Abstract:Homicides of dependent elderly and nonelderly adults by their caregivers violate trust and have long-term consequences for families. A better understanding of the characteristics of homicide by caregivers may provide insights that can inform prevention efforts. Data collected in the National Violent Death Reporting System (NVDRS) between 2003 and 2007 are used to characterize victims, perpetrators, and caregiver roles, and circumstances that precipitated homicides by a caregiver. A total 68 incidents are categorized into either homicide by neglect ( n = 17), intentional injury of the victim only (n = 21), or homicide followed by suicide of the perpetrator (n = 30). Demographics, mechanism of injury, location of injury, and victim-suspect relationship variables are supplemented by narrative accounts of incidents. In general, findings show that adult homicide victims of a caregiver were widowed (42.6%), non-Hispanic (97.1%), White (88.2%), women (63.2%) killed in their homes (92.6%) with a firearm (35.3%) or by intentional neglect (25.0%) by a husband (30.9%) or a son (22.1%). Nearly half were aged 80 years and older (48.5%), 42.6% were aged 50 to 79 years, and 0.9% were aged 20 to 49 years. Many homicide by caregiver incidents are precipitated by physical illness of the victim or caregiver, opportunity for perpetrator financial gain, mental illness of the caregiver, substance use by the caregiver, or an impending crisis in the life of the caregiver not related to illness. Understanding the vulnerabilities of victims, the characteristics of suspects, and the multiple types of motivations is key to developing effective prevention efforts.
Elder homicide in the north of Portugal

Abstract:Introduction: The increasing average life expectancy leads to population aging, and growing numbers in the elder population, a population that suffers from great vulnerability. Therefore, a rise in elder homicide is expected. This phenomenon has not been thoroughly described in Portugal. This study aims to analyse and characterize elder homicide, in order to achieve a better understanding of this form of violence. Material and methods: We conducted a retrospective study, reviewing autopsy reports along with data regarding circumstances of death of suspected homicides occurring between 1992 and 2007 (n = 78), whose victims were 65 years old or above, autopsied in the medical-legal services of the north of Portugal. Results: During this period, an overall of 615 suspected homicides occurred, 13% being perpetrated against elders. The age range of the victims was 65-96 years, with an average of 74 years; 58% of them were males and 45% were married. In 61% of the documented cases, the perpetrator was known to the victim, and a family member in 36%. A considerable number of deaths occurred at the victim's residence and in a rural scenario, the motive being robbery in 51% of the documented cases. The supposed perpetrator ran away from the scene in 64% of the cases. The most frequent mechanisms of death were blunt force trauma (31%), followed by stabbing (22%) and gunshot wounds (21%). Fatal wounds were observed mainly in the head (55%) and thorax (35%). Conclusions: In the north of Portugal, elder homicide seems to be uncommon. This study suggests an association between elder homicide and robbery, in which elders, due to their vulnerability, are a preferential target. Contrary to other reports worldwide, there were no cases of death in nursing homes, possibly due to underreport or misidentification. In a large number of the cases, it wasn't possible to collect all the necessary data, due to lack of information regarding circumstances of the events.
Family violence homicide rates: a state-wide comparison of three data sources in Victoria, Australia

Abstract:Background: Family violence homicide (FVH) is a major public health and social problem in Australia. FVH trend rates are key outcomes that determine the effectiveness of current management practices and policy directions. Data source-related methodological problems affect FVH research and policy and the reliable measurement of homicide trends. Objective: This study aimed to determine data reliability and temporal trends of Victorian FVH rates and sex and relationship patterns. Method: FVH rates per 100,000 persons in Victoria were compared between the National Coronial Information System (NCIS), Coroners Court of Victoria (CCoV) Homicide Register, and the National Homicide Monitoring Program (NHMP). Trends for 2001-2017 were analysed using Joinpoint regression. Crude rates were determined by sex and relationship categories using annual frequencies and Australian Bureau of Statistics population estimates. Results: NCIS closed FVH cases totalled 360, and an apparent downward trend in the FVH rate was identified. However, CCoV and NHMP rates trended upwards. While NCIS and CCoV were case-based, NHMP was incident-based, contributing to rate variations. The NCIS-derived trend was particularly impacted by unavailable case data, potential coding errors and entry backlog. Neither CCoV nor NHMP provided victim-age in their public domain data to enable age-adjusted rate comparison. Conclusion: Current datasets have limitations for FVH trend determination; most notably lag times for NCIS data. Implications: This study identified an indicative upward trend in FVH rates in Victoria, suggesting insufficiency of current management and policy settings for its prevention and control.

Abstract:Journal of Interpersonal Violence, Ahead of Print. This qualitative descriptive analysis examines 33 cases of missing and murdered Indigenous women aged 50 years and older. The cases encompassed single murders, multiple murders, and mass stabbing events. The study found that the offender was known and/or identified in the majority of cases, with a significant portion resulting in guilty pleas or jury convictions. However, a notable portion of cases remained unsolved or ended with the offender's suicide. Alcohol and/or illicit substance abuse was prevalent; known substance abuse history was identified in victim and/or offender for nearly 70% of cases. Most murders occurred off tribal land and were perpetrated by men, typically younger than their victims, with some form of relationship to them. Themes for the resolved cases varied, including familial violence, sexual violence, and financial gain. The findings underscore the need for intervention strategies such as addressing substance abuse in adolescence, intervening early in relationship conflicts, training law enforcement in elder sexual homicide investigations, and providing clinical care for mental illness in cases involving family and partners. Additionally, the study highlights the necessity for a national database to track homicides involving elder Indigenous women, facilitating more effective prevention and response efforts.

Abstract:The World Health Organization recently presented the priorities for tackling abuse of older people in a coordinated and strategic way. However, data on the forensic scenario is still lacking. In this context, the aim of the present work was to provide a comprehensive literature review of this inherently complex phenomenon in the post-mortem setting, in order to better characterize it from a forensic point of view. A comprehensive literature search was performed in three electronic databases following the PRISMA guidelines. Sociodemographic and medical data of victims and perpetrators, post-mortem data, types of abuse and risk factors were extracted from non-aggregated data. Forty-eight papers dealing with abuse in the post-mortem setting were included, with a predominance of case reports and case series. The review showed that neglect was the most common type of abuse and victims are predominantly older women who are abused in a domestic setting by trusted family member. To generate more and better data, expanded research in the forensic field requires standardized methods and the raise of professional awareness about abuse of older people.

Comparing child and adult sexual homicides in Australia and New Zealand: A retrospective study

Abstract:The present study examined distinctions between child (n = 30) and adult (n = 212) sexual homicide offenders (SHOs) in Australia and New Zealand, contributing to the limited international research on the subject. Data, primarily sourced from judges' sentencing comments on AustLII and New Zealand Legal Information Institute, revealed significant differences. Child SHOs displayed elevated rates of pedophilia, sexual deviance, and adverse childhood experiences, including sexual abuse. They were more likely to be married, cohabitate, and target familial victims. Their crimes were more often committed during daylight and outdoors, involving tactics such as victim conning, restraints, strangulation, and hiding victim's bodies. No significant group differences emerged regarding offenders' psychopathy or sexual sadism scores. Results were interpreted in line with child SHOs' deviant sexual preferences and the routine activity theory. The study, as the first investigating child sexual homicides in Australia and New Zealand, sets the foundation for an evidence-based approach to policy and practice.
Developing a Comprehensive Understanding of Older Person Abuse in Canadian Immigrant Communities: An Integrative Review

Abstract:Older immigrants represent 28% of the Canadian population who are over the age of 60. World-wide 1 in 6 older persons experiences abuse. Due to population aging, attention must be paid to the abuse and victimization of older immigrant persons, and the concept of elder abuse. The purpose of this integrative review was to understand elder abuse from the perspective of older immigrants, who came to Canada in their 60s or older as dependents of families or sponsors. Whittemore and Knafl's (2005) method of review resulted in six articles that met the inclusion criteria. Results revealed three themes: conceptualization of abuse, post-immigration stressors and cultural factors, and barriers to access support and protection. The perpetrators were often close family members including intimate partners, spouses, children, children-in-laws and grandchildren. Contextual factors that influenced abuse included: power imbalance, change in social status from head of the families to legal and financial dependents due to immigration, culture, ethnicity, gender role expectations and language barrier. More research is needed to understand the diverse older immigrants experiences of elder abuse.

Abstract:Although often perceived to be a problem of the young, violence commonly affects older adults, a rapidly growing segment of the population. Violence can be directed toward older adults (elder abuse and intimate partner violence), self-directed (suicide), or perpetrated by older adults against others (intimate partner violence and violence in dementia). Across forms of violence, firearm access increases lethality, and veterans may be a particularly high-risk population. The forms of violence in older adults have some common risk factors (such as medical or psychiatric illness) and common challenges for prevention (such as balancing autonomy and well-being in vulnerable adults). The integration of prevention strategies across the life span, disciplines, and forms of violence offers promise for promoting older adult health and well-being. Looking forward, key areas for attention will include raising awareness about these topics and prioritizing funding for the implementation and evaluation of violence prevention interventions in health care settings and the community.

Abstract:Adult child to parent violence is a growing international social problem that needs to be better understood to develop clinical interventions. An exploratory study on the antecedents of early life adversity on adult child to parent violence/elder mistreatment was carried out using secondary analysis. Directed content analysis was used based on life course theory with a data set of older mothers all > 57 years old (J. R. Smith 2021, 2022). Measurement of instances of child abuse, domestic violence, and birth trauma among abused older mothers was conducted. All the older women had reported being negatively affected by their adult children's problems and 56% reported being physically attacked. A case example is presented to illustrate how repeated incidents of abuse or neglect in early life might explain an older woman's inability to take actions for her own safety.

Abstract:Although domestic violence is seen as a serious public health issue for women worldwide, international evidence suggests that women aged over 50 who are victims are suffering in silence because the problem is often ignored by health professionals. More U.K. research is needed to identify the extent of the problem, and services to meet the needs of older women. This study aims to bridge this gap by gaining a deeper understanding of how 'older women' cope with domestic violence and how it affects their wellbeing. Eighteen older women who were currently, or had been in an abusive relationship were recruited. Semi-structured interview schedules were used to discuss the personal nature of DV and its effects on wellbeing, ways of coping and sources of support. Findings suggest that living in a domestically violent context has extremely negative effects on older women's wellbeing leading to severe anxiety and depression. Three-quarters of the women defined themselves as in 'very poor' mental and physical health and were using pathogenic coping mechanisms, such as excessive and long-term use of alcohol, prescription and non-prescription drugs and cigarettes. This negative coping increased the likelihood of these women experiencing addiction to drugs and alcohol dependence and endangered their health in the longer term. Our findings suggest that health professionals must receive appropriate education to gain knowledge and skills in order to deal effectively and support older women experiencing domestic violence.
Wolf in Sheep's Clothing? Violent, Abusive, and Harmful Behavior by the Older Person Toward Their Family Caregivers: A Qualitative Study

Abstract:Violent, abusive, and harmful behavior enacted by older adults upon their caregivers represents a distressing and frequently disregarded facet within the domain of caregiving. This qualitative study aims to (a) explore family caregivers' experiences of violent, abusive, and harmful behavior by the older person and (b) explore how violent, abusive, and harmful behavior by the older person affects family caregivers' mental health. This qualitative study encompassed 393 participants, with a diverse age range spanning from 40 to 72 years. All the interviews went through the process of content analysis. For the first objective, findings indicated six emerging themes: Frequent and extreme verbal violence (77.3%); feeling manipulated and controlled by older adults (74.7%); experiencing unpredictable illegal circumstances provoked by older adults (62.1%); experiencing damaging financial issues provoked by older adults (43.1%); experiencing physical violence (34.2.%); and experiencing sexual violence (31.1%). The second objective highlighted four themes: depression and anxiety (89.9 %), anger (81.2%), feeling morally isolated (78.3%), and emotional outbursts (65.1%). Brazilian participants mainly experienced frequent and extreme verbal violence (62.4%). Moreover, depression and anxiety were mainly verbalized by English participants (84.3%). These findings underscore the significant toll that older individuals' violent, abusive, and harmful behavior can have on the mental well-being of family caregivers. This study sheds light on the complex experiences faced by family caregivers and emphasizes the urgent need for targeted interventions to foster healthier caregiving environments. Older individuals' violent, abusive, and harmful behavior toward their caregivers has received limited attention in research and public discourse. The findings of this study call attention to the pressing need of addressing this issue, given its detrimental impact on the mental health of family carers. Recognizing the significance of this topic demands a comprehensive and targeted approach to ensure the well-being and safety of caregivers and older adults.
Violence against older people and associations with mental health: A national probability sample survey of the general population in England

Abstract:Background: Official estimates of violence prevalence in England exclude older people. There are few studies of elder abuse and these excluded violence from acquaintances and strangers and lack comparability with younger adults. Objectives: To estimate prevalence of past-year violence victimisation in older people, identify factors associated with violence in older age, quantify the extent to which experience of violence in older people was associated with common mental disorder (CMD). Study design/methods: Analysis of a 2014 general population probability sample survey of 2570 adults aged 60+ and 4484 16-59 year olds. Modified version of the Conflict Tactics Scale measured domestic violence and List of Threatening Experiences captured bullying and serious assault. CMD were assessed using the revised Clinical Interview Schedule. Associations were examined using regression models adjusted for childhood victimisation and other adversities. Results: 2.0 % (n = 52,CI:1.4-2.6) of older people experienced violence in the past year, with intimate partner violence the most prevalent form. Older people of non-white ethnicity, those who were socially isolated or lonely, and the formerly married were more likely to experience violence. Violence was associated with CMD in older people (adjusted odds ratio 2.2, CI:1.0-4.8), controlling for impairments, adversities and other factors. Conclusion: Violence, especially from an intimate partner, is evident in later life and strongly associated with poor mental health. Better instruments for the identification of violence and abuse in older people in research and safe enquiry in practice settings are needed, with recognition of and attention to ethnic and other inequalities among older people in exposure.
Implicit and explicit dehumanization of older family members: Novel determinants of elder abuse proclivity.

Abstract:Elder abuse affects one in six older persons globally. Three limitations impede progress in prevention: most research is victim- rather than perpetrator-based; the reliance on explicit, self-reported factors; and failure to account for psychological factors, such as dehumanization, that motivate abuse. The current study addressed these gaps by examining whether implicit and explicit dehumanization of t could explain elder abuse proclivity. In a web-based survey of 585 family caregivers of older persons, dehumanization was found to be prevalent with 51% of the caregivers implicitly and 31% explicitly dehumanizing older persons. As predicted, implicit and explicit dehumanization contributed to elder abuse proclivity (OR = 1.23, 95% CI = 1.02-1.50, <i>p</i> = .03) and (OR = 1.26, 95% CI = 1.05-1.51, <i>p</i> = .01), respectively, after adjusting for relevant covariates including caregiver burden, and caregivers' and care-recipients' health. Developing caregiver-based interventions to humanize older persons may complement ongoing efforts in reducing elder abuse.
Children bereaved by fatal intimate partner violence: A population-based study into demographics, family characteristics and homicide exposure

Abstract:BACKGROUND: In the context of violence against women, intimate partner homicide increasingly receives research and policy attention. Although the impact of losing a parent due to intimate partner homicide is intuitively obvious, little is known about the children involved. We aimed to identify all children bereaved by parental intimate partner homicide in the Netherlands in the period 2003-2012, describe their demographics and family circumstances, and assess their exposure to prior violence at home and to the homicide itself.METHODS AND FINDINGS: We cross-examined 8 national data sources and extracted data about children's demographics and circumstances prior to, and during the homicide. Our primary outcomes were prior violence at home (child maltreatment, neglect or domestic violence) and homicide witness status (ranging from being at a different location altogether to being present at the scene). During the decade under study, 256 children lost a biological parent due to 137 cases of intimate partner homicide. On average, the children were 7.4 years old at the time of the homicide (51.1% were boys; 95% CI 47.3-54.7) and most lost their mother (87.1%; full population data). Immigrant children were overrepresented (59.4%; 95% CI 52.8-66.0). Of the children for whom information about previous violence at home was gathered, 67.7% (95% CI 59.7-73.7) were certainly exposed and 16.7% (95% CI 11.3-22.2) probably. Of the children who had certainly been exposed, 43.1% (95% CI 41.1-60.9) had not received social services or mental health care. The majority of the children (58.7%; 95% CI 52.1-65.3) were present at the location of the homicide when the killing took place, with varying levels of exposure. Homicide weapons mostly involved cutting weapons and firearms, leading to graphic crime scenes.CONCLUSIONS: Care providers need capacity not only to help children cope with the sudden loss of a parent but also with unaddressed histories of domestic violence and exposure to graphic homicide scenes, in a culture-sensitive way. Future directions include longitudinal monitoring of children's mental health outcomes and replication in other countries.

Abstract:This article is based on the lecture for the 2014 American Geriatrics Society Outstanding Scientific Achievement for Clinical Investigation Award. Elder abuse is a global public health and human rights problem. Evidence suggests that elder abuse is prevalent, predictable, costly, and sometimes fatal. This review will highlight the global epidemiology of elder abuse in terms of its prevalence, risk factors, and consequences in community populations. The global literature in PubMed, MEDLINE, PsycINFO, BIOSIS, Science Direct, and Cochrane Central was searched. Search terms included elder abuse, elder mistreatment, elder maltreatment, prevalence, incidence, risk factors, protective factors, outcomes, and consequences. Studies that existed only as abstracts, case series, or case reports or recruited individuals younger than 60; qualitative studies; and non-English publications were excluded. Tables and figures were created to highlight the findings: the most-detailed analyses to date of the prevalence of elder abuse according to continent, risk and protective factors, graphic presentation of odds ratios and confidence intervals for major risk factors, consequences, and practical suggestions for health professionals in addressing elder abuse. Elder abuse is common in community-dwelling older adults, especially minority older adults. This review identifies important knowledge gaps, such as a lack of consistency in definitions of elder abuse; insufficient research with regard to screening; and etiological, intervention, and prevention research. Concerted efforts from researchers, community organizations, healthcare and legal professionals, social service providers, and policy-makers should be promoted to address the global problem of elder abuse.

Abstract:The homeless population remains understudied, and their victimisation is unreported, especially homeless victims of homicide. With the number of people faced with homelessness increasing, the heightened rates of victimisation for violent crimes throughout this population becomes even more concerning. A review of the literature revealed an absence of meaningful research beyond basic descriptive statistics of rates of homeless homicide in the UK. The current study explores the behavioural variation and risk factors associated with the victims and perpetrators of 19 cases of homeless homicide in the UK. A content analysis was first conducted to derive 22 case variables. Smallest Space Analysis was then employed to analyse the cases according to the variables selected. The themes produced in the SSA output were comparable to that of Canter's Victim Role Model: Victim as Object, Victim as Person and Victim as Vehicle. The current study lays a foundation for developing an understanding of the variation in behaviour across cases of homeless homicide and may serve to inform preventative measures.

Objectives: The current study examined the associations between lifetime abuse victimization and prospective health outcomes in late adulthood. Methods: Data from 4907 older adults (mean age = 80) from the Wisconsin Longitudinal Study were analyzed. Multivariate analyses examined the associations of lifetime abuse victimization with depression, physical health status, and memory. Results: Greater exposure to lifetime abuse was associated with a significantly higher risk of depression (OR = 1.13, CI: [1.08, 1.19], p < .001) and a greater number of limitations in physical functioning (b = .08, SE = .02, p .05). Discussion: Our results support the interrelations of interpersonal violence across the life course and the lasting health effects of exposure to lifetime abuse. Findings highlight the need for a life course-based, trauma-informed approach in prevention and intervention programs for older adults.

Abstract:Elder abuse in family settings has increased in recent years for a variety of reasons, including the increasing proportion of older adults in the total population, the related increase in chronic disabling diseases, and the increasing involvement of families in caregiving relationships with elders. Future trends indicate not only continued growth of the older population but suggest, as well, an increased demand for family caregiving which may, in turn, be accompanied by increasing rates of elder abuse. It is important to consider issues associated with such caregiving and elder abuse in families from an ecological perspective as a basis both for framing conceptually relevant and effective prevention strategies as well as for understanding the specific character of the broader issue of the intergenerational nature of the quality of life in an aging society. Using an applied ecological model, the article focuses on the contextual risk factors of elder abuse. Specifically, five levels of environment--microsystem, mesosystem, exosystem, macrosystem, and chronosystem--will be utilized to organize and interpret existing research on the risk factors asociated with elder abuse (Bronfenbrenner, 1979, 1986, 1997). The configuration of the risk factors provides a useful framework for understanding the intergenerational character of the quality of life for older adults, for developing recommendations for empirically-based action research, and for the development of community-based prevention and intervention strategies. The application of a contextual perspective to the development of intervention and prevention programs will be addressed, the latter in relation to primary, secondary, and tertiary prevention.
