Abstract:OBJECTIVE:To investigate the methodology and efficacy of endoscopic sphenopalatine artery diathermy in management of refractory epistaxis.METHOD:Twenty patients with severe intractable posterior epistaxis, who had underwent various procedures like nasal packing, electro- or chemical cautery or others and still exacerbated, were subjected to this study. Under general anesthesia, according to the endoscopic anatomy of the sphenopalatine foramen, the sphenopalatine artery was identified under the posterior tip of the middle turbinate. After the pedicle of the artery was delineated, a bipolar cauterized the artery and transected it.RESULT:Twenty patients have been followed up for 5 to 20 months with an average of 10 months. No patients suffered recurrent epistaxis during the period. Only 1 patients in this series reported minor nasal adhesion.CONCLUSION:Diathermy of sphenopalatine artery, as a purely endonasal procedure, is an effective and microinvasive means of achieving long-term control of refractory epistaxis. It has few complications associated with other forms of arterial ligation.

Abstract:Objective To evaluate the efficacy of endoscopic ligation of the sphenopalatine artery in the treatment of severe posterior epistaxis.Method 25 patients were hospitalized for the treatment of severe posterior epistaxis,in whom posterior nasal packing was not effective.Endoscopic ligations of the sphenopalatine artery were performed under general anesthesia in 24 patients and local anesthesia in 1.Results In the procedure,all the sphenopalatine arteries were successfully identified and ligated where the artery left the sphenopalatine foramen.After surgery,22 patients were successfully controlled without any bleeding.Recurrent epistaxis occurred in one patient after 6 hours after surgery and was successfully treated with posterior nasal packing and discharged after two days.Two patients suffered minor late epistaxis that only required medical treatment.There were no further bleedings in all patients after 3 to 6 months follow-up.No complications were found except slight intranasal adhesion in 2 patients.Conclusion Endoscopic ligation of the sphenopalatine artery is a safe and an effective surgical treatment for patients with severe posterior epistaxis.

Posterior Epistaxis: Common Bleeding Sites and Prophylactic Electrocoagulation.

Abstract:Posterior epistaxis is a frequent emergency, and the key to efficient management is identification of the bleeding point. We performed a retrospective study of 318 patients with posterior epistaxis treated with endoscopic bipolar electrocautery during a 4-year period. Distribution of the bleeding sites was recorded. Patients with no definite bleeding sites in the first operation were assigned to Group A (n = 39) and Group B (n = 34). Patients in Group A were only observed in the ward. Patients in Group B were given prophylactic electrocoagulation at the common bleeding points. Of the 318 patients, bleeding sites were successfully identified and coagulated in 263 patients. All of them were located posteriorly, with 166 on the lateral nasal wall, 86 on the septum, and 11 on the anterior face of the sphenoid sinus. The rebleeding rate of Group B (8.8%) was lower than that of Group A (38.5%) (p < 0.01).

Abstract:Abstract Purpose The control of epistaxis has always posed a significant challenge for otolaryngologists. One of the most viable options to address refractory cases is the ligation or cauterization of the sphenopalatine artery. The objective of this study was to assess the efficacy, safety, and long-term outcomes of these interventions. Materials and methods Two independent otolaryngologists conducted a comprehensive search for studies dealing with management of uncontrolled/recurrent epistaxis by consulting the main scientific databases on the web, including PubMed, Google Scholar, Medline, EMBASE, Web of Science, and the Cochrane Library. The systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) statement. The criteria for considering studies for the review were based on the population, intervention, comparison, outcome, timing and setting (PICOTS) framework. Results Sixteen studies were included in the systematic review, comprising a total of 454 patients. Among these, 289 individuals underwent ligation of the sphenopalatine artery, while 100 underwent cauterization of the same artery. Additionally, 56 patients underwent both ligation and cauterization of the sphenopalatine artery during the same surgery. The incidence of rebleeding and complications was respectively 12.1% (55/454) and 3% (14/454), resulting in relatively low rates in both cases. Conclusions Our review emphasizes the increasing importance of surgical approaches, specifically ligation or cauterization of the sphenopalatine artery, in addressing refractory cases. The low incidence of complications, predominantly temporary decreased lacrimation in patients undergoing ligation of the sphenopalatine artery, highlights the safety and feasibility of these interventions.

Abstract:Objective To confirm the curative effect of plasma on perennial allergic rhinitis (PAR) and to study the therapical mechanism.Methods The anterior-ethmoid neurotomy was performed in 46 cases (81 procedures) using plasma by intranasal endoscopic surgery.Clinical symptoms and physical sign before and after operation were evaluated,and the pathologic change of the incision sections was observed.Results In operative group with plasm(46 cases,81 sides),the apparent rate was 32.6%(15 cases),the effective rate was 67.4%(31 cases),the average counting-score reduce from 12.93 pre-operation to 6.14 post-operation(P0.001).The sample of nasal cavity tissue showed complete aerification on the incision areas and no evident denaturation outside 0.1mm of it.Conclusion The anterior-ethmoid neurotomy with plasma is an effective and simple therapical technique for treating PAR,it can also be applied in hemostasis,incision and aerification in nasal cavity including nasal septum mallet.
Utility of Nasal Access Guides in Endoscopic Endonasal Skull Base Surgery: Assessment of Use during Cadaveric Dissection and Workflow Analysis in Surgery

Abstract:Abstract Background A nasal access guide (NAG) for endoscopic endonasal approaches (EEAs) to the skull-base has been developed and approved for clinical use but its utility has not been formally investigated. Objective The study aims to assess the effect of a NAG on endoscopic visualization during cadaveric dissection and to perform a workflow analysis with process-based performance measures in the operating room and their effect on clinical outcomes. Methods Skull-base course participants were observed during hands-on cadaveric dissection with and without NAG. Instances of endoscope withdrawal for lens cleaning and inadequate visualization due to lens soiling were tabulated. Participants completed a Likert-scale survey examining the NAG utility and provided an overall grading. Surgical workflow and process-based performance on patients undergoing EEA to the skull-base was analyzed. Passage of powered and dissecting instruments, removal of endoscopes for cleaning, and dislodgment or migration of the device were reviewed. Postoperative assessments included mucosal trauma and synechiae formation. Results Instances of endoscope soiling and manual cleaning were significantly reduced by 40% and 61% with the NAG during cadaveric dissection. The overall grading of the device was 2.75/3. Surgical workflow was observed in 35 patients. Average number of passes of endoscopes, instruments, and powered tools during a 10-minute observation period were 3,17, and 5 during the surgical approach, and 3, 18, and 1 during tumor dissection. Dislodgement of the device occurred in 25.7% and migration of the device in 2.8% of cases. Postoperative synechiae, exposed cartilage or septal perforation was not observed in follow up. Conclusion NAG can significantly reduce inadequate visualization during EEA to the skull-base and has the potential to reduce instances of nasal trauma. Participants assessed its overall utility as being “excellent.”

Abstract:Introduction Radiofrequency (RF) ablation of parahisian accessory pathways (PHAPs) is a serious challenge, which is associated with high recurrence and fluoroscopy exposure. According to previous studies and case reports, catheter ablation of PHAPs is associated with a high post-ablation recurrence. The aim of this study is to assess the efficacy, safety and long-term outcome of radiofrequency ablation in patients with recurrent PHAPs after prior ablations with zero or extremely low fluoroscopy. Methods and Results A total of 58 patients (43 male, 75.4%) with PHAPs that had one or more previously unsuccessful RF ablations were enrolled. RF ablation procedures were performed under a three-dimensional guiding system (EnSite NavX, St. Jude Medical, MN) with extremely low exposure of fluoroscopy to femoral venous or arterial routes. The acute success rate was 96.6% (56/58). Two procedures were terminated during ablation because of high risk of complete AVB. In addition to a case of transient complete AVB during RF energy delivery, no minor or major complications were recorded during the procedure or long-term follow up. The mean fluoroscopy time was 98 seconds (0-129), and the mean radiation dose was 1.98 mGy (0-3.38) in all 58 cases. Fourteen patients underwent the EPS and RF ablation with zero X-ray exposure. The long-term success rate was 98.2% with a median follow-up period of 30 (12-45) months. Conclusions With a near zero X-ray exposure, RF ablation of recurrent PHAPs can be performed with a high acute success rate and convincing safety during long-term follow-up.
Risk factors of epistaxis after endoscopic endonasal skull base surgeries

Abstract:Background: Epistaxis after endoscopic endonasal skull base surgeries does not typically occur as an accidental circumstance but often results from multiple factors. We aimed to assess the possible risk factors associated with postoperative epistaxis. Methods: Patients who underwent endoscopic endonasal skull base surgery at Huashan hospital from August 2018 to November 2019 were enrolled in the study. Postoperative epistaxis was defined as severe, persistent, or recurrent arterial nosebleed, which required therapeutic intervention. Patients were divided into the epistaxis and no epistaxis groups. The incidence, clinical characteristics, management, and intraoperative findings were recorded. Results: A total of 762 patients were included in the study. 20 (2.6%) patients experienced postoperative epistaxis, which happened in a delayed fashion between 6 and 30 postoperative days. Multivariate logistic regression analysis showed that arterial hypertension (OR=3.394, 95%CI: 1.094-10.531, P = 0.034) and preoperative systolic blood pressure (SBP) (OR=1.035, 95%CI: 1.002-1.068) were identified as predictors for postoperative epistaxis. The bleeding sites were identified at the left posterior septal artery (PSA) in 10 (62.5%) cases, the right PSA in 4 (25%) cases, the left palatovaginal artery in 1 (6.25%) case, and the right inferior turbinate branch of the posterior lateral nasal artery in 1 (6.25%) case, respectively. Direct bipolar cauterization was used to stop the bleeding. After definitive treatment, no patients had recurrent epistaxis. Conclusions: Arterial hypertension and preoperative SBP were associated with postoperative epistaxis after endoscopic endonasal skull base surgery, and the left PSA contributed to the dominant site of bleeding during surgical re-exploration.

Abstract:Objective:To discuss surgical methods and experience of the excision of the intranasal and paranasal papilloma via the nasal endoscope and observe the curative effect.Methods:Retrospective analysis was conducted to clinical data of 48 patients suffering from the intranasal and paranasal papilloma treated in the hospital from February 2010 to February 2015. All the patients had tumor resection by low-temperature plasma,Caldwell-Luc maxillary sinus approach or lacrimal recess approach under the assistance of nasal endoscope. Among them,there are 29 pa-tients with the nasal tumor and 19 patients with nasal sinuses tumor. The 24-to 36-month follow-up for clinical effects was conducted,to summarize and analyze the surgical methods and skills.Results:48 patients of this group had the diseased tissue removed successfully via surgery.4 cases of them relapsed during follow-up,with a rate of 8.33%. Compared to 22% postoperative relapse rate in the nasal inverted papilloma via the nasal endoscope reported in China,P＜0.05. 3 patients of the 4 cases were found with relapse during the nasal endoscopic reexamination and cured after local resection and local electrocoagulation via the plasma knife and 1 patient of the 4 cases was cured by thorough debridement in a second operation. None of them suffered from serious intraoperative or postoperative com-plications and good clinical effects were reached.Conclusion:Preferable clinical effects can be obtained for the in-tranasal and paranasal papilloma by resection of the low-temperature plasma knife with the aid of the nasal endo-scope. It is of great significance to ensure successful operation by selecting appropriate surgery based on extent and staging of the disease and accurate judgment of the root and base of the tumor for resection during the surgery,togeth-er with postoperative close follow-up and low-temperature plasma treatment,so as to achieve the aim of curing and lower the probability of second surgery.

Abstract:OBJECTIVE:To report a new approach to maxillary sinus without ablation of nasolacrimal duct and inferior turbinate by endoscopic nasal lateral wall dissection (ENLWD). The preliminary clinical application results, the operative technique and indications were discussed.METHODS:Twelve hospitalized patients, aged from 42 years to 68 years, were enrolled in this paper, 9 patients were male and 3 were female. Among the 12 patients, 8 were inverted papilloma (IP), 2 were nasal polyps, 1 was Kubo's postoperative cyst of maxillary sinus (MS) and 1 was recurrent bone cyst of maxilla. Two IP patients were excluded from this group for the follow-up time was less than 12 months. Preoperative nasal endoscopy and CT scan were done in all patients, 6 patients with tumor also received MRI examination. The operation began with a mucosal incision in front of inferior turbinate (IT) and then the lacrimal duct (LD) was dissected, thus a IT-LD flap was formed. The MS lesion was removed under endoscope when the flap was replaced medially. The inferior antrostomy was performed when the flap was repositioned.RESULTS:All 10 patients were unilateral lesion. Six MS IP patients were T3 Krouse stage. The tumor was found originating from the every part of the MS wall in the operation, especially from the anterior and medial wall. The same situation was seen in 2 patients with nasal polyps. The follow-up ranged from 7 months to 60 months (average 22 months). The shape of IT was good, and well-epithelized operative cavities were observed. Only 1 patients of IP had a local recurrence in its inferior antrostomy 6 months after operation and no recurrence was seen after local debridement. All of them had no epiphora and other complications.CONCLUSIONS:ENLWD is a new and minimally invasive approach not only to MS, in which the lesion can be thoroughly removed, but also to orbital floor and pterygopalatine fossa without ablation of lacrimal duct and IT.

Abstract:Objective To evaluate the effect of nasal endoscopic surgery by low-temperature plasma radiofrequency for benign tumor resection.Methods The clinical data of 17 patients with nasal sinus benign tumor treated between Janu-ary 2012 and May 2013 under nasal endoscopic surgery by the low-temperature plasma radiofrequency were retrospec-tively studied.All were followed up in a 1-year process.In which,8 cases of inverted papilloma,1 case of angiofibro-ma of nasopharynx,3 cases of nasal hemangioma,and 5 cases of hermorrhagic and necrotic polyps of maxillary sinus were studied.Results The blood loss was 5 to 150 mL with an average of 30 mL.The operation time was 20 to 55 minutes with an average of 35 minutes. During the follow-up period,cavity epithelial change was good and no recur-rence was found.Conclusion Endoscopic surgery by low-temperature plasma radiofrequency is a viable surgical meth-od for benign tumor and has advantages of less bleeding,minimal invasion and short operation time.
Prospective Study on Coblation Vs. Cautery in Endoscopic Trans Sphenoidal Surgery for Pituitary Macroadenoma: Impact on Nasal Function

Abstract:Endoscopic endonasal Trans-sphenoidal surgery for sellar-suprasellar tumors is considered the preferred route. The ideal instruments in skull base surgery should be able to manipulate adequately through the narrow corridors with effective tissue removal and bleeding control and reduce collateral damage to nearby vital structures. The present study sought to compare the nasal morbidity following transnasal sphenoidal surgery for pituitary macroadenoma using Coblation and Electrocautery. 160 undergoing Endoscopic TSS for non-functioning PAs were enrolled. The patients were randomly allocated into Coblation (n = 80) and electrocautery (n = 80) groups. All operations were performed by the same group of surgeons skilled in their surgical techniques. There were 59 and 53 patients with Knosp grade II tumors in the cautery and coblation group, respectively, while 21 and 27 patients had Knosp grade III tumors. The average duration of surgery using cautery was 96+/-4.5 min, while with coblation, the average time was 83+/-3.5 min, and the difference was statistically significant. Nasal crusting and granulation severity were observed more in the electrocautery group. The coblation group patients had clear surgical fields with few ooze points; the difference between the two groups was significant. Nasal synechiae and decreased nasal patency were common in the electrocautery group. There were no statistical differences in nasal bleeding, nasal deformity, or sense of smell parameters. Advanced medical tools like coblation are safe and effective for the dissection and ablation of the mucosa. They result in better intra-operative visualization and lesser postoperative nasal morbidity, which will help patients achieve a better quality of life.
Extended transcavernous posterior clinoidectomy in endoscopic endonasal surgery

Abstract:Objective: Mastery of the posterior clinoidectomy technique is of utmost importance for neurosurgeons who specialize in endoscopic endonasal approaches, because the posterior clinoid process (PCP) is commonly involved in chondroid tumor resection. Three main techniques for posterior clinoidectomy have been developed: intradural, extradural, and transcavernous interdural. The authors introduce here a novel technical variant in which the transcavernous approach is extended to the dorsal clinoidal space after transection of the caroticoclinoid ligament, and they elaborate on its clinical application on the basis of anatomical dissections and radiological studies. Methods: The authors reviewed CT angiography images and 3D reconstruction of the PCP in 50 adults to analyze the height and presence of ossified ligament attachments. In addition, endoscopic endonasal posterior clinoidectomy was performed in 20 lightly embalmed postmortem human heads. Three techniques, including extradural, transcavernous, and extended transcavernous posterior clinoidectomy, were performed sequentially, and anatomical landmarks and areas exposed with each technique were investigated and compared. Results: Using radiological studies, the authors categorized the PCPs as 1 of 2 types: 1) normal, defined as less than or equal to 8 mm high with no ossified ligament attachments; or 2) complex, defined as greater than 8 mm high with or without an ossified ligament attachment. Compared with extradural (exposed PCP height 4.7 ± 0.5 mm) and transcavernous (exposed PCP height 7.3 ± 0.8 mm) posterior clinoidectomies, the extended transcavernous posterior clinoidectomy provided the maximally exposed PCP height (9.6 ± 0.4 mm; p < 0.0001). Conclusions: This report details the extended transcavernous posterior clinoidectomy as a novel technical variant for achieving maximal exposure of the PCP in endoscopic endonasal surgery. In addition, the positive results establish the importance of preoperative skull base imaging for surgical planning.

Abstract:Objective: To explore the effects of endoscopic treatment for refractory nasal bleeding.Methods: Twenty-eight cases of refractory nasal bleeding were treated by nasal endoscopy combined with electro-coagulation and stuffing of resorbed materials.Results: Twenty-six cases were cured by one treatment,one was cured by two treatments,and one was cured by conservative treatment.Conclusion: The regions of the olfactory cleft and posterior part of the inferior meatus are the common areas in cases of refractory nasal bleeding.Bleeding points can be precisely searched by nasal endoscopy.Endoscopic treatment for refractory nasal bleeding is a microinvasive and effective method.
Applying the Full Endoscopic Keyhole Technique to CPA Lesions: A Single-Center Study

Abstract:In recent years, endoscopy has become an increasingly common tool used during neurosurgical procedures. However, its application in treating cerebellopontine angle (CPA) lesions has not progressed as rapidly. In this study, the authors present their initial experience with surgically treating CPA lesions using a fully endoscopic keyhole retrosigmoid approach. They conducted a retrospective analysis of clinical data from patients who underwent endoscopic keyhole CPA surgery at their center between May 2017 and April 2022. They provide a comprehensive explanation of this method and an overview of the strategies that have been developed to achieve better clinical outcomes. The study included 107 patients, consisting of 10 cases of vestibular schwannoma, 21 cases of epidermoid cyst, 32 cases of trigeminal neuralgia, and 44 cases of hemifacial spasm. The authors analyzed the clinicodemographic details of the patients. Among the 31 tumor cases, gross total resection was achieved in 25 patients (80.6%), while near-total resection was performed in 6 patients (19.4%). In patients with trigeminal neuralgia, facial pain resolved in 31 out of 32 patients (96.9%). Similarly, facial convulsions disappeared or were relieved in all 44 patients (100%) with hemifacial spasms after the operation. Postoperative complications included facial nerve paresis (n=9, 8.4%), with improvement observed in 6 cases during follow-up, transient facial hypoesthesia (n=3, 2.8%), cerebrospinal fluid rhinorrhea (n=3, 2.8%), transient abducens paresis (n=1, 0.9%), and postoperative hemorrhage (n=1, 0.9%). Endoscopy provides improved deep illumination and, combined with close-up observation, enhances the visualization of structures within the CPA region. The fully endoscopic keyhole technique is a safe and effective method for managing CPA lesions.
The interventional treatment of intractable epistaxis

Abstract:Objective:To evaluate selective angiographic diagnosis and superselective embolization of intractable epistaxis in 21 cases.Methods:The seldinger technique was used in all patients. The catheter was introduced via femoral artery on the right side. Selective angiographic analysis and diagnosis were performed.Superselective angiographic embolization with silk thread in diamend form 140 to 250 um was performed in bleeding vessels.Results:All cases achieved satisfactory effect. During the follow-up 2 to 12 month, no patients recurrence nose bleeding. 8 cases were operated in 1 week. Bleeding reduced sharply in operation,and operative difficulty cut down and operative success rate was increased. Conclusion:Selective angiographic diagnosis,and Superselective angiographic embolization,and suitive size of silk thread granular,and appropriate embolic speed are critical for successful treatment of massive epistais,Maxillary artery embolization is safe,effective,and uncomplicated Interventional embolization has afforded a new methed for acoustic-nasola-larynxologic operation.

Endoscopic Study for the Pterygopalatine Fossa Anatomy: Via the Middle Nasal Meatus-Sphenopalatine ForamenApproach

Abstract:Objective: The purposes of this study were to locate the constant anatomic landmarks, which are very important and helpful for endoscopic surgery and not well described for the pterygopalatine fossa (PPF) surgery via the middle nasal meatus-sphenopalatine foramen approach to establish a safe surgical mode. Methods: Eight cases of adult skull specimens were selected for the simulated surgery. The Messerklinger surgical approach was used under the endoscope. The uncinate process was removed successively, and the anterior ethmoid sinus and posterior ethmoid sinus were opened. The opening of the maxillary sinus was identified and was expanded forward and backward. The ethmoidal crest was found and was used as an anatomic landmark to find the sphenopalatine foramen. The sphenopalatine artery was protected and was used as a guide to enter the PPF region. The sphenopalatine artery was followed conversely to anatomize the blood vessels and nerves in the PPF. Results: It was found that our surgical procedure provides a clear view of the constant anatomic landmark including ethmoidal crest and sphenopalatine foramen. By retrograde dissection, following the sphenopalatine artery, which runs out of the sphenopalatine foramen behind the ethmoidal crest, the internal maxillary artery (IMA) and the branches of the IMA in the PPF were exposed. Posterior to the sphenopalatine artery, the typical Y-shaped structure with the pterygopalatine ganglion as the center was visible when the IMA and its branches were moved downward and outward. The Y structure, which is consisted of the pterygopalatine ganglion, branches of the internal maxillary nerve, vidian nerve, and descending palatine nerve, served as the other anatomic landmark. By following the Y structure, it was easy to locate the pterygoid canal, foramen rotundum, and the infraorbital nerve, and the integrity of the nerve structure could be protected. Conclusion: Endoscopic PPF surgery via the middle nasal meatus-sphenopalatine foramen approach is safe, and the ethmoidal crest, sphenopalatine foramen, and Y structure with the pterygopalatine ganglion in the center are important anatomic landmarks that can be referred to during the surgery.
