Abstract:Objective To analyze the prevalence rate,diagnosis,treatment,prognosis of heterotopic pregnancy(HP) after in vitro fertilization and embryo transfer(IVF-ET).Methods The clinical data of 14 patients diagnosed HP after IVF-ET were reviewed.Results Eight cases had history of tubal or chronic pelvic inflammation.One case had chronic pelvic inflammation and bicornuate uterus.Three cases had chronic pelvic inflammation and oligozoo-spermia.One case had azoospermia.One case had oligo-ovulation and oligozoo-spermia.All of them were diagnosed by sonography before surgery.Ten patients had salpingectomy by laparoscopy and one by laparotomy.Two patients of corneal pregnancy had laparoscopy and laparotomy respectively.one had salpingectomy in other hospital.After surgery,9 cases had ongoing intrauterinc pregnancy,5 cases were inevitable abortion.Conclusions Previous tubal or pelvic inflammation disease are the most common risk factors for HP.The sonographic examination is reliable in the diagnosis of HP.Early treatment can improve the prognosis obviously.Laparoscopic salpingectomy is still considered to be the best method for the treatment of HP.
OC29.05: A Comparison of the Pregnancy Outcomes Between Women with a Didelphus Uterus and a Unicornuate Uterus after in Vitro Fertilisation‐embryo Transfer

Abstract:To investigate the pregnancy outcomes between women with a didelphus uterus and those with a unicornuate uterus after in vitro fertilisation-embryo transfer (IVF-ET). This was a retrospective analysis of 50 infertile women with a didelphus uterus and 280 cases with a unicornuate uterus from January 2004 to December 2017 at our reproductive centre. All women conceived a singleton pregnancy after IVF-ET at our reproductive centre. The diagnoses of uterine malformations were based on American Fertility Society (AFS) classification system. The outcome measures included pregnancy and obstetric outcomes, which were compared between the didelphus uterine group and the unicornuate uterine group. The didelphus uterine group and the unicornuate uterine group were statistically similar regarding maternal age ( 30.4 ± 4.7 vs. 29.9 ± 4.5 years; P = 0.505), infertility type (P = 0.462), cause of infertility (P = 0.070) and insemination methods (P = 0.261). The didelphus uterine group and the unicornuate uterine group had similar rates of miscarriage (18.0 vs. 20.7%; P = 0.660), ectopic pregnancy (0 vs. 1.1%; P = 1.000), preterm delivery (16.0 vs. 13.6%; P = 0.648), term delivery (66.0 vs. 64.6%; P = 0.853), very preterm birth (2.4 vs. 2.7%; P = 0.913), low live birth weight (12.2 vs. 11.6%; P = 0.918), very low live birth weight (2.4 vs. 0.5%; P = 0.295), Caesarean section (85.4% vs. 84.0%; P = 0.828), live birth (82.0 vs. 76.8%; P = 0.415) and perinatal mortality (0 vs. 1.8%; P = 1.000); additionally, the gestational age at delivery (38.2 ± 2.1 vs. 37.9 ± 2.7 weeks; P = 0.465) and the live birth weight (3026 ± 573 vs. 3072 ± 518 g; P = 0.608) had no significant differences between these two groups. For singleton pregnancy conceived via IVF-ET, women with a didelphus uterus and those with a unicornuate uterus had similar pregnancy and obstetric outcomes.
Hepatitis B Virus in Oocytes and Embryos: Pregnancy Outcomes and Children’s Health

Abstract:Objective To investigate whether the presence of hepatitis B virus (HBV) in oocytes and embryos affects pregnancy outcomes for in vitro fertilization and embryo transfer (IVF-ET) and is related to the vertical transmission of HBV to children. Design Retrospective cohort study. Setting A university-affiliated fertility center. Patient(s) This study included 167 couples with at least one HBsAg-seropositive partner. These couples underwent IVF-ET and the discarded oocytes and embryos had been tested for HBV. Couples with HBV-positive oocytes or embryos were categorized as the positive group, whereas those couples with HBV-negative oocytes and embryos served as the negative group. Intervention(s) None. Main Outcome Measure(s) Pregnancy outcomes and the rate of children’s HBV infected. Result(s) The pregnancy outcomes of fresh and frozen ET were not associated with the presence of HBV in the oocytes and embryos. Out of the 106 babies born, one child whose mother tested positive for HBsAg but had negative oocytes and embryos was infected with HBV. Additionally, 26.09% children who had been administered passive immunization and active vaccinations did not reach protective levels of anti-HBV antibodies (HBsAb) and became non-responders. The negative rate of children’s HBsAb was associated with the presence of HBV in oocytes and embryos (OR=3.01, 95% CI=1.04-9.25, P < 0.05). Conclusion(s) The presence of HBV in oocytes and embryos did not affect pregnancy outcomes or result in the vertical transmission of HBV to the offspring of HBV carriers. Follow-up is needed for HBV-vaccinated children with an HBV-infected parent. Booster vaccinations are necessary for continued protection.
Treatment Effects of Laparoscopy Versus Laparotomy on Heterotopic Pregnancy after in Vitro Fertilization and Embryo Transfer.

Abstract:ObjectiveTo compare the treatment effects of laparoscopy versus laparotomy on heterotopic pregnancy (HP) after in vitro fertilization-embryo transfer (IVF-ET). MethodsThe retrospective case-control study enrolled 109 patients diagnosed with HP after IVF-ET treatment in our hospital from January 2009 to March 2020. All patients received surgical treatment by either laparoscopy or laparotomy. Data for general characteristics, diagnostic features, surgical parameters, as well as perinatal and neonatal outcomes were collected. ResultsSixty-two patients received laparoscopy and 47 received laparotomy. Significantly lower percentage of large hemoperitoneum (P = 0.001), shorter surgery duration (P < 0.001), less intraoperative blood loss (P = 0.001), higher rates of general anesthesia (P < 0.001), and lower cesarean section rates for singletons (P = 0.003) were found in the laparoscopy group. The perinatal and neonatal outcomes were comparable between the two groups. When interstitial pregnancy was considered alone, the surgical blood loss was significantly reduced in the laparoscopy group (P = 0.021), but there was no significant difference in hemoperitoneum, surgery duration, or perinatal and neonatal outcomes in singletons. ConclusionBoth laparoscopy and laparotomy are effective surgical treatments for HP after IVF-ET. Laparoscopy is minimally invasive but laparotomy can be an alternative in emergency situations.
[Management and Pregnancy Outcomes of Heterotopic Pregnancy].

Abstract:Objective: To investigate the clinical features, diagnosis and treatments of heterotopic pregnancy (HP) and demonstrate the outcomes of HP after the surgical treatment of ectopic pregnancies. Methods: A retrospective analysis was performed on 144 cases of HP in Women's Hospital, School of Medicine, Zhejiang University from January 2003 to December 2016. Results: (1) Clinical features of HP:the average age of patients was (30.8±3.8) years old, body mass index (BMI) was (21.9±2.9) kg/m(2), and was diagnosed at (6.5±1.3) weeks gestational age. Four patients (2.8%) were naturally pregnant, 10 patients (6.9%) received ovulation induction, and 130 patients (90.3%) had received infertility treatments. Fifty-one patients (35.4%) had no clinical symptoms, and 93 patients (64.6%) had clinical symptoms included vaginal hemorrhage,abdominal pain and hemorrhagic shock caused by intraperitoneal hemorrhage. The location of ectopic pregnancy was most common in the fallopian tubes (59.0%, 85/144) and the interstitial part (33.3%,48/144) . (2) Ectopic pregnancy treatment of HP:thirteen patients underwent expectation treatment, and the remaining 131 cases underwent surgical treatment, including laparoscopy (n=56) , laprotomy (n=52) , and fetal reduction (n=23) . Among the 131 patients underwent surgery, intrauterine pregnancy were found inevitable abortion in 6 cases preoperatively. The total early abortion rate after surgery was 14.4% (18/125) ; the second operation rate was 3.1% (4/131) . (3) Intrauterine pregnancy outcome of HP: 120 intrauterine fetal were survival, the total live birth rate was 83.3% (120/144) .One hundred and seven intrauterine fetal were survival after operation and the live birth rate after operation was 85.6% (107/125) . Twenty-nine cases were premature delivery and the premature delivery rate was 24.2% (29/120) . There was no significant differences between tubal HP and interstitial HP group in the preterm birth rate [25.8% (16/62) vs 26.3% (10/38) ; χ(2)=0.003, P>0.05]. Cesarean section rate of delivery in interstitial HP group was significantly higher than that in tubal HP group [97.4% (37/38) vs 59.7% (37/62) ], and the difference was statistically significant (χ(2)=17.400, P<0.05) . Conclusions: The clinical manifestations of HP are diversified, combining of high risk factors, clinical symptoms and ultrasonography could improve the accuracy of diagnosis. Different method has been used to treat HP, such as laparoscopic or laparotomy and fetal reduction, and there are varying degrees of failure rate and postoperative abortion rate. We should consider carefully to adopt expectant management. Through individualized treatment, most HP could get good perinatal outcomes.

Abstract:Abstract Study question What are the ultrasonic characteristics of heterotopic intramural pregnancy (HIMP)? How to manage and what about the clinical outcomes of HIMP? Summary answer Expectant management may be a considerable choice for an non-viable intramural pregnancy (IMP). Most intrauteine pregnancies (IUPs) of HIMPs seems to have good outcomes. What is known already Heterotopic pregnancy (HP) post in vitro fertilization is very rare in infertility women, with a prevalence of 0.04%. HIMP is one of the rarest types of HP, where one gestational sac (GS) is embedded within the endometrial cavity and the other one GS is implanted in the myometriun. HIMP was firstly and only described by Jiangtao Lyu et al. in 2018. So far, little is known about its natural history and ultrasonic imaging characteristics. And there is no consensus regarding the ultrasound diagnosis and clinical management for HIMP due to few evidence-based medicine records. Study design, size, duration A retrospective observational study was conducted of 6 infertile women who obtained a HIMP through in vitro fertilization-embryo transfer (IVF-ET) between January 2009 and December 2019 at our reproductive centre. Participants/materials, setting, methods Six infertile women conceived a HIMP via IVF-ET were retrospectively retrieved between January 2009 and December 2019 at the Reproductive and Genetic Hospital of CITIC-Xiangya (Changsha City, China). The ultrasound diagnosis, clinical management and pregnancy outcome of these cases were analysed. The ultrasound findings, therapeutic methods and clinical outcomes were obtained from the hospital’s electronic medical records. This study was approved by the local ethics committee. Main results and the role of chance: Six women with HIMPs were retrospectively analysed. Among them, 5 cases were revealed by ultrasound scans; however, one case was misdiagnosed. The diagnostic accuracy was 83.3%. Five cases of HIMP were diagnosed at initial scan. The diagnostic time ranged from 22 to 38 days after ET (5 + 6 - 7 + 6 weeks). Among them, an intramural GS was observed in all 5 cases; embryonic cardiac activity (ECA) was detected in one case by the followed-up scans; there was a yolk sac only in one case; an empty GS was noted in 3 cases. An IUP was revealed in all 6 cases, and ECA was observed in 5 cases at the initial diagnosis or later. A GS with a yolk sac only was showed in one case. Among the 5 diagnostic women, one case with a live IMP was treated with laparoscopy at 8 + 1 weeks, 4 cases were managed expectantly. Of them, the IUPs of 4 cases delivered live infants and one case managed expectantly experienced miscarriage. In one case, IMP was misdiagnosed as interstitial pregnancy at day–28 scan. Exploratory laparoscopy and foetal reduction were performed at 8 + 2 weeks. Laparoscopy confirmed an IMP and the retained IUP delivered a live infant. Limitations, reasons for caution The case numbers are too few to draw any objective conclusions, because of the extreme rarity of HIMP. Thus, a further multi-centre larger prospective study will help to confidently illustrate the clinical significance, and effective and appropriate management method for women with a HIMP. Wider implications of the findings: Our study showed that HIMP may not be as rare as previously reported. Increased awareness of this condition, better comprehension of the diagnostic criteria and improved resolution of ultrasound equipment may result in more frequent and accurate detection of HIMP, which will be helpful for early management to preserve IUP. Abstract:Objectives: The purpose of this study was to evaluate the clinical characteristics, prenatal diagnosis, and management of patients with heterotopic pregnancy after ovulation induction or embryo transfer. Methods: This was a retrospective study of fifty cases with heterotopic pregnancy, in which the fertilization way, type and number of embryo transferred, gestational age, clinical presentation and outcome of intrauterine, ultrasound presentation and site of ectopic pregnancy, bilateral fallopian tube and treatment were evaluated. Results: Six patients had spontaneous pregnancy and two had artificial insemination after ovulation induction. Sixteen had fresh and nineteen had frozen embryo transfer with seven patients unrecorded its embryo type and number. The average days from transplantation (or intercourse/insemination) to diagnosing heterotopic pregnancy was thirty-seven with the earliest eighteen and the latest more than 70 days. Although the most common presentation was vaginal bleeding or abdominal pain, more than 21% was found by ultrasound and rare individuals even presented with gastrointestinal symptom which may imply ruptured EP and hemorrhagic shock. Giving proper treatment (surgery or local drug injection), the majority of them had a successful intrauterine pregnancy with only seven miscarried. Conclusions: Ovulation induction or embryo transfer increased the risk of HP greatly and clinician should raise high suspicious during the whole first trimester. Although the most ectopic site was ampullary, other sites such as cornual, cervical, abdominal especially interstitial or tubal stumps should also be assessed by ultrasound even in patients with bilateral salpingectomy or tubal obstructed. Repeated ultrasound tests 2 weeks after the diagnosis of intrauterine pregnancy with heart beating was very necessary to find the missed ones in suspicious patients. Compared with surgery, embryo suction with or without proper local drug injection would be more advisable for patients with cervical, cornual, or interstitial pregnancy in order to reserve the intrauterine pregnancy.

Abstract:Objectives:To investigate the clinical manifestations,diagnosis,therapy and outcomes of heter-otopic pregnancy after in vitro fertilization and embryo transfer (IVF -ET).Methods:3 cases of heterotopic preg-nancy after IVF -ET were analyzed retrospectively between January 2001 and December 2013 in Peking University First Hospital.Results:The 1st patient received emergency laparotomy bilateral tubal resection for the broken of sac caused by ruptured ectopic pregnancy.And the pregnancy continues until found abortion without any obvious incen-tive after 16 weeks.The 2nd one found intrauterine fetal demise when admitted into the hospital.And the ultrasound found ectopic gestational sac,then the patient received curettage and laparoscopic tubal resection.The last patient was found heterotopic pregnancy by transvaginal ultrasound and was received salpingectomy (right tube removed) with laparoscopic.Now the intrauterine gestational sac continued to develop.Conclusion:It is difficult to diagnose at the early stage of heterotopic pregnancy for the shortage of typical clinical manifestations.Clinicians should pay more attention to it and take timely treatment based on surgery,so as to protect patients,their fertility and embryo.
The indicator of clinical outcomes for patients with heterotopic pregnancy following in-vitro fertilization with embryo transfer

Abstract:ObjectiveTo explore the early predictors for pregnancy outcomes in patients with heterotopic pregnancy (HP) following in-vitro fertilization with embryo transfer (IVF-ET).Material and methodsThis retrospective study reviewed 81 patients with HP following IVF-ET in our institution between January 2003 and September 2017. The relationships between clinical outcomes and general characteristics, sonographic features and different management options were analyzed by logistic regression analysis.ResultsThe gestational age at the time of diagnosis was 50.9 ± 12.3 days. Among these cases, 76 were accurately diagnosed by TVS, 1 was misdiagnosed as adnexal torsion by TVS, and 4 were confirmed to have IUPs after the surgical treatment. Hence, the sensitivity of TVS for detecting HP was 93.8% (76/81). However, forty-seven patients (58.0%) had suspected HP when they underwent the initial TVS. Among these patients, live births occurred for 60 patients, 11 of whom delivered preterm. The miscarriage rate was 58.3% (14/24) for patients without IUP cardiac activity at HP diagnosis, and 12.3% (7/57) for patients with IUP cardiac activity; a significant correlation was identified (χ2 = 18.651, P < 0.001). Additionally, the abortion rate of patients following fresh non-donor embryo was higher than patients after frozen-thawed embryo (χ2 = 10.437, P = 0.001). Further by logistic regression analysis, patients following frozen-thawed embryo and an IUP with cardiac activity at HP diagnosis were identified as two independent factors of pregnancy outcome. (OR = 0.060, 95%CI = 0.008–0.471, P = 0.007; OR = 0.010, 95%CI = 0.001–0.124, P<0.001).ConclusionsPatients following frozen-thawed embryo and an IUP with cardiac activity at HP diagnosis could be the independent predictors for a favorable prognosis.

Abstract:Background Heterotopic interstitial pregnancy is a rare variant of heterotopic pregnancies, and it poses challenges in treating the heterotopic pregnancy and preserving the intrauterine pregnancy. However, there is no clear consensus regarding the optimal management. The aim of this study was to investigate the pregnancy outcomes of women diagnosed with heterotopic interstitial pregnancy. Methods A total of 17 women diagnosed with heterotopic interstitial pregnancy between July 2010 and December 2015 were included. General characteristics of each patient, including age, gravidity and parity, history of pelvic inflammatory disease or surgery, and especially the corresponding therapeutic interventions, were retrospectively analyzed. Moreover, pregnancy outcomes were further followed by face-to-face interview. Results Of the 17 patients, 10 (58.5%) underwent surgical treatment (7 laparoscopic cornual resection, and 3 laparotomy); and 3 cases simultaneously terminated the intrauterine pregnancy by suction evacuation. Compared with laparotomy, laparoscopic cornual section showed shorter operative time (median 40 vs. 70 min), less blood loss (150 vs. 400 ml) and shorter hospital stay (2 vs. 4 days). In addition, 4 (23.5%) patients underwent selective embryo reduction under transvaginal ultrasound guidance. Expectant management was chosen in the remaining 3 patients. In the follow-up study, other than a case of missed miscarriage, the other 13 women who remained committed to their pregnancies all delivered healthy babies either by caesarean section or vaginal birth. No congenital anomalies were reported, and all the infants were in good growth and development. Conclusions Laparoscopic cornual resection is a feasible approach with favorable surgical and long-term pregnancy outcomes. Additionally, medical or expectant management may be a viable treatment option for selected symptom-free patient. Although the survival of the intrauterine pregnancy could not always be assured, the prognosis for a woman with heterotopic interstitial pregnancy is generally good.
Recurrent Heterotopic Pregnancy Following Frozen-Thawed Embryo Transfer: A Case Study and Comprehensive Literature Review

Abstract:BACKGROUND Recurrent heterotopic pregnancy is a serious and rare pregnancy complication that occurs after in vitro fertilization and embryo transfer, posing a great threat to the safety of the mother and intrauterine fetus. We report a case of recurrent heterotopic pregnancy after in vitro fertilization and embryo transfer. We also reviewed the literature to explore the causes, diagnosis, treatment, and prevention of recurrent heterotopic pregnancy. CASE REPORT A 32-year-old woman with tubal factor infertility underwent in vitro fertilization and embryo transfer. Oocyte extraction was performed twice, while embryo transfer was performed 4 times, with 2 embryos transferred each time. No pregnancies occurred during the 2 fresh embryo transfer cycles; however, heterotopic pregnancies occurred during both frozen-thawed embryo transfer cycles. Ultrasonography detected only the first heterotopic pregnancy, at 6 weeks after embryo transfer. As the intrauterine embryo had stopped developing, laparoscopically monitored dilatation and curettage and removal of the affected fallopian tubes were performed. The second heterotopic pregnancy was detected 3 weeks after embryo transfer, guided by the ultrasound examination conducted earlier. After timely surgical treatment, a live birth occurred. CONCLUSIONS This case and review of the literature elucidates the importance of considering the possibility of a heterotopic pregnancy during ultrasound examinations, especially in patients who utilized assisted reproductive technology to transfer multiple embryos.
Clinical Outcomes of Laparoscopic Management of Heterotopic Interstitial Pregnancy After in vitro Fertilization-embryo Transfer

Abstract:Objective To analyze the safety and clinical outcomes of laparoscopic surgery for heterotopic interstitial pregnancy patients after in vitro fertilization-embryo transfer (IVF-ET).Methods Clinical data of 6 cases of heterotopic interstitial pregnancy after in vitro fertilization-embryo transfer (IVF-ET) who underwent general anaesthetic laparoscopic surgery in our hospital were analyzed.Under the laparoscopic exploration, intraperitoneal blood clots were removed, and the incision of interstitial fallopians were continuous sutured after the ectopic pregnancy focal were cleaned up.Results The mean surgery time was (63.7±14.2) min (range, 55-80 min).All the patients underwent ultrasound examinations 2 weeks after the surgery.All the intrauterine fetal heart beats could be seen and no obvious blood flow signal was seen in the interstitial part of uterus.Follow-ups for 23-41 months (mean, 31.8±5.9 months) found all the 6 patients had childbirth without uterine rupture or neonatal malformation.ConclusionLaparoscopic management may lead to favorable prognoses in patients with heterotopic interstitial pregnancy.
Laparoscopic surgical treatment and obstetric outcomes of women with interstitial heterotopic pregnancy in IVF-ET

Abstract:Objective To investigate the clinical efficacy of laparoscopic treatments for women with interstitial heterotopic pregnancy(IHP) after in vitro fertilization and embryo transfer(IVF-ET),and the obstetric outcomes.Methods The women who had IHP after fresh or frozen-thawed embryo transfer were retrospectively studied,and they were all treated with laparoscopic surgery at the Center for Reproductive Medicine of Peking University Third Hospital from January,2011 to December,2015.The laparoscopic surgery was conducted by opening the interstitial pregnancy mass,removing the gestational tissue,electrodessicating the inner wall of interstitial cavity,and suturing the cornual.Patients' genenral conditions,peri-surgical data and pregnancy outcomes were reviewed.Results All the 18 women with the diagnosis of IHP were successfully treated by laparoscopic surgery.Ruptured interstitial pregnancy and internal bleeding were found in 9 cases,and 6 of them had the history of salpingectomy at the same side.The average operating time was (62.1 ± 16.4) minutes,no peri-operative complications.All the 18 patients had live-birth deliveries.Two patients had preterm births at 35-and 36-gestation week separately,and the other 16 patients had full term deliveries.No signs of persistent ectopic pregnancy and uterine rupture were found.Conclusions The laparoscopic technique was feasible and safe for IHP with satisfactory obstetric outcomes.To the patients of interstitial pregnancy with salpingectomy history,more attention should be paid and earlier intervention should be taken to prevent the ectopic pregnancy rupture.

Abstract:Objective: To characterize general attributes of women with heterotopic pregnancy (HP) in order to establish an optimal management regimen.Study design: A retrospective analysis was conducted on 25 HPs recorded at the Women's Hospital of Zhejiang University between 2007 and 2011. Data on patients' symptoms, risk factors, diagnostic features, treatment and outcome were collected and analysed.Results: All patients had conceived via assisted reproductive technology (ART), 72% had a history of pelvic inflammatory disease and 68% of patients were symptomatic. Fifty-six percent of patients were diagnosed with HP via routine transvaginal ultrasound examination at approximately 6-7 weeks of gestation. Eighty percent of patients underwent surgical treatment (35% laparoscopic salpingectomy, 65% laparotomy), and 20% of patients underwent medical treatment with local injection of methotrexate (MTX) or MTX in combination with potassium chloride (KCl) into ectopic sites such as the cervix or interstitium. Eighty-eight percent of patients delivered live infants without congenital anomalies, and three patients (all of whom underwent surgical treatment) miscarried.Conclusions: Routine transvaginal ultrasound examination at approximately 6-7 weeks of gestation could facilitate the diagnosis of HP, although repeat ultrasound is necessary to avoid misdiagnosis. Prompt diagnosis and correct treatment, including local injection of MTX or MTX in combination with MCI into various ectopic sites, led to favourable prognoses. Abstract:Objective:To investigate the diagnosis and treatment of heterotopic pregnancy after IVF-ET.Methods:To analyze a case of heterotopic pregnancy after IVF ET retrospectively.Results:The patient had heterotopic pregnancy after IVF-ET,and followed operation approved a stale ectopic pregnancy.Although all efforts were done,the patient had a late abortion at the second trimester because of chorioamnionitis.Conclusion:Heterotopic pregnancy is very uncommon and easy to be misdiagnosed.If we found a gestation sac in the patient's uterus that undergoing assisted reproductive technology, should alarm the possibility of combining ectopic pregnancy.

Abstract:Objective:To investigate the incidence rate,risk factors,diagnosis,treatment methods and prognosis of heterotopic pregnancy(HP).Methods:We restrospectively analyzed the clinical data of 32 patients with HP in Gynecology Department of our hospital from Jan.2013 to Dec.2015.Resluts:The rate of HP after in vitro fertilization and embryotransfer(IVF-ET) was 1.54% (32/2076).All patients were transferred 2 embryoes.The rate of patients with tubal diseases was 71.88%(23/32).The diagnostic accurate rate of HP by transvaginal ultrasound was 84.38%(27/32).The intrauterine pregnancy in 29 patients were reserved,and 24 patients had pre-term or in-term delivery.The total successful pregnancy rate was 82.76%.The successful pregnancy rate in conservative observation group,fetal reduction group,laparoscopic surgery group and laparotomy group was 80.00%(4/5)、100%(3/3)、83.33%(5/6)、80.00%(12/15),respectively.The intrauterine pregnant outcomes were not significantly different among the 4 groups(P>0.05).Conclusion:Tubal diseases and IVF-ET were the main risk factors of HP,and transvaginal ultrasound was the most important examination method.Laparoscopic surgery is the first line therapeutic method.The prognosis of HP was not bad.

Abstract:Background: Heterotopic cervical pregnancy is a rare event of ectopic pregnancy with an incidence rate of < 1%. Herein, we report a rare case of successful treatment of heterotopic pregnancy following an in vitro fertilization-embryo transfer using ultrasound-guided hysteroscopy. In order to choose the best treatment option, we reviewed the clinical treatments and discussion of heterotopic cervical pregnancy over the last 15 years. Methods: The heterotopic pregnancy was terminated using ultrasound-guided hysteroscopy; however, the intrauterine pregnancy was maintained. We searched for the keywords "cervical pregnancy combined with intrauterine pregnancy," "compound pregnancy," "assisted reproductive technology," "cervical pregnancy," and "ectopic pregnancy" on PubMed to include articles published in the last 15 years. Results: The patient underwent an emergency cervical cerclage at 22 weeks' gestation for cervical insufficiency and delivered a healthy newborn at 38 weeks' gestation by transvaginal compliance. Twenty-one relevant case reports were selected. After analysis and discussion, we found that assisted reproductive technology is more likely to lead to heterotopic pregnancy than unassisted reproduction. Most women requesting the preservation of intrauterine embryos opted for surgical termination of cervical pregnancy and achieved the ideal outcomes. Conclusion: More attention should be paid to the diagnosis and treatment of heterotopic pregnancies to obtain the most optimal pregnancy outcome and long-term prognosis. Hysteroscopic surgery is a completely feasible cervical pregnancy treatment option with less postoperative impact on the mother and the intrauterine fetus.

Abstract:The incidence of HP has risen dramatically with the widespread application of assisted reproductive technology, and the diagnosis and management of HP remain challenging. With increased awareness of HP in patients who have undergone IVF-ET, early diagnosis and appropriate surgical treatment may lead to a favorable prognosis.

Abstract:Objective This retrospective study aims to analyze the influence of different treatment modalities on viable intrauterine pregnancy and to summarize the clinical features of heterotopic pregnancy (HP) patients. Material and methods All patients diagnosed with HP between January 2012 and December 2022 in Tianjin Central Obstetrics and Gynecology Hospital were reviewed retrospectively. Results This study diagnosed 65 patients using transvaginal ultrasound (TVS), including two cases of natural pregnancy, seven cases of ovulation induction pregnancy, and 56 cases after in vitro fertilization and embryo transfer (IVF-ET). The gestational age was 50.2 ± 13.0 days at the time of diagnosis. The most frequent manifestations were abdominal pain (61.5%) and vaginal bleeding (55.4%), while 11 patients (16.9%) had no symptoms before the diagnosis. The primary treatment was expectant and surgical management, including laparotomy and laparoscopic surgery. In the expectant management group, four patients were transferred to surgery due to rupture of ectopic pregnancy or ectopic pregnancy mass gradually enlarged. In the surgical management group, 53 patients underwent laparoscopic surgery, and six underwent laparotomy. The laparoscopic group's mean operation time was 51.3 ± 14.2 min (range: 15–140 min), and the median intraoperative blood loss was 20 mL (range 5–200 mL). In contrast, the laparotomy group's mean operation time was 80.0 ± 25.3 min (range 50–120 min), and the median intraoperative blood loss was 22.5 mL (range 20–50 mL). Four patients had postoperative abortions. Sixty-one newborns had no birth abnormalities, and no developmental malformations were discovered after a median follow-up of 32 months. Conclusion Expectant treatment has a high failure rate in HP, and laparoscopic surgery is a safe and effective treatment for removing ectopic pregnancy without increasing the risk of abortion or newborn birth defects.
