Abstract:(1) Background: The purposes of this study were to develop a physical fitness evaluation program for new firefighters, to investigate whether there is a quality difference in performing CPR for cardiac arrest patients according to physical strength, and to provide basic data to improve CPR quality. (2) Methods: The subjects of this study were fire trainees who were appointed as firefighters for the first time in G province from 3 March 2021 to 25 June 2021. The age of the subjects was 25–29 years old, and their experience of working as a firefighter was less than three months. According to the purposes of the study, the researcher composed the Physical Fitness Evaluation Program, including the physical fitness evaluation method and steps, and requested a content expert group to modify and supplement the ‘physical fitness assessment program’. The subjects were divided into four groups according to their levels of physical strength, and CPR was performed for 50 min in groups of two. A high-end Resuscitation Anne Simulator (Laeadal, Norway) mannequin was used to evaluate the quality of CPR. (3) Results: When comparing the difference in CPR quality, there were statistically significant differences in the number of chest compressions and compression depth, but all groups met the CPR guidelines. In the case of this study, it is thought that high-quality CPR could be performed because the subjects’ average age was low and they continued to exercise to improve their physical strength for their role. (4) Conclusions: It was concluded that the fitness level of new firefighters confirmed by this study was sufficient for general high-quality CPR. In addition, for high-quality CPR, continuous management is required by developing a continuous CPR education and physical training program for all firefighters.

Evaluation of Comprehensive Cardio-Pulmonary Resuscitation Training in Non-Medical Undergraduates

Abstract:Total 280 non-medical undergraduates (174 males and 106 females) were enrolled in the study from September 2015 to January 2016.Among all participants,132 received comprehensive cardio-pulmonary resuscitation (CPR) training,including knowledge teaching,video playing and simulation practice (study group),and 148 received knowledge teaching and simulation practice only (control group).Changes of CPR knowledge,skills and willingness were evaluated before and after training,and compared between two groups.The theory scores were increased after the training in both groups (study group: 87.7±5.6 vs.47.9±4.7,P<0.001;control group: 71.6±5.2 vs.48.4±4.9,P<0.001);there was no significantly difference between two groups before the training (P>0.05),however,there was significant difference after the training (P<0.001).The qualified rates of CPR skills were increased after the training in both groups (study group 81.8% vs.26.5%,P<0.001;control group: 68.2% vs.26.4%,P<0.001);there was no significant difference in qualified rates before the training between two groups (P>0.05),however,there was significant difference after the training (P<0.01).The willingness rates of performing CPR were increased after the training in both groups (study group: 64.4% vs.13.6%,P<0.001;control group: 46.6% vs.14.2%,P<0.001);there was no significant difference in willingness level before the training between two groups (P>0.05),however,there was significantly difference after the training (P<0.01).The study shows that comprehensive training can enhance CPR knowledge,skills and willingness for non-medical undergraduates,indicating that it may also be effective for other social groups.
Analysis of Physiological Response during Cardiopulmonary Resuscitation with Personal Protective Equipment: A Randomized Crossover Study

Abstract:The use of personal protective equipment (PPE) is required for the self-protection of healthcare workers during cardiopulmonary resuscitation (CPR) in patients at risk of aerosol transmission of infectious agents. The aim of this study was to analyze the impact of personal protective equipment on physiological parameters during CPR. A randomized, quasi-experimental, crossover design was used. The study was carried out in a training and simulation emergency box and the total sample consisted of 20 healthcare professionals. Two CPR tests were compared with the recommended sequence of 30 chest compressions and 2 ventilations. The duration of each test was 20 min. One of the CPR tests was carried out without using any PPE (CPR_control), i.e., performed with the usual clothing of each rescuer. The other test was carried out using a CPR test with PPE (i.e., CPR_PPE). The main variables of interest were: CPR quality, compressions, ventilations, maximum heart rate, body fluid loss, body temperature, perceived exertion index, comfort, thermal sensation and sweating. The quality of the CPR was similar in both tests. The maximum heart rate was higher in the active intervals (compressions + bag-valve-mask) of the test with PPE. CPR_PPE meant an increase in the perceived effort, temperature at the start of the thermal sensation test, thermal comfort and sweating, as opposed to CPR performed with usual clothing. Performing prolonged resuscitation with PPE did not influence CPR quality, but caused significant physiological demands. Rescuers were more fatigued, sweated more and their thermal comfort was worse. These results suggest that physical preparation should be taken into account when using PPE and protocols for physiological recovery after use should also be established.

Abstract:Cardiopulmonary Resuscitation (CPR) is a critical life-saving skill, especially in healthcare settings where timely and proficient intervention can significantly impact patient outcomes. While theoretical CPR training provides foundational knowledge, integrating practical experience is essential for bridging the gap between theoretical understanding and practical real-world application. The primary objectives include evaluating how practical experience influences CPR proficiency between individuals with theoretical training only and those with one year of practical (real-time) experience. The study aims to provide valuable insights into the effectiveness of CPR training programs and the significance of practical exposure in improving outcomes. Seventy healthcare professionals, comprising 35 doctors with CPR training only (Group A) and 35 doctors with one year of practical CPR experience (Group B), were subjected to standardized Basic Life Support (BLS) testing on a manikin. Various CPR performance indicators were meticulously assessed, such as compression depth, rate, hand positioning, effective ventilations, and hands-off time. Statistical analyses were conducted to draw comparisons and highlight the impact of practical experience. With practical experience, Group B demonstrated significantly superior CPR performance compared to Group A. This was evident in shorter start-up time, enhanced compression depth and hand positioning, higher rates of effective ventilations, and reduced hands-off time. While compression rates remained similar between the groups, practical experience emerged as a crucial factor influencing diverse CPR quality indicators. The study concludes that practical experience is pivotal in improving CPR proficiency, emphasizing the need for hands-on components in training programs. These findings contribute to refining CPR training strategies for enhanced real-world application and improved emergency survival rates. Ongoing research in this field is crucial for continuous advancements in CPR protocols.
Acquisition of Knowledge and Practical Skills after a Brief Course of BLS-AED in First-Year Students in Nursing and Physiotherapy at a Spanish University

Abstract:Out-of-hospital cardiorespiratory arrest is one of the leading causes of death in the Western world. Early assistance with quality Cardiopulmonary Resuscitation (CPR) and the use of a defibrillator may increase the percentage of survival after this process. The objective of this study was to evaluate the effect of CPR training and the management of an Automatic External Defibrillator (AED). A descriptive, cross-sectional, observational study was carried out among students in the first year of a Nursing and Physiotherapy degree of the University of León. To achieve this goal, a theoretical-practical educational intervention of four hours’ duration which included training on CPR, AED and Basic Life Support (BLS) was carried out. A total of 112 students were included. The results showed an increase in theoretical knowledge on BLS as well as on CPR and AED, and practical skills in CPR and AED management. A theoretical exposition of fifteen minutes and the practical training of CPR wasenough for the students to acquire the necessary theoretical knowledge, although the participants failed to reach quality criteria in CPR. Only 35.6% of students reached the right depth in compressions. Also, ventilation was not performed properly. Based on the results, we cannot determine that the percentage of overall quality of CPR was appropriate, since 57.6% was obtained in this respect and experts establish a value higher than 70% for quality CPR. There was a clear relationship between sex, weight, height and body max index (BMI), and quality CPR performance, being determinant variables to achieve quality parameters. Currently, Basic Life Support training in most universities is based on training methods similar to those used in the action described. The results obtained suggest implementing other training methods that favour the acquisition of quality CPR skills.

The data tables described in this article summarise the impact that specific training interventions, number of times trained, and retention testing intervals have on laypeople's CPR psychomotor skills, as reported by peer-reviewed journal articles. The psychomotor skills included are: compression rate, compression depth, duration of interruptions to compressions, chest recoil, hand placement, proportion of adequate or 'correct' compressions, ventilation volume, compression-to-ventilation ratio, duty cycle and overall skills. Abstract:Background. The quality of cardiopulmonary resuscitation (CPR) is pivotal in improving the outcome of cardiac arrest. Nevertheless, there is evidence that even professional rescuers may deliver ineffective chest compressions (CCs). We sought to evaluate the impact of a CPR feedback device on the quality of CC performed by a supposedly highly trained and skilled population of attendees to the national annual congress of the Italian Resuscitation Council. Methods. A total of 202 congress attendees were enrolled to perform 2 min CC without feedback, followed by a 5 min rest and another 2 min interval of CC with feedback. Moreover, participants were randomly assigned to two study groups: "feedback later", in which the first 2 min CCs were delivered without the feedback, and "feedback first", in which the first 2 min CCs were aided by feedback. CPR quality has been analyzed in terms of the percentage of CC with adequate depth (CC, %), complete chest release (CR, %), and the CC rate (CC/min). Results. Approximately 60% of correct CCs were performed without feedback, which significantly increased to 79% with the use of feedback. In the "feedback later" group, the percentage of correctly performed CC and CR significantly increased during the second 2 min interval of CC with feedback (median value 51% vs. 86%, p < 0.0001 and 71% vs. 84%, p = 0.025, respectively). In the "feedback first" group, the percentage of correct CC remained stable during the two CC intervals (71% vs. 80%, p = 0.06), while CR was better without the help of the feedback (70% vs. 93%, p < 0.0001). CC/min was in the recommended range (100–120/min) in all the CC events. Conclusions. In this population of expected highly skilled CPR providers, the overall quality was inconsistent and, in many cases, did not reach guidelines recommendations. The use of a feedback device significantly improved the quality of CC. When the feedback device was used in the first CC attempt, it had a learning effect that was reflected in maintaining quality during the second CC series.

Abstract:The American Heart Association's (AHA) recommendation for biyearly recertification and annual mandatory CPR training may be suboptimal for first responders (nurses and technicians) working in outpatient clinics (American Heart Association, 2013). To determine the efficacy of the AHA guidelines, 40 simulated sudden cardiac arrest (SCA) encounters were conducted followed by debriefing and a subsequent SCA to determine a basic level of CPR proficiency. First responders' CPR skills were evaluated using a 19-item assessment form to quantify the event. A comparison of scores using two different viewing modalities was performed to provide an assessment of the training program. Of the 40 sessions, group mean performance scores for the first encounter were just above the organization's minimum required score of 24. Performance scores increased slightly (27-28) after the second encounter. Proficiency of skills was poor and frequent basic life support training may be indicated to help first responders provide high-quality CPR.

Abstract:Aim: The aim of this paper is an attempt to analyse the determinants of effective cardiopulmonary resuscitation (CPR) performed by medical personnel over a five-year span. Material and methods: An original research questionnaire was used to collect and process data for the purposes of this retrospective observational study. The research sample consisted of 167 patients aged between 16 and 102 who had in-hospital sudden cardiac arrest (SCA) and underwent cardiopulmonary resuscitation (CPR). Results: The most common causes of SCA in the study group of patients were cardiovascular diseases, and the mechanism of asystole was responsible for half of the recorded sudden cardiac arrests. While performing CPR, chest compressions were the most frequently applied procedure, whereas defibrillation or the Esmarch (jaw-thrust) maneuver were used less often. The effectiveness of resuscitation in almost a half of the cases was unsatisfactory, whereas the average time of performing resuscitation was 27,83 minutes. Pronouncement of death was made with regard to patients on whom resuscitation was performed for the longest time. On the other hand, among patients who were resuscitated for less than 20 minutes, breathing, blood circulation and con¬sciousness were restored, or only breathing and circulation were restored. Conclusions: The main cause of sudden cardiac arrest in the study period were non-shockable heart rhythms, whereas the restoration of spontaneous cir¬culation and breathing was achieved in the cases when resuscitation was performed for less than 20 minutes. The survival rate after successful resuscitation is comparable to the results obtained in other hospitals in Poland and Europe, as well as statistical data from scientific publications.

Abstract:BackgroundPerformance of sufficient cardiopulmonary resuscitation (CPR) by medical personnel is critical to improve outcomes during cardiac arrest. It has however been shown that even health care professionals possess a lack of knowledge and skills in CPR performance. The optimal method for teaching CPR remains unclear, and data that compares traditional CPR instructional methods with newer modalities of CPR instruction are needed. We therefore conducted a single blinded, randomised study involving medical students in order to evaluate the short- and long-term effects of a classical CPR education compared with a bilateral approach to CPR training, consisting of problem-based learning (PBL) plus high fidelity simulation.MethodsOne hundred twelve medical students were randomized during a curricular anaesthesiology course to a control (n = 54) and an intervention (n = 58) group. All participants were blinded to group assignment and partook in a 30-min-lecture on CPR basics. Subsequently, the control group participated in a 90-min tutor-guided CPR hands-on-training. The intervention group took part in a 45-min theoretical PBL module followed by 45 min of high fidelity simulated CPR training. The rate of participants recognizing clinical cardiac arrest followed by sufficiently performed CPR was the primary outcome parameter of this study. CPR performance was evaluated after the intervention. In addition, a follow-up evaluation was conducted after 6 months.Results51.9% of the intervention group met the criteria of sufficiently performed CPR as compared to only 12.5% in the control group on the day of the intervention (p = 0.007). Hands-off-time as a marker for CPR continuity was significantly less in the intervention group (24.0%) as compared to the control group (28.3%, p = 0.007, Hedges' g = 1.55). At the six-month follow-up, hands-off-time was still significantly lower in the intervention group (23.7% vs. control group: 31.0%, p = 0.006, Hedges' g = 1.88) but no significant difference in sufficiently performed CPR was detected (intervention group: 71.4% vs. control group: 54.5%, p = 0.55).ConclusionPBL combined with high fidelity simulation training leads to a measurable short-term increase in initiating sufficient CPR by medical students immediately after training as compared to classical education. At six month post instruction, these differences remained only partially.

Abstract:Cardiopulmonary resuscitation (CPR) education for the public may improve bystander intention to perform CPR on cardiac arrest patients. Studies have shown that different CPR education intervention methods can improve learning performance, with key indicators including attitude toward to CPR, intention to perform CPR, and degree of CPR knowledge and skills. The present study compared the traditional face-to-face method to hybrid and virtual reality (VR) methods to observe difference in learning performance and length of performance retention. This study adopted randomized controlled trial to compare CPR learning performance between traditional face-to-face, hybrid, and VR methods. Participants from each intervention group completed a pretest and 2 posttests. The measurement tools included an attitude and intention questionnaire, knowledge examination, and skill examination with a RESUSCI ANNE QCPR ® manikin. The performance among all participants in pretest showed no significant difference between the intervention groups, indicating no difference in their background attitude, knowledge, and skill level. Significant differences were observed in the average degree of intention to perform CPR between the hybrid and traditional groups in 1st and 2nd posttest. Compared to the pretest results, the posttests revealed significantly higher attitude toward CPR, intention to perform CPR, knowledge examination results, accuracy of overall chest compression, accuracy of CPR procedure, accuracy of AED usage, accuracy of chest compression rate, and accuracy of chest compression depth. The average time to reattending CPR learning and practice session was 11–12 weeks reported by participants. The hybrid and VR methods to CPR education resulted in the same level of improvement in learning performance as traditional face-to-face teaching. The suggested frequency for renewing CPR knowledge and skills is 12 weeks which may be considered in new strategies aimed at promoting CPR education and exposure to the public.

Cardiorespiratory Fitness in Spanish Firefighters: Age Differences and Associations Between Fitness-Related Parameters

Abstract:Objective: To evaluate the cardiorespiratory fitness and other fitness-related measures, and to analyze the differences among age groups. Methods: This study was conducted on 104 firefighters. Maximal oxygen uptake (VO2 max) was assessed using the Shuttle Test and fitness was evaluated using appropriate physical tests. Results: Mean VO2 max was 45.7 mL/kg/min and 60.6% of firefighters were over the minimum accepted standpoint to do effective work, however, the VO2 max decreased with increasing age. 35.6% of the firefighters were overweight. Age, weight, and total body fat were found to be significant predictors of VO2 max level in this population, with total body fat as strongest predictor. Conclusions: Healthy habits-based interventions tailored by age groups should be promoted, to improve cardiorespiratory fitness and body composition for safe work and to improve firefighters' health.
Comparison of Acute Arterial Responses Following a Rescue Simulation and Maximal Exercise in Professional Firefighters

Abstract:Cardiovascular events are the leading cause of on-duty deaths among firefighters. Screening firefighters for risk of sudden cardiac event is a critical element of a comprehensive medical program. Although intense physical exertion has been shown to trigger sudden cardiac events in the general population, it is unclear how hemodynamic responses following clinical exercise testing compare to that of performing firefighting tasks in personal protective equipment. Therefore, the purpose of this study was to compare hemodynamic responses following rescue simulation (RS) and maximal exercise in firefighters. This was a cross-over repeated measures study. Thirty-eight professional firefighters (31.8 ± 5.2 yr; VO2peak: 57.9 mL/kg/min) completed a maximal aerobic exercise test (MAET) and an RS. Pulse wave velocity (PWV), pulse pressure (PP), and brachial and central mean arterial pressure (MAP) were measured before and 5 and 15 min post-exercise. The findings indicated that femoral PWV decreased after MAET and RS at both time points (p < 0.005). No significant differences were found in aortic and carotid PWV over time or between conditions (p ≥ 0.05). Significant increases in brachial and central PP and MAP were noted 5 min post-MAET and RS (p = 0.004). In conclusion, the present study demonstrated that peripheral arterial stiffness (AS) decreased in firefighters following both conditions, with no differences in central AS. Our findings provide valuable information on hemodynamic responses similar between RS and MAET, and are important for controlling CVD risk and the AS response.

Abstract:Background: The most recent international guidelines recommended support training of chest compression (CC) using feedback devices. This study aimed to compare the training efficacy of a simplified feedback trainer with the traditional cardiopulmonary resuscitation (CPR) simulator in CPR training. Methods: A total of 60 soldiers were randomly allocated into three groups equally, trained with a simplified external cardiac massage (ECM) trainer named Soul Sheath TM (SS) (SS group), a Resusci Anne manikin (RA group), or traditional simulation training with instructor feedback, respectively. After 7 days of training, the CPR skills were tested blindly in a 2-min assessment session. The primary outcome was the proportion of effective CC, and the secondary outcome included CC rate, depth, compression position, and extent of the release. Results: The percentage of effective CC achieved in the SS group was comparable with the RA group (77.0 ± 15.52 vs. 77.5 ± 10.73%, p = 0.922), and significantly higher than that in the control group (77.0 ± 15.52 vs. 66.8 ± 16.87%, p = 0.037). Both the SS and RA groups showed better CC performance than the control group in terms of CC rate (SS group vs. control group, P = 0.032 and RA group vs. control group, P = 0.026), the proportion of shallow CC (SS group vs. control group, P = 0.011 and RA group vs. control group, P = 0.017). No difference between the SS group and RA group was found in all the CC parameters. Conclusions: The simplified ECM trainer (SS) provides a similar efficacy to the traditional manikin simulator with feedback in CC training to improve the quality of CPR skills.

Abstract:Cardiopulmonary resuscitation (CPR) skills are a crucial aspect of handling medical emergencies, especially in a general hospital environment. In this context, CPR training is important to increase the competency of medical personnel in responding to emergencies involving cardiac or respiratory arrest. However, the direct impact of CPR training on the performance of medical practitioners in emergency management still requires more in-depth research. This study aimed to evaluate the effect of CPR skills training on emergency management in public hospitals. The primary focus is to determine whether CPR training improves medical practitioners' readiness and performance in responding to emergencies, as well as whether it helps in increasing patient survival rates. This research method uses a case study approach in a particular public hospital. The results showed that CPR skills training significantly improved the performance of medical practitioners in handling emergencies in public hospitals. The group that had undergone CPR training showed a faster and more effective response rate in providing emergency resuscitation measures. In addition, there is a significant increase in the survival rate of patients who receive treatment from medical practitioners who have been trained in CPR. This research concludes that CPR skills training has a significant positive impact on handling emergencies in public hospitals. The increased performance of medical practitioners in responding to emergencies and the increase in patient survival rates are strong evidence of the importance of CPR training in increasing the readiness and effectiveness of medical treatment in the hospital environment. Therefore, it is recommended that CPR training continue to be improved and routinely integrated into medical personnel training programs.
The application of Borg scale in cardiopulmonary resuscitation: An integrative review

Abstract:The study of human performance and perception of exertion constitutes a fundamental aspect for monitoring health implications and enhancing training outcomes such as cardiopulmonary resuscitation (CPR). It involves gaining insights into the varied responses and tolerance levels exhibited by individuals engaging in physical activities. To measure perception of exertion, many tools are available, including the Borg scale. In order to evaluate how the Borg scale is being used during CPR attempts, this integrative review was carried out between October/2020 and December/2023, with searches from PubMed, CINAHL, Web of Science, Embase, PsycINFO and VHL. Full publications relevant to the PICO strategy were included and letters, editorials, abstracts, and unpublished studies were excluded. In total, 34 articles were selected and categorised into three themes: a) CPR performed in different contexts; b) CPR performed in different cycles, positions, and techniques; c) CPR performed with additional technological resources. Because CPR performance is considered a strenuous physical activity, the Borg scale was used in each study to evaluate perception of exertion. The results identified that the Borg scale has been used during CPR in different contexts. It is a quick, low-cost, and easy-to-apply tool that provides important indicators that may affect CPR quality, such as perception of exertion, likely improving performance and potentially increasing the chances of survival.
Different Resting Methods in Improving Laypersons Hands-Only Cardiopulmonary Resuscitation Quality and Reducing Fatigue: A Randomized Crossover Study

Abstract:OBJECTIVE:To determine the effects of different resting methods with various rest-start points or rest-compression ratios on improving cardiopulmonary resuscitation (CPR) quality and reducing fatigue during continuous chest compressions (CCC) in 10-min hands-only CPR scenario.METHODS:This prospective crossover study was conducted in 30 laypersons aged 18-65. Trained participants were randomized to follow different orders to perform following hands-only CPR methods: (1) CCC, 10-min CCC; (2) 4+6, 4-min CCC + 6-min of 10-s pause after 60-s compressions; (3) 2+8 (10/60), 2-min CCC + 8-min of 10-s pause after 60-s compressions; (4) 5/30, 2-min CCC + 8-min of 5-s pause after 30-s compressions; (5) 3/15, 2-min CCC + 8-min of 3-s pause after 15-s compressions. CPR quality (depth, rate, hands-off duration, chest compression fraction (CCF)) and participants' fatigue indicators (heart rate, blood pressure, rating of perceived exertion (RPE)) were compared among methods of different rest-start points and different rest-compression ratios with CCC.RESULTS:Twenty-eight participants completed all methods. All resting methods reduced the trend of declining compression depth and the trend of increasing RPE while maintaining CCF of more than 86%. In methods with different rest-start points, the 2+8 method showed no difference in overall CPR quality or fatigue, but better CPR quality of every minute than 4+6 method. In methods with different rest-compression ratios, the 3/15 method showed the best CPR quality and the highest heart rate increment.CONCLUSION:During prolonged hands-only CPR, appropriate transient rests were associated with higher CPR quality and lower subjectively perceived fatigue in laypersons.
Fitness Fights Fires: Exploring the Relationship between Physical Fitness and Firefighter Ability

Abstract:Background: Regular exercise in firefighters may be effective in preventing or attenuating ill health (e.g., hypertension, diabetes, and obesity), as well as improving their firefighting ability. The purpose of this study was to investigate the relationship between physical fitness and firefighting ability. Methods: Male firefighter recruits’ (n = 89; age = 27.1 ± 4.2 yrs) physical fitness and firefighting ability were assessed at Week 1 and Week 7 during a basic firefighting training academy. Physical fitness was assessed via 1.5 mile run time, sit-up and push-up repetitions, the Young Men’s Christian Association (YMCA) bench press test, vertical jump height, and sit-and-reach flexibility, while firefighting ability was assessed via completion time on a firefighting skills test. Results: Fitness predicted significant variance in firefighting ability at Week 1 (R2= 0.46; p < 0.01) and Week 7 (R2 = 0.46; p < 0.01), after accounting for age and body mass index. Cardiovascular endurance accounted for 22.4% (FΔ (1, 85) = 25.75) and 39.3% (FΔ (1, 85) = 55.53) while muscular endurance accounted for an additional 19.0% (FΔ (3, 82) = 10.34) and 6.3% (FΔ (3, 82) = 3.2) unique variance in firefighting ability at Week 1 and Week 7, respectively. Conclusions: Given the strong association between fitness and firefighting performance, municipal departments may want to focus on increasing fitness levels among firefighters.
