemiology and risk factors, and the need for behavioral change interventions in prevention and treatment. However, it can only be confirmed that NAFLD and cardiovascular disease are closely related and that NAFLD can lead to and exacerbate the development of AS, although the exact mechanisms are unknown. Bioinformatics approaches allow us to rapidly gain insight into the common links between the 2 diseases concerning both genetic mechanisms and signaling pathways. [ 11 ] In a systems biology analysis, 64 core genes were identified for hypertension and NAFLD, and enrichment analysis revealed an association of the genes with inflammatory and metabolic biological functions. The purpose of the present study was to provide novel insights into the mechanisms underlying AS and NAFLD by analyzing genetic data from public databases. Our study can further identify the common mechanisms between NAFLD and AS, which can help develop therapeutic targets for AS as well as prevent and treat AS in patients with early-stage NAFLD.
