Abstract:Enhanced recovery after surgery (ERAS) is associated with shorter hospital length-of-stay and fewer complications, according to a robust meta-analysis and systematic review by Sauro and colleagues 1 published in this issue of JAMA Network Open . In the meta-analysis, which included 74 randomized clinical trials and 495 studies from 2009 to 2021 across multiple countries and surgical specialties, the authors found that trials comparing ERAS with traditional postoperative care incorporated a mean (SD) of 11.1 (3.0) ERAS elements with a mean (SD) compliance of 74.7% (10.2%). Pooled outcomes showed decreased hospital and postoperative lengths of stay, as well as fewer 30-day complications for ERAS patients. There was no difference between ERAS and traditional care for postoperative hospital readmission rates or 30-day mortality. Most studies (>80%) were conducted in high- and middle-income countries. Approximately 40% of studies were conducted in China and India; and 6.8% of the studies were from the United States. Gastrointestinal and gynecologic surgery were the most-represented specialties, accounting for approximately 60% of the included studies. This analysis adds to the existing evidence supporting the incorporation of ERAS into standard surgical recovery programs. As noted by Sauro et al, 1 ERAS is a heterogeneous set of interventions with varying levels of compliance, and this may challenge the interpretation of regression estimates, knowing that some ERAS components are supported by stronger evidence than others. 2 However, in defining the exposure as the presence or absence of any program, and not a specific set of interventions, Sauro and colleagues 1 zoom out from protocols and highlight the culture of enhanced recovery. Knowing that the principles (and culture) of ERAS (eg, early ambulation, multimodal pain control, early enteral feeding) have diffused into many of our personal paradigms for standard postoperative care, the next set of questions about optimizing care after surgery will invariably run into the modern tension in biomedical science: protocols or precision? In a commentary in Anesthesiology, Schreiber and Muehlschlegel ask this exact question in response to a study characterizing expanded pain phenotypes and what the implications might be for ERAS pain control. 3 They highlight that ERAS protocols are often designed based on the surgical procedure, and alternatively propose that ERAS protocols should be created based on "person type" rather than surgical type in order to best manage postoperative pain and other perioperative metrics. 3 This approach would consider patient-level risk factors for poorly controlled pain, outside of just the surgical procedure, to predict and prevent postoperative pain most effectively. Their commentary underscores the importance of efforts toward designing and implementing patient-centered approaches and that the procedure-patient dyad may be the unit of interest. Clearly, the introduction and incorporation of ERAS was a critical wake-up call for improving surgical outcomes across specialties, but as the lessons of ERAS become more ubiquitous, how can we make the next paradigm shift in surgical care and postoperative outcomes more targeted, nuanced, and patient-centered while retaining the principles we know to be of great value? Surgeons, anesthesiologists, and specialists across the perioperative spectrum continue to broaden the questions we ask and the data we collect. The evolution of technology and expansion of the electronic health record to gather individualized data from patients in the perioperative setting, and in turn capture quality-of-life metrics, has improved our ability to optimize patient-reported outcome measures. 4 From prehabilitation to prior experiences of pain, ileus, posttraumatic stress disorder, and others, not to mention the specifics of the planned operation, the expanse of measurable data leading into and continuing after surgery continues to grow. These data may have considerable implications for several postoperative domains such as nutrition, pain control, psychological safety, and physical activity. The specifics, though, about how best to incorporate this knowledge into practice with the confidence to recommend certain approaches over others remains somewhat elusive. With the availability of this data and burgeoning analytic methods, there may be a new horizon in perioperative medicine toward the goals of reducing morbidity and mortality, improving patient experiences, delivering efficient health care services, and reducing health disparities that both retains ERAS principles and expands our ability to provide precision medicine.
