Abstract:AIM To study the effects of intrathecal magnesium sulfate on anesthesia in severe pre-eclampsia patients undergoing cesarean section. METHODS Sixty severe pre- eclampsia primiparas undergoing caesarean section were included and randomly divided into two groups(n = 30). The patients in the magnesium group were received spinal anesthesia with 0.5% bupivacaine 2 m L + sufentanil 2.5 μg + magnesium sulfate 50 mg and the other patients in the control group received bupivacaine and sufentanil only. Intra- operative pain was rescued by intravenous fentanyl 1 μg·kg-1. Postoperative pain was rescued by patient control intravenous analgesia(PCIA). Onset, duration of sensory and motor block, duration of spinal anesthesia and postoperative analgesia requirements were studied. The adverse drug reactions and the Apgar score of infants were observed.RESULTS The duration of spinal anesthesia((233.4 ± 17.2) min vs.(192.4 ± 12.4) min) and motor block((204.8 ± 20.1) min vs.(173.6 ± 17.5) min) were significantly longer in the magnesium group compared with the control group(P < 0.05). The VAS at 4 h time point after cesarean delivery in the magnesium group was significantly lower than that in the control group(P < 0.05). Fentanyl requirement for 24 h following surgery was significantly lower in the magnesium group than in the control group(( 157.3 ± 45.5) μg vs.( 243.5 ±48.7) μg, P < 0.05). The incidence rate of adverse drug reactions, such as hypotension, bradycardia,nausea, vomiting and pruritus, was similar between the two groups(P > 0.05). The Apgar score at 1 min and5 min after babies’ delivery was above seven points in the two groups. CONCLUSION In parturients with severe pre- eclampsia undergoing caesarean delivery, the addition of intrathecal magnesium sulfate to bupivacaine and sufentanil prolongs the duration of analgesia and reduces postoperative analgesic requirements without additional adverse drug reactions.

Abstract:BACKGROUND: The pharmacokinetic basis of magnesium sulphate (MgSO4 ) dosing regimens for eclampsia prophylaxis and treatment is not clearly established.OBJECTIVES: To review available data on clinical pharmacokinetic properties of MgSO4 when used for women with pre-eclampsia and/or eclampsia.SEARCH STRATEGY: MEDLINE, EMBASE, CINAHL, POPLINE, Global Health Library and reference lists of eligible studies.SELECTION CRITERIA: All study types investigating pharmacokinetic properties of MgSO4 in women with pre-eclampsia and/or eclampsia.DATA COLLECTION AND ANALYSIS: Two authors extracted data on basic pharmacokinetic parameters reflecting the different aspects of absorption, bioavailability, distribution and excretion of MgSO4 according to identified dosing regimens.MAIN RESULTS: Twenty-eight studies investigating pharmacokinetic properties of 17 MgSO4 regimens met our inclusion criteria. Most women (91.5%) in the studies had pre-eclampsia. Baseline serum magnesium concentrations were consistently <1 mmol/l across studies. Intravenous loading dose between 4 and 6 g was associated with a doubling of this baseline concentration half an hour after injection. Maintenance infusion of 1 g/hour consistently produced concentrations well below 2 mmol/l, whereas maintenance infusion at 2 g/hour and the Pritchard intramuscular regimen had higher but inconsistent probability of producing concentrations between 2 and 3 mmol/l. Volume of distribution of magnesium varied (13.65-49.00 l) but the plasma clearance was fairly similar (4.28-5.00 l/hour) across populations.CONCLUSION: The profiles of Zuspan and Pritchard regimens indicate that the minimum effective serum magnesium concentration for eclampsia prophylaxis is lower than the generally accepted level. Exposure-response studies to identify effective alternative dosing regimens should target concentrations achievable by these standard regimens.TWEETABLE ABSTRACT: Minimum effective serum magnesium concentration for eclampsia prophylaxis is lower than the generally accepted therapeutic level.

Abstract:Magnesium (Mg) is an essential ion for life and is involved in many important biological processes. Mg deficiency may have serious consequences and has been implicated in many diseases. Assessment of Mg status is difficult because Mg is essentially intracellular and there is no simple, rapid and accurate laboratory test to determine total body Mg. New, non-invasive techniques are being developed that could be used in the near future. Mg therapy is clearly indicated in "torsade de pointes" and preeclampsia. Mg therapy is not recommended in the treatment of acute myocardial infarction, stroke or non-severe acute asthma. Nevertheless, Mg administration may be useful to reduce cerebral ischemic events after aneurysmal subarachnoid hemorrhage or in the treatment of severe acute asthma, specifically in children. Finally, the potential post-operative analgesic effect of Mg is still debated and results from new, larger clinical trials may help clarify this issue. This article reviews physiological functions and pharmacology of Mg status as well as the causes, clinical manifestations, and treatment of Mg deficiency. The possible analgesic effects of Mg in the postoperative period are also reviewed.
Oral Magnesium Supplementation for the Prevention of Preeclampsia: a Meta-analysis or Randomized Controlled Trials

Abstract:Previous studies evaluating the role of magnesium supplementation as a preventative strategy for preeclampsia showed inconsistent results. We performed a meta-analysis or randomized controlled trials (RCTs) to evaluate the influence of oral magnesium supplementation on the incidence of preeclampsia in pregnant women. Relevant studies were obtained by search of Medline, Embase, and Cochrane’s Library databases. A random effects model was used to pool the results. Influences of study characteristics of the outcome were evaluated by predefined subgroup analyses. Seven RCTs with 2653 pregnant women were included. Pooled results showed that oral magnesium supplementation during pregnancy significantly reduced the risk of preeclampsia (risk ratio [RR]: 0.76, 95% confidence interval [CI]: 0.59 to 0.98, P = 0.04) with no significant heterogeneity (P for Cochrane’s Q test = 0.42, I2 = 1%). The result was not significant for the outcome of severe preeclampsia (RR: 0.54, 95% CI: 0.18 to 1.69, P = 0.29; I2 = 0%), although only two studies were available. Subgroup analysis showed that oral magnesium supplementation did not significantly reduce the risk of preeclampsia in normal pregnant women (RR: 0.91, 95% CI: 0.67 to 1.25, P = 0.57), but the preventative effect was significant in studies with normal and high-risk pregnant women (RR: 0.54, 95% CI: 0.35 to 0.83, P = 0.005; P for subgroup difference = 0.04). In conclusion, oral supplementation of magnesium may reduce the risk of preeclampsia, which may be more remarkable in high-risk pregnant women.

Abstract:Eclampsia is a common complication of preeclampsia patients and can be life-threatening for both the mother and the fetus. Hence, timely intervention and appropriate management of this detrimental condition are extremely crucial. Eclampsia is described as the occurrence of generalized convulsions in patients with preeclampsia. Magnesium sulfate (MgSO4) is the drug of choice for treating and preventing eclampsia. This review aims to study and analyze the available literature on the pathogenesis of eclampsia, the pharmacology of MgSO4, and its effectiveness in the management of eclampsia. Other proposed treatments and their comparative study with MgSO4 are also discussed. Additionally, we examine the data regarding the impact of eclampsia, its public health burden, and the cost-effectiveness of MgSO4. One of the major drawbacks associated with the use of MgSO4 in low-income countries has been the cost of treatment and the lack of resources. We have analyzed the trials that have proposed alternate treatment regimens which could shape new guidelines to resolve these issues. For this review, we extensively studied abstract and full-text articles from multiple databases. This article discusses the pathophysiology of eclampsia, the pharmacology of MgSO4, the issues surrounding eclampsia management, and how MgSO4 benefits these patients.

Abstract:Preeclampsia is a heterogeneous pregnancy-specific syndrome associated with abnormal trophoblast invasion and endothelial dysfunction. Magnesium (Mg2+) level may be normal or decreased in women with preeclampsia. However, the use of Mg2+ salts, such as Mg2+ sulphate, are useful in reducing the pathophysiological consequences of preeclampsia with severe features and eclampsia. Although the mechanism of action of this Mg2+ salt is not well understood, the available evidence suggests a beneficial effect of Mg2+ for the mother and foetus. The mechanisms include a lower level of soluble fms-like tyrosine kinase 1 and endoglin, blockage of brain N-methyl-D-aspartate receptors, decreased inflammation mediators, activation of nitric oxide synthases, blockage of arginases, and reduced free radicals level. The maintenance of Mg2+ homeostasis in pregnancy is crucial for an appropriate pregnancy progression. Oral Mg2+ salts can be used for this purpose which could result in mitigating the deleterious consequences of this syndrome to the mother, foetus, and newborn.

Abstract:Severe pre-eclampsia is managed by using Magnesium sulphate to prevent its progress into eclampsia with a success rate of more than 50%. This study aimed at establishing reliable data pertaining factors influencing pharmacologic serum concentration of Magnesium and their correlation to obstetric outcomes. This study was carried out at Iringa regional referral hospital, spanning six months from November 2023 to April 2024. A convenient sampling technique was used to recruit 121 pre-eclamptic patients. Recruited women were tested serum Magnesium at admission, 4 and 12 hours post initial administration of MgSO4. IBM SPSS version 29 was used for analysis where, Continuous variables were analyzed by using median and interquartile range while categorical variables were analyzed using frequency and percentages. The associations between variables were determined using chi-square tests, univariate, and multivariate binary regression models. A p value<0.05 was considered statistically significant. Results: The median age of participants were (29±10 IQR), the median serum Mg at admission was (0.8±0.29 IQR) Mmol/L majority with low 53(43.8%) levels. Proportion of subtherapeutic serum Mg was 38.8% with median of (1.9±1.02 IQR) Mmol/L. Multivariate regression showed that; low serum Mg at admission [p<0.001, OR= 9.17, 95% CI (3.07- 27.37)], Creatinine level [p = 0.012, OR= 4.49, 95% CI (1.38- 14.6)], Overweight [p= 0.002, OR= 5.52, 95% CI (1.83- 16.68)] and proteinuria +++[p-value = 0.004, OR= 7.13, 95% CI (1.84- 26.86)] were significantly associated with subtherapeutic level. After adjusting with other factors, it was found that subtherapeutic level [p<0.001, OR=25.44(5.73-112.90)], elevated creatinine [p=0.040, OR=3.59(1.66-12.15)], Reduced urine output [p=0.001, OR=6.69(2.15-20.90)] were significant predictors of adverse maternal outcomes. Conclusion: Subtherapeutic serum Magnesium which significantly predict adverse obstetric outcomes, is influenced by factors which can be monitored and be useful in effective management of severe pre-eclampsia. This can help in administering tailored dose of MgSO4 for better outcomes.
Effects of Magnesium Sulfate on Oxidative Stress Injury and Hypoxia-Inducible Factor 1α Expression Level in Placental Tissues of Preeclampsia Rats

Abstract:Objective To investigate the effects of magnesium sulfate on oxidative stress injury and hypoxia-inducible factor 1α (HIF-1α) expression level in placental tissues of preeclampsia rats. Methods Thirty SD rats were randomly divided into the blank control group, preeclampsia (PE) group, and magnesium sulfate group (n=10 each) . Except for the blank control group, endotoxin was used in the other groups to establish PE rat model. The magnesium sulfate group was given intraperitoneal injection of 80 mg/kg magnesium sulfate. The blank control group and PE group were injected with the same dose of normal saline. The cesarean section was performed at 21 d after pregnancy, and the placenta tissues were isolated. The level of HIF-1α in the placenta tissues was measured by real-time fluorescence quantitative polymerase chain reaction (RT-PCR) method. The levels of glutathione peroxidase (GPX) , malondialdehyde (MDA) and superoxide dismutase (SOD) in the placenta tissues were determined by ultraviolet colorimetry. The tissue homogenate method was used to determine the reactive oxygen species (ROS) level in the placenta tissues. Results (1) At 10 d after the pregnancy, there were no statistically significant differences in the blood pressure and 24 h urinary protein quantification between the groups (all P>0.05) . At 15 d and 21 d after the pregnancy, there were no significant change in the blood pressure and 24 h urinary protein quantification in the blank control group, whereas the above indexes increased in the PE group and magnesium sulfate group (all P<0.05) , and there were statistically significant differences compared with those at 10 d after pregnancy in the same group (all P<0.05) . The blood pressure and 24 h urine protein quantification in the PE group and magnesium sulfate group were higher than those in the blank control group during the same time period (all P<0.05) . The blood pressure and 24 h urine protein quantification in the magnesium sulfate group were lower than those in the PE group (all P<0.05) . (2) The placenta weight, and weight and length of newborn rats in the PE group were lower than those in the blank control group (all P<0.05) , whereas the indexes in the magnesium sulfate group were higher than those in the PE group (all P<0.05) . (3) The levels of GPX and SOD in placental tissues in the PE group and magnesium sulfate group were lower than those in the blank control group (all P<0.05) , whereas the levels of MDA, ROS and HIF-1α mRNA were higher than those in the blank control group (all P<0.05) . The levels of GPX and SOD in the magnesium sulfate group were higher than those in the PE group, whereas the levels of MDA, ROS and HIF-1α mRNA were lower than those in the PE group (all P<0.05) . Conclusion PE rats show significant oxidative stress injury, and the mRNA level of HIF-1α is significantly high in placenta tissues. The intervention with magnesium sulfate may reduce oxidative stress injury and down-regulate the mRNA expression level of HIF-1α. Validation of a Model for Predicting Magnesium Concentration in Women with Preeclampsia: A Retrospective Cohort Study

Abstract:Objective: To validate a model for predicting magnesium concentration in magnesium sulfate treatment in preeclampsia. Design: Retrospective cohort study. Setting. Three secondary care hospitals, one accepting neonates from gestational week 28 + 0. Population. Women with preeclampsia undergoing magnesium sulfate treatment. Subjects initially received Zuspan treatment (4 g bolus and 1 g/h maintenance dose), commonly increased by individual titration. Main Outcome Measures. Difference in mean between measured and predicted magnesium concentration. Proportion of women reaching target concentration (>2 mM) in 25 h. Results: 56 women were included, with 356 magnesium measurements available. Mean magnesium concentration was 1.82 mM. The prediction model overestimated magnesium concentration by 0.10 mM (CI 0.04-0.16) but exhibited no bias for weight, creatinine, or treatment duration. Weighted mean infusion rate was 1.22 g/h during 30 hours. Overall success rate in reaching target concentration was 54%, decreasing to 40% in women > 95 kg. Overall success rate at 8 hours was 11%. No toxic concentrations were found. Conclusions: Zuspan regimen is very safe, but slow to reach therapeutic concentrations-despite efforts of individual titration. Success rate is lower in heavy women, which is of particular importance considering their predisposition to develop preeclampsia. The validated pharmacokinetic model performs well and may be used to individually tailor treatment from the outset.

Abstract:Preeclampsia is the second leading cause of maternal death in Uganda. WHO recommends using magnesium sulphate (MgSO4) to prevent and treat preeclampsia with severe features (PEC) and eclampsia. MgSO4 is used to prevent eclampsia and treat women who experience an eclamptic convulsion to avoid severe maternal/infant illnesses and death. We set out to assess MgSO4 administration patterns in women with PEC or eclampsia and the immediate newborn outcomes of neonates.

Abstract:Background: Preeclampsia and eclampsia are among the leading direct causes of maternal death and morbidity worldwide. Up to 34% of maternal deaths in Tanzania are due to preeclampsia/ eclampsia. Magnesium sulfate is recommended for preventing and treating convulsions in women with Preeclampsia or eclampsia. However, evidence suggests limited knowledge of its dosage and proper toxicity assessment after administration among health care providers. Aim of the study: This study explored nurse-midwives' perspectives on providing MgSO4 to patients with preeclampsia or eclampsia in Tanzania. Materials and methods: A descriptive exploratory qualitative study using in-depth interviews was conducted to understand nurse-midwives' perspectives on providing magnesium sulfate to patients with PE/E. Nineteen nurse-midwives were interviewed from three hospitals in the Dar es Salaam region. We used a semi-structured interview guide in Kiswahili language to collect data. All interviews were digitally recorded and transcribed verbatim. We analyzed data using inductive content analysis. Results: This study revealed that nurse-midwives provide magnesium sulfate to save the lives of women and their unborn children. Nurse-midwives reasoned that confidence in their skill enhances provision of magnesium sulfate. However, they were concerned about its effect on the progress of labour. Ineffective use of magnesium sulfate emerged from inadequate training, an unsupportive work environment, and underutilization of the existing guidelines. Conclusion: Nurse-midwives have clear drive to provide magnesium sulfate to women with preeclampsia or eclampsia. However, inadequate training, underutilization of guidelines and unsupportive work environment lead to ineffective use of magnesium sulfate. Targeted practical training should be emphasized for nurse-midwives mastery of clinical competencies.
Magnesium Sulfate Decreases Cerebral Perfusion Pressure in Preeclampsia

Abstract:OBJECTIVE: To determine the cerebral hemodynamic effect of magnesium sulfate (MgSO4) in preeclampsia.DESIGN: Prospective observational study.SETTING: Tertiary Care Medical Center.POPULATION: Preeclamptic patients (systolic < 160, diastolic < 110 mmHg).METHODS: Transcranial Doppler (TCD) of the middle cerebral arteries (MCA) of 15 preeclamptics, before and after IV MgSO4. No vasoactive drugs other than MgSO4 were given. 11 patients with mild range BP (140/90-160/110 mmHg) had measurements at baseline, 30 and 120 minutes after MgSO4, and 7 patients with elevated CPP had baseline and 30 minute measurements. Hemodynamic parameters were compared with normative curves.MAIN OUTCOME MEASURES: Mean arterial pressure (MAP), heart rate (HR), cerebral perfusion pressure (CPP), resistance index (RI), resistance area product (RAP), and cerebral flow index (CFI).RESULTS: Eight women had normal baseline CPP, and 11 had normal CFI. 11 had mild preeclampsia range blood pressure and MgSO4 had no significant effect on CPP, CFI, HR, MCA velocities, or RAP but did decrease the MAP in the first 30 minutes (107 +/- 8 to 100 +/- 9 mmHg; p = 0.035), mainly due to a drop in diastolic pressure (87 +/- 10 to 82 +/- 9 mmHg; p = 0.004). 7 patients with elevated baseline CPP had a significant reduction in CPP, but no change in CFI, after MgSO4.CONCLUSIONS: MgSO4 does not significantly effect CPP or CFI in preeclamptics with baseline blood pressure in the mild range, but does significantly reduce CPP in those with high baseline CPP. This may be important in the prevention of hypertensive encephalopathy.

Abstract:The role of magnesium sulfate for treatment of eclampsia is well established. The medication proved to be superior to other anticonvulsants to reduce the incidence of recurrent convulsions among women with eclampsia. Additionally, magnesium sulfate has been indicated for women with preeclampsia with different severe features. However, despite these recommendations, many clinicians are still not confident with the use of magnesium sulfate, even in settings with high incidence of preeclampsia and unacceptable rates of maternal mortality. This review brings basic science and clinical information to endorse recommendations to encourage clinicians to use magnesium sulfate for patients with all severe features of preeclampsia, not only for women with neurological symptoms. Additionally, other benefits of magnesium sulfate in anesthesia and fetal neuroprotection are also presented. Finally, a comprehensive algorithm presents recommendations to manage patients with preeclampsia with severe features between 34 and 36 +6 weeks.

Abstract:The curative effect of magnesium sulfate combining with low molecular weight heparin are stud-ied on early onset preeclampsia .Methods:The subjects included 23 early onset preeclampsia patients of singleton preg-nancy from August 2011 to July 2012 .With single blinded tecknique ,the patients were divided into two groups at ran-dom :magnesium sulfate group and magnesium sulfate combining with low molecular weight heparin group .The curative effects of two treatments were studied .Results:Data analyses indicated that APTT in joint group was obviously length-ened more than magnesium sulfate group (P=0 .030) .TT in joint group was shortened after treatment and there was significant difference(P= 0 .027) .Significant difference lied in the scope of the changes between two groups (P=0.039) .Significant difference was also found in the increase of concentration of serum uric acid after the treatment in joint group (P=0 .03) .SCr in joint group was increased and significantly different before and after the treatment (P=0.035) .The lengthening of expected pregnancy in average was significant difference between two groups (P< 0 .05) . Perinatals weighing implied a statistical significance between two groups (P<0 .05) .Conclusion:The study demonstra-ted magnesium sulfate combining low molecular weight heparin showed an advantageous effect on early onset pre-eclampsia ,compared with traditional magnesium sulfate only treatment .The combined treatment could inhibit the hy-per-coagulant state without adverse effect on anti-hypertension ,thus promoting renal function and microcirculation . The combined treatment could improve the placenta blood stream irrigation ,nurturing fetal growth and enlarging the gestational age and fetal weight .
Effect of Magnesium Sulfate on Epidural Labor Analgesia with Ropivacaine in Patients with Preeclamp-Sia

Abstract:Objective To investigate the effect of magnesium sulfate on epidural labor analgesia with ropivacaine in the patients with preeclampsia. Methods Seventy nulliparous parturients with pre?eclampsia, aged 23-34 yr, of American Society of Anesthesiologists physical status Ⅱ or Ⅲ, weighing 63-81 kg, with a singleton fetus in vertex presentation, without contradications to neuraxial anesthesia, waiting for vaginal delivery, received epidural analgesia for labor. The patients were divided into magnesi?um sulfate group and control group using a random number table, with 35 patients in each group. In magne?sium sulfate group, magnesium sulfate 50 mg∕kg ( 30 ml) was infused intravenously over 15 min when their cervical dilation was 3 cm, while the equal volume of normal saline was given in control group. Epi?dural labor analgesia was performed with ropivacaine. The up?and?down sequential allocation was used to determine the median effective concentration of epidural ropivacaine ( EC50 ) . The severity of pain was as?sessed with visual analogue scale score. Effective analgesia was defined as visual analogue scale score of≤1. The initial concentration of ropivacaine was 0.15%. Each time the concentration was increased∕decreased according to whether or not analgesia was effective, and the ratio between the two successive concentrations was 0.9. The EC50 and 95% confidence interval ( CI) of ropivacaine for epidural labor analgesia was calcu?lated. Results The EC50 (95% CI) of ropivacaine for epidural labor analgesia was 0.066% (0.062%-0.071%) in magnesium sulfate group. The EC50 (95% CI) of ropivacaine for epidural labor analgesia was 0.078% (0.072%-0.085%) in control group. The EC50 of ropivacaine was significantly lower in magnesi?um sulfate group than in control group ( P<0.05) . Conclusion Magnesium sulfate can enhance the effica?cy of ropivacaine for epidural labor analgesia in the patients with preeclampsia.

Abstract:Background: In this study, the effect of magnesium sulfate and labetalol in treating pregnancy-induced hypertension (PIH) and its influence on anxiety and depression in patients are observed, and new ideas for treating anxiety and depression in PIH are introduced. Methods: A retrospective cohort study was conducted to select patients with PlH diagnosed from July 2020 to July 2023 from Affiliated Hospital of Electronic Science and Technology University and Chengdu Women' s and Children's Central Hospital in Chengdu of Sichuan Province. The changes in blood pressure, Edinburgh Postnatal Depression Scale (EPDS), and generalized anxiety disorder 7 (GAD-7) in patients with hypertensive pregnancy were collected and analyzed. Results: In our investigation, 219 patients completed the study, and 36.1% (79/219) of them developed anxiety and depression. According to whether the patients were treated with magnesium sulfate and labetalol, 49 cases were assigned to the magnesium sulfate and labetalol treatment (MSLT) group, and 30 cases were assigned to the conventional treatment (CT) group. Edinburgh Postnatal Depression Scale scores and GAD-7 scores in the MSLT group were significantly lower than those in the CT group, indicating that magnesium sulfate and labetalol can improve anxiety and depression in hypertensive patients during pregnancy. The difference was statistically significant (P < .05). According to the changes in systolic blood pressure, the clinical efficacy of patients was evaluated, and no significant difference in efficacy existed between the MSLT and CT groups. Conclusion: Magnesium sulfate and labetalol can control the blood pressure of patients with PIH and indirectly improve anxiety and depression in patients with PIH, thereby introducing new ideas for the treatment of PIH accompanied by anxiety and depression.
Examining the Use of Magnesium Sulfate to Treat Pregnant Women with Preeclampsia and Eclampsia: Results of a Program Assessment of Emergency Obstetric Care (EmOC) Training in India

Abstract:Background: The aim of this study is to examine rates of magnesium sulfate utilization by emergency obstetric care trainees to treat preeclampsia-eclampsia in India. Secondarily, structural barriers are identified which limit the use of magnesium sulfate, highlighting limitations of emergency obstetric care training, which is a commonly implemented intervention in resource-poor settings. Methods: Trainees' curriculum specified magnesium sulfate treatment for eclampsia and severe preeclampsia. Case records were analyzed for preeclampsia-eclampsia diagnosis, magnesium sulfate utilization, delivery route, and maternal and neonatal outcomes from 13,238 reported deliveries between 2006 and 2012 across 75 district hospitals in 12 Indian states. Results: Of 1320 cases of preeclampsia-eclampsia, 322 (24.4%) had eclampsia. Magnesium sulfate was given to 12.9% of preeclamptic and 54.3% of eclamptic women, with lower usage rates in rural communities. Among the 1308 women with preeclampsia-eclampsia, only 24 deaths occurred (1.8%). In contrast, among the 17,179 women without preeclampsia-eclampsia, there were 95 reported deaths (0.6%). Both maternal mortality ratios were found to be much higher than the Millennium Development Goal target of 0.15%. Magnesium sulfate administration was associated with a higher death rate in preeclamptic but not eclamptic women, representing possible confounding by severity. Conclusion: To optimize resources spent on emergency obstetric care training, the consistent availability of magnesium sulfate should be improved in India. Increasing drug availability, implementing clinical guidelines around its administration, and training health-care providers on the identification and treatment of preeclampsia-eclampsia could lead to notable improvements in maternal and infant mortality.

Abstract:BACKGROUND Preeclampsia increases maternal and perinatal mortality and is affected by calcium and magnesium levels. Reduced extracellular levels of calcium and magnesium constitute the pathogenesis of eclampsia. A reduction in the calcium-magnesium ratio may aid in the detection and prevention of preeclampsia. MATERIAL AND METHODS This was an analytical observational study with a cross-sectional design, including patients with and without preeclampsia (inpatient and outpatient). A total of 246 patients were included in this research; 138 patients had preeclampsia and 108 patients did not. All examinations of magnesium and calcium levels at the Hasan Sadikin Hospital Clinical Pathology laboratory were conducted using an ion selective electrode modified with methylthymol blue complexometric titration. RESULTS Patients with preeclampsia had significantly higher average serum magnesium and calcium levels than did patients without preeclampsia (2.85 vs 2.09, P=0.0001; 4.45 vs 4.85, P=0.025, respectively). Patients with preeclampsia demonstrated significantly lower serum calcium-magnesium ratios than did patients without preeclampsia (1.98 vs 2.60, P=0.0001). Receiver operating characteristic curve analysis on the serum calcium-magnesium ratio showed an area under the curve of 68.0% (P=0.0001), with a cutoff value of 2.36 (sensitivity 64.8%, specificity 62.3%), indicating that patients with serum calcium-magnesium ratios of <2.36 were predicted to have a risk of preeclampsia. CONCLUSIONS Patients with preeclampsia had significantly lower serum calcium-magnesium ratios than did patients without preeclampsia; therefore, a low calcium-magnesium ratio could be a risk factor for preeclampsia.

Abstract:Abstract Background Preeclampsia is pregnancy-specific syndrome of reduced organ perfusion secondary to vasospasm and endothelial activation. Aim of the Work to assess the effect of injecting magnesium sulfate on Doppler parameters of uterine artery, umbilical and fetal middle cerebral arteries (MCA) in women with severe preeclampsia. Patients and Methods This study included eighty pregnant with severe preeclampsia or eclampsia who were given magnesium sulphate therapy in Ain Shams University Maternity hospital. The Study group included 80 pregnant patients Doppler flow velocity parameters (resistance index [RI], pulsatility index [PI] and systolic/diastolic [S/D] ratio) were evaluated in the uterine, fetal umbilical and middle cerebral arteries before and 20 minutes after intravenous administration of 6 grams of magnesium sulfate (Loading dose). Results Magnesium sulfate had significant hemodynamic effects with significant reduction of systolic and diastolic blood pressure. Regarding the Doppler studies, it was found that after administration of magnesium sulfate there was significant reduction in the Doppler velocimetry parameters (Resistance index[RI], Pulsatility index[PI] and systolic/diastolic[S/D] ratio) in the uterine, umbilical and fetal middle cerebral arteries denoting decreased vascular resistance with increased blood flow in these vessels. Conclusion Intravenous administration of magnesium sulfate in pregnant women with severe preeclampsia and eclampsia resulted in a decrease in uterine artery, fetal umbilical and middle cerebral arteries Doppler indices with reduced resistance to blood flow in these vessels.
