Abstract:Precocious puberty in girls is defined by the appearance of secondary sex characteristics before the age of 8 years. Precocious puberty is either peripheral or central. In most cases, the cause of central precocious puberty is unknown. Rapidly progressive forms of central precocious puberty may benefit from puberty suppression to prevent compromised final height and psychosocial problems related to the physical changes. Idiopathic central precocious puberty in girls can be a challenging diagnosis. Clinical examination, evaluation of growth charts, bone age, and hormonal tests are part of the workup. The role of pelvic ultrasound to evaluate pubertal features of the internal genitalia in this context is controversial. In this paper, we have reviewed the literature to find the scientific bases for several proposed ultrasound parameters and cutoff values for the determination of onset of puberty in girls with suspected central precocious puberty. There are no reliable cutoffs for the measurements of uterus and ovaries for the diagnosis of central precocious puberty and there is overlap in the appearance of the organs in both prepubertal and pubertal girls. The interpretation of the findings on pelvic ultrasound must be done with caution and always in close communication with the referring clinicians.

Early breast development in girls: the power of greyscale sonography and sonoelastography

Abstract:Abstract Objective Accurate distinction between central pubertal precociousness (PP) and premature thelarche (PT) is important to guide treatment. Both greyscale ultrasonography (US) and sonoelastography can be used to examine breast tissue. The aim of this study is to investigate the performance of breast US and strain elastographic (SE) in the diagnosis of increased breast volume in girls. Methods Sixty-three girls with breast development up to 8 years of age and diagnosed with PP and PT were included in the prospective study. Basal luteinizing hormone (LH), follicle-stimulating hormone (FSH), and estradiol (E2) values were obtained. Each bud was considered as a unit in US. Mediolateral (ML) and anteroposterior diameters (AP) were measured, and US grading was performed. Breast SE was examined, and strain index (SI) was calculated. US and laboratory findings were compared. Results Of the 121 buds examined, 39 (32.2%) were with PP (6.97 ± 2.44 years) and 82 (67.8%) were with PT (6.51 ± 2.52 years). Diameters were correlated with bone age, LH, FSH, and US grade. The mean AP showed a moderate difference in favour of PP between the groups (P < .06). The mean ML was higher in PP (P < .01). There was a difference in mean SI values (P < .004). Sensitivity and specificity were 71% and 61% for ML and 72% and 56% for SI, respectively. Conclusion Both ML and US grading may help discriminate PP from PT. The role of sonoelastography requires further investigation. Advances in knowledge Ultrasound and SE show significant differences between PP and PT, but these are not sufficiently reliable to be of clinical use. The contribution of sonoelastography requires further study before it can be recommended. However, SI of breast tissue can be helpful in distinguishing PP and PT from other causes of early increase in breast volume.

Abstract:Objectives To analyze clinical characteristics of precocious puberty attributed to tumors. Methods Hospital-based analysis was performed for 24 adolescents with precocious puberty associated with tumors. Results Percen- tage of precocious puberty attributed to tumors in boys was 12.93%, while the percentage in girls was 0.50%. Significantly advanced bone age (BA), and obviously higher levels of LH and FSH (before and after the GnRH stimulation test) were found after the transformation from peripheral precocious puberty (PPP) to central precocious puberty (CPP). Conclu- sions The incidence of precocious puberty attributed to tumors was significantly higher in boys than that in girls. Adolescents with PPP associated with tumors will transform to CPP after operation.

Abstract:Objective To explore the significance of the cut point of peak level of luteinizing hormone (LH) in gonadotropin-releasing hormone (GnRH) agonist test in the diagnosis of precocious puberty in obese girls.Methods According to the diagnostic criteria of children's central precocious puberty diagnosis consensus (2015),796 cases of precocious puberty (peak level of LH ≥3.3 IU/L) in Children's Hospital of Zhejiang University School of Medicine from January 2014 to December 2015 were divided into normal weight group (573 cases),overweight group (170 cases) and obesity group (53 cases).The 3 groups were compared in terms of basic level of LH,basic level of follicle stimulating hormone (FSH),peak level of LH and FSH,ratio of LH (peak)/FSH (peak),sex hormone binding protein (SHBG),and children with 3.3 IU/L ≤peak level of LH ＜5.0 IU/L were followed up.The accuracy of the diagnosis of central precocious puberty using peak level of LH was analyzed by using receiver operating characteristic (ROC) curve.Results The median of LH (peak) in the obese group was 6.92 IU/L,the median of SHBG was 46.52 nmol/L,the median of LH (peak) in normal weight group was 8.92 IU/L,and the median of SHBG was 87.28 nmol/L.There were significant differences between the 2 groups (P ＜ 0.05,0.001).A total of 89 cases in normal weight group and 65 cases of obesity/overweight group (3.3 IU/L ≤peak level of LH ＜5.0 IU/L) were followed up for 1 year,and 18 cases in normal weight group and 33 cases in obesity/overweight group developed to central precocious puberty,while the number of cases in the 2 groups was significantly different (P ＜0.001).The ROC curve analysis showed that the cut point of peak level of LH was 4.16 IU/L,the sensitivity was 0.606,the specificity was 0.656,and the Jordan index was the largest (0.344).Conclusions The peak level of LH in GnRH agonist test is important for the diagnosis of central precocious puberty in girls,but the effect of body mass index on its cut point needs to be considered.

Abstract:Objectives: To assess the performance of elastography (ES) and ultrasound (US) in predicting the malignancy of breast lesions and to compare their combined diagnostic value with that of magnetic resonance imaging (MRI). Materials and Methods: The study prospectively enrolled 242 female patients with dense breasts treated in 35 heath care facilities in China between November 2018 and October 2019. Based on conventional US and elastography, radiologists classified the degree of suspicion of breast lesions according to the US Breast Imaging Reporting and Data System (BI-RADS) criteria. The diagnostic value was compared between US BI-RADS and MRI BI-RADS, with pathological results used as the reference standard. Results: The results demonstrated that irregular tumor shape, a nonparallel growth orientation, indistinct margins, angular contours, microcalcifications, color Doppler flow and ES score on US imaging were significantly related to breast cancer in dense breasts (P=0.001; P=0.001; P=0.008; P<0.001; P=0.019; P=0.008; P=0.002, respectively). The sensitivity, specificity, PPV, NPV, accuracy and AUC of US BI-RADS category were 94.7%, 90.7%, 95.8%, 88.0%, 93.4% and 0.93 (95%CI, 0.88-0.97), respectively, while those of MRI BI-RADS category were 98.2%, 57.5%, 84.3%, 83.3%, 86.0% and 0.78 (95%CI, 0.71-0.85), respectively. MRI BI-RADS showed a significantly higher sensitivity than US BI-RADS (98.2% vs 94.7%, P=0.043), whereas US BI-RADS showed significantly higher specificity (90.7% vs 57.5%, P<0.001). US BI-RADS showed better diagnostic efficiency in differentiating nodules in dense breasts than MRI BI-RADS (AUC 0.93 vs 0.78, P<0.001). Conclusion: By combining the use of ES and conventional US, US BI-RADS had better diagnostic efficiency in differentiating nodules in dense breasts than MRI. For the diagnosis of malignant tumors in patients with dense breasts, MRI and US BI-RADS can be used as supplemental diagnostic tools to detect lesions, with US BI-RADS considered the preferred adjunctive resource.

Abstract:Objective:To explore the value of pelvic ultrasound in evaluating precocious puberty in girls and different types of precocious puberty.Methods:Thirty girls with central precocious puberty (CPP) and 30 girls with peripheral precocious puberty (PPP) diagnosed clinically from August 2019 to August 2021 were selected, and 30 girls with no significant abnormalities in pelvic ultrasound, gonadotropin releasing hormone (GnRH) stimulation test, and bone age measurement were selected as a normal control group. All the children underwent pelvic ultrasound examination. The differences in age, body mass index, uterine thickness, uterine length diameter/cervical length diameter, uterine volume, ovarian volume, and maximum follicular length diameter were compared among the three groups of girls, and the ROC curves of different ultrasound parameters for CPP, PPP, and normal subjects were drawn to evaluate the evaluation efficiency of each parameter.Results:The uterine thickness, uterine volume, ovarian volume, and maximum follicular length diameter in the CPP group were significantly higher than those in the control group and PPP group (P＜0.05). The uterine thickness, uterine volume, ovarian volume, and maximum follicular length diameter in the PPP group were significantly higher than those in the control group (P＜0.05). Taking the GnRH stimulation test as the gold standard, the areas under the ROC curves of uterine thickness, uterine volume, ovarian volume, and maximum follicle length diameter to distinguish CPP and PPP were 0.900, 0.849, 0.843, and 0.755; the areas under the ROC curves to distinguish CPP from normal girls were 0.941, 0.887, 0.926, and 0.769; and the areas under the ROC curves to distinguish PPP from normal girls were 0.814, 0.811, 0.706, and 0.813, respectively. The area under the ROC curve of uterine thickness was the largest, and the cut-off value of uterine thickness＞1.39 cm yielded an 86.7% sensitivity and 90.0% specificity in identifying girls with CPP and PPP. Taking the uterine thickness＞1.07 cm as the cut-off value, the sensitivity and specificity for distinguishing CPP from normal girls were 96.7% and 90.0%, respectively. Taking the uterine thickness＞0.96 cm as the cut-off value, the sensitivity and specificity for distinguishing PPP from normal girls were 80.0% and 86.7%.Conclusion:Multiple parameters of pelvic ultrasound have good clinical application value in the evaluation of girls' precocious puberty and the identification of different types. As a new evaluation parameter, uterine body thickness deserves further attention. These parameters can be used as the primary screening method for precocious puberty to provide a reliable basis for clinical diagnosis.
Predictive value of transabdominal pelvic ultrasonography for the diagnosis of central precocious puberty: A single-center observational retrospective study

Abstract:This single-center, observational, retrospective study aimed to evaluate the diagnostic accuracy of pelvic ultrasonographic parameters for detecting central precocious puberty (CPP) in a cohort of female pediatric patients undergoing gonadotropin stimulation tests. The study population consisted of 47 female patients with a suspicion of CPP. Thirty four out of 47 patients (72.34%) were subsequently diagnosed with CPP based on the current laboratory diagnostic criteria (LH peak > 5 IU/L). The ultrasonography results of 39 out of 47 patients (82.97%) were categorized as pubertal, while 31 out of 34 participants (91.17%) in the CPP group exhibited pubertal ultrasonography features. In 13 out of 47 girls (27.65%), a CPP diagnosis was ruled out; however, among these 13 patients, eight exhibited pubertal ultrasonography features suspicious of CPP. We observed a robust concordance between the GnRH test results indicative of pubertal activation and the presence of pubertal pelvic ultrasonographic features in 31 out of 34 children (91.17%). A significant correlation was found between ovarian volume and basal LH and LH/ FSH ratio, and also for basal LH, LH peak, LH/FSH ratio and peak LH/FSH ratio (p = 0.026, p = 0.011, p = 0.031, p = 0.004, respectively). Pelvic ultrasonography had a sensitivity of 91.17% and a specificity of 38.46% in differentiating CPP from premature thelarche.

Abstract:Background Pelvic ultrasound is a simple, non-invasive, available, and low-cost test, with an important role in the assessment of precocious puberty. The objective of the study was to analyze the uterine and ovarian volume of patients with Central Precocious Puberty (CPP). Methods A cross-sectional and retrospective study was carried out with patients with CPP treated from November 2021 to October 2022. They were considered to have CPP if baseline LH > 0.3 IU/mL or pubertal test with leuprorelin 3.75 mg (LH after 2 hours of intramuscular injection > 8 mIU/mL), advancement >= 12 months or 2 standard deviations on bone age, thelarche before 8 years of age and progression of secondary sexual characteristics. Uterine volume >= 3.5 ml and ovary volume >= 2 ml were considered suggestive of pubertal stimulation. To check the existence of correlation between the tested variables, we used the Spearman correlation coefficient as they are variables without normal distribution. The significance level was α=0.05. The study was approved by the Research Ethics Committee (CAAE 39249220.5.000.5345). Results The mean age was 7.39 years, the mean height was 132 cm and the mean BMI was 17.34 kg/m2. The mean age of the thelarche and pubarche was 7 years and 11.62 years for the mothers' menarche. The average measures were respectively: uterine volume was 7.59 ml; volume of the largest ovary was 3.55 ml and the pulsatility index of the uterine arteries was 4.06. The average difference between chronological age and bone age was 18.75 months. The mean values of hormonal basal status were SDHEA 74.94 mcg/dL, LH 3.13 mIU/mL and estradiol 31.86 pg/mL. Tanner's median was M3P2. Analyzing uterine volume, a positive and statistically significant correlation was observed between it and baseline estradiol (r=0.501; p=0.007), basal LH (r=0.715; P = 3.5ml was found in 75% of our patients (21/28). Analyzing the volume of the largest ovary, there was no correlation between it and SDHEA (r=0.354; p=0.137), basal LH (r=0.350; p=0.073) or basal Estradiol (r=0.127; p=0.526) but was positive between it and the uterine volume (r=516; p=0.006). Besides this, the volume of the largest ovary >= 2 ml was found in 81% of our patients (22/27). Conclusions Uterine volume seems to be a good parameter for expressing estrogenic stimulus and ovarian volume was a good parameter for expressing gonadotrophic stimulus. Both, the uterine volume and the ovary volume can help in the diagnosis of CPP and also in the follow up to show progression or stabilization of puberty.

Diagnostic potential of Shear wave elastography for central precocious puberty with breast development

Abstract:Purpose: The aim of this study was to assess the accuracy and diagnostic use of shear wave elastography (SWE) in differentiating central precocious puberty (CPP) with breast development and to analyze the correlations between sex hormone levels and SWE parameters. Methods: A total of 227 participants were included in this retrospective case-control study, including 113 girls with genuine precocious puberty breast development (the CPP group) and 114 with non-genuine precocious puberty breast development (the non-CPP group). The participants underwent clinical assessment, hormonal assays, and SWE using advanced ultrasound equipment. Statistical analyses, including t-tests, correlation analysis, logistic regression, and receiver operating characteristic (ROC) analysis, were performed to evaluate the diagnostic value of SWE and sex hormone levels in differentiating CPP with breast development. Results: There were no significant differences in clinical characteristics between the two groups. The sex hormone levels of estradiol, testosterone, luteinizing hormone (LH), follicle-stimulating hormone (FSH), and prolactin in the CPP group were significantly higher than those in the non-CPP group. Shear wave velocity (SWV) Maximum (Max), SWV Minimum (Min), SWV Mean, SWV standard deviation (SD), and SWV coefficient of variation in the CPP group were significantly higher than in the non-CPP group. Correlation analysis demonstrated significant positive correlations between LH, FSH, estradiol, and testosterone levels with various SWE parameters, indicating their clinical relevance. Logistic regression analysis identified substantial predictive potential of sex hormone levels and SWE parameters for genuine precocious puberty breast development. Additionally, the ROC analysis highlighted a high predictive value of the combined model of SWE parameters, with an area under the curve (AUC) of 0.903. Conclusion: The study underscores the correlations between sex hormone levels and SWE parameters. The superior predictive performance of the combined model of SWE parameters emphasizes the value of integrated SWE assessments for improving the accuracy of diagnosing genuine precocious puberty breast development.
Pathologic breast conditions in childhood and adolescence: evaluation by sonographic diagnosis

Abstract:Objective: The growing awareness of female breast cancer has led to increased sensitivity toward pathologic breast conditions in children and adolescents. Thus, approximately 15% of patients in child and adolescent gynecology are referred for the first time because of conspicuous features of the breast such as pain, palpable masses, and other findings on visual inspection. The aim of this study was to analyze the underlying diagnoses and diseases and determine the status of breast sonography in the diagnostic process. Methods: The study population consisted of 62 female patients between 8 weeks and 20 years of age (1997-2002) who were examined clinically, followed by standardized sonography (7.5-13 MHz, conventional B-mode panoramic sonography). Presumed diagnoses were confirmed by biopsy in some patients (n = 16) and by follow-up with clinical examination and sonography in most cases (n = 46). Results: The clinical and sonographic evaluation confirmed 4 main groups of diagnoses: benign tumors (15), developmental disturbances (14), cystic changes (11), and inherent defects (7). In the remaining cases, the findings were no abnormality (9), nipple discharge without evidence of pathologic or morphologic correlates (3), abscesses (2), and epidermoid cyst (1). Conclusions: Knowledge of regular breast development and its variants is essential for the physician. Given knowledge of the sonographic appearance of physiologic breast development and specific lesions, breast sonography is most helpful in identifying and characterizing abnormalities and guiding further investigation.
Diagnostic Accuracy of Female Pelvic Ultrasonography in Differentiating Precocious Puberty From Premature Thelarche: A Systematic Review and Meta-analysis

Abstract:Background The gonadotropin-releasing hormone (GnRH) stimulation test is the benchmark for diagnosing precocious puberty (PP). However, it is invasive, time-consuming, costly, and may create an unpleasant experience for participants. Moreover, some overlaps may occur between PP and premature thelarche (PT) in the early stage of PP. Female pelvic ultrasonography may provide additional information to help differentiate PP from PT and subsequently initiate early treatment. In this study, we aimed to first directly compare pelvic ultrasonography parameters between PP and PT groups and secondly, investigate their diagnostic accuracy compared with the GnRH stimulation test. Methods A systematic search of the PubMed/MEDLINE, EMBASE, Scopus, and Cochrane Library databases was performed up to March 31, 2021. All types of studies, except for case reports and review articles, were included. The GnRH stimulation test was used to confirm PP diagnosis. Those whose organic conditions might cause PP were excluded. The mean, standard deviation, sensitivity, and specificity of each parameter were documented. Forest plots were constructed to display the estimated standardized mean differences (SMDs) from each included study and the overall calculations. A bivariate model was used to calculate the pooled sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), and diagnostic odds ratio (DOR). Results A total of 13 studies were included for analysis. The SMDs (95% confidence interval – CI) in ovarian volume, fundal-cervical ratio, uterine length, uterine cross-sectional area, and uterine volume between PP and PT groups were 1.12 (0.78–1.45; p < 0.01), 0.90 (0.07–1.73; p = 0.03), 1.38 (0.99–1.78; p < 0.01), 1.06 (0.61–1.50; p < 0.01), and 1.21 (0.84–1.58; p <0.01), respectively. A uterine length of 3.20 cm yielded a pooled sensitivity of 81.8% (95% CI 78.3%–84.9%), specificity of 82.0% (95% CI 61.0%–93.0%), PLR of 4.56 (95% CI 2.15–9.69), NLR of 0.26 (95% CI 0.17–0.39), and DOR of 19.62 (95% CI 6.45–59.68). The area under the summary receiver operating characteristics curve was 0.82. Conclusion Female pelvic ultrasonography may serve as a complementary tool to the GnRH stimulation test in differentiating PP from PT. Abstract:Objectives: Pubertal gynecomastia (PG) is defined as benign proliferation of male breast tissue. Ultrasonography is frequently used for follow-up of symptomatic gynecomastia. Shear wave elastography (SWE) is an auxiliary tool for the diagnosis of breast disorders. Our aim is to evaluate SWE properties of breast tissue in PG. Methods: Fifty consecutive adolescents, aged 10-18 with a diagnosis of gynecomastia were prospectively included in the study. Following clinical and laboratory evaluation, participants underwent B-mode ultrasound and classified as nodular, dendritic, and diffuse pattern. SWE and hormonal values of all groups were recorded. Then, statistical analysis was performed. Results: Briefly, 92 breasts (43 left and 49 right; 42 bilateral and 8 unilateral) were included in the analyses. There were no significant differences in clinical parameters including age, BMI-SDS, estradiol (E2), testosterone (T), and E2/T levels within groups (p > 0.05). Breast volume and SWE levels were significantly different in all groups (p < 0.05). SWE values were significantly lower for those of dendritic pattern than in nodular pattern (p < 0.05). ROC analysis revealed that for the best the cut-off value of 13.7 kPa, area under the curve value was 0.922 with 84% sensitivity and 87% specificity. Conclusions: SWE values were significantly lower in patients with dendritic gynecomastia than those with nodular gynecomastia. Knowledge of the SWE values for the initial diagnosis and follow-up values could help avoid unnecessary interventions.

Abstract:BackgroundAn underlying identifiable organic cause is present in up to 50% cases of central precocious puberty in male patients.Case characteristicsA 7-years-8-months-old presented with delayed puberal development. Analytical examinations showed suppressed basal and stimulated levels of testosterone, LH and FSH. Abdominal ultrasound and contrast cranial magnetic resonance results were initially negative.OutcomeGerminoma was found on cranial computer tomography.ConclusionThere is often a wide time-lapse between symptoms and diagnosis of germinoma, so frequent monitoring is vital.

Abstract:Puberty is one of the most astonishing periods of human life, when significant physical alterations occur along with psychosocial maturation. Precocious Puberty (PP) is defined as the appearance and progressive development of secondary sexual characteristics at a younger age than the general population, i.e. for Caucasian girls before 8 years of age. Untreated precocious puberty usually leads to short stature and can also cause significant emotional and behavioral issues. In recent years, an increased incidence of PP has been found in many countries although several studies now suggest that this trend has slowed down over the last decade in most industrialized countries, while persisting in other countries. Some girls with idiopathic precocious puberty may also have slowly progressive pubertal development without deterioration of their predicted height over a 2-year follow-up period. It is important to determine which girls to treat and the role of the clinician remains crucial. The clinician also needs to be familiar with the terminology of pubertal progression. The aim of this review was to examine the diagnosis of central precocious puberty (CPP) taking in account clinical practice and international literature.

Abstract:Objective To discuss the efficiency of ultrasonographic score evaluation system in the differential diagnosis of breast mass. Methods The software of ultrasonographic score evaluation system was designed according to the parameter of Breast Imaging Reporting and Data System(BI-RADS) categories,axillary region lymph node imaging and Doppler ultrasonographic bloodstream image features.Five-hundred and seventy patients of breast mass were evaluated by the system. The breast benign diseases and malignant tumor was analyzed based on the diameter respectively. The Receiver Operating Characteristic Curve(ROC curve) was used in appraising the efficiency of ultrasonographic score evaluation system. Results No significant difference between the benign breast mass and malignant tumor was shown in the group I of diameter lower than 1.0cm(P>0.05) ,and there were significant differences in the other three groups of diameter greater than 1.0cm. Area under the ROC curve of total groups was 0.884±0.016;with a sensitivity of 86.8% and specificity of 97.5%,the best critical score for differential diagnosis of total groups was 13.50. Conclusions Ultrasonographic score evaluation system is a concise method to differentiate the breast benign mass lesion and malignant tumor before operation. It is of value in the characteristic diagnosis of breast mass,especially in masses of diameter greater than 1.0 centimeter.

Abstract:BACKGROUND: Most epidemiologic studies of puberty have only 1 source of pubertal development information (maternal, self or clinical). Interpretation of results across studies requires data on reliability and validity across sources. METHODS: The LEGACY Girls Study, a 5-site prospective study of girls aged 6 to 13 years (n = 1040) collected information on breast and pubic hair development from mothers (for all daughters) and daughters (if ≥10 years) according to Tanner stage (T1–5) drawings. At 2 LEGACY sites, girls (n = 282) were also examined in the clinic by trained professionals. We assessed agreement (κ) and validity (sensitivity and specificity) with the clinical assessment (gold standard) for both the mothers’ and daughters’ assessment in the subcohort of 282. In the entire cohort, we examined the agreement between mothers and daughters. RESULTS: Compared with clinical assessment, sensitivity of maternal assessment for breast development was 77.2 and specificity was 94.3. In girls aged ≥11 years, self-assessment had higher sensitivity and specificity than maternal report. Specificity for both mothers and self, but not sensitivity, was significantly lower for overweight girls. In the overall cohort, maternal and daughter agreement for breast development and pubic hair development (T2+ vs T1) were similar (0.66, [95% confidence interval 0.58–0.75] and 0.69 [95% confidence interval 0.61–0.77], respectively), but declined with age. Mothers were more likely to report a lower Tanner stage for both breast and pubic hair compared with self-assessments. CONCLUSIONS: These differences in validity should be considered in studies measuring pubertal changes longitudinally when they do not have access to clinical assessments.

Abstract:Objective: To find the frequency of precocious puberty in children and to compare the clinical and laboratory parameters of central & peripheral precocious puberty. Study Design: Cross-sectional study. Setting: Karachi's National Institute of Child Health's Paediatric & endocrinology Division. Period: December 2016 and 2021. Material & Methods: All patients with precocious puberty will be taken from files through non-probability convenience sampling method. Data was analyzed on SPSS version 22.0. Results: Total 65 patients were included. The mean age was 6+3.35years. Precocious puberty was classified as peripheral precocious puberty in 38 (58.4%), central precocious puberty in 20 (30.76%), premature thelarche in 5(7.69%) and premature1pubarche in 2 (3.07%). In the peripheral precocious1puberty group, CAH was found in 22(78.5%), out of which 2 patients were of rare mutation of CAH presenting with peripheral precocious puberty (DAX mutation and 11 B hydroxylase mutation, adenocarcinoma was observed in 2(7.14%) followed by Mu-cane- Albright syndrome was in 4(14.28%) and van wykgrumbach syndrome in 10 patients. Central precocious puberty was found in 20 patients hypothalamic harmartoma in 4(20%), craniopharyngioma 3(15%), hypothalamic astrocytoma 1(5.0%), genetically proven neurofibromatosis in 1(5.0%) patient and hydrocephalus 1(5.0%) and in 10(50%) patients no cause was found. Conclusion: The conclusion of the study that Peripheral precocious puberty was more common than central precocious puberty. The common cause of peripheral precocious puberty was CAH, while central precocious puberty was idiopathic. Children with central precocious puberty had greater height SDS, weight SDS, FSH, and LH levels compared to children who had peripheral precocious puberty.
Ultrasonographic review of pediatric breast masses among Nigerian children in a tertiary hospital

Abstract:Background: Breast masses occur infrequently in children and adolescents. Most of these masses have proved that benign and conservative approach is the management of choice. Consequently, imaging has become crucial. The knowledge of the ultrasonographic features of childhood and adolescent breast masses is needed for age appropriate medical care. Aim: The aim of this study was to describe and document the ultrasonographic spectrum of breast masses in children and adolescents (0-19) years seen in the Ahmadu Bello University teaching hospital, Zaria. Materials and methods: A 2-year retrospective review of breast sonograms of 25 consecutively presenting children and adolescents (3 males and 22 females) who had palpable breast masses. Ultrasound scans were performed with a Mindray Machine DC-8 using the linear transducer at 7.5-12 MHz transducer frequency. Histopathological confirmation of the solid masses was also obtained. The statistical analysis of the data collected was done using the SPSS software version 20 (SPSS Inc., Chicago, IL, USA). Results: The patients were between 40 days and 19 years old (mean 14.8 years and standard deviation 4.1). The ultrasonographic findings were those of infections, benign tumoral lesions and pubertal endocrine changes. Overall, fibroadenoma was the most common mass, seen in 14 (56%) of the patients. Other findings were simple cysts, non-puerperal and puerperal mastitis, juvenile papillomatosis and normal glandular pubertal breast tissue. The three male patients had gynaecomstia, pseudogynaecomastia and cystic lymphangioma of the chest wall presenting as unilateral breast masses, respectively. There was no malignancy recorded in any of the patients. Conclusion: Ultrasonographically, benign masses predominate which is concordant with surgical findings. Familiarity with these features would obviate the need for unnecessary invasive procedures which should be reserved for only deserving cases.

Abstract:The study aimed to evaluate the correlation between self-reported pubertal developmental scale (PDS) and physically assessed Tanner staging by an experienced pediatrician among girls. In a school population-based study in Zhongshan, China, we recruited 1,722 girls in grades 1–3 by a multistage stratified cluster random sampling method. Participants completed self-reported PDS questionnaire prior to physical examination. Breast development was evaluated by a female pediatrician combined with ultrasound examination for overweight/obese girls; pubic hair development was evaluated. Otherwise, we tested follicle-stimulating hormone (FSH) and luteinizing hormone (LH) for some participants. We observed a weak association between Tanner-derived composite stage (TDCS) and puberty category scores (PCS) (τ=0.288, p<0.001) among all girls. There was correlation (τ=0.314, p=0.001) between ultrasound-derived composite stage (UDCS) and PCS among overweight/obese girls. Moreover, among overweight/obese girls, PCS was positively correlated with LH (r=0.265, p=0.008), but not FSH (r=0.155, p=0.123), and when the basal LH value was greater than 0.3 mIU/mL, the proportion of PCS stage ≥2 (9/18) was higher than the proportion of TDCS ≥2 (5/18). As for the determination of pubertal onset, when UDCS was used as the gold standard, the specificity of PCS was 0.86 and positive predictive value was 90.00 %. There was a weak correlation between PCS and TDCS among girls early adolescence. Moreover, among overweight/obese girls, combining hormone values, ultrasonographic stage of breast, and the positive predictive value of PCS, we posit that self-reported PDS might be a more reliable method than TDCS to evaluate pubertal development among overweight/obese girls.

Abstract:Objective: To assesse the clinical value of ultrasound in diagnosis of gynecomastia. Methods: Ultrasonographic characteristics of 106 gynecomastia patients confirmed pathologically or clinically during Jan 2005 to Dec 2012 were retrospectively analyzed. The patients were divided 5 groups according to the age, i.e. ≤19 years group, 20-39 years, 40-59 years, 60-79 years and ≥80 years group. Meanwhile, the patients were divided 4 groups according to the year of diagnosis: Jan 2005 to Dec 2006 group, Jan 2007 to Dec 2008 group, Jan 2009 to Dec 2010 group and Jan 2011 to Dec 2012 group. Data were analyzed with Chi-square analysis. Results: Ultrasound showed the thickened gland proliferation below the nipple or around the areola was well defined and hypoechoic or slightly hyperechoic, not encapsulated, well circumscribed from the anterior subcutaneous adipose tissue and the posterior pectoral muscle, the internal echogenicity was either homogeneous or heterogenous, no strong echo calcification presented. CDFI showed that there was light vascularity in and around the gland. Gynecomastia mostly occurred in 20-39 age and 2011-2012 year. Unilateral gynecomastia was most common (66/106, 62.26%). Conclusion: The incidence of gynecomastia has a growth trend in the past few years. Being an ideal inspection method, ultrasound has value in diagnosis and differential diagnosis of gynecomastia.
