Abstract:Genome sequencing (GS) has emerged as a powerful diagnostic tool in several patient populations. For example, in critically ill neonates, genetic disease often presents with few clinical features and disease progression can be rapid. Standard genetic testing strategies can take months or weeks to complete and many neonates die or are discharged before a diagnosis is obtained. Outside of China, application of rapid GS in critically ill infants has been shown to have both diagnostic and clinical utility. In this case report, we sought to determine the genetic etiology of a critically ill neonate using a novel 24-hour ultra-rapid GS protocol and to explore the clinical utility of the diagnosis. A critically ill male neonate was admitted to the Neonatal Intensive Care Unit (NICU) of Children's Hospital of Fudan University in June 2021 due to preterm birth (35+2 weeks gestation). He presented with edema, asphyxia, increased body weight and head circumference (> P97), hypoglycemia (0.8-1.1mmol/L), abnormal heart development, cardiac insufficiency and respiratory failure. Notably, fetal B-ultrasound performed at 6 months gestation showed polyhydramnios, large biparietal diameter and pleural effusion. On his fourth day of admission, 24-hour ultra-rapid GS was ordered and initiated at 2:00 PM. Blood samples were obtained from the patient and parents and sequencing was performed as previously described by our lab. To accelerate the data analysis process, we used the DRAGEN Bio-IT Platform as an all-in-one solution. A NovaSeq platform was used to generate the BCL files and output variant calling produced VCF files for single nucleotide variants (SNVs), small insertions and deletions (INDELs), copy number variants (CNVs), structural variants (SVs), and mitochondrial variants. Sequencing reads were aligned to the reference human genome (GRCh37/hg19). The process from BCL to FastQ was completed in 10 minutes. A bypass annotation and filtering process of SNVs, INDELs, and mitochondrial variants began when the VCFs for small variants were ready. This process occurred in parallel to variant calling for CNVs and SVs. Processing of VCFs for small variants took approximately 30 minutes for each sample, which were run separately on 3 servers. Annotation and filtering of small variants took approximately 30 minutes to complete and were then combined with CNV and SV results for manual interpretation. GS detected a heterozygous de novo pathogenic variant in the HRAS gene (NM_005343.4, c.35G>A, p.Gly12Asp) .The total time from sample collection to result delivery was 23.5 hours. Based on the GS results, the boy was diagnosed with Costello syndrome and, at the request of the parents, he was switched to a palliative support plan. For the family, genetic counselling been arranged to help guide their next reproductive planning. The all-in-one solution of BCL-FastQ-VCF process combined with parallel computing greatly shortened the bioinformatics pipeline. This case report represents the first successful execution of 24hr ultra-rapid GS in China. The pipeline provided a rapid and efficient method for determining the genetic etiology of a critically ill neonate and was conducive to early individualized management. There are additional cases being analyzed in our rapid pipeline so that more Chinese neonates will have the opportunity to benefit from this technology.
