Abstract:DNA damage and oxidative stress induced by chemotherapy are important factors in the onset of premature ovarian insufficiency (POI). Studies have shown that mitochondria derived from mesenchymal stem cells (MSC-Mito) are beneficial for age-related diseases, but their efficacy alone is limited. Pyrroloquinoline quinone (PQQ) is a potent antioxidant with significant antiaging and fertility enhancement effects. This study aimed to investigate the therapeutic effect of MSC-Mito in combination with PQQ on POI and the underlying mechanisms involved.

What problem does this paper attempt to address?
The paper primarily explores the effects and potential mechanisms of combined treatment with mitochondria derived from human mesenchymal stem cells (MSC-Mito) and pyrroloquinoline quinone (PQQ) on chemotherapy-induced premature ovarian insufficiency (POI). ### Research Background - DNA damage and oxidative stress caused by chemotherapy are important factors leading to premature ovarian insufficiency. - Mitochondria derived from mesenchymal stem cells (MSC-Mito) have been shown to be beneficial for age-related diseases, but their effects alone are limited. - Pyrroloquinoline quinone (PQQ) is a powerful antioxidant with significant anti-aging and fertility-enhancing effects. ### Research Objectives - To explore the therapeutic effects and underlying molecular mechanisms of the combined use of MSC-Mito and PQQ on POI. ### Experimental Design - A POI model was established in C57BL/6J mice induced by cyclophosphamide and busulfan. - The effects of MSC-Mito and PQQ on the estrous cycle, ovarian pathological damage, sex hormone secretion, and oxidative stress in mice were evaluated. - Western blot and immunohistochemistry techniques were used to detect the expression of SIRT1, PGC-1α, and ATM/p53 pathway-related proteins in ovarian tissues. - A KGN cell model was constructed, and DNA damage and apoptosis were studied using comet assay and flow cytometry. ### Main Findings - Combined treatment with MSC-Mito and PQQ significantly restored ovarian function and antioxidant capacity in POI mice. - This treatment reduced the loss of follicles at various stages and improved the disrupted estrous cycle. - In vitro experiments showed that PQQ promoted the proliferation of MitoTracker-labeled MSC-Mito, synergistically restored mitochondrial function, and inhibited oxidative stress. - The combined treatment increased mitochondrial biogenesis mediated by SIRT1 and PGC-1α and inhibited the activation of ATM and p53, thereby reducing DNA damage-mediated apoptosis. - All the above changes induced by the combined treatment were reversed after KGN cells were pretreated with SIRT1 siRNA. ### Conclusion - The study results indicate that PQQ promotes the proliferation of MSC-Mito and, when used in combination with MSC-Mito, improves chemotherapy-induced POI through the SIRT1/ATM/p53 signaling pathway.
