Abstract:The extensive degeneration of functional somatic cells and the depletion of endogenous stem/progenitor populations present significant challenges to tissue regeneration in degenerative diseases. Currently, a cellular reprogramming approach enabling directly generating corresponding progenitor populations from degenerative somatic cells remains elusive. The present study focused on intervertebral disc degeneration (IVDD) and identified a three-factor combination (OCT4, FOXA2, TBXT [OFT]) that could induce the dedifferentiation-like reprogramming of degenerative nucleus pulposus cells (dNPCs) toward induced notochordal-like cells (iNCs). Single-cell transcriptomics dissected the transitions of cell identity during reprogramming. Further, OCT4 was found to directly interact with bromodomain PHD-finger transcription factor to remodel the chromatin during the early phases, which was crucial for initiating this dedifferentiation-like reprogramming. In rat models, intradiscal injection of adeno-associated virus carrying OFT generated iNCs from in situ dNPCs and reversed IVDD. These results collectively present a proof-ofconcept for dedifferentiation-like reprogramming of degenerated somatic cells into corresponding progenitors through the development of a factor-based strategy, providing a promising approach for regeneration in degenerative disc diseases.
Exosome-laden Injectable Self-Healing Hydrogel Based on Quaternized Chitosan and Oxidized Starch Attenuates Disc Degeneration by Suppressing Nucleus Pulposus Senescence

Abstract:Disc degeneration is the common pathology underlying various degenerative spinal disorders and currently there is no effective cure. Here, we found nucleus pulposus (NP) cell senescence was closely associated with the severity of disc degeneration, and exosomes (Exos) derived from mesenchymal stem cells (MSCs) ameliorated NP cell senescence and promoted extracellular matrix (ECM) deposition. As chitosan-based hydrogels have been widely used as vehicles to deliver Exos due to their prominent antibacterial capacity, biocompatibility, and biodegradability, we developed an Exos-laden hydrogel based on quaternized chitosan (QCS) and oxidized starch (OST) to treat disc degeneration. The synthesized QCS-OST hydrogel is injectable, self-healing, biocompatible, and demonstrated desirable pore size, injectable properties, and sustainable release of Exos. In a rat model of disc degeneration, the QCS-OST/Exos hydrogel was able to rejuvenate NP cell senescence, promote ECM remodeling, and partially restore the structures of NP and annulus fibrosis. Our findings suggested that the novel QCS-OST/Exos hydrogel is an effective therapeutic strategy for treating disc degeneration via alleviating NP cell senescence.

Abstract:The intervertebral disc degeneration (IDD) with increasing aging mainly manifests as low back pain (LBP) accompanied with a loss of physical ability. These pathological processes can be preliminarily interpreted as a series of changes at cellular level. In addition to cell death, disc cells enter into the stagnation with dysfunction and deteriorate tissue microenvironment in degenerative discs, which is recognized as cell senescence. During aging, many intrinsic and extrinsic factors have been proved to have strong connections with these cellular senescence phenomena. Growing evidences of these connections require us to gather up critical cues from potential risk factors to pathogenesis and relative interventions for retarding cell senescence and attenuating degenerative changes. In this paper, we try to clarify another important cell state apart from cell death in IDD and discuss senescence-associated changes in cells and extracellular microenvironment. Then, we emphasize the role of oxidative stress and epigenomic perturbations in linking risk factors to cell senescence in the onset of IDD. Further, we summarize the current interventions targeting senescent cells that may exert the benefits of antidegeneration in IDD.

Abstract:Low back pain (LBP) is one of the world’s most common musculoskeletal diseases and is frequently associated with intervertebral disc degeneration (IDD). While the main cause of IDD is commonly attributed to a reduced number of nucleus pulposus (NP) cells, current treatment strategies (both surgical and more conservative) fail to replenish NP cells or reverse the pathology. Cell replacement therapies are an attractive alternative for treating IDD. However, injecting intervertebral disc (IVD) cells, chondrocytes, or mesenchymal stem cells into various animal models of IDD indicate that transplanted cells generally fail to survive and engraft into the avascular IVD niche. Whereas pluripotent stem cells (PSCs), including induced pluripotent stem cells (iPSCs) and embryonic stem cells (ESCs), hold great potential for revolutionizing regenerative medicine, current protocols for differentiating these cells into NP-like cells are inadequate. Nucleus pulposus progenitor cells (NPPCs), which are derived from the embryonic notochord, can not only survive within the harsh hypoxic environment of the IVD, but they also efficiently differentiate into NP-like cells. Here we provide an overview of the latest progress in repairing degenerated IVDs using PSCs and NPPCs. We also discuss the molecular pathways by which PSCs differentiate into NPPCs in vitro and in vivo and propose a new, in vivo IDD therapy.

Abstract:With a booming aging population worldwide, bone and joint degenerative diseases have gradually become a major public health focus, attracting extensive scientific attention. However, the effective treatments of these degenerative diseases have been confined to traditional medications and surgical interventions, which easily lead to the possibility of drug abuse or loss of physiological function to varying degrees. Recently, given that the development of reprogramming has overcome shackles in the field of degenerative diseases, direct reprogramming would provide a new concept to accelerate progress in the therapy of bone and joint degenerative diseases. The process of direct reprogramming would directly induce ordinary somatic cells to the desired targeted cells without passing through pluripotent cell states. In this review, we summarize some direct reprogramming of cells that has been attempted for the repair of common bone and joint degenerative diseases, such as osteoarthritis, osteoporosis-related fracture and intervertebral disc degeneration. However, it is inevitable that some obstacles, such as accurate transcription factors, an appropriate extracellular microenvironment and efficient delivery carriers in vivo, need to be resolved. In addition, developmental and promising directions associated with direct reprogramming have attracted public attention. Investigation of the regulation of the transient genome, metabolic conversion and cellular skeleton would provide superior potential candidates for the revolution of direct reprogramming. The aim of direct reprogramming is to directly provide target cells for cell therapy and even tissue reconstruction in bone and joint degenerative diseases. Moreover, the development of direct reprogramming have potential to achieve repair and even reconstruct in situ, which would be breakthrough effect for the repair of bone and joint degenerative diseases. The advance of direct reprogramming has opened numerous opportunities for new therapeutic strategies in regenerative medicine.

Abstract:Osteoarthritis (OA) and intervertebral disc degeneration (IVDD) are the most common degenerative bone and joint diseases, posing a major threat to patients' physical and mental health due to the occurrence of chronic pain and disability. Within this context, the absence of efficacious therapies has led to a growing interest in regenerative medicine. In particular, as a method that can erase the memory of differentiation and re-endow cells with pluripotency, cell reprogramming technologies have ushered in a new era of personalized therapy, which not only show great potential for the treatment of degenerative osteoarthropathies but also promise to achieve tissue regenerative and repair. However, compared to other areas of research, reprogramming technologies to treat OA and IVDD are still in the preliminary stages and require further investigation. This paper briefly introduces the characteristics of cell reprogramming; summarizes the pathological mechanisms of reprogramming to improves energy metabolism, aging, inflammation, oxidative stress, and immune imbalance in OA and IVDD under the background of microenvironment and immunity; highlights the significant advantages of reprogramming-derived cells compared to embryonic stem cells and mesenchymal stem cells, based on these advances, providing important strategies for its development and clinical application in OA and IVDD.

Intervertebral disc cell fate during aging and degeneration: apoptosis, senescence, and autophagy

Abstract:Background: Degenerative disc disease, a major cause of low back pain and associated neurological symptoms, is a global health problem with the high morbidity, workforce loss, and socioeconomic burden. The present surgical strategy of disc resection and/or spinal fusion results in the functional loss of load, shock absorption, and movement; therefore, the development of new biological therapies is demanded. This achievement requires the understanding of intervertebral disc cell fate during aging and degeneration. Methods: Literature review was performed to clarify the current concepts and future perspectives of disc cell fate, focused on apoptosis, senescence, and autophagy. Results: The intervertebral disc has a complex structure with the nucleus pulposus (NP), annulus fibrosus (AF), and cartilage endplates. While the AF arises from the mesenchyme, the NP originates from the notochord. Human disc NP notochordal phenotype disappears in adolescence, accompanied with cell death induction and chondrocyte proliferation. Discs morphologically and biochemically degenerate from early childhood as well, thereby suggesting a possible involvement of cell fate including age-related phenotypic changes in the disease process. As the disc is the largest avascular organ in the body, nutrient deprivation is a suspected contributor to degeneration. During aging and degeneration, disc cells undergo senescence, irreversible growth arrest, producing proinflammatory cytokines and matrix-degradative enzymes. Excessive stress ultimately leads to programmed cell death including apoptosis, necroptosis, pyroptosis, and ferroptosis. Autophagy, the intracellular degradation and recycling system, plays a role in maintaining cell homeostasis. While the incidence of apoptosis and senescence increases with age and degeneration severity, autophagy can be activated earlier, in response to limited nutrition and inflammation, but impaired in aged, degenerated discs. The phosphatidylinositol 3-kinase (PI3K)/Akt/mammalian target of rapamycin (mTOR) is a signal integrator to determine disc cell fate. Conclusions: Cell fate and microenvironmental regulation by modulating PI3K/Akt/mTOR signaling is a potential biological treatment for degenerative disc disease.

Abstract:Abstract Intervertebral disc degeneration (IDD) is a common disease that increases with age, and its occurrence is stressful both psychologically and financially. Stem cell therapy for IDD is emerging. For this therapy, stem cells from different sources have been proven in vitro, in vivo, and in clinical trials to relieve pain and symptoms, reverse the degeneration cascade, delay the aging process, maintain the spine shape, and retain mechanical function. However, further research is needed to explain how stem cells play these roles and what effects they produce in IDD treatment. This review aims to summarize and objectively analyse the current evidence on stem cell therapy for IDD.

Abstract:Inflammatory cytokines and cellular senescence collectively contribute to the intervertebral disc degeneration. Enzyme‐instructed self‐assembly supramolecular nanofibers (Fbp‐GFFYPLGLAG(E)nRGD) are developed to target the overexpression of two crucial enzymes, MMP‐2 and COX‐2. These nanofibers can effectively block the synergistic effect of the inflammatory cascade and aging‐related secretion phenotype and reduce the extracellular matrix damage caused by inflammation and aging cell accumulation. Finally, it can mitigate the progression of disc degeneration. As an age‐related disease, intervertebral disc degeneration is closely related to inflammation and aging. Inflammatory cytokines and cellular senescence collectively contribute to the degradation of intervertebral disc. Blocking this synergy reduces disc extracellular matrix damage caused by inflammation and aging. In this study, drug‐loaded nanofibers with sequential targeting functions are constructed through intelligent response, hydrophilicity, and in situ self‐assembly empowerment of flurbiprofen. The peptide precursor responds to the cleavage of overexpressed MMP‐2 in the degenerative intervertebral disc microenvironment (intracellular and extracellular), resulting in the formation of self‐assembled nanofibers that enable the on‐demand release of flurbiprofen and COX‐2 response. In vitro, Comp. 1 (Flurbiprofen‐GFFYPLGLAGEEEERGD) reduces the expression of inflammation‐related genes and proteins and the polarization of M1 macrophages by competitively inhibiting COX‐2 and increases the expression of extracellular matrix proteins COL‐2 and aggrecan. Additionally, it can reduce the expression of Senescence‐Associated Secretory Phenotype and DNA damage in aged nucleus pulposus cells and promote the recovery of proliferation and cell cycle. In vivo, drug‐loaded nanofibers delay intervertebral disc degeneration by inhibiting inflammation and preventing the accumulation of senescent cells. Therefore, the sequentially targeted self‐assembled drug‐loaded nanofibers can delay intervertebral disc degeneration by blocking the synergistic effect of inflammatory cytokines and cellular senescence.

Abstract:Intervertebral disc (IVD) degeneration is a pathological process that affects the molecular, cellular , tissue levels, but neither conservative nor surgical treatments currently address its root causes. Recent developments in genetic engineering , tissue engineering technologies indicate that a biomedical treatment for IVD degener-ation may be possible. Lentivirus has been utilized in this study as a gene vector to introduce insulin-like growth factor-1 (IGF-1) and transforming growth factor-beta 3 (TGF-beta 3) into nucleus pulposus mesenchymal stem cells (NPMSCs) for the first time. This resulted in the construction of dual-gene engineered seed cells that could differentiate into nucleus pulposus cells overexpressing both IGF-1 and TGF-beta 3 growth factors. A bicomponent polymer network (BCN) hydrogel based on gelatin methacrylate (GelMA) and hyaluronic acid methacrylate (HAMA) was prepared and used as a carrier to deliver genetically engineered NPMSCs to the target tissues. BCN hydrogels effectively upregulated the extracellular matrix expression and nucleus pulposus differentiation after being loaded with NPMSCs overexpressing IGF-1 and TGF-beta 3. The novel biotherapeutic modality presented could be used to repair degenerated IVD tissues, replenish seed cells with a rich potential, and promote extracellular matrix synthesis and nucleus pulposus differentiation, thereby providing a theoretical basis and experimental rationale for the etiological treatment of IVD degeneration.

Abstract:Low back pain is a chronic health problem. It is the most common cause of limited activity. 90% of people worldwide will develop low back pain during their life. It is due, in the most cases, to the intervertebral disc degeneration (IDD). The first cause of IDD is aging. In all tissues aging is a condition characterized by mutations and genome instability, mitochondrial dysfunction, oxidative damage and consequential decline of cellular functions. Decline in tissues function is related to the loss of stem cells. These characteristics translate into different pathologies, with increasing costs for the healthcare system. In 2050 around 2 billion people worldwide will be older than 65. People are living longer but they are not healthier. Worldwide research is focused on identify the molecular pathways that can regulate expression of pro-survival networks. The purpose of this study was to determinate whether blood chimerism with a young wild type mouse (WT) could delay or reverse aging in a prematurely aging mouse model (Ercc1-/Δ). Blood chimerism was obtained by way of parabiosis, an experimental method in which two animals are surgically connected and develop a united circulation. 3 kinds of parabiotic pairs were generated: WT + WT at 40 days old (hisochronic parabiosis), Ercc1-/Δ + Ercc1-/Δ at 64 days old (hisochronic parabiosis) and WT + Ercc1-/Δ (heterochronic parabiosis). After surgery the couples develop anastomosis that make possible the blood chimerism. Administration of a dye (Evans blue) and fluorescent nano-beads into a single symbiont animal were used to test shared circulation in the parabiosis couples. Mice were sacrificed after 4 weeks; entire intervertebral discs were removed from the surrounding vertebral bodies by an incision along the endplate. Lumbar discs from each mouse were analyzed histologically and for nucleous polposus protein content. Expression of pro-survival networks in senescent cells and in stem cells were analyzed. Compared with the hisochronic controls, Ercc1-/Δ mice heterochronically paired with young WT mice had a significally improved protein content in the disc (95% CI). Histological analysis (EE & SaffO) revealed an improvement in the disc for the Ercc1-/Δ individual in the heterochronic pair relative to the hisochronic ones. We observed a reactivation of stem cells when exposed to younger serum and a migration of young stem cells to the old mice (using GFP protein expressed in the younger mouse). Furthermore µCT highlighted an improvement of the bone quality. These results indicate that spine of the progeroid mice were rejuvenated in the older mice when exposed to a serum derived from younger organism. Senescent cells secrete pro-inflammatory cytokines, and chemokines, which together constitute the senescence-associated secretory phenotype. We believe that stem cells are quiescent in old organism and that the environment can reactivate them. Moreover this factor can attract young stem cells from the young mice. The factor responsible for this, this elixir of youthful, although currently unidentified, could potentially be used as an effective treatment to rescue IDD as well other pathologies connected to aging.

Abstract:As a permanent state of cell cycle arrest, cellular senescence has become an important factor in aging and age-related diseases. As a central regulator of physiology and pathology associated with cellular senescence, the senescence associated secretory phenotype can create an inflammatory and catabolic environment through autocrine and paracrine ways, ultimately affecting tissue microstructure. As an age-related disease, the correlation between intervertebral disc degeneration and cellular senescence has been confirmed by many studies. Various pathological factors in the microenvironment of intervertebral disc degeneration promote senescent cells to produce and accumulate and express excessive senescence associated secretory phenotype. In this case, senescence associated secretory phenotype has received considerable attention as a potential target for delaying or treating disc degeneration. Therefore, we reviewed the latest research progress of senescence associated secretory phenotype, related regulatory mechanisms and intervertebral disc cell senescence treatment strategies. It is expected that further understanding of the underlying mechanism between cellular senescence pathology and intervertebral disc degeneration will help to formulate reasonable senescence regulation strategies, so as to achieve ideal therapeutic effects.The translational potential of this article: Existing treatment strategies often fall short in addressing the challenge of repairing intervertebral disc Intervertebral disc degeneration(IVD) degeneration. The accumulation of senescent cells and the continuous release of senescence-associated secretory phenotype (SASP) perpetually impede disc homeostasis and hinder tissue regeneration. This impairment in repair capability presents a significant obstacle to the practical clinical implementation of strategies for intervertebral disc degeneration. As a result, we present a comprehensive overview of the latest advancements in research, the associated regulatory mechanisms, and strategies for treating SASP in IVD cells. This article aims to investigate effective interventions for delaying the onset and progression of age-related intervertebral disc degeneration. In an era where the aging population is becoming increasingly prominent, this endeavor holds paramount practical and translational significance.

Abstract:Articular cartilage injury and degeneration causing pain and loss of quality-of-life has become a serious problem for increasingly aged populations. Given the poor self-renewal of adult human chondrocytes, alternative functional cell sources are needed. Direct reprogramming by small molecules potentially offers an oncogene-free and cost-effective approach to generate chondrocytes, but has yet to be investigated. Here, we directly reprogrammed mouse embryonic fibroblasts into PRG4+ chondrocytes using a 3D system with a chemical cocktail, VCRTc (valproic acid, CHIR98014, Repsox, TTNPB, and celecoxib). Using single-cell transcriptomics, we revealed the inhibition of fibroblast features and activation of chondrogenesis pathways in early reprograming, and the intermediate cellular process resembling cartilage development. The in vivo implantation of chemical-induced chondrocytes at defective articular surfaces promoted defect healing and rescued 63.4% of mechanical function loss. Abstract:The progress, challenges, and future prospects of repair strategies and materials for the intervertebral disc are reviewed, highlighting and summarizing the latest advances of main repairing strategies and functional materials for disc degeneration repair, to provide a reference for the following relevant studies. Intervertebral disc (IVD) degeneration is the leading cause of low back pain, which represents a highly prevalent chronic aging‐associated disorder with a large socio‐economic burden and a great decline in the quality of life, contributing to the pressing need for IVD treatments. Thus, finding effective disease‐modifying approaches for postponing the progression of IVD degeneration and repairing degenerated intervertebral discs is of great importance, even facing various challengeable obstacles. Bioactive functional materials exhibit excellent advances in modulating cell activity and remodeling tissue homeostasis, which acts as a promising and accessible therapeutic approach to tissue repair and organ reconstruction. Recently, these have been notably explored to investigate bioactive functional materials‐based repair strategies for intervertebral disc recovery and regeneration, including cell‐based tissue engineering, the development of implants for disc recovery, and therapeutic methods for remodeling the functional integrity of the IVD. This review summarizes the advanced progress of bioactive functional materials in IVD repair and regeneration, and discusses their regulatory mechanisms, limitations, challenges as well as their clinical translational prospects in the future.

Abstract:Intervertebral disc degeneration (IVDD) is a frequent and intractable chronic condition in orthopedics that causes enormous discomfort in patients' lives and thoughts, as well as a significant economic burden on society and the nation. As a result, understanding the pathophysiology of IVDD is critical. The pathophysiology of IVDD has been linked to numerous variables, including oxidative stress, apoptosis, matrix metalloproteinases, and inflammatory factors. Cellular senescence has recently attracted a lot of attention in the study of age-related diseases. It has been discovered that IVDD is intimately linked to human senescence, in which nucleus pulposus cell senescence may play a significant role. Previously, our group did a comprehensive and systematic clarification of the pathogenesis of IVDD from an immune perspective and discovered that the fundamental pathogenesis of IVDD is inflammatory upregulation and nucleus pulposus cell death caused by an imbalance in the immune microenvironment. In this review, we will treat nucleus pulposus cell senescence as a novelty point to clarify the pathophysiology of IVDD and further explore the probable relationship between senescence and immunity along with the dysregulation of the immunological microenvironment to propose new therapeutic approaches for IVDD.

[Research progress of cellular senescence and senescent secretary phenotype in intervertebral disc degeneration]

Abstract:Objective: To summarize the role of cellular senescence and senescent secretary phenotype in the intervertebral disc (IVD) degeneration. Methods: Relevant articles that discussed the roles of cellular senescence in the IVD degeneration were extensively reviewed, and retrospective and comprehensive analysis was performed. The senescent phenomenon during IVD degeneration, senescent secretary phenotype of the disc cells, senescent pathways within the IVD microenvironment, as well as the anti-senescent approaches for IVD regeneration were systematically reviewed. Results: During aging and degeneration, IVD cells gradually and/or prematurely undergo senescence by activating p53-p21-retinoblastoma (RB) or p161NK4A-RB senescent pathways. The accumulation of senescent cells not only decreases the self-renewal ability of IVD, but also deteriorates the disc microenvironment by producing more inflammatory cytokines and matrix degrading enzymes. More specific senescent biomarkers are required to fully understand the phenotype change of senescent disc cells during IVD degeneration. Molecular analysis of the senescent disc cells and their intracellular signaling pathways are needed to get a safer and more efficient anti-senescence strategy for IVD regeneration. Conclusion: Cellular senescence is an important mechanism by which IVD cells decrease viability and degenerate biological behaviors, which provide a new thinking to understand the pathogenesis of IVD degeneration.

Abstract:Intervertebral disc degeneration (IVDD) refers to the aging and degenerative diseases of intervertebral disc components such as nucleus pulposus, annulus fibrosus, and cartilage endplate, and is the main cause of chronic low back pain. Over the past few years, many researchers around the world concerned that the degeneration of nucleus pulposus (NP) cells plays the main role in IVDD. The degeneration of NP cells is caused by a series of pathological processes, including oxidative stress, inflammatory response, apoptosis, abnormal proliferation, and autophagy. Interestingly, many studies have found a close relationship between the senescence of NP cells and the progression of NP degeneration. The classical aging pathways also have been confirmed to be involved in the pathological process of IVDD. Moreover, several anti-aging drugs have been used to treat IVDD by inhibiting NP cells senescence, such as proanthocyanidins, resveratrol and bone morphogenetic protein 2. Therefore, this article will systematically list and discuss aging, cell senescence, the pathogenesis and targeted therapies of IVDD, in order to provide new ideas for the treatment of IVDD in the future.
