Abstract:Die gastroösophageale Refluxkrankheit (GERD) stellt einer der häufigsten Erkrankungen des Gastrointestinaltraktes dar und gilt als Volkskrankheit. Neben der hohen Prävalenz und der deutlichen Einschränkung der Lebensqualität liegt die Bedeutung der Erkrankung im breiten klinischen Spektrum an Symptomen, Komplikationen und Folgeerkrankungen. Im Gegensatz zur GERD-Leitlinie 2014 wird bei der vorliegenden Aktualisierung das Krankheitsbild der Eosinophilen Ösophagitis integriert, sodass mit der vorliegenden Leitlinie die beiden häufigsten Krankheitsbilder der Speiseröhre abgehandelt werden. Abstract:It is an honor and a great pleasure for us to be guest editors for this special issue of Hämostaseologie – Progress in Haemostasis , which addresses important issues surrounding the complex of venous thromboembolism (VTE). In February 2023, the revised guideline on "Diagnostics and Therapy of Venous Thrombosis and Pulmonary Embolism" has been published on the website of the Association of the Scientific Medical Societies in Germany (AWMF)[1]. This guideline was drawn up under the leadership of the German Society of Angiology (DGA), and representatives of 17 scientific societies contributed to its content. As an S2k guideline, its recommendations are consensus based and are the result of a systematic review and evaluation of current evidence and consideration of the benefits and harms of diagnostic and therapeutic options. In this special issue, guideline authors provide a comprehensive overview of selected guideline topics which might be of clinical relevance to our readers and our community of haemostaseologists. Abstract:Esophageal ambulatory reflux monitoring is the current gold standard for the diagnosis of gastroesophageal reflux disease (GERD). In order to facilitate standardized procedure and improve diagnostic accuracy, clinical guidelines for ambulatory esophageal reflux monitoring were developed based on thorough literature search and working group conference by experts in gastrointestinal motility. Indications, contraindications, methodology, and reporting of ambulatory esophageal reflux monitoring were discussed in these clinical guidelines.

Abstract:Background Eosinophilic oesophagitis (EoE) is an increasingly common cause of dysphagia in both children and adults, as well as one of the most prevalent oesophageal diseases with a significant impact on physical health and quality of life. We have provided a single comprehensive guideline for both paediatric and adult gastroenterologists on current best practice for the evaluation and management of EoE. Methods The Oesophageal Section of the British Society of Gastroenterology was commissioned by the Clinical Standards Service Committee to develop these guidelines. The Guideline Development Group included adult and paediatric gastroenterologists, surgeons, dietitians, allergists, pathologists and patient representatives. The Population, Intervention, Comparator and Outcomes process was used to generate questions for a systematic review of the evidence. Published evidence was reviewed and updated to June 2021. The Grading of Recommendations, Assessment, Development and Evaluation (GRADE) system was used to assess the evidence and make recommendations. Two rounds of voting were held to assess the level of agreement and the strength of recommendations, with 80% consensus required for acceptance. Results Fifty-seven statements on EoE presentation, diagnosis, investigation, management and complications were produced with further statements created on areas for future research. Conclusions These comprehensive adult and paediatric guidelines of the British Society of Gastroenterology and British Society of Paediatric Gastroenterology, Hepatology and Nutrition are based on evidence and expert consensus from a multidisciplinary group of healthcare professionals, including patient advocates and patient support groups, to help clinicians with the management patients with EoE and its complications.

Abstract:Esophageal eosinophilia and eosinophilic esophagitis (EoE) are increasingly recognized and prevalent conditions, which now represent common clinical problems encountered by gastroenterologists, pathologists, and allergists. The study of EoE has become a dynamic field with an evolving understanding of the pathogenesis, diagnosis, and treatment. Although there are limited data supporting management decisions, clinical parameters are needed to guide the care of patients with eosinophilic-esophageal disorders. In this evidence-based review, recommendations developed by adult and pediatric gastroenterologists are provided for the evaluation and management of these patients. New terminology is emphasized, particularly the concepts of esophageal eosinophilia and proton-pump inhibitor-responsive esophageal eosinophilia (PPI-REE) as entities distinct from EoE.

Ziel der Leitlinie ist es, einen Handlungskorridor für häufige klinische Entscheidungen zu etablieren, um eine konsistente und qualitativ hochwertige Patientenversorgung zu sichern. The S2k guideline "Liver Transplantation", jointly developed by the German Society of Gastroenterology, Digestive and Metabolic Diseases (DGVS) and the German Society of General and Visceral Surgery (DGAV), represents the first German-language guideline for the care of adult patients before and after liver transplantation. This guideline has been crafted through a collaborative, consensus-based approach, involving experts from various disciplines. It integrates current scientific insights and clinical experience to ensure optimal care for patients undergoing liver transplantation. Targeting health care professionals in diagnostics and therapy, patient advocates, affected individuals, and their families, the guideline aims to establish a framework for common decisions in clinical practice. Abstract:Significant variability in the technical performance of gastrointestinal (GI) endoscopy, overuse of endoscopy for surveillance, an underuse of more advanced endoscopic interventions, such as endoscopic submucosal dissection or endoscopic ultrasound-guided biliary drainage, are only a few of the challenges that the European Society of Gastrointestinal Endoscopy (ESGE) has sought to address through society publications. Over the past decade, ESGE has developed a rigorous, evidence-based, and reproducible standard for ESGE publications. ESGE is an amalgam of 41 national gastroenterology/endoscopy member societies; thus, ESGE publications must be of sufficiently high quality to be either directly accepted as the equivalent to the local standards of our member societies or to influence the local standard of care and thereby standardize practice within our large and diverse ESGE community. ESGE has strived to advance and improve the methodology of its publications to keep up with the technological advancements and innovations in GI endoscopy. For example, when the ESGE Guideline Committee was first established, its focus was exclusively on producing clinical guidelines as guidance on whether and when GI endoscopy was indicated. Later, quality standards in endoscopy were introduced and a more rigorous methodology for ESGE publications was created. Most recently, technology innovations, such as bariatric endoscopy, third-space endoscopy, and therapeutic endoscopic ultrasound, have resulted in additional training challenges for advanced endoscopy procedures; thus, ESGE has created procedure-specific curricula. Today, ESGE publications cover the entire spectrum of guidance and training in GI endoscopy ([Fig. 1]), addressing the following fundamental questions. When is an endoscopic diagnostic evaluation and/or therapeutic intervention recommended? What is the level of competence required for the endoscopist undertaking such intervention(s)? How can we train an endoscopist to achieve such a level of competence? How can we measure and improve quality of GI endoscopy performance? These questions deserve to be addressed by specific methodologies for two main reasons: 1) the amount of published high level evidence is much lower for the level of competence and training required for endoscopic procedures than for clinically oriented issues; and 2) the output of such information has been primarily qualitative in the case of clinical recommendations (to do vs. not to do endoscopy) and quantitative for defining the level of endoscopist competence required and its acquisition. Thus, ESGE publications are currently divided into 1) clinical guidelines, 2) performance (quality) measures, and 3) curricula. Article published online: 30 January 2024 © 2024. Abstract:The present document constitutes Part 2 of the EoETALY Consensus Statements guideline on the diagnosis and management of eosinophilic esophagitis (EoE) developed by experts in the field of EoE across Italy (i.e., EoETALY Consensus Group). Part 1 was published as a different document, and included three chapters discussing 1) definition, epidemiology, and pathogenesis; 2) clinical presentation and natural history and 3) diagnosis of EoE. The present work provides guidelines on the management of EoE in two final chapters: 4) treatment and 5) monitoring and follow-up, and also includes considerations on knowledge gaps and a proposed research agenda for the coming years. The guideline was developed through a Delphi process, with grading of the strength and quality of the evidence of the recommendations performed according to accepted GRADE criteria.This document has received the endorsement of three Italian national societies including the Italian Society of Gastroenterology (SIGE), the Italian Society of Neurogastroenterology and Motility (SINGEM), and the Italian Society of Allergology, Asthma, and Clinical Immunology (SIAAIC). The guidelines also involved the contribution of members of ESEO Italia, the Italian Association of Families Against EoE.

Experts in the field of EoE across Italy (i.e., EoETALY Consensus Group) including gastroenterologists, endoscopists, allergologists/immunologists, and paediatricians conducted a Delphi process to develop updated consensus statements for the management of patients with EoE and update the previous position paper of the Italian Society of Gastroenterology (SIGE) in light of recent evidence. Grading of the strength and quality of the evidence of the recommendations was performed using accepted GRADE criteria. The guideline is divided in two documents: Part 1 includes three chapters, namely 1) definition, epidemiology, and pathogenesis; 2) clinical presentation and natural history, and 3) diagnosis, while Part 2 includes two chapters: 4) treatment and 5) monitoring and follow-up. Abstract:Introduction Eosinophilic esophagitis (EoE) is a chronic inflammatory disease of the esophagus characterized by symptoms of esophageal dysfunction and histologically by predominantly eosinophilic infiltration of the squamous epithelium. European Society for Pediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) published a guideline in 2014; however, the rapid evolution of knowledge about pathophysiology, diagnostic criteria, and therapeutic options have made an update necessary. Methods A consensus group of pediatric gastroenterologists from the ESPGHAN Working Group on Eosinophilic Gastrointestinal Diseases (ESPGHAN EGID WG) reviewed the recent literature and proposed statements and recommendations on 28 relevant questions about EoE. A comprehensive electronic literature search was performed in MEDLINE, EMBASE, and Cochrane databases from 2014 to 2022. The Grading of Recommendations Assessment, Development and Evaluation system was used to assess the quality of evidence and formulate recommendations. Results A total of 52 statements based on the available evidence and 44 consensus‐based recommendations are available. A revision of the diagnostic protocol, options for initial drug treatment, and the new concept of simplified empiric elimination diets are now available. Biologics are becoming a part of the potential armamentarium for refractory EoE, and systemic steroids may be considered as the initial treatment for esophageal strictures before esophageal dilation. The importance and assessment of quality of life and a planned transition to adult medical care are new areas addressed in this guideline. Conclusion Research in recent years has led to a better understanding of childhood EoE. This guideline incorporates the new findings and provides a practical guide for clinicians treating children diagnosed with EoE.

Abstract:Background: Gastroesophageal reflux is defined as a common physiologic phenomenon encountered intermittently by a large population, especially following a feed. Gastroesophageal reflux disease (GERD) happens if the number of gastric juice that refluxes into the esophagus passes the accepted limit, making symptoms with or without correlated esophageal mucosal injury (i.e., esophagitis). Diagnosis and treatment guidelines of gastroesophageal reflux disease (GERD) were announced in 1995 and updated in 2005 and lastly reviewed in 2013 by the American College of Gastroenterology (ACG). These and other guidelines undergo periodic review. Advances continue to be made in GERD, leading us to review and revise previous guideline statements. Here we aimed to review the different updated guidelines of diagnosis and treatment of Gastroesophageal reflux disease (GERD). A web-based search utilizing the advanced characteristics of different databases like PubMed, Google Scholar, Embase, Scopus, and Cochrane electronic databases was carried out. Abstract:This Executive summary of the Guideline on pediatric gastrointestinal endoscopy from the European Society of Gastrointestinal Endoscopy (ESGE) and the European Society for Paediatric Gastroenterology Hepatology and Nutrition (ESPGHAN) refers to infants, children, and adolescents aged 0 - 18 years. The areas covered include: indications for diagnostic and therapeutic esophagogastroduodenoscopy and ileocolonoscopy; endoscopy for foreign body ingestion; endoscopic management of corrosive ingestion and stricture/stenosis; upper and lower gastrointestinal bleeding; endoscopic retrograde cholangiopancreatography, and endoscopic ultrasonography. Percutaneous endoscopic gastrostomy and endoscopy specific to inflammatory bowel disease (IBD) have been dealt with in other Guidelines and are therefore not mentioned in this Guideline. Training and ongoing skill maintenance will be addressed in an imminent sister publication.

Abstract:Background: Eosinophilic esophagitis (EoE) is recognized as a chronic type 2 inflammatory disease characterized by the eosinophilic infiltration of the esophageal tissue, posing a significant disease burden and highlighting the necessity for novel management strategies to address unmet clinical needs. Objectives: To critically evaluate the existing literature on the epidemiology and management of EoE, identify evidence gaps, and assess the efficacy of current and emerging treatment modalities. Design: An extensive literature review was conducted, focusing on the epidemiological trends, diagnostic challenges, and therapeutic interventions for EoE. This was complemented by a survey among physicians and consultations with a scientific expert panel, including a patient's association (ESEO Italia), to enrich the study findings. Data sources and methods: The review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, scrutinizing epidemiological studies and management research to compile comprehensive insights into the disease's landscape. The physician survey and expert panel discussions aimed to bridge identified evidence gaps. Results: The review included 59 epidemiological and 51 management studies, uncovering variable incidence and prevalence rates of EoE globally, with an estimated diagnosed prevalence of 41 per 100,000 in Italy. Diagnostic challenges were identified, including nonspecific symptoms and the lack of definitive biomarkers, which complicate the use of endoscopy. Treatment options such as elimination diets, proton-pump inhibitors, and swallowed corticosteroids were found to have varying success rates, while Dupilumab, an emerging therapy targeting interleukin (IL)-4 and IL-13, shows promise. Conclusion: Despite advancements in understanding and managing EoE, significant unmet clinical needs remain, particularly in biomarker identification, therapy personalization, and cost-effectiveness evaluation. A comprehensive, multidimensional approach to patient management is required, emphasizing the importance of early symptom recognition, accurate diagnosis, and tailored treatment strategies. Dupilumab offers potential as a novel treatment, underscoring the need for future research to explore the economic and social dimensions of EoE care pathways.
