Abstract:As the country with the largest number of older adults with dementia, China is faced with unprecedented challenges in dementia care. In consideration of the complexity, professionality, and high dependency of caring for the elderly with dementia, the burden outweighs the capacity of informal caregivers which leads to the corresponding increment of demand in caring in institutions. While formal caregivers caring for older adults with dementia are exposed to mounting caregiving burdens, the characteristics of resilience composed of “risk factors” and “protective factors” influence the origin of the burden and their coping strategies. The characteristics of caregivers and care recipients and their interactive relationship form the internal burden, while the trust between caregivers and relatives of the elderly as well as the management within institutions are two major elements resulting in the outer burden. Personal characteristics, spiritual beliefs, behavioral strategies, and received family support of formal caregivers together shape their burden adaptation and coping strategies. In the construction of caring for the older adults with dementia in institutions, it is an urgent need to reflect on the “feminization of care”, reconstruct the significance of trust in resilience strategies, and build up a comprehensive social support system for formal caregivers at the same time. Relieving the burden of formal caregivers and responding actively is an important guarantee for stabilizing the team of caregivers and improving the quality of care for the elderly with dementia.
Burden of Caregivers Who Care for Oldest-Old Parents with Disability: A Cross-Sectional Study.

Abstract:Objective: To describe the characteristics of oldest-old Chinese with disability and their adult-child caregivers, and the extent to which these characteristics were associated with caregiver burden. Methods: The study was based on 168 pairs of disabled oldest-old adults and their adult-child caregivers, derived from the Chinese Longitudinal Healthy Longevity Survey. Descriptive analyses of care recipients' and caregivers' characteristics were conducted respectively, in reference to caregiver burden. Statistically significant characteristics identified in these bivariate analyses were then jointly evaluated in multiple linear regression models with caregiver burden as the outcome. Results: Care recipients positive emotion status [(beta =-0.227 (-0.412,-0.042)], multiple chronic disease [(beta = 0.513 (0.081, 0.945)], and caregivers spent more caregiving time [(beta = 0.225 (0.061, 0.389)] were main factors associated with caregiver burden. Conclusion: Adult-children caregivers perceived heavier burden if care recipients had low positive emotions, had multiple chronic diseases, and caregivers spent more time caregiving. Abstract:Background Dementia is a chronic progressive disease seriously affecting the patient's daily life and working skills and may cause the patient disability and dependence. Thus, caring for dementia patients inevitably falls on families in the mainland of China. Unfortunately, there are rarely enough reports available about mental health and social support in family caregivers for domestic dementia patients. This study aimed to investigate the changes in psychological status and social support in domestic dementia family caregivers and hope the government and relevant departments pay more attention to the family caregiver's psychological change, which may be better for dementia patients. Methods Fifty-eight immediate family caregivers (family group) for dementia patients, including 21 Alzheimer's disease (AD) and 37 vascular dementia inpatients, were recruited for interview. Fifty-eight age- and sex-matched normal volunteers (control group) were also interviewed in the same period. The psychiatric distress was assessed by the Symptom Checklist 90 (SCL-90), family functioning was assessed by the Family Assessment Device (FAD), and the social support was assessed by the Multidimensional Scale of Perceived Social Support (MSPSS). Group differences were analyzed using unpaired t test for comparison of SCL-90, FAD, and MSPSS mean scores. Pearson's correlation coefficient was used to find the association between the various dimensions of FAD and the social support from dementia family caregivers. Results Except phobic anxiety dimension, the other dimensions of SCL-90 mean scores were significantly higher in the family group than those in the control group (P <0.05). In the family group, except affective involvement dimension, the other dimensions of FAD mean scores were within the scope of unhealthy family functioning. Significant differences in problem solving, communication, roles, affective responsiveness, behavioral control, general functioning, and the total MSPSS mean scores were seen between the family group and the control group (P <0.01). Except affective involvement dimension, the other dimensions of the family functioning correlated with the perceived social support in the family group (P <0.05 or 0.01). Conclusions Dementia patients have a negative effect on families' mental health in the mainland of China. Dementia family caregivers experience defected family functioning and low levels of social support.
Family burden and psychological distress among Chinese caregivers of elderly people with dementia: a moderated mediation model

Abstract:Background: Although the Stress-Coping Model (SCM) has been widely used to explain the coping process of individuals facing stressful situations, its applicability to caregivers of elderly people with dementia (PwD) in China needs to be further investigated. Furthermore, the role of external resources in caregivers stress coping is not yet clear. Therefore, our study aimed to investigated the mediating and moderating mechanisms between family burden and psychological distress in PwD caregivers based on the SCM. Methods: A cross-sectional study, with 193 pairs of PwD and caregivers completed the self-designed questionnaire, Family Burden Scale of Disease, Kessler Psychological Distress Scale, General Self-Efficacy Scale, Simplified Coping Style Question, The Family Adaptation and Cohesion Evaluation Scales II-CV and Social Support Rating Scale. Partial least squares-structural equation modeling (PLS-SEM) analyzed the mediating and moderating effects. Results: Family burden positively correlated with psychological distress; the chain mediation effects of self-efficacy and positive coping between family burden and psychological distress was significant; the interaction term (family function_positive coping) did not but (social support_positive coping) had a significant positive impact on psychological distress. Conclusions: The findings provides a practical basis for the use of SCM in informal caregivers of elderly PwD, assists understanding the mechanism of the relationship between family burden and psychological distress. And it supplies new perspectives for reducing the negative psychological health status and a theoretical basis for designing interventions for caregivers.
Caregiver Distress and Associated Factors in Dementia Care in the Community Setting in China.

Abstract:The aim of this study was to investigate caregiver distress in reacting to the care recipient's behavioral and psychological symptoms of dementia (BPSD) and factors contributing to caregiver distress in the community setting in China. One hundred and fifty-two family caregivers of people with dementia in community settings were assessed using the Chinese version of the Neuropsychiatric Inventory-Questionnaire and the Social Support Rating Scale. The prevalence of BPSD and caregiver distress in reacting to BPSD was higher in China than those reported in high income countries. The most common individual BPSD were apathy/indifference, depression/dysphoria and night-time behaviors. Delusions, hallucinations and apathy/indifference were rated as the most distressing to caregivers. BPSD contributed most to caregiver distress. The high level of caregiver distress identified in this study suggests that dementia services and caregiver support should be established in the public healthcare system to target the needs of people with dementia and their caregivers.

Abstract:Objectives: Numerous studies have demonstrated that informal caregivers of people living with dementia (PLWD) have serious adverse psychological problems, but little research has been done on the role of integrating caregivers' various coping resources in reducing psychological effects. Therefore, the current study aimed to investigate the impact of caregivers' internal (self-efficacy and positive coping) and external coping resources (family function and social support) on the family burden and psychological distress. Methods: A cross-sectional study, and a self-designed questionnaire, Family Burden Scale of Disease, Kessler Psychological Distress Scale, General Self-Efficacy Scale, Simplified Coping Style Question, The Family Adaptation, Partnership, Growth, Affection, Resolve Scale and Social Support Rating Scale were completed by 193 pairs of PLWD and caregivers. Partial least squares-structural equation modeling (PLS-SEM) was used to analyze the mediating effects of coping resources. Results: Self-efficacy, positive coping, family function, and social support respectively mediated the relationship between family burden and psychological distress (β = 0.402, β = 0.059, β = -0.111, β = 0.053; P < 0.05). Besides, the serial mediation effects of self-efficacy and positive coping, family function and social support were also significant (β = 0.104, β = 0.045; P < 0.05). Conclusions: This study confirmed the mediating role of internal and external coping resources between family burden and psychological distress in caregivers of older PLWD, providing a theoretical basis for promoting the,mental health of caregivers.

Abstract:This study aimed to explore the relationships between social capital, family functioning, and caregiver burden in caregivers of older adults with post-stroke dementia (PSD). A cross-sectional correlational study was conducted between November 2017 and April 2018 among 105 caregivers of older adults with PSD using a demographic questionnaire, the Social Capital Scale, Family APGAR Index (APGAR), and Caregiver Burden Inventory (CBI).An independent samples t-test was also utilized to compare caregiver burden based on demographic characteristics. Statistical analysis comprised Pearson correlation analysis and multiple regression analysis using SPSS version 22.0 to identify influencing factors. Significant negative correlations were found between caregiver burden and variables such as social capital ( p < .001) and family functioning ( p < .01). The results of the multivariate analysis revealed that social capital, physical condition, and duration of caregiving were the main influencing factors (β = 58.162, ΔR2 = 0.289, p < .001). These findings suggest that nurses should continue to prioritize the well-being of patients' families, and collaborative efforts should be made to establish care facilities that cater to both medical and social needs.

'It's a lonely journey': caregiving experiences and psychosocial distress among Chinese American dementia family caregivers

Abstract:Objectives: Chinese American family caregivers of persons with Alzheimer's disease and related dementia (ADRD) are a vulnerable but understudied population. The goal of this qualitative study was to examine their caregiving experiences and psychosocial distress process and explore intervention strategies. Methods: In-depth individual interviews were conducted with 18 Chinese American dementia caregivers. All interviews were transcribed verbatim; thematic content analysis was conducted to construct a conceptual framework. Results: All participants reported high levels of caregiving stress associated with care-recipients' advanced symptoms and required assistance in activities in daily living. The relationship of caregiver and care-recipient was strained in their roles transition. The complex healthcare system, insurance policies, and a lack of linguistically appropriate services aggravated their psychosocial distress. Chinese cultural norms on 'family harmony' hindered their seeking of social support. Prolonged caregiving stress led to physical and mental impairment, including poor sleep, depression, and chronic conditions. Participants described their caregiving experience as 'a lonely journey' with a pervasive sense of hopelessness and withdrawal; their distress process was positively or negatively influenced by their coping strategies. All participants were eager for any kind of support; especially culturally appropriate programs that could improve their caregiving skills, self-care, and access to services. Conclusion: Our data suggest that Chinese American dementia caregivers, especially those with limited English proficiency, experience elevated psychosocial distress, which was aggravated by the barriers to social support and health services due to their immigrant and minority status. Culturally appropriate targeted intervention is urgently needed for this underserved and vulnerable population.

Abstract:Western Journal of Nursing Research, Ahead of Print. Background:Various factors drive a care setting shift from institutional to home settings for dementia care until the latest stage of the disease, suggesting the critical role of family caregivers living with persons with dementia.Objectives:This study explored the characteristics and correlates of self-reported overall physical and psychological (ie, depressive symptoms and stress) health among family caregivers living with persons with dementia in Sweden.Methods:This cross-sectional, descriptive study used baseline data from an existing music-based intervention study of persons with dementia and their family caregivers (N = 76). Multivariable linear regression analyses were performed.Results:On average, caregivers (n = 38; mean age: 74.8 years) were slightly younger than persons with dementia (n = 38; mean age: 78.6 years). Most caregivers were female (n = 24; 63.2%) and spouses or partners of persons with dementia (n = 37; 97.4%). Caregivers' perceived relationship with their family members with dementia was the only factor associated with caregivers' self-reported overall physical health (b = −0.655, p = .046). This suggests caregivers' more frequent feeling of a good relationship with the persons with dementia was linked to better self-rated physical health among family caregivers living with persons with dementia.Conclusions:This study highlights the importance of family caregivers' perceived relationship with persons with dementia in the context of caregivers' self-reported physical health. Future research is needed to explore the perceived relationship from the perspectives of persons with dementia and the determinants of caregiving dyads' (persons with dementia and family caregivers) perceived relationship with each other.

Abstract:Objectives: This study aimed to describe caregiving stress among family caregivers of Chinese older adults living with disabilities, and explore how care intensity, financial expenses, and care difficulties are associated with caregiving stress. Methods: Data of 220 older adult-caregiver dyads were collected from 6 urban districts and 6 rural counties from Shandong province, China. Descriptive analyses and multivariate ordinal logistic regression analyses were performed. Results: Family caregivers providing nine or more hours of care per day reported higher caregiving stress than those who provided fewer than nine hours. Caregivers who experienced insufficient care abilities, economic hardships, or time conflicts were more likely to report caregiving stress. Financial support provided to older adults was not associated with caregiving stress. Conclusions: Family caregivers of Chinese older adults with disabilities are experiencing excessive caregiving stress. Social support groups and China's long-term care insurance system should be promoted to better assist family caregivers. (C) 2022 Elsevier Inc. All rights reserved.

Abstract:Abstract Family caregivers are critical sources of support to older adults living with disabilities, but they also need to cope with caregiving stress and difficulties brought by the caregiving process. This study aimed to explore the associations between family transfers (i.e., time transfer and financial transfer), perceived difficulties of caregiving, and caregiving stress among family caregivers of older adults living with disabilities. Respondents were randomly selected from 6 urban districts and 6 rural towns in Shandong province, China. Data from 220 older adults living with disabilities and their family caregivers were analyzed. Caregiving stress were divided into three categories (mild stress, moderate stress and high stress). Difficulties were divided into four categories (insufficient care abilities, economic hardship, time conflicts, and inadequate nursing equipment). Family caregivers reported moderate to high levels of caregiving stress. Results from multivariate ordinal logistic regression shows that family caregivers providing nine or more hours of care per day reported higher levels of caregiving stress than those who provided 8 or less hours of care per day (9 to 12 hours: OR=2.92, p<.05; 13 to 16 hours: OR=2.66, p<.05). Caregivers who perceived insufficient care abilities, economic hardship, and time conflicts were more likely to report caregiving stress. Respites in the forms of caregiver support group and day care attendance for older adults living with disabilities may reduce caregiver stress. Educational program and professional knowledge training courses may be helpful in reducing caregiving stress. The long-term care insurance system should be promoted to provide social support for family caregivers.

Abstract:Objectives: Caring for a family member with dementia may have important negative psychological consequences on caregivers. The present study aims to analyze the existence of different profiles in family caregivers of people with dementia according to the levels manifested in different psychosocial variables, which are grouped into psychosocial variables (dysfunctional thoughts, familism, experiential avoidance) and resources (leisure and social support). In addition, it aims to study whether there are differences among those profiles in the levels of distress (depressive, anxious and guilt symptomatology) depending on the relationship of kinship with the cared-for person. Methods: 288 family caregivers of people with dementia divided into four kinship groups (wives, husbands, sons and daughters) participated. Face-to-face interviews were conducted assessing sociodemographic variables, familism (family obligations), dysfunctional thoughts, experiential avoidance, leisure activities, perceived social support and depressive, anxious and guilt symptomatology. A hierarchical cluster analysis was performed using Ward's method and contingency tables were run between the clusters obtained and the variable of kinship and distress variables. Results: Five clusters were obtained: Low psychosocial vulnerability-High resources (mostly daughters), Low psychosocial vulnerability-Low resources (mostly daughters), Mixed (mostly sons), High psychosocial vulnerability-High resources (mostly husbands) and High psychosocial vulnerability-Low resources (mostly wives). Although with nuances, the clusters associated with lower distress are the Low psychosocial vulnerability-High resources profile and the High psychosocial vulnerability-High resources profile, and with higher distress the Low psychosocial vulnerability-Low resources profile and the Mixed profile. Conclusions: High levels of dysfunctional thoughts, familism and experiential avoidance are not always associated with greater psychological distress. In fact, profile 2 (Low psychosocial vulnerability-Low resources), in which most caregiving daughters are distributed, seems to be particularly vulnerable to presenting higher levels of emotional. Therefore, the identification of profiles of potential protection and vulnerability to psychological distress in family caregivers could help to increase the effectiveness of interventions aimed at this population.

Abstract:The study identified factors associated with depressive symptoms in family caregivers of persons with dementia with Lewy bodies, Alzheimer’s disease, or Parkinson’s disease dementia. An Internet-based survey generated 606 response sets. Regression analysis indicated that care recipients’ memory and behavioral problems, caregivers’ self-efficacy, grief, and tangible, emotional, and affectionate support were significantly associated with caregivers’ depressive symptoms. Self-efficacy mediated caregiver burden to improve psychological well-being. Social support mediated the association between caregiver grief and psychological well-being. Social workers can provide psychoeducational interventions to help caregivers to manage emotional distress and improve capacity to manage tasks related to the dementia.

Abstract:PurposeTo examine a predictive theoretical model of psychological distress based on the following variables reflected on family caregivers of patients with cancer: the unmet supportive care needs, subjective caregiving burden, social support, and the positive aspects of caregiving.MethodsA cross-sectional descriptive study was conducted on a sample of 484 dyads of patients and their family caregivers. The caregivers completed structured questionnaires designed to measure psychological distress, unmet supportive care needs, subjective caregiving burden, positive aspects of caregiving, and social support. Patients’ demographic variables and medical data were collected from a medical record review. We used a structural equation modeling to test the predictive theoretical model.ResultsPath analysis results partially supported the proposed model with satisfactory fit indices. Specifically, family caregivers with an increasing number of unmet needs or a heavier caregiving burden were more likely to have more severe psychological distress. Bootstrapping results supported that the caregiving burden and social support were significant mediators. Greater unmet supportive care needs predicted higher psychological distress through increasing caregiving burden. Stronger social support predicted lower psychological distress through decreasing caregiving burden. Positive aspects of caregiving predicted lower caregiving burden through the increasing perceived social support, which in turn eliminated psychological distress.ConclusionsUnmet supportive care needs could cause psychological distress through increasing caregiving burden. The positive aspects of caregiving reduced caregiving burden through increasing social support, which subsequently alleviated psychological distress. Interventions that aim to satisfy supportive care needs, to reduce caregiving burden, and to strengthen social support ties may boost the mental health of family caregivers.

Abstract:Informal caregivers are crucial to the care of persons with dementia (PWD), but their role is often reported as challenging. This study aims to examine caregiver stress and depressive symptoms among informal caregivers of PWD using a modified transactional stress and coping model. Path analysis was conducted among a sample of informal caregivers of PWD in Singapore ( n = 281) using information on functional dependence and memory and behavioral problems of PWD, and self-reported measurements on caregivers' knowledge of dementia, perceived positive aspects of caregiving, social support, coping patterns, caregiving self-efficacy, caregiving burden and depressive symptoms. The initial model reflecting our assumptions for the transactional stress and coping model showed a poor fit (model 1 - CFI = 0.858, TLI = 0.665, RMSEA = 0.118). Logical modifications were made until sufficient model fit was achieved (model 2 - CFI = 0.987, TLI = 0.955, RMSEA = 0.043). We then removed the insignificant paths in model 2 and obtained our final model (model 3 - CFI = 0.990, TLI = 0.974, RMSEA = 0.033). The final model supported our hypotheses, with some adjustments. This study advances our understanding of caregiver distress by modifying the transactional stress and coping model, including (1) the key role of caregiver self-efficacy in the primary appraisal process, (2) the dynamic assessment of coping resources across all stages of the model, and (3) the importance of coping patterns. Future studies could explore the generalizability of these findings.

In addition, it aims to study whether there is a greater representation of each kinship group in each of the profiles and if there are differences in emotional distress among such profiles considering the kinship with the care-recipient. Participants were 288 family dementia caregivers, divided into four kinship groups (wives, husbands, sons and daughters). Psychosocial (familism, dysfunctional thoughts and experiential avoidance), resource (leisure activities and social support) and outcomes (depressive, anxious and guilt symptomatology) variables were collected. A hierarchical cluster analysis using Ward's method, an exploratory factor analysis of two fixed factors and contingency tables were performed. Five clusters were obtained: Low psychosocial vulnerability-High resources, Low psychosocial vulnerability-Low resources, Mixed, High psychosocial vulnerability-High resources, and High psychosocial vulnerability-Low resources. Results suggested that clusters associated with lower distress were the Low psychosocial vulnerability-High resources and the High psychosocial vulnerability-High resources. Clusters associated with higher distress were the Low psychosocial vulnerability-Low resources and Mixed. High levels of dysfunctional thoughts, familism and experiential avoidance do not always have a maladaptive function. This could depend on sociocultural and resource variables such as the kinship with the caregiver or perceived social support. The identification of profiles of family caregivers potentially needing protection and vulnerable to psychological distress could help to increase the effectiveness of interventions aimed at this population.

Social Capital as a Moderator for Caregivers’ Psychological Distress: A Dynamic Panel Data Model Analysis in Japan

Abstract:Background: The adverse impact of caregiving on caregivers’ mental health and the positive impact of social capital (SC) on health are both well understood. This study examined the moderating effect of SC on the association between family caregiving and caregivers’ psychological distress (PD).Methods: We used longitudinal data from 27,869 individuals born between 1946 and 1955. The data were collected from a 14-wave nationwide longitudinal survey conducted from 2005 to 2018. We estimated dynamic panel data (DPD) models, which could control for an individual’s time-invariant attributes in a dynamic framework. We did this to examine how SC moderated the association between informal caregiving and a caregiver’s PD (defined by a Kessler score of 13 or higher). We also examined how the results varied over time, as the caregiver’s age advanced.Results: Of the respondents aged 50–73 years, 12.5% of women and 8.4% of men provided care to their older parents or spouses. The DPD model results showed that the onset of caregiving increased the probability of PD (M 3.4%) by 2.1% (95% confidence interval [CI]: 1.6%–2.7%) and 1.1% (95% CI: 0.5%–1.6%) for women and men, respectively. SC moderated the association between caregiving and a caregiver’s PD by 53.4% (95%: 30.4%–76.4%) and 84.9% (95% CI: 62.0%–107.8%) for women and men, respectively. We also observed that the moderating effect of SC on a caregiver’s PD increased as the caregiver’s age advanced in both women and men, preventing a deterioration in the psychological impact of caregiving. Conclusions: The results underscore the moderating effect of SC on the association between caregiving and PD. This suggests the need to keep family caregivers from being socially isolated, especially as they get older.

Study on burden of family caregivers of patients with dementia in community and influencing factors

Abstract:Objective:To investigate caregiver burden of caregivers of dementia patients and explore its influen-cing factors.Methods:Multi plot cross sectional questionnaire survey was used to collect data.General appli-cation data table,caregivers burden questionnaire (CBI),Chinese version of neural psychiatric questionnaire and social support Edition (NPI Q)and society support scale (SSRS)were used for evaluation on community pri-mary family caregivers of 152 cases of dementia patients,then to analyze the influence factors of the family care-givers burden.Results:152 cases of primary family caregivers spent an average of 127.60 h per week in dementia patients.More than half (55.3%)of the caregivers were under pressure to take care of patients with dementia. Subjective care burden score was 38.89±12.94.Multiple stepwise regression analysis showed that the influen-cing factors of caregivers’subjective burden included dementia severity behavioral and psychological symptoms, patients with dementia whether or not to live with the patient and family to share number of caregiving tasks, seeking emotional support and caregivers’health status(P <0.05).Conclusion:The burden of family caregivers of patients with dementia in China was significantly higher than that in other countries or regions.The severity of psychiatric symptoms in patients with dementia,whether or not to live with the patient,to share the number of tasks in the home,to seek emotional support behavior and the health status of caregivers,were the main sub-jective burdens of caregivers.
Resilience Moderates the Association Between Stigma and Psychological Distress among Family Caregivers of Patients with Schizophrenia

Abstract:The aims of this study were to describe the prevalence of psychological distress, to further validate the influence of caregivers' stigma and resilience on their psychological distress, and to explore the moderating effect of resilience in the relationship between stigma and psychological distress in caregivers. Participants were 125 family caregivers of individuals with schizophrenia from one provincial Mental Health Center in China. The Kessler Psychological Distress Scale (K10), Perceived Devaluation and Discrimination Scale (PDD) and the Ten-item Connor-Davidson Resilience Scale (CD-RISC10) were used to measure their psychological distress, stigma, and resilience, respectively. A total of 33.6% of caregivers had high levels of psychological distress. Lower levels of resilience and higher levels of stigma among caregivers contributed to greater psychological distress. Furthermore, the moderating effect of resilience in the relationship between stigma and psychological distress in family caregivers of patients with schizophrenia was firstly verified. This study also highlighted that intervention to promote resilience, especially in caregivers who had higher levels of stigma, might be helpful to alleviate their psychological distress.
