Abstract:PurposeTo develop and validate a machine learning-based CT radiomics method for preoperatively predicting the stages (stage I and non-stage I) of Wilms tumor (WT) in pediatric patients. MethodsA total of 118 patients with WT, who underwent contrast-enhanced computed tomography (CT) scans in our center between 2014 and 2021, were studied retrospectively and divided into two groups: stage I and non-stage I disease. Patients were randomly divided into training cohorts (n = 94) and test cohorts (n = 24). A total of 1,781 radiomic features from seven feature classes were extracted from preoperative portal venous-phase images of abdominal CT. Synthetic Minority Over-Sampling Technique (SMOTE) was used to handle imbalanced datasets, followed by a t-test and Least Absolute Shrinkage and Selection Operator (LASSO) regularization for feature selection. Support Vector Machine (SVM) was deployed using the selected informative features to develop the predicting model. The performance of the model was evaluated according to its accuracy, sensitivity, and specificity. The receiver operating characteristic curve (ROC) and the area under the ROC curve (AUC) was also arranged to assess the model performance. ResultsThe SVM model was fitted with 15 radiomic features obtained by t-test and LASSO concerning WT staging in the training dataset and demonstrated favorable performance in the testing dataset. Cross-validated AUC on the training dataset was 0.79 with a 95 percent confidence interval (CI) of 0.773-0.815 and a coefficient of variation of 3.76%, while AUC on the test dataset was 0.81, and accuracy, sensitivity, and specificity were 0.79, 0.87, and 0.69, respectively. ConclusionsThe machine learning model of SVM based on radiomic features extracted from CT images accurately predicted WT stage I and non-stage I disease in pediatric patients preoperatively, which provided a rapid and non-invasive way for investigation of WT stages.

Abstract:Up to 20% of renal masses ≤4 cm is found to be benign at the time of surgical excision, raising concern for overtreatment. However, the risk of malignancy is currently unable to be accurately predicted prior to surgery using imaging alone. The objective of this study is to propose a machine learning (ML) framework for pre-operative renal tumor classification using readily available clinical and CT imaging data. We tested both traditional ML methods (i.e., XGBoost, random forest (RF)) and deep learning (DL) methods (i.e., multilayer perceptron (MLP), 3D convolutional neural network (3DCNN)) to build the classification model. We discovered that the combination of clinical and radiomics features produced the best results (i.e., AUC [95% CI] of 0.719 [0.712–0.726], a precision [95% CI] of 0.976 [0.975–0.978], a recall [95% CI] of 0.683 [0.675–0.691], and a specificity [95% CI] of 0.827 [0.817–0.837]). Our analysis revealed that employing ML models with CT scans and clinical data holds promise for classifying the risk of renal malignancy. Future work should focus on externally validating the proposed model and features to better support clinical decision-making in renal cancer diagnosis.

Abstract:Purpose: By using a radiomics-based approach, multiple radiomics features can be extracted from regions of interest in computed tomography (CT) images, which may be applied to automatically classify kidney tumors and normal kidney tissues. The study proposes a method based on CT radiomics and aims to use extracted radiomics features to automatically classify of kidney tumors and normal kidney tissues and to establish an automatic classification model. Methods: CT data were retrieved from the 2019 Kidney and Kidney Tumor Segmentation Challenge (KiTS19) in The Cancer Imaging Archive (TCIA) open access database. Arterial phase-enhanced CT images from 210 cases were used to establish an automatic classification model. These CT images of patients were randomly divided into training (168 cases) and test (42 cases) sets. Furthermore, the radiomics features of gross tumor volume (GTV) and normal kidney tissues in the training set were extracted and screened, and a binary logistic regression model was established. For the test set, the radiomic features and cutoff value of P were consistent with the training set. Results: Three radiomics features were selected to establish the binary logistic regression model. The accuracy (ACC), sensitivity (SENS), specificity (SPEC), area under the curve (AUC), and Youden index of the training and test sets based on the CT radiomics classification model were all higher than 0.85. Conclusion: The automatic classification model of kidney tumors and normal kidney tissues based on CT radiomics exhibited good classification ability. Kidney tumors could be distinguished from normal kidney tissues. This study may complement automated tumor delineation techniques and warrants further research.

Abstract:Background: With the increasing incidence of renal lesions, pretreatment differentiation between benign and malignant lesions is crucial for optimized management. This study aimed to develop a machine learning model utilizing radiomic features extracted from various regions of interest (ROIs), intratumoral ecological diversity features, and clinical factors to classify renal lesions. Methods: CT images (arterial phase) of 1,795 renal lesions with confirmed pathology from three hospital sites were split into development (1184, 66%) and test (611, 34%) cohorts by surgery date. Conventional radiomic features were extracted from eight ROIs of arterial phase images. Intratumoral ecological diversity features were derived from intratumoral subregions. The combined model incorporating these features with clinical factors was developed, and its performance was compared with radiologists' interpretation. Results: Combining intratumoral and peritumoral radiomic features, along with ecological diversity features yielded the highest AUC of 0.929 among all combinations of features extracted from CT scans. After incorporating clinical factors into the features extracted from CT images, our combined model outperformed the interpretation of radiologists in the whole (AUC = 0.946 vs 0.823, P < 0.001) and small renal lesion (AUC = 0.935 vs 0.745, P < 0.001) test cohorts. Furthermore, the combined model exhibited favorable concordance and provided the highest net benefit across threshold probabilities exceeding 60%. In the whole and small renal lesion test cohorts, the AUCs for subgroups with predicted risk below or above 95% sensitivity and specificity cutoffs were 0.974 and 0.978, respectively. Conclusions: The combined model, incorporating intratumoral and peritumoral radiomic features, ecological diversity features, and clinical factors showed good performance for distinguishing benign from malignant renal lesions, surpassing radiologists' diagnoses in both whole and small renal lesions. It has the potential to save patients from unnecessary invasive biopsies/surgeries and to enhance clinical decision-making.

Abstract:Objectives This study was conducted in order to design and develop a framework utilizing deep learning (DL) to differentiate papillary renal cell carcinoma (PRCC) from chromophobe renal cell carcinoma (ChRCC) using convolutional neural networks (CNNs) on a small set of computed tomography (CT) images and provide a feasible method that can be applied to light devices. Methods Training and validation datasets were established based on radiological, clinical, and pathological data exported from the radiology, urology, and pathology departments. As the gold standard, reports were reviewed to determine the pathological subtype. Six CNN-based models were trained and validated to differentiate the two subtypes. A special test dataset generated with six new cases and four cases from The Cancer Imaging Archive (TCIA) was applied to validate the efficiency of the best model and of the manual processing by abdominal radiologists. Objective evaluation indexes [accuracy, sensitivity, specificity, receiver operating characteristic (ROC) curve, and area under the curve (AUC)] were calculated to assess model performance. Results The CT image sequences of 70 patients were segmented and validated by two experienced abdominal radiologists. The best model achieved 96.8640% accuracy (99.3794% sensitivity and 94.0271% specificity) in the validation set and 100% (case accuracy) and 93.3333% (image accuracy) in the test set. The manual classification achieved 85% accuracy (100% sensitivity and 70% specificity) in the test set. Conclusions This framework demonstrates that DL models could help reliably predict the subtypes of PRCC and ChRCC.

Abstract:Classification of ultrasound (US) kidney images for diagnosis of congenital abnormalities of the kidney and urinary tract (CAKUT) in children is a challenging task. It is desirable to improve existing pattern classification models that are built upon conventional image features. In this study, we propose a transfer learning-based method to extract imaging features from US kidney images in order to improve the CAKUT diagnosis in children. Particularly, a pre-trained deep learning model (imagenet-caffe-alex) is adopted for transfer learning-based feature extraction from 3-channel feature maps computed from US images, including original images, gradient features, and distanced transform features. Support vector machine classifiers are then built upon different sets of features, including the transfer learning features, conventional imaging features, and their combination. Experimental results have demonstrated that the combination of transfer learning features and conventional imaging features yielded the best classification performance for distinguishing CAKUT patients from normal controls based on their US kidney images.

Abstract:Kidney tumor (KT) is one of the diseases that have affected our society and is the seventh most common tumor in both men and women worldwide. The early detection of KT has significant benefits in reducing death rates, producing preventive measures that reduce effects, and overcoming the tumor. Compared to the tedious and time-consuming traditional diagnosis, automatic detection algorithms of deep learning (DL) can save diagnosis time, improve test accuracy, reduce costs, and reduce the radiologist's workload. In this paper, we present detection models for diagnosing the presence of KTs in computed tomography (CT) scans. Toward detecting and classifying KT, we proposed 2D-CNN models; three models are concerning KT detection such as a 2D convolutional neural network with six layers (CNN-6), a ResNet50 with 50 layers, and a VGG16 with 16 layers. The last model is for KT classification as a 2D convolutional neural network with four layers (CNN-4). In addition, a novel dataset from the King Abdullah University Hospital (KAUH) has been collected that consists of 8,400 images of 120 adult patients who have performed CT scans for suspected kidney masses. The dataset was divided into 80% for the training set and 20% for the testing set. The accuracy results for the detection models of 2D CNN-6 and ResNet50 reached 97%, 96%, and 60%, respectively. At the same time, the accuracy results for the classification model of the 2D CNN-4 reached 92%. Our novel models achieved promising results; they enhance the diagnosis of patient conditions with high accuracy, reducing radiologist's workload and providing them with a tool that can automatically assess the condition of the kidneys, reducing the risk of misdiagnosis. Furthermore, increasing the quality of healthcare service and early detection can change the disease's track and preserve the patient's life.

Abstract:The precise classification of kidneys and kidney tumors is of great guiding significance for the diagnosis and surgical planning of clinical doctors. Artificial segmentation of kidneys and renal tumors not only increases the workload of doctors, but also has a certain degree of subjectivity. Due to the varying morphology and pathological characteristics of various kidney tumors, conventional imaging analysis methods have certain limitations in their application. This study aims to propose a novel optical imaging device based on deep learning to achieve better results in kidney tumor image segmentation and recognition. The study collected a batch of image data on kidney tumors and trained the data using deep learning algorithms. After many experiments and optimization, a model with excellent performance in renal tumor segmentation and recognition was obtained. Compared with several existing methods, the method proposed in this paper has better segmentation accuracy for kidney tumors. This can not only avoid mutual interference between the network and the resolution network, but also improve the recognition efficiency between the two networks. In addition, the system can also provide reliable data feedback for clinical doctors, helping them continuously correct problems in subsequent visual examinations, thereby achieving higher diagnostic accuracy. The experimental results show that the neural network is effective in image segmentation and quantitative analysis, and can be applied in clinical practice.

Abstract:Purpose The objective of this study is to develop a novel diagnostic tool using deep learning and radiomics to distinguish bone tumors on CT images as metastases from breast cancer. By providing a more accurate and reliable method for identifying metastatic bone tumors, this approach aims to significantly improve clinical decision-making and patient management in the context of breast cancer. Methods This study utilized CT images of bone tumors from 178 patients, including 78 cases of breast cancer bone metastases and 100 cases of non-breast cancer bone metastases. The dataset was processed using the Medical Image Segmentation via Self-distilling TransUNet (MISSU) model for automated segmentation. Radiomics features were extracted from the segmented tumor regions using the Pyradiomics library, capturing various aspects of tumor phenotype. Feature selection was conducted using LASSO regression to identify the most predictive features. The model's performance was evaluated using ten-fold cross-validation, with metrics including accuracy, sensitivity, specificity, and the Dice similarity coefficient. Results The developed radiomics model using the SVM algorithm achieved high discriminatory power, with an AUC of 0.936 on the training set and 0.953 on the test set. The model's performance metrics demonstrated strong accuracy, sensitivity, and specificity. Specifically, the accuracy was 0.864 for the training set and 0.853 for the test set. Sensitivity values were 0.838 and 0.789 for the training and test sets, respectively, while specificity values were 0.896 and 0.933 for the training and test sets, respectively. These results indicate that the SVM model effectively distinguishes between bone metastases originating from breast cancer and other origins. Additionally, the average Dice similarity coefficient for the automated segmentation was 0.915, demonstrating a high level of agreement with manual segmentations. Conclusion This study demonstrates the potential of combining CT-based radiomics and deep learning for the accurate detection of bone metastases from breast cancer. The high-performance metrics indicate that this approach can significantly enhance diagnostic accuracy, aiding in early detection and improving patient outcomes. Future research should focus on validating these findings on larger datasets, integrating the model into clinical workflows, and exploring its use in personalized treatment planning.

A Deep Learning-Based Radiomics Model for Differentiating Benign and Malignant Renal Tumors

Abstract:OBJECTIVES: To investigate the effect of transfer learning on computed tomography (CT) images for the benign and malignant classification on renal tumors and to attempt to improve the classification accuracy by building patient-level models.METHODS: One hundred ninety-two cases of renal tumors were collected and identified by pathologic diagnosis within 15 days after enhanced CT examination (66% male, 70% malignant renal tumors, average age of 62.27 ± 12.26 years). The InceptionV3 model pretrained by the ImageNet dataset was cross-trained to perform this classification. Five image-level models were established for each of the Slice, region of interest (ROI), and rectangular box region (RBR) datasets. Then, two patient-level models were built based on the optimal image-level models. The network's performance was evaluated through analysis of the receiver operating characteristic (ROC) and five-fold cross-validation.RESULTS: In the image-level models, the test results of model trained on the Slice dataset [accuracy (ACC) = 0.69 and Matthews correlation coefficient (MCC) = 0.45] were the worst. The corresponding results on the ROI dataset (ACC = 0.97 and MCC = 0.93) were slightly better than those on the RBR dataset (ACC = 0.93 and MCC = 0.85) when freezing the weights before the mixed6 layer. Compared with the image-level models, both patient-level models could discriminate better (ACC increased by 2%-5%) on the RBR and Slice datasets.CONCLUSIONS: Deep learning can be used to classify benign and malignant renal tumors from CT images. Our patient-level models could benefit from 3D data to improve the accuracy.

Abstract:PurposeComputed tomography (CT) images can display internal organs of patients and are particularly suitable for preoperative surgical diagnoses. The increasing demands for computer-aided systems in recent years have facilitated the development of many automated algorithms, especially deep convolutional neural networks, to segment organs and tumors or identify diseases from CT images. However, performances of some systems are highly affected by the amount of training data, while the sizes of medical image data sets, especially three-dimensional (3D) data sets, are usually small. This condition limits the application of deep learning.MethodsIn this study, given a practical clinical data set that has 3D CT images of 20 patients with renal carcinoma, we designed a pipeline employing transfer learning to alleviate the detrimental effect of the small sample size. A dual-channel fine segmentation network (FS-Net) was constructed to segment kidney and tumor regions, with 210 publicly available 3D images from a competition employed during the training phase. We also built discriminative classifiers to classify the benign and malignant tumors based on the segmented regions, where both handcrafted and deep features were tested.ResultsOur experimental results showed that the Dice values of segmented kidney and tumor regions were 0.9662 and 0.7685, respectively, which were better than those of state-of-the-art methods. The classification model using radiomics features can classify most of the tumors correctly.ConclusionsThe designed FS-Net was demonstrated to be more effective than simply fine-tuning on the practical small size data set given that the model can borrow knowledge from large auxiliary data without diluting the signal in primary data. For the small data set, radiomics features outperformed deep features in the classification of benign and malignant tumors. This work highlights the importance of architecture design in transfer learning, and the proposed pipeline is anticipated to provide a reference and inspiration for small data analysis.

Abstract:Considering that the kidneys segmentation challenge for image processing because of the gray level from abdominal computer tomography (CT) scans is a great similarity of adjacent organs, partial volume effects and so on, a novel multi-feature sparse constraints strategy is proposed to diagnose the benign and malignant renal tumors, which can improve the accuracy and reliability of segmentation. The weighted sparse measure is defined by introducing weights in the l1-norm of vectors. The weight is inversely proportional to the similarity between data, therefore the weighted l1-norm penalty on the linear representation coefficients tends to force similar data be involved while dissimilar data uninvolved in the linear representation of a datum. The resulted representation can overcome the drawbacks of l1-norm penalty that the presentation coefficients are usually over sparse and not robust for highly correlated data. Experimental results and objective assessment indexes show that the proposed method can effectively segment CT images with good visual consistency. In addition, the dice coefficients of renal and renal tumors were 0.933 and 0.854, respectively. In addition, our method can also be used for the diagnosis of renal tumors, and has also achieved good performance.

Abstract:Early intervention in kidney cancer helps to improve survival rates. Abdominal computed tomography (CT) is often used to diagnose renal masses. In clinical practice, the manual segmentation and quantification of organs and tumors are expensive and time-consuming. Artificial intelligence (AI) has shown a significant advantage in assisting cancer diagnosis. To reduce the workload of manual segmentation and avoid unnecessary biopsies or surgeries, in this paper, we propose a novel end-to-end AI-driven automatic kidney and renal mass diagnosis framework to identify the abnormal areas of the kidney and diagnose the histological subtypes of renal cell carcinoma (RCC). The proposed framework first segments the kidney and renal mass regions by a 3D deep learning architecture (Res-UNet), followed by a dual-path classification network utilizing local and global features for the subtype prediction of the most common RCCs: clear cell, chromophobe, oncocytoma, papillary, and other RCC subtypes. To improve the robustness of the proposed framework on the dataset collected from various institutions, a weakly supervised learning schema is proposed to leverage the domain gap between various vendors via very few CT slice annotations. Our proposed diagnosis system can accurately segment the kidney and renal mass regions and predict tumor subtypes, outperforming existing methods on the KiTs19 dataset. Furthermore, cross-dataset validation results demonstrate the robustness of datasets collected from different institutions trained via the weakly supervised learning schema.
Machine learning based identification of hub genes in renal clear cell carcinoma using multi-omics data

Abstract:Renal cell carcinoma is one of the most universal urinary system cancers in the world. The most common renal cell carcinoma subtype is renal clear cell carcinoma. It is usually associated with high rates of metastasis and mortality. Therefore, finding effective therapeutic targets and prognostic molecular markers is of great significance to improve the early diagnosis rate and prognostic accuracy of renal clear cell carcinoma. In this work, we successfully identified six hub genes that are closely related to the occurrence, development and prognosis of renal clear cell carcinoma and proposed three new potential prognostic markers, namely ATP4B, AC144831.1 and Tfcp2l1 through differentially expressed genes (DEGs) analysis, GO functional enrichment and KEGG pathway analysis, WGCNA analysis, and survival analysis. In addition, we established machine learning models to predict the occurrence of tumors through the gene expression data of patients. It is expected that the results of this study can provide reference value for the treatment of renal clear cell carcinoma.

Application of Radiomics Model of CT Images in the Identification of Ureteral Calculus and Phlebolith

Abstract:Objective. To investigate the clinical application of the three-dimensional (3D) radiomics model of the CT image in the diagnosis and identification of ureteral calculus and phlebolith. Method. Sixty-one cases of ureteral calculus and 61 cases of phlebolith were retrospectively investigated. The enrolled patients were randomly categorized into the training set (n = 86) and the testing set (n = 36) with a ratio of 7 : 3. The plain CT scan images of all samples were manually segmented by the ITK-SNAP software, followed by radiomics analysis through the Analysis Kit software. A total of 1316 texture features were extracted. Then, the maximum correlation minimum redundancy criterion and the least absolute shrinkage and selection operator algorithm were used for texture feature selection. The feature subset with the most predictability was selected to establish the 3D radiomics model. The performance of the model was evaluated by the receiver operating characteristic (ROC) curve, and the area under the ROC curve (AUC) was also calculated. Additionally, the decision curve was used to evaluate the clinical application of the model. Results. The 10 selected radiomics features were significantly related to the identification and diagnosis of ureteral calculus and phlebolith. The radiomics model showed good identification efficiency for ureteral calculus and phlebolith in the training set (AUC = 0.98; 95%CI: 0.96–1.00) and testing set (AUC = 0.98; 95%CI: 0.95–1.00). The decision curve thus demonstrated the clinical application of the radiomics model. Conclusions. The 3D radiomics model based on plain CT scan images indicated good performance in the identification and prediction of ureteral calculus and phlebolith and was expected to provide an effective detection method for clinical diagnosis.

Abstract:Background: Assessment of renal lesions and deficiency accurately remains critical in the diagnosis of congenital anomalies of the kidneys and urinary tracts (CAKUT) in children. Advanced imaging such as Magnetic resonance Imaging (MRI) and Diffusion weighted Imaging (DWI) allows structural and functional insufficiency to be detected. Currently, radiomics machine learning models are being explored as full-automated diagnostic tools. We aimed to develop a machine learning integrated radiomics model to predict renal anomalies and deficiency in children. Methods: A retrospective study of 280 children with MRI/DWI were enrolled between 2018 and 202 at a children's hospital. A total of 1,037 radiomics features were extracted from the DWI images of each participant, which were divided into training set and test set (8:2 split). Using 5-fold cross-validated method, multiple machine learning algorithms were employed to predict renal lesions and deficiency when compared with the radiologist's diagnosis based on DWI, Tc-99m-labeled dimercaptosuccinic acid (DMSA) SPECT cortical renal scintigraphy or Tc-99m-labeled diethylenetriamine pentaacetate (DTPA) renal scan. Results: For detecting the kidney lesions, the LASSO + Random Forest algorithm outperformed other classifiers with an accuracy of 0.750 (95% confidence interval, 0.734-0.766) and area under the curve (AUC) of 0.765 (95% confidence interval, 0.700-0.831). The performance of classifiers did not show a significant difference (p > 0.05) in detecting bilateral or unilateral kidney lesions by DWI scanning. The classifiers performed significantly better in bilateral kidney deficit than in unilateral kidney deficit (p < 0.05). We next built prediction models for renal deficiency using the radiomics signature and clinical features compared to renal scintigraphy. The ensemble model had a high-test accuracy of 80.9% +/- 4.2% and a sensitivity of 91.7% +/- 7.1% with a moderate calibration. Conclusion: An ensemble model integrated with DWI-radiomic and clinical features can be utilized to predict renal lesions and deficiency in children with CAKUT.

Abstract:This study was aimed to discuss the feasibility of distinguishing benign and malignant breast tumors under the tomographic ultrasound imaging (TUI) of deep learning algorithm. The deep learning algorithm was used to segment the images, and 120 patients with breast tumor were included in this study, all of whom underwent routine ultrasound examinations. Subsequently, TUI was used to assist in guiding the positioning, and the light scattering tomography system was used to further measure the lesions. A deep learning model was established to process the imaging results, and the pathological test results were undertaken as the gold standard for the efficiency of different imaging methods to diagnose the breast tumors. The results showed that, among 120 patients with breast tumor, 56 were benign lesions and 64 were malignant lesions. The average total amount of hemoglobin (HBT) of malignant lesions was significantly higher than that of benign lesions (P < 0.05). The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of TUI in the diagnosis of breast cancer were 90.4%, 75.6%, 81.4%, 84.7%, and 80.6%, respectively. The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of ultrasound in the diagnosis of breast cancer were 81.7%, 64.9%, 70.5%, 75.9%, and 80.6%, respectively. In addition, for suspected breast malignant lesions, the combined application of ultrasound and tomography can increase the diagnostic specificity to 82.1% and the accuracy to 83.8%. Based on the above results, it was concluded that TUI combined with ultrasound had a significant effect on benign and malignant diagnosis of breast cancer and can significantly improve the specificity and accuracy of diagnosis. It also reflected that deep learning technology had a good auxiliary role in the examination of diseases and was worth the promotion of clinical application.
