Abstract:Although China has eliminated absolute poverty, the effects of sickness still pose a threat to the prospect of returning to poverty in western rural areas. However, poverty governance extends beyond solving absolute poverty, and should enhance the family's ability to resist risks, proactively identify the existence of risks, and facilitate preventive measures to reduce the probability of falling into poverty again. This study aimed to assess the health poverty vulnerability of rural households in western China and decompose its determinants.

What problem does this paper attempt to address?
### Problems the Paper Attempts to Solve This paper aims to assess the vulnerability of health poverty among households in rural areas of Western China and analyze its determinants. Although China has eradicated absolute poverty, diseases still pose a threat to the return of poverty in rural Western regions. Therefore, the research focuses on: 1. **The current status of health poverty vulnerability in rural Western China**: Identifying households with high health poverty vulnerability to reduce their risk of falling into poverty and consolidate poverty alleviation achievements. 2. **Reconstructing the Andersen Behavioral Model**: By extending the Andersen Behavioral Model, exploring various factors affecting health poverty vulnerability in rural Western regions, and using the Shapley decomposition method to identify the main contributing factors. ### Background - **Health Poverty**: Health poverty refers not only to economic poverty but also to the lack of physical health, mental health, and the ability to afford healthcare. - **Dynamic Risk**: Health poverty vulnerability is a dynamic concept that connects the well-being level of households with potential future health risks. - **Policy Needs**: Health poverty alleviation should shift from treatment to prevention, formulating forward-looking, precise, and efficient intervention policies to prevent large-scale return to poverty due to illness. ### Methods - **Data Source**: The study used annual tracking data from the "Innovative Payment System to Improve Health Benefits Project" in 2022, a collaboration between the Ningxia Hui Autonomous Region Health Commission and Harvard University. - **Sample Selection**: Through multi-stage stratified random sampling, 5,455 households and 20,347 individuals were selected for the study. - **Model Construction**: The three-stage feasible generalized least squares (FGLS) method by Chaudhuri was used to measure health poverty vulnerability, and Tobit regression and censored least absolute deviations (CLAD) methods were used for robustness testing. - **Statistical Methods**: Epidata 3.1 was used to input survey data, and Stata 16.0 was used for data processing and statistical analysis. ### Results - **Health Poverty Vulnerability Index**: The average health poverty vulnerability index of the sample households was 0.3000 ± 0.2223, with 16.9% of households having a vulnerability index ≥0.5, indicating a high risk of health poverty. - **Influencing Factors**: Through Shapley decomposition, it was found that predisposing factors (such as age, ethnicity, household size, etc.), enabling factors (such as education level, per capita income, etc.), and need factors (such as types of chronic diseases, days of hospitalization, etc.) are the main factors affecting health poverty vulnerability. ### Conclusion - **Health Poverty Risk in Rural Areas**: Rural areas still face the risk of falling into or returning to poverty due to residents' health issues. - **Policy Recommendations**: Health poverty alleviation should gradually shift from treatment to prevention, formulating forward-looking, precise, and efficient intervention policies to consolidate health poverty alleviation achievements and prevent large-scale return to poverty due to illness. ### Significance of the Study - **Theoretical Contribution**: Enriches the application of the Andersen Behavioral Model in the field of health poverty, providing empirical research support. - **Practical Guidance**: Provides a theoretical basis and data support for health poverty alleviation work in other regions and countries.
