Abstract:Despite the known beneficial effects of creatine in treating exercise-induced muscle damage (EIMD), its effectiveness remains unclear. This study investigates the recovery effect of creatine monohydrate (CrM) on EIMD. Twenty healthy men (21–36 years) were subjected to stratified, randomized, double-blind assignments. The creatine (CRE) and placebo (PLA) groups ingested creatine and crystalline cellulose, respectively, for 28 days. They subsequently performed dumbbell exercises while emphasizing eccentric contraction of the elbow flexors. The EIMD was evaluated before and after exercise. The range of motion was significantly higher in the CRE group than in the PLA group 24 h (h) post exercise. A similar difference was detected in maximum voluntary contraction at 0, 48, 96, and 168 h post exercise (p = 0.017–0.047). The upper arm circumference was significantly lower in the CRE group than in the PLA group at 48, 72, 96, and 168 h post exercise (p = 0.002–0.030). Similar variation was observed in the shear modulus of the biceps brachii muscle at 96 and 168 h post exercise (p = 0.003–0.021) and in muscle fatigue at 0 and 168 h post exercise (p = 0.012–0.032). These findings demonstrate CrM-mediated accelerated recovery from EIMD, suggesting that CrM is an effective supplement for EIMD recovery.

What problem does this paper attempt to address?
### What problem does this paper attempt to solve? This paper aims to investigate the effects of Creatine Monohydrate (CrM) on the recovery from Exercise-Induced Muscle Damage (EIMD). Specifically, the study explores whether a 28-day continuous supplementation of CrM can reduce the extent of EIMD and accelerate its recovery process through a double-blind, randomized, placebo-controlled trial. ### Research Background and Objectives - **Background**: Intense exercise, especially eccentric contractions, can lead to EIMD, characterized by symptoms such as decreased muscle strength, limited range of motion, muscle stiffness, and pain. These symptoms typically appear within 24 hours post-exercise and may last for several days. Previous studies have suggested that creatine can improve muscle strength and reduce damage, but there is a lack of large-scale double-blind randomized controlled trial evidence. - **Objectives**: To determine, with a statistically robust sample size, whether pre-supplementation with CrM can reduce the damage from EIMD and promote the recovery process. ### Main Findings - **Study Design**: 20 healthy males were recruited and randomly divided into a creatine group (CRE) and a placebo group (PLA), with 10 participants in each group. Both groups consumed either creatine or a placebo for 28 days, followed by a dumbbell eccentric exercise test. - **Results**: - The range of motion in the creatine group was significantly higher than in the placebo group within 24 hours post-exercise. - The maximum voluntary contraction force in the creatine group was significantly higher than in the placebo group at 0, 48, 96, and 168 hours post-exercise. - The upper arm circumference in the creatine group was significantly lower than in the placebo group at 48, 72, 96, and 168 hours post-exercise. - The biceps brachii shear modulus in the creatine group was significantly lower than in the placebo group at 96 and 168 hours post-exercise. - The muscle fatigue level in the creatine group was significantly lower than in the placebo group immediately and at 168 hours post-exercise. ### Conclusion The study indicates that a 28-day continuous supplementation of Creatine Monohydrate can effectively promote the recovery from EIMD, particularly in terms of range of motion, maximum voluntary contraction force, upper arm circumference, and muscle fatigue. This suggests that CrM is an effective supplement for EIMD recovery.
