Abstract:Background: Mycoplasmas are the smallest prokaryotic microorganisms found in nature. Mycoplasma pneumoniae (M. pneumoniae) is the most commonly studied among human mycoplasmas. Objectives: In this review, we briefly focus on the recent developments that have enhanced our understanding of M. pneumoniae, one of the smallest pathogenic bacteria of great clinical importance in children. Content: M. pneumoniae infections may involve either upper or lower respiratory tract or both of them. Extrapulmonary manifestations have been reported in almost every organ, including the skin and the hematologic, cardiovascular, musculoskeletal, and nervous system due to direct local effects, after dissemination of bacteria or indirect effects. The correct identification of M. pneumoniae infections is vital for prescription of the appropriate therapy.There are scarce specific findings of clinical laboratory results for the diagnosis of M. pneumoniae infection. Detection of M. pneumoniae infections can be achieved using culture, serology, or molecular-based methods. Culture is time-consuming, laborious, and expensive. The major types of serological tests for M. pneumoniae include the microtiter plate enzyme immunoassay (EIA), the membrane EIA, indirect immunofluorescence, and particle agglutination. Nucleic acid amplification tests (NAATs) include traditional PCR, nested PCR, real-time quantitative PCR, nucleic acid sequence-based amplification (NASBA), loop-mediated isothermal amplification (LAMP) technology, and RNA simultaneous amplification and testing (SAT). Macrolides have been the drug of choice for treating M. pneumoniae infection in past years. Clinically significant acquired macrolide-resistant M. pneumoniae (MRMP)has emerged worldwide which may be associated with more extrapulmonary complications, and severe clinical and radiological features. Since molecular-based assays can detect M. pnueumoniae in clinical specimens, there is a need for real point of care testing for fast detection of M. pneumoniae or its DNA and mutations in macrolide resistance gene. It is necessary to develop safe vaccines that provide protective immunity against M.pneumoniae infection.

Abstract:A clinical association between exacerbation of asthma symptoms and Mycoplasma pneumoniae ( M. pneumoniae) infection has long been suspected. We studied 80 children aged 5–15 years; 50 with asthma (Group 1) and 30 without an acute exacerbation of asthma (Group 2) for detection of M. pneumoniae by serology and polymerase chain reaction (PCR) on nasopharyngeal aspirates. Our study confirms that lower respiratory tract infections with M. pneumoniae are frequently associated with exacerbations of asthma in children.

Abstract:Mycoplasma pneumoniae is a major cause of communityacquired respiratory infections in children and adults. A number of severe pulmonary complications have been reported with M. pneumoniae infection. These complications include obliterative bronchitis, bronchiectasis, pleural effusion, interstitial fibrosis, lung abscess, acute respiratory distress syndrome (ARDS) and necrotizing pneumonitis. Necrotizing pneumonitis, which has been described as a suppurative complication of Streptococcus pneumoniae infection, is a rare manifestation of M. pneumoniae infection. Herein we report on five pediatric patients with necrotizing pneumonitis due to M. pneumoniae and review the relevant literature.

Abstract:OBJECTIVE:To study the pathogenic bacteria of lower respiratory tract infection (LRTI), and age and gender distribution and drug resistance of the pathogenic bacteria in children.METHODS:Sputum specimens for bacterial cultures were collected in sterile tubes from all of the children with LRTI who had been admitted to the Children's Hospital of Zhejiang University between August 2001 and July 2002. Antibiotic susceptibility tests were performed using the Vitek system, the Kirby-Bauer diffuse method and the Etest method after bacteria were identified.RESULTS:Among the 4,238 patients with LRTI during the study period, 1,181 patients were bacteria-positive, with a positive rate of 27.9%. Streptococcus pneumoniae (S. pneumoniae) was the most common (222 strains), followed by Haemophilus influenzae (H. influenzae) (216 strains), Klebsiella pneumoniae (K. pneumoniae) (216 strains), Escherichia coil (E. coli) (169 strains) and Staphylococcus aureus (S. aureus) (89 strains). The isolation rate of S. pneumoniae in females was significantly higher than in males (6.2% vs 4.7%; P < 0.05). However, the isolation rates of K. pneumoniae and S. aureus in males were higher than in females (5.1% vs 4.1% and 2.5% vs 1.5%, respectively; P < 0.05). A higher incidence of LRTI due to S. pneumoniae and H. influenzae was found in the 1-3 years group, while the incidence of LRTI due to K. pneumoniae, E. coli, S. aureus and E. cloacae was higher in patients under 1 year of age. Antibiotic susceptibility tests showed that rates of penicillin non-susceptible S. pneumoniae, ampicillin resistant H. influenzae, oxacillin-resistant S. aureus and ESBL-positive K. pneumoniae and E. coli were 55.0%, 16.5%, 41.2%, 42.6% and 4.5%, respectively.CONCLUSIONS:S. pneumoniae, H. influenzae, K. pneumoniae, E. coli and S. aureus were common pathogens of LRTI in children. The infection rate varied with age and gender. Antibiotics for treating LRTI should be selected based on the drug susceptibility test.

Abstract:Mycoplasma pneumoniae(MP)remains to be an important pathogen of childhood community-acquired pneumonia,and also has close relationship to asthma exacerbation. There is a much higher incidence of MP infection in older children than younger children. The latter,although with a lower infection rate,usually show a more severe clinical picture,and is prone to have tachypnea and retractions. High fever and irritative cough are characteristics of MP infections in older children. Serological method is the standard one for diagnosing MP infection,while PCR is more suitable for early diagnosis of MP infection in younger and immunocompromised patients. Macrolide is still the drug of choice for treatment of childhood MP pneumonia,but how to manage the refractory MP pneumonia has become a big challenge to clinical researchers.

Abstract:Background: Mycoplasma pneumoniae (M. pneumoniae) is an important pathogen of community-acquired pneumonia in children, but the epidemiology and clinical features of M. pneumoniae infection in infants are reported in only a few studies. This study aims to evaluate possible age-related differences in the presenting clinical features in infants with M. pneumoniae infection.Methods: 24-month longitudinal study on lower respiratory tract infection (LRTI) caused by M. pneumoniae, confirmed by both serology and polymerase chain reaction, was performed. Medical records of patients were reviewed for demographic, clinical and microbiological characteristics.Results: Out of 2174 infants with LRTI admitted to the Children's Hospital Affiliated to Soochow University in Jiangsu Province, 80 were diagnosed with M. pneumoniae infection. We found that 15 were aged 1 to < 5 months; 29 were aged 5 to < 9 months; and 36 were aged 9 to < 12 months. M. pneumoniae infection mainly occurred in August to October. The presence of fever with a maximum temperature of > 39.0 degrees C for >= 3 days was more common in the 9 to < 12 month age group (P < 0.05). Laboratory tests showed that the infants aged 9-12 months had a higher peripheral leukocyte (P = 0.035) and neutrophil (P = 0.015) count and a higher CRP level (P = 0.041). Moreover, the median length of hospitalization for infants aged 1 to < 5 months was shorter than that in the other two groups.Conclusion: Our work provides important clinical information about infants with M. pneumoniae infection and highlights that younger infants with M. pneumoniae infection may have a milder clinical course than older infants. Abstract:To investigate the incidence and clinical features of mixed infections in children with Mycoplasma pneumoniae (MP) pneumonia.A total of 201 cases diagnosed as MP pneumonia were investigated for mixed infections by sputum bacterial culture, respiratory virus antigen detection and serum Chlamydia pneumoniae antibody test. For those with the indications for bronchoscopy, we also did bronchoalveolar lavage and lavage bacterial culture.A high incidence (103/201, 51.2%) of mixed infections in children with MP pneumonia was revealed. The most frequent co-infected pathogen was Chlamydia pneumoniae (52, 25.9%), followed by viruses (29, 14.4%), and bacteria (22, 10.9%). Among viruses, respiratory syncytial virus was the most common (17, 8.5%), followed by adenovirus (6, 3.0%), parainfluenza virus type III (4, 2.0%) and influenza virus type B (2, 1.0%). Sputum bacterial culture was positive in 14/201 (7.0%) cases, Streptococcus pneumonia being most common (6, 3.0%). BALF culture yielded positive results in 11.6% (8/69), Streptococcus pneumonia was also common (5, 7.3%). Among 29 cases with MP and virus coinfection, 26 were younger than 3 years (89.7%), while for MP and Chlamydia pneumoniae coinfection, most of them were older than 3 years (40/52, 76.9%). Compared with non-mixed infections, those with mixed infections had longer fever duration (24.5% and 40.8% longer than 10 d), more frequently developed pleural effusion (11.2%, 23.3%) and large area of shadow in chest imaging (35.7%, 51.5%). White blood cell [(14.28 ± 4.99) × 10(9)/L], C-reactive protein (CRP) [69(32.5 - 99.5) mg/L] and neutrophil ratio in BALF [0.86 (0.63 - 0.91)] were much higher in children with mixed bacterial infections than that in non-mixed infections [(9.06 ± 3.47) × 10(9)/L, 3 (0 - 31.0) mg/L, 0.44 (0.03 - 0.88)]. But no significant difference was found in peripheral blood neutrophil proportion between mixed bacterial infections (0.38 ± 0.25) and non-mixed infections (0.51 ± 0.19).More than half of cases with MP pneumonia had mixed infections, most commonly caused by Chlamydia pneumonia followed by viruses. The incidence of mixed infections with bacteria was low. Mixed infections with virus were more common in young children, while mixed infection with Chlamydia pneumoniae was more common in older ones. Bacterial infections should be paid more attention, especially those caused by Streptococcus pneumoniae, for those with high peripheral white blood cell counts, high CRP levels and high proportion of neutrophils in BALF.

Abstract:Mycoplasma pneumoniae ( M. pneumoniae ) is an important community-acquired pneumonia pathogen. Serological test and polymerase chain reaction (PCR) assay are the two main laboratory tests to detect M. pneumoniae now. Little information was compared about the sensitivity and specificity of PCR using different specimens including bronchoalveolar lavage (BAL) and nasopharyngeal aspirate (NPA). The aim of the present study was to evaluate diagnostic values of different specimens by fluorescence quantitative real-time PCR and to find clinical features helpful to diagnose M. pneumoniae pneumonia (MPP). Four hundred and six hospitalized pneumonia children were studied. M. pneumoniae DNA in NPA and BAL samples were detected by fluorescence quantitative real-time PCR. M. pneumoniae -specific IgM was tested by ELISA. MPP were diagnosed based on positive M. pneumoniae -specific IgM in 101 (24.9%) children. The median ages of MPP and non-MPP children were 4.1 and 2.4 years, respectively, with significant difference between them ( p < 0.001). Laboratory results including leukocyte count, neutrophil percentage, immunoglobulins, except serum IgM, subgroups of T lymphocyte, and BAL cell count had no significant differences in MPP and non-MPP. BAL macrophage cell percentage was lower in BAL-PCR positive children ( p = 0.003), while BAL neutrophil percentage was higher in BAL-PCR positive children ( p = 0.007). PCR from NPA and BAL were similar in diagnostic parameters, including sensitivity, specificity, PPV, and NPV (78.6%, 63.4%, 39.8%, and 90.6% for NPA-PCR, respectively; 70.3%, 58.7%, 36.0%, and 85.6% for BAL-PCR, respectively). Conclusions: NPA is better than BAL as PCR sample in MPP diagnosis for similar performance in PCR assay, cheap, and less invasive. BAL is useful in defining local inflammatory condition. Age is the only prefigurative factor in MPP.

Abstract:The purpose of this study was to determine the clinical features of viral pathogens and Mycoplasma pneumoniae in children hospitalized with community-acquired pneumonia (CAP). M. pneumoniae infection was diagnosed by both serology and polymerase chain reaction (PCR). Respiratory viruses were detected by direct immunofluorescence or PCR. Medical records of children younger than 5 years diagnosed with 5-day-old CAP were reviewed. Viral pathogens and/or M. pneumoniae were detected in 388 (15.59%) children in the following three groups: viral monoinfection (n = 321), M. pneumoniae with viral coinfection (n = 17), and M. pneumoniae monoinfection (n = 50). M. pneumoniae monoinfection was characterized by older age, fever, higher neutrophil count, and chest X-ray showing lobar consolidation. Wheezing was more common in children with viral infections. Elevated alanine aminotransferase and aspartate aminotransferase were commonly seen in children with Mycoplasma infections. The median symptom duration in children with viral coinfection was shorter than in the other two groups (both p < 0.05). M. pneumoniae and respiratory viruses are important etiologic agents for CAP in children younger than 5 years, with characteristic clinical features. M. pneumoniae and viral coinfection are associated with shorter duration of symptoms before admission.

Compared with non-mixed infections,those with mixed infections had longer fever duration (24.5％ and 40.8％ longer than 10 d),more frequently developed pleural effusion ( 11.2％,23.3％ ) and large area of shadow in chest imaging (35.7％,51.5％ ).White blood cell[ ( 14.28 ±4.99) × 109/L],Creactive protein (CRP) [69(32.5-99.5) mg/L] and neutrophil ratio in BALF[0.86(0.63-0.91 ) ] were much higher in children with mixed bacterial infections than that in non-mixed infections [ (9.06 ± 3.47) ×109/L,3 (0-31.0) mg/L,0.44 (0.03-0.88 ) ].But no significant difference was found in peripheral blood neutrophil proportion between mixed bacterial infections (0.38 ± 0.25 ) and non-mixed infections(0.51 ±0.19).Conclusion More than half of cases with MP pneumonia had mixed infections,most commonly caused by Chlamydia pneumonia followed by viruses.The incidence of mixed infections with bacteria was low.Mixed infections with virus were more common in young children,while mixed infection with Chlamydia pneumoniae was more common in older ones.Bacterial infections should be paid more attention,especially those caused by Streptococcus pneumoniae,for those with high peripheral white blood cell counts,high CRP levels and high proportion of neutrophils in BALF.

Abstract:Mycoplasma pneumoniae is a leading cause of community-acquired pneumonia among school-aged children and young adults. Infections occur throughout the year but tend to surge during winter months across Europe. A characteristic epidemic cycle, where a substantial surge in the number of infections occurs, is seen approximately every 1–4 years and hypothesized to be driven by changes in immunity and a shift in circulating variants. Once thought to be an organism of low virulence, it has now been found to possess several virulence factors, including toxin production, biofilm formation and evasion of antibody-mediated immunity. The lack of a cell wall and reduced metabolic pathways limit the options for antibiotic treatment. Acquired macrolide resistance is a growing concern, with >80% of cases in China being macrolide-resistant. Although efforts have been made to develop a vaccine, there are still substantial hurdles to overcome in relation to vaccine-enhanced disease, which results from an inappropriate immune response among vaccinated individuals.

Abstract:Objective To summarize the epidemiology and evaluate possible age-related differences in the presenting clinical features in three year-old children with Mycoplasma pneumoniae pneumonia(MPP)during 2009 to 2014.Methods The medical records of 17855 children with community-acquired pneumonia enrolled by Children′s Hospital of Soochow University during 2009 to 2014.Totally 1145 younger than three year-old children with MPP were enrolled,and they were classified into three groups of 1 month to 1 year-old group (n=512),1 to 2 year-old group (n=393) and 2 to 3 year-old group (n=240).The epidemiology and possible age-related differences in the presenting clinical features,main laboratory and imaging results in three year-old children with MPP were summarized.Results (1) The highest infection rate of different reasons in Suzhou was in autumn(10.46%),and the lowest was in spring(6.95%),The highest infection rate of different ages was 2 to 3 year-old group(11.61%),and the difference was statistically significant (P<0.05).(2) Compared with 1 month to 1 year-old group(n=512,4.31%) and 1 to 2 year-old group (n=39,10.09%),2 to 3 year-old group (n=240,11.61%) had higher infection rate,more patients with fever(53.9% vs. 77.1% vs. 85.4%) and high fever(16.8% vs. 30.5% vs. 41.4%),longer time to normalization of temperature(3d vs. 5d vs. 6d),but less patients with wheezing(61.3% vs. 52.4% vs. 42.9%) and dyspnea(7.6% vs. 4.6% vs. 3.8%).(3) The differences were statistically significant in the percentage of neutrophils,lymphocytes,CRP and platelet count between different ages(P<0.001).(4) The incidences of large area of lesions in chest X-ray examination among different ages had significant differences(P<0.001).Conclusions MP is one of the important pathogens of respiratory tract infection in younger than 3 year-old children.It can occur in every season,and the highest infection rate of MP is in autumn and spring.In younger than 3 year-old,older patients are more vulnerable to infection of MP,the number of fever and high fever are more,fever duration is longer,but younger patients are more vulnerable to virus infection and prone to dyspnea.Chest X-ray examination shows small patchy shadow in most cases,the younger children are more easily to have large area of lesions and pleural effusion.

Abstract:Objectives: This retrospective study was aimed to explore the epidemiological and clinical profiles of Mycoplasma pneumoniae infection in neonates.Methods: From 2011 to 2014, 1322 hospitalized neonates with lower respiratory tract infections were screened for Mycoplasma pneumoniae by detection of Mycoplasma pneumoniae antibodies using Serion ELISA classic Mycoplasma pneumoniae kits.Results: Mycoplasma pneumoniae was identified in 89 (6.7%) patients. The age ranged from 1 day to 28 days with a median of 22 days. The male to female ratio was 1.15:1. Mycoplasma pneumoniae infection peaked in spring (from March through May) and winter (from December through February). Compared with non-Mycoplasma pneumoniae infected neonates, those with Mycoplasma pneumoniae infection were older, presented fever more frequently, and had less tachypnea.Conclusions: Mycoplasma pneumoniae could be an important etiologic agent for respiratory tract infection in neonates. In neonates Mycoplasma pneumoniae infection was usually associated with older age, presence of fever, and less tachypnea. Mycoplasma pneumoniae infection in neonates tends to be a mild process. Abstract:Mycoplasma pneumoniae is an important cause of respiratory tract infections in children as well as adults that can range in severity from mild to life-threatening. Over the past several years there has been much new information published concerning infections caused by this organism. New molecular-based tests for M. pneumoniae detection are now commercially available in the United States, and advances in molecular typing systems have enhanced understanding of the epidemiology of infections. More strains have had their entire genome sequences published, providing additional insights into pathogenic mechanisms. Clinically significant acquired macrolide resistance has emerged worldwide and is now complicating treatment. In vitro susceptibility testing methods have been standardized, and several new drugs that may be effective against this organism are undergoing development. This review focuses on the many new developments that have occurred over the past several years that enhance our understanding of this microbe, which is among the smallest bacterial pathogens but one of great clinical importance.
Clinical Characteristics of Pneumonia Caused by Mycoplasma Pneumoniae in Children of Different Ages.

Abstract:To study clinical characteristics of Mycoplasma pneumoniae pneumonia (MPP) in children of different ages. We investigated the medical records of MPP patients admitted at Children's Hospital of Soochow University from January 2006 and December 2013. The presence of MP was confirmed by real-time PCR and ELISA. There were overall 3358 children with MPP. 412 (12.3%) were <6 months old. Six months to 1 year, 1-3 years, 3-5 years and ≥5 years old children constituted 423 (12.6%), 1033 (30.8%), 733 (21.8%) and 757 (22.5%) respectively. Fever was less frequent in <6 months old infants, with frequency increasing in older patients. The proportion of patients with wheeze was highest (57.9%) in patients aged 6 months to 1-year-old. Tachypnea, cyanosis and hypoxemia were more prevalent in <6 months old infants (P=0.00). Radiologically, bronchopneumonia was frequent among infants, while segmental/lobar pneumonia was frequent in ≥5 years old patients. White blood cell and platelet counts of infants were significantly higher than other children. Neutrophil count, c-reaction protein levels was significantly different amongst age groups. Our study revealed MPP patients of different ages presented with different clinical findings especially <6 months old and ≥5 years of age, which may indicate different pathological findings in age groups of patients with MPP.

Abstract:Mycoplasma pneumoniae (MP) is a common pathogen that causes respiratory infections. Mycoplasma pneumoniae pneumonia (MPP) accounts for 10% to 40% of community-acquired pneumonia in children under 18 years old. [ 1 ] MPP usually has mild symptoms and manifests as a self-limited disease. However, in recent years, an increasing number of severe MPP [ 2 , 3 ] and refractory MPP (RMPP) [ 4 ] have emerged in clinical practice, posing great challenges to pediatricians. RMPP is a disease that does not improve after 7 days of treatment with standard macrolide drugs. [ 5 ] Macrolides are the preferred drugs for treating childhood MP infections, but with increasing clinical use, drug resistance issues have become more severe, and RMPP cases are also increasing year by year. [ 6 ] The most common clinical manifestations of MPP include dry cough and fever, usually accompanied by headache, myalgia, sore throat, as well as laboratory tests (mainly elevated inflammatory markers) and abnormal imaging tests (chest computed tomography shows thickening of the bronchial wall, central lobular nodules, lymph node enlargement or ground glass attenuation, etc). [ 7 ] The duration of fever and hospitalization in RMPP is much longer, and the likelihood of developing extrapulmonary complications, including pleural effusion and multiple organ dysfunction, may also lead to more serious long-term sequelae, including obliterative bronchiolitis and bronchiectasis, [ 8 , 9 ] posing a serious threat to the life and health of children. [ 10 ]

Abstract:Background: Mycoplasma pneumoniae is a common cause of community-acquired pneumonia (CAP) in children and adolescents. Identifying the incidence and severity of this infection in individuals under the age of 18 can assist in improving the treatment and prevention process. Objectives: The present study aimed to evaluate the frequency of M. pneumoniae infection in young patients through a systematic review and meta-analysis. Methods: Three major databases (Medline (via PubMed), Embase, and Web of Science) were searched for research about M. pneumoniae infection in children. STATA (version 14) software was used to interpret the data. Results: Seventy-seven studies fulfilled the eligibility criteria to evaluate the prevalence of M. pneumoniae infection among children. The prevalence of this infection in young patients was 22.5%. The results revealed that children and adolescents in the age range of 10‒18 years old had the highest prevalence of M. pneumoniae (23.1%, 95% confidence interval [CI]=6.7‒39.4), followed by the 5‒10-year-old age group (21.6%, 95% CI=17.9‒25.3] and the under 5-year group (20.9%, 95% CI=16.7‒25.1). Conclusion: The global rate of M. pneumoniae infection has been gradually increasing since 2001. Finally, more extensive studies on the prevalence of M. pneumoniae infections in children throughout the world are needed to assess its exact prevalence and antibiotic resistance trend.
