Abstract:Obstructive sleep apnea (OSA) is characterized by repeated episodes of upper airway obstruction during sleep, and it is closely linked to several cardiovascular issues due to intermittent hypoxia, nocturnal hypoxemia, and disrupted sleep patterns. Pulmonary hypertension (PH), identified by elevated pulmonary arterial pressure, shares a complex interplay with OSA, contributing to cardiovascular complications and morbidity. The prevalence of OSA is alarmingly high, with studies indicating rates of 20–30% in males and 10–15% in females, escalating significantly with age and obesity. OSA's impact on cardiovascular health is profound, particularly in exacerbating conditions like systemic hypertension and heart failure. The pivotal role of hypoxemia increases intrathoracic pressure, inflammation, and autonomic nervous system dysregulation in this interplay, which all contribute to PH's pathogenesis. The prevalence of PH among OSA patients varies widely, with studies reporting rates from 15% to 80%, highlighting the variability in diagnostic criteria and methodologies. Conversely, OSA prevalence among PH patients also remains high, often exceeding 25%, stressing the need for careful screening and diagnosis. Treatment strategies like continuous positive airway pressure (CPAP) therapy show promise in mitigating PH progression in OSA patients. However, this review underscores the need for further research into long-term outcomes and the efficacy of these treatments. This review provides comprehensive insights into the epidemiology, pathophysiology, and treatment of the intricate interplay between OSA and PH, calling for integrated, personalized approaches in diagnosis and management. The future landscape of OSA and PH management hinges on continued research, technological advancements, and a holistic approach to improving patient outcomes.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is to explore the complex relationship between obstructive sleep apnea (OSA) and pulmonary hypertension (PH), especially the causal relationship between these two diseases and their impact on cardiovascular health. Specifically, the paper focuses on the following aspects: 1. **Epidemiology**: - Explore the prevalence of OSA and PH in different populations, and how these prevalences are affected by factors such as age, gender, and body mass index. - Analyze the prevalence of PH in OSA patients and the prevalence of OSA in PH patients. 2. **Pathophysiological mechanisms**: - Describe the complex physiological interactions between OSA and PH, including the effects of intermittent hypoxia, nocturnal hypoxemia, and sleep pattern disturbances on the cardiovascular system. - Explore in detail how mechanisms such as hypoxic - induced pulmonary vasoconstriction, sympathetic nervous system activation, inflammatory response, and oxidative stress lead to or exacerbate the development of PH. - Analyze the role of right ventricular dysfunction in the development of OSA and PH. 3. **Diagnosis and treatment**: - Discuss the currently used methods for diagnosing OSA and PH, including polysomnography, echocardiography, and right - heart catheterization. - Evaluate the effectiveness of treatment strategies such as continuous positive airway pressure (CPAP) in alleviating the progression of PH in OSA patients. - Emphasize the need for further research on long - term treatment effects and the effectiveness of different treatment methods. 4. **Future research directions**: - Call for more research to understand the long - term interactions between OSA and PH, especially in the areas of personalized diagnosis and management. - Emphasize the need for comprehensive and personalized diagnosis and treatment methods to improve the prognosis of patients. Through these discussions, the paper aims to provide a comprehensive perspective to help clinicians better understand and manage the complex relationship between OSA and PH, thereby improving the treatment effectiveness and quality of life of patients.
