Abstract:IntroductionSpastic esophageal motility disorders induced significant symptoms including dysphagia, retrosternal pain and regurgitation. Conventional surgical treatment for esophageal motility disorders is myotomy, and recently most of these myotomies are performed by laparoscopy. P.O.E.M. is a novel approach to perform myotomy through the esophagus with long submucosal tunnel. This study aimed to investigate the feasibility and safety of P.O.E.M.MethodWe first conducted a prospective study on the performance of P.O.E.M. in animal model. After familiarizing the technique of P.O.E.M., we applied P.O.E.M. for treatment of esophageal motility disorders in human. For the technique of P.O.E.M., the gastroesophageal junction (GEJ) was first identified and the site of submucosal entrance was estimated to be 15 cm from GEJ. After submucosal injection with adequate elevation, a 2cm mucosa incision was made longitudinally. The endoscope with an oblique hood was introduced into the submucosa. Further dissection in the submucosa was performed using a prototype small triangle tip (TT) knife to create the tunnel. The myotomy started 10cm above GEJ and extending to 2cm below. The inner circular muscles were hook up by TT knife before cutting. After myotomy, the entrance sites were closed with endoclips.ResultsThe techniques of P.O.E.M. was first examined in non-survival porcine models which were successfully performed in 2 pigs. Seven 30 kg porcine models were then employed in the survival study. The mean operative time was 45.1 minutes. All the pig survived for 1 week except one which sustained severe pneumomediastinum and succumbed after the procedure. Post-mortem examination confirmed healing of all the GI entrance with no evidence of leakage. P.O.E.M. was then performed for treatment of esophageal motility disorders in four patients, one with Achalasia and three with hypertensive lower esophageal sphincter (LES). The mean preoperative LES pressure was 70.3mmHg. The mean operative time was 92 minutes. All patients tolerated diet on day 2 after the operation with contrast study confirming no leakage. There was no major complication.
