Percutaneous nephroscopy combined with ultrasound-guided negative-pressure suction for the treatment of perirenal hematoma

Abstract:Perirenal hematoma refers to the hematoma located in the perirenal fascia after renal parenchyma or capsule hemorrhage. If the hematoma is located between the renal parenchyma and the capsule, it is called subcapsular hematoma. Many literatures have included it in the study of perirenal hematoma (PRH). PRH is often caused by urinary surgery, and other causes include spontaneity or trauma. [ 1–3 ] If the huge hematoma is not drained or cleared thoroughly in time, the prognosis of patients with secondary infection is often poor. According to previous literature reports, it is generally considered that the largest diameter of the PRH is >6 cm. [ 4 ] At present, the main treatment of PRH includes conservative treatment, surgical intervention (including open surgery, perirenal puncture, and drainage), some scholars try to combine urokinase and other drugs to enhance the effect of clot drainage. [ 5 ] However, at present, it is difficult to drain the huge PRH or multilocular hematoma only by puncture and drainage, while open surgery has great trauma, long hospital stay, and may even lead to incision nonunion and secondary systemic infection. some scholars are also looking for laparoscopic removal of PRH, the effect is similar to that of open surgery, but the trauma is still large. [ 6 ] For huge PRH requiring surgical intervention, it is urgent to find a more minimally invasive and effective treatment. This paper reports 5 cases of giant PRH treated by percutaneous nephroscopy combined with ultrasound negative pressure aspiration under general anesthesia. The clinical data were summarized and analyzed to explore the safety and efficacy of this percutaneous nephroscopy minimally invasive technique in the treatment of symptomatic giant PRH. To provide a feasible choice for the treatment of PRH.
