Abstract:Sir: Since the proposal of the term “porokeratotic eccrine ostial and dermal duct nevus” by Abell and Read in 1980, fewer than 30 cases have been published. The typical presentation has been congenital, asymptomatic papules and plaques on one or more of the extremities, rarely involving the trunk, face, or scalp.1,2 Lesions follow Blaschko’s lines with variable extent of involvement. The Blaschkoid configuration reflects clinical mosaicism attributed to a novel postzygotic mutation, chromosomal alterations during embryogenesis or, less frequently, functional mosaicism of an X-linked defect. Kroumpouzos et al. reported a similar pattern of clinical presentation in a rare case of systemized porokeratotic eccrine and hair follicle nevus.3 The histologic hallmark of porokeratotic eccrine ostial and dermal duct nevus is cornoid lamellae packed within epidermal invaginations overlying eccrine units. This has been attributed to either a hyperplastic acrosyringium with relative sparing of the glandular portion or a hyperplastic epidermal invagination associated with an eccrine-like duct.4 Although it has been speculated that an abnormal clone of epidermal cells at the base of the parakeratotic columns may be prone to malignant degeneration, we found no documentation of associated malignancy. We present a case of congenital porokeratotic eccrine ostial and dermal duct nevus in a 39-year-old woman whose ulcerated lesions within the porokeratotic eccrine ostial and dermal duct nevus revealed squamous cell carcinoma. The patient was born with diffuse, whirled hyperpigmentation of her trunk and extremities. Since the age of 6, these lesions had undergone intermittent pruritic blistering and gradually evolved into painful, nonhealing wounds. Two years before her current presentation, she was evaluated for worsening symptoms. Biopsy specimens obtained at that time were benign, revealing columns of cornified parakeratotic cells overlying epidermal invaginations and adnexal ostia. When referred for surgical evaluation, hyperpigmented patches and plaques were noted in a Blaschkoid arrangement throughout the trunk and upper and lower extremities (Fig. 1). The distribution pattern of the nevus had not changed since birth. Both discrete and clustered superficial ulcerations were noted within these lesions.Fig. 1.: Blaschkoid distribution of lesions extending inferiorly to include the posterior aspect of the right leg and plantar surface of the right foot. Multiple oozing ulcers are present.Over an 11-month period, she underwent staged excision of all ulcerated lesions, followed by either primary closure or split-thickness skin grafts. A total of 36 specimens were sent for pathologic examination. Thirty-one (86 percent) of these revealed squamous cell carcinoma adjacent to the cornoid lamella (Fig. 2). Seven of these 31 specimens were described as invasive squamous cell carcinoma, whereas the rest were labeled as either superficial or multifocal squamous cell carcinoma in situ.Fig. 2.: Cornoid lamellae adjacent to squamous cell carcinoma. Note the angled column of parakeratosis, diminished granular layer, and dyskeratosis (hematoxylin and eosin; original magnification, ×200).A case of diffuse, painful, nonhealing ulcers revealing squamous cell carcinoma within congenital porokeratotic eccrine ostial and dermal duct nevus lesions is presented. This is an unusual finding in what is otherwise known as a benign clinical entity that rarely requires surgical therapy. One may consider malignant transformation of an abnormal clone of epidermal cells at the base of the parakeratotic columns. Alternatively, malignant transformation has been documented in rare cases of linear epidermal nevus—an entity that bears close resemblance to porokeratotic eccrine ostial and dermal duct nevus clinically. However, the distinction between linear epidermal nevus with parakeratotic features and porokeratotic eccrine ostial and dermal duct nevus has not been adequately established in the current literature.5 Further investigation into the genetic and molecular specifics of porokeratotic eccrine ostial and dermal duct nevus pathogenesis is required.
