Abstract:BackgroundIn the emergency department, it is important to identify and prioritize who requires an urgent intervention in a short time. Triage helps recognize the urgency among patients. An accurate triage decision helps patients receive the emergency service in the most appropriate time. Various triage systems have been developed and verified to assist healthcare providers to make accurate triage decisions. The triage accuracy can represent the quality of emergency service, but there is a lack of review studies addressing this topic.MethodsA literature search was conducted in four electronic databases where ‘emergency nursing’ and ‘triage accuracy’ were used as keywords. Studies published from 2008 January to 2018 August were included as potential subjects. Nine studies were included in this review after the inclusion and exclusion criteria were applied.ResultsWritten case scenarios and retrospective review were commonly used to examine the triage accuracy. The triage accuracy from studies was in moderate level. The single-center studies which held better results than those from multi-center studies revealed the need of triage training and consistent training between emergency departments.ConclusionsRegular refresher triage training, collaboration between emergency departments and continuous monitoring were necessary to strengthen the use of triage systems and improve nurse’s triage performance.

Abstract:Triage refers to the process of patient prioritisation in the emergency department (ED). This is based on the severity of the patient's illness and is performed by emergency nurses (ENs). This has a pivotal role in ensuring patient safety and in ensuring that the ED operates smoothly – so continuous and accurate training are essential. As Emergency Nursing has been formally established in Greece since 2019, it is of the uppermost importance that all Greek ENs should be trained in the use of a standardised triage system. The present study aimed to evaluate the effect of triage training of ENs in the use of the Swiss Triage System (STS) after an intervention of one week.

Abstract:The Emergency Department (ED) is a facility meant to treat patients in need of medical assistance. The choice of triage system hugely impactsed the organization of any given ED and it is important to analyze them for their effectiveness. The goal of this review is to briefly describe selected triage systems in an attempt to find the perfect one. Papers published in PubMed from 1990 to 2022 were reviewed. The following terms were used for comparison: "ED" and "triage system". The papers contained data on the design and function of the triage system, its validation, and its performance. After studies comparing the distinct means of patient selection were reviewed, they were meant to be classified as either flawed or non-ideal. The validity of all the comparable segregation systems was similar. A possible solution would be to search for a new, measurable parameter for a more accurate risk estimation, which could be a game changer in terms of triage assessment. The dynamic development of artificial intelligence (AI) technologies has recently been observed. The authors of this study believe that the future segregation system should be a combination of the experience and intuition of trained healthcare professionals and modern technology (artificial intelligence).

Abstract:Sax and colleagues have continued their in-depth evaluation of the Emergency Severity Index (ESI), the most commonly used triage tool that prioritizes care for patients seeking emergency services in the US. Triage (meaning "to sort" in French) using the ESI is a multistep process that first identifies critical or high-risk patients, who are assigned the highest priority levels: ESI 1, for patients requiring lifesaving interventions, and ESI 2, for high-risk patients needing immediate care. For the remaining stable patients, ESI priority is based on anticipated use of resources for care: ESI 3 (many resources), ESI 4 (1 resource), and ESI 5 (none). Triage is primarily performed by nurses and requires rapid assessment and anticipation of emergency department (ED) resources, which include diagnostic tests, consultations, and procedures. Predicted resource use is based on clinical judgement, which leaves room for practice variability and potential bias. The ESI has performed poorly for the last 2 decades, and there are continued calls for improving triage, particularly for vulnerable populations.

Abstract:Natural or man-made medical disasters have repeatedly affected human communities. The impact on health care resources may vary depending on the magnitude of each crisis, catastrophe or pandemic, and the resources available. Medical triage protocols serve as invaluable tools to address clinical needs, particularly when resources, including supplies, equipment, and personnel, are limited. Although resources should be allocated to maximize the benefit, resource allocations need to be ethically sound. Existing triage protocols have inherent limitations.

Abstract:Introduction: Perceived quality of patient care is essential in healthcare, particularly in hospital emergency departments (EDs), where the triage process, performed by nurses, is fundamental yet under-researched. The aim is to explore patient satisfaction in triage, identify improvement opportunities, and provide guidance for future research to enhance ED care. Methodology: A scoping review was conducted across databases including PubMed, CINAHL, Scopus, Cochrane, Joanna Briggs, Google, and Google Scholar. Empirical articles from January 2014 to December 2023 were selected, excluding non-face-to-face triage, pediatrics, and aspects not related to nursing care. Results: Six studies were included, demonstrating a high level of patient satisfaction with triage, especially regarding nurses' skills and professional competence, courtesy, respect, interest, and empathy. However, pain management and confidence in treatment scored lower. Patient satisfaction was related to age, gender, reason for consultation, and perception of nurses' professional competence. Discussion: Similar opinions were observed regarding areas for improvement in care and nurses' skills in triage. The need for qualitative research and more rigorous sampling methods was highlighted. Conclusions: Nurse training and education in triage are crucial for patient satisfaction. Further research is needed to evaluate the effectiveness of nursing interventions in triage and address remaining challenges.
The reliability and validity of triage tools in geriatric emergency departments: A scoping review

Abstract:Background: The nurse's ability to accurately identify urgent conditions and triage this vulnerable population tends to be complex and challenging. Little is known about the reliability and validity of common triage tools in geriatric patients. Aim: To determine the reliability and validity of triage tools in geriatric emergency care and summarize the specific content of current triage tools for geriatric patients. Methods: The eligible literature was searched from the MEDLINE, CINAHL, EMBASE, and Cochrane Database using targeted search strategies. We defined the objectives and questions, set standards for article inclusion criteria, and conducted literature searching and screening. The mixed methods assessment tool (MMAT) appraised the article's quality. Finally, we extracted and analyzed the data from the included articles, summarizing the results. Endnote X9 was used for data extraction and collation. Results: Nine articles were eligible. These included six triage tools: CTAS, JTAS, KTAS, MTS, SETS, and ESI. The reliability of the CTAS was good when applied to triage geriatric patients. The SETS performed well in prehospital simulated triage. The ESI has moderate to excellent reliability. The CTAS has good to excellent validity, while the JTAS, KTAS, MTS, and ESI have fair to good results. Conclusion: Several triage tools are useful in geriatrics, but the reliability and validity of these tools have mixed results. Applying triage tools to triage geriatric patients still has limitations.

Abstract:Background Patients are increasingly turning to the Internet for health information. Numerous online symptom checkers and digital triage tools are currently available to the general public in an effort to meet this need, simultaneously acting as a demand management strategy to aid the overburdened health care system. The implementation of these services requires an evidence-based approach, warranting a review of the available literature on this rapidly evolving topic. Objective This scoping review aims to provide an overview of the current state of the art and identify research gaps through an analysis of the strengths and weaknesses of the presently available literature. Methods A systematic search strategy was formed and applied to six databases: Cochrane library, NICE, DARE, NIHR, Pubmed, and Web of Science. Data extraction was performed by two researchers according to a pre-established data charting methodology allowing for a thematic analysis of the results. Results A total of 10,250 articles were identified, and 28 publications were found eligible for inclusion. Users of these tools are often younger, female, more highly educated and technologically literate, potentially impacting digital divide and health equity. Triage algorithms remain risk-averse, which causes challenges for their accuracy. Recent evolutions in algorithms have varying degrees of success. Results on impact are highly variable, with potential effects on demand, accessibility of care, health literacy and syndromic surveillance. Both patients and healthcare providers are generally positive about the technology and seem amenable to the advice given, but there are still improvements to be made toward a more patient-centered approach. The significant heterogeneity across studies and triage systems remains the primary challenge for the field, limiting transferability of findings. Conclusion Current evidence included in this review is characterized by significant variability in study design and outcomes, highlighting the significant challenges for future research. An evolution toward more homogeneous methodologies, studies tailored to the intended setting, regulation and standardization of evaluations, and a patient-centered approach could benefit the field.

A preliminary study of a novel emergency department nursing triage simulation for research applications

Abstract:Background: Studying the effect on functioning of the emergency department of disasters with a potential impact on staff members themselves usually involves table top and simulated patient exercises. Computerized virtual reality simulations have the potential to configure a variety of scenarios to determine likely staff responses and how to address them without intensive utilization of resources. To decide whether such studies are justified, we determined whether a novel computer simulation has the potential to serve as a valid and reliable model of on essential function in a busy ED. Methods: Ten experienced female ED triage nurses (mean age 51) mastered navigating a virtual reality model of triage of 4 patients in an ED with which they were familiar, after which they were presented in a testing session with triage of 6 patients whose cases were developed using the Emergency Severity Index to represent a range of severity and complexity. Attitudes toward the simulation, and perceived workload in the simulation and on the job, were assessed with questionnaires and the NASA task load index. Z-scores were calculated for data points reflecting subject actions, the time to perform them, patient prioritization according to severity, and the importance of the tasks. Data from questionnaires and scales were analyzed with descriptive statistics and paired t tests using SPSS v. 21. Microsoft Excel was used to compute a correlation matrix for all standardized variables and all simulation data. Results: Nurses perceived their work on the simulation task to be equivalent to their workload on the job in all aspects except for physical exertion. Although they were able to work with written communications with the patients, verbal communication would have been preferable. Consistent with the workplace, variability in performance during triage reflected subject skill and experience and was correlated with comfort with the task. Time to perform triage corresponded to the time required in the ED and virtual patients were prioritized appropriately according to severity. Conclusions: This computerized simulation appears to be a reasonable accurate proxy for ED triage. If future studies of this kind of simulation with a broader range of subjects that includes verbal communication between virtual patients and subjects and interactions of multiple subjects, supports the initial impressions, the virtual ED could be used to study the impact of disaster scenarios on staff functioning.
Optimizing triage education for emergency room nurses: A scoping review

Abstract:Aims: Accurate triage decisions by emergency room nurses are pivotal for patient prognosis and efficient utilization of resources. This study aimed to identify teaching methods, contents, intervention characteristics, and initial consideration of educational design for the development of triage education, targeting triage nurses. Design: A scoping review. Data sources: PubMed, CINAHL, Web of Science, Embase, and RISS were searched for studies in either English or Korean, regardless of publication year. Review methods: The review was conducted according to the Joanna Briggs Institute guidelines. Studies published before November 15, 2023 were selected, based on the following index terms in each database: nurses, triage, education, and emergency services, hospital. Results: Of the 20 studies included in this review, five focused on severity classification of patients with cardiovascular diseases, one addressed infectious diseases, two examined pediatric patients, one explored patients with trauma, and the remaining eleven were not limited to specific diseases. Eleven studies (55 %) employed face-to-face (offline) education, whereas six (30 %) used non-face-to-face (online) education. The teaching methods were classified as teacher-centered learning and student-centered learning. The educational strategies included in-person lectures, online classes, demonstrations, simulations, mobile technology or web-based programs, group discussions, role-plays, and flipped learning. Outcome variables, such as triage accuracy, knowledge, performance ability, self-efficacy, satisfaction, wait time, and competency were measured as intervention effects. Conclusions: This review demonstrates the key characteristics and contents of triage education interventions, along with key considerations in the initial design stages. Triage education covers a wide range of contents and diverse teaching methods pertinent to severity classification in triage practice. Effective educational programs hinge on the meticulous planning of objectives, optimal selection of the target population, needs assessment, and suitable teaching methods and materials. Future triage education for emergency room nurses should be tailored to specific participants while anticipating and planning all potential circumstances of implementation.
Unique triage methods in the paediatric emergency roomof a low resource setting

Abstract:Introduction: Despite its importance in emergency medicine, the practice of triage in low- and middle-income countries remains rudimentary, due to dearth of human and material resources, among other reasons. Subjects and methods: In the Children Emergency Room of a teaching hospital, some nonclinical staff were intermittently deployed to coordinate patient traffic and provide brief first- contact triage on arrival. For this, they were internally trained in triage. The present study was aimed at assessing the value and accuracy of first-contact triage provided by nonclinical staff in a children emergency department of a tertiary hospital. Results: Data of 45 patients seen during the period were obtained from the records of the triage officers and included their age, gender, presenting complaints and the findings of the paediatric assessment triangle. There was a statistically significant relationship between the triage ratings of the nonCC –BY 4.0 clinical staff and the qSOFA / L- qSOFA scores obtained by the emergency room doctors, the time spent in CHER and the total duration of admission (R2= 0.36 - 0.44; p = 0.003), (R2= 0.23 - 0.28; p = 0.018) and (R2= 0.33 - 0.40; p = 0.007) respectively. Conclusion: This study shows that to a high degree, the nonclinical staff correctly categorized the severity of illness in the Paediatric Emergency Department of a tertiary level hospital, thus limiting undue tertiary delay in treatment and potential harm from such delays. Hence, the study exposes a potential source of human resources for paediatric emergency triage in LMICs where there is a dearth of professional medical personnel.
Safety in Teletriage by Nurses and Physicians in the United States and Israel: Narrative Review and Qualitative Study

Abstract:Background: The safety of telemedicine in general and telephone triage (teletriage) safety in particular have been a focus of concern since the 1970s. Today, telehealth, now subsuming teletriage, has a basic structure and process intended to promote safety. However, inadequate telehealth systems may also compromise patient safety. The COVID-19 pandemic accelerated rapid but uneven telehealth growth, both technologically and professionally. Within 5-10 years, the field will likely be more technologically advanced; however, these advances may still outpace professional standards. The need for an evidence-based system is crucial and urgent. Objective: Our aim was to explore ways that developed teletriage systems produce safe outcomes by examining key system components and questioning long-held assumptions. Methods: We examined safety by performing a narrative review of the literature using key terms concerning patient safety in teletriage. In addition, we conducted system analysis of 2 typical formal systems, physician led and nurse led, in Israel and the United States, respectively, and evaluated those systems' respective approaches to safety. Additionally, we conducted in-depth interviews with representative physicians and 1 nurse using a qualitative approach. Results: The review of literature indicated that research on various aspects of telehealth and teletriage safety is still sparse and of variable quality, producing conflicting and inconsistent results. Researchers, possibly unfamiliar with this complicated field, use an array of poorly defined terms and appear to design studies based on unfounded assumptions. The interviews with health care professionals demonstrated several challenges encountered during teletriage, mainly making diagnosis from a distance, treating unfamiliar patients, a stressful atmosphere, working alone, and technological difficulties. However, they reported using several measures that help them make accurate diagnoses and reasonable decisions, thus keeping patient safety, such as using their expertise and intuition, using structured protocols, and considering nonmedical factors and patient preferences (shared decision-making). Conclusions: Remote encounters about acute, worrisome symptoms are time sensitive, requiring decision-making under conditions of uncertainty and urgency. Patient safety and safe professional practice are extremely important in the field of teletriage, which has a high potential for error. This underregulated subspecialty lacks adequate development and substantive research on system safety. Research may commingle terminology and widely different, ill-defined groups of decision makers with wide variation in decision-making skills, clinical training, experience, and job qualifications, thereby confounding results. The rapid pace of telehealth's technological growth creates urgency in identifying safe systems to guide developers and clinicians about needed improvements.
Enhancing triage accuracy: The influence of nursing education on risk prediction

Abstract:Aim: This study aimed to compare the performance in risk prediction of various outcomes between specially trained triage nurses and the Manchester Triage System (MTS). Design: Prospective observational study. Methods: The study was conducted from June 1st to December 31st, 2023, at the Emergency Department of Merano Hospital. Triage nurses underwent continuous training through dedicated courses and daily audits. We compared the risk stratification performed by expert nurses with that of MTS on various outcomes such as mortality, hospitalisation, and urgency defined by the physicians. Comparisons were made using the Areas Under the Receiver Operating Characteristic curve (AUROC). Results: The agreement in code classification between the MTS and the expert nurse was very low. The AUROC curve analysis showed that the expert nurse outperformed the MTS in all outcomes. The triage nurse's experience led to statistically significant better stratification in admission rates, ICU admissions, and all outcomes based on the physician's assessment. Conclusions: The continuous training of nurses enables them to achieve better risk prediction compared to standardized triage systems like MTS, emphasizing the utility and necessity of implementing continuous training pathways for these highly specialised personnel.
A novel triage framework for emergency department based on machine learning paradigm

Abstract:Emergency departments, crucial in managing patient emergencies, are often challenged by overcrowding and diagnostic errors during triage—the process that assesses the urgency of patients' conditions. Traditional triage systems, heavily dependent on human judgement, are prone to errors like under‐triage, where severe conditions are missed, delaying treatment, and over‐triage, where less severe conditions are overly prioritized, causing unnecessary resource use and morbidity. This thesis presents a novel multi‐model machine‐learning framework designed to enhance triage accuracy by evaluating multiple medical conditions concurrently, including chronic illnesses like heart disease. The framework employs various machine‐learning techniques—such as logistic regression, support vector machines, random forests, deep neural networks, and decision trees—to analyze different medical conditions in two comprehensive phases. In the first phase, patients' conditions are categorized, and a multi‐tiered analysis using multiple classifiers refines the assessment by considering probabilistic outcomes from each classifier. The second phase synthesizes these insights into a unified and precise triage decision, distinguishing between patients who require critical care and those needing less urgent hospitalization. This integration of diverse machine learning models allows for a fused and precise triage decision, overcoming traditional triage systems' limitations that usually focus on single conditions and rely on isolated model predictions. A hybrid feature selection method is also utilized to identify critical predictors, enhancing the decision‐making process. The framework is validated through a specially curated dataset that simulates multiple triage scenarios, evaluated by medical experts for efficacy. Comparative analysis with traditional triage methods demonstrates significant improvements in decision accuracy, as evidenced by higher area under curve (AUC) values—0.95 for critical care and 0.90 for hospitalization. The implementation of this data‐driven approach substantially reduces human error, boosts operational efficiency, and aids medical staff in making rapid, informed decisions. This research represents a significant advancement in emergency medical care, illustrating the benefits of integrating sophisticated machine learning techniques to improve patient outcomes and resource management.

Abstract:Objectives Emergency department overcrowding could be improved by upstream telephone triage. Emergency telephone triage aims at managing and orientating adequately patients as early as possible and distributing limited supply of staff and materials. This complex task could be improved with the use of Clinical decision support systems (CDSS). The aim of this scoping review was to identify literature gaps for the future development and evaluation of CDSS for Emergency telephone triage. Materials and Methods We present here a scoping review of CDSS designed for emergency telephone triage, and compared them in terms of functional characteristics, technical design, health care implementation and methodologies used for evaluation, following the PRISMA-ScR guidelines. Results Regarding design , 19 CDSS were retrieved: 12 were knowledge based CDSS (decisional algorithms built according to guidelines or clinical expertise) and 7 were data driven (statistical, machine learning, or deep learning models). Most of them aimed at assisting nurses or non-medical staff by providing patient orientation and/or severity/priority assessment. Eleven were implemented in real life, and only three were connected to the Electronic Health Record. Regarding evaluation , CDSS were assessed through various aspects: intrinsic characteristics, impact on clinical practice or user apprehension. Only one pragmatic trial and one randomized controlled trial were conducted. Conclusion This review highlights the potential of a hybrid system, user tailored, flexible, connected to the electronic health record, which could work with oral, video and digital data; and the need to evaluate CDSS on intrinsic characteristics and impact on clinical practice, iteratively at each distinct stage of the IT lifecycle.

Enhancing the fairness of pandemic critical care triage

Abstract:Historically, the triage of temporarily scarce health resources has served narrow utilitarian ends. The recent H1N1 pandemic experience provided an opportunity for expanding the theoretical foundations/understandings of critical care triage in the context of declared infectious pandemics. This paper briefly explores the ethics-related challenges associated with the development of modern critical care triage protocols and provides descriptions of some 'enhanced fairness' features which were developed through the use of an inclusive deliberative engagement process by a Canadian provincial Department of Health.

Abstract:Background and Objectives: Triage systems help provide the right care at the right time for patients presenting to emergency departments (EDs). Triage systems are generally used to subdivide patients into three to five categories according to the system used, and their performance must be carefully monitored to ensure the best care for patients. Materials and Methods: We examined ED accesses in the context of 4-level (4LT) and 5-level triage systems (5LT), implemented from 1 January 2014 to 31 December 2020. This study assessed the effects of a 5LT on wait times and under-triage (UT) and over-triage (OT). We also examined how 5LT and 4LT systems reflected actual patient acuity by correlating triage codes with severity codes at discharge. Other outcomes included the impact of crowding indices and 5LT system function during the COVID-19 pandemic in the study populations. Results: We evaluated 423,257 ED presentations. Visits to the ED by more fragile and seriously ill individuals increased, with a progressive increase in crowding. The length of stay (LOS), exit block, boarding, and processing times increased, reflecting a net raise in throughput and output factors, with a consequent lengthening of wait times. The decreased UT trend was observed after implementing the 5LT system. Conversely, a slight rise in OT was reported, although this did not affect the medium-high-intensity care area. Conclusions: Introducing a 5LT improved ED performance and patient care.

Abstract:Minoritized patients are disproportionately represented in low-acuity emergency department (ED) visits in the United States in part caused by lack of timely access to primary and urgent care. However, there is also the possibility that implicit bias during triage could contribute to disproportionate representation of minority groups in low-acuity ED visits. Triage discordance, defined as when ED resources used are different from initial triage score predictions, can be used as a proxy for triage accuracy. Recent data suggest that discordant triage may be common, although little is known about the interaction with race, ethnicity, and language for care.

Abstract:Aim Although there has been a significant amount of research on nurse workloads in hospital environments, there has been a lack of comprehensive investigation of the workload of triage nurses. This study assesses the workload fluctuations of triage nurses in various emergency departments in the same region and examines how workload varies based on emergency department visit volumes and seasonal fluctuations. Design The study is a retrospective, multicentre, observational study from January 1, 2019, to December 31, 2023, in seven Italian emergency departments, gathering information on all patients who went through the triage process. Methods We documented the number of nurses present throughout each shift and adjusted the data to account for the number of patients attended to by each triage nurse per shift. The EDs were classified into high‐inflow, medium‐inflow and low‐inflow categories according to the annual number of patients they served. Results The study included 1,223,331 emergency department triage assessments. During daytime shifts, triage nurses in high‐inflow emergency departments assessed a median of 74 patients, while those in medium‐inflow and low‐inflow emergency departments assessed 63 and 32 patients, respectively. During nighttime shifts, triage nurses in high‐inflow emergency departments assessed 26 patients, compared to 14 in medium‐inflow and 5 in low‐inflow emergency departments. Significant seasonal fluctuations in the workload of triage nurses were observed. Conclusions This study reveals substantial disparities in the number of patients triage nurses assess, which vary according to emergency department patient inflow volumes and seasonal factors. The findings emphasise the need for more standardised and equitable workload distribution among triage nurses. Future research should aim to establish a systematic nurse staffing model for triage to ensure manageable workloads and maintain patient safety.
