Abstract:ObjectiveTo summarize the experience in diagnosis and treatment of malignant change in choledochal cyst patients in the past 20 years. MethodsThe clinical data of 16 patients admitted from 1980 to 2000 were analyzed retrospectively. Results9 patients had had a previous internal drainage procedure,12 patients had biliary tract infection, 4 cases presented with abdominal masses. All suffered body weight loss and general malaise. ERCP was performed in 5 cases with no previous operation, in which abnormal pancreatobiliary duct junction was found in 4 patients. Laparotomy plus metastatic lymph node biopsy was performed in 4 patients, choledochotomy with T-tube drainage for 4 patients, cyst excision and pancreatoduodenectomy for 3 cases, partial cyst excision with left lobectomy for 2 patients, cyst excision with Roux-en-Y hepaticojejunostomy for 3 patients. Pathology proved carcinoma was located in cyst wall in most cases.Postoperative survival time ranged from 4 to 31 months with a mean of 12.7 months. ConclusionsThe clinical symptoms of malignant change in congenital choledochal cyst were non-specific. The preoperative diagnosis for canceration was difficult, and the prognosis was poor.Total extrahepatic choledochocele resection should be adopted for the prevention of canceration.Intraoperative frozen section is helpful to confirm diagnosis. Cyst excision with pancreatoduodenectomy is the treatment of choice for carcinoma invading pancreatic head.
[A Case of Adenocarcinoma of Duct of Luschka]

Abstract:A 75-year-old man was transferred to our hospital with a diagnosis of acute cholecystitis. On next day, a laparoscopic cholecystectomy was performed. Histologic examination confirmed an adenocarcinoma arising from duct of Luschka. Patient underwent additional resection of the liver bed. No residual adenocarcinoma was seen in the surgical specimen. Adenocarcinoma of duct of Luschka is rare, but it is important during cholecystectomy to consider the presence of duct of Luschka, possibility of malignant disease and careful dissection of the gallbladder from its fossa staying close to the gallbladder wall.

A rare gastric metastasis secondary to residual cystic duct carcinoma: case report and literature review

Abstract:An unusual gastric metastasis from residual cystic duct carcinoma was reported, which was easily mistaken as primary gastric carcinoma before the surgery. A 50-year-old Chinese man presented with right upper abdominal discomfort. Based on the biopsy and computed tomography results, an advanced gastric antrum adenocarcinoma was primarily diagnosed. Intraoperatively, there were other findings: residual cystic duct with chronic hyperplasia, a suspected purulent cavity filled with grayish-brown cloudy liquid at the distal end of the cystic duct and the gallbladder socket. The patient underwent radical operation. Histopathological findings finally suggested that adenocarcinoma of the residual cystic duct infiltrated into the whole layer of the gastric wall. Postoperative adjuvant chemotherapy and immunotherapy were administered. The patient has achieved 20-month recurrence-free survival. The comprehensive treatment including radical surgery, adjuvant chemotherapy and immunotherapy may improve the prognosis of such patients.
Metachronous Pancreatic Head Ductal Carcinoma Three Years After Resection Of Gallbladder Cancer

Abstract:We report a rare case of female patient with metachronous gallbladder cancer and pancreatic head ductal carcinoma. At the age of 53 years, the patient underwent a cholecystectomy and resection of the liver bed for gallbladder cancer. The post-operation diagnosis was a well-differentiated adenocarcinoma with serosa involvement (T3N0M0, stage IIA). Three years later, an irregular and enhanced 2.4 cm mass in the pancreatic head with obviously pancreatic duct dilated was found by abdominal imaging. We considered it as pancreatic head cancer and performed pancreaticoduodenectomy. The histological diagnosis was a pancreatic ductal carcinoma (T2N0M0, stage I). No recurrence was found after thirty-three months follow up.
A Resected Gallbladder Carcinoma Coexisting With Adenomyomatosis Involving Varied Degrees of Intraepithelial Dysplasia: A Case Report and Literature Review

Abstract:A 59-year-old asymptomatic man underwent ultrasonography, which revealed gallstones and thickened gallbladder wall. Abdominal computed tomography (CT) showed a slightly swollen bilocular gallbladder and a soft tissue mass in the fundus site. Segmental adenomyomatosis (ADM) was suspected because numerous fundic cystic lesions were seen on magnetic resonance imaging. Endoscopic ultrasonography revealed numerous Rokitansky-Aschoff sinuses (RAS) and a papillary soft tissue shadow surrounded with irregular and remarkably thickened fundic gallbladder wall. Fluoro-2-deoxy-D-glucose-positron emission tomography/CT demonstrated slightly increased fluoro-2-deoxy-D-glucose uptake in the corresponding lesion. Surgery was performed under a diagnosis of gallbladder carcinoma (GBC) with concomitant ADM, and histopathology revealed a 30-mm papillotubular adenocarcinoma extending from the gallbladder body to fundus with invasion into the subserosa. Numerous RAS were present throughout the gallbladder showing various degrees of dysplasia. Ki67 and p53-labeling index (LI) was significantly higher in the dysplastic epithelium compared with normal fundic epithelium. p53-LI was also markedly increased (72.1%) in tissue in front of tumor invasion. Interestingly, these hyperproliferation indicators were extremely high (Ki67-LI: 28.8%; p53-LI: 91.9%) in RAS with low-grade dysplasia even in the gallbladder neck. Although, generally, tumors do not develop in the gallbladder neck with segmental ADM, our results suggest that a gallbladder with ADM has potential for carcinogenesis regardless of location, with segmental ADM. On the basis of histopathology, our patient was diagnosed with GBC arising from RAS with multicentric and multistep growth. A relationship between GBC and ADM, especially segmental ADM, has been suggested but remains controversial. Our experience is very suggestive of carcinogenesis developing from ADM.

Abstract:Hepatobiliary cystadenomas are rare cystic neoplasms that often occur in middle aged women. The exact etiology of these tumors is unknown. Diagnosis is often delayed in these cases. However, misdiagnosis and inappropriate treatment may result in unfavorable outcome. We report a case of hepatobiliary cystadenoma with pleural effusion. We also review the literature and discuss the current diagnostic and treatment modalities.

Abstract:Introduction and importance: Cystadenoma and cystadenocarcinoma of the biliary duct remain a rare diagnosis, the localization is intrahepatic in 97% of the cases and extrahepatic in 3%of them [1]. Cystadenocarcinoma of the gallbladder is the rarest localization, it's usually diagnosed late because of the poor symptoms and the nonspecific lesions in the different imaging.Only hundreds of cases have been reported for cystadenocarcinoma of the gallbladder.This case report has been reported in line with the SCARE Criteria. Case presentation: We present a 70-year-old woman, with no family or personal history, who was complaining of pain in the right hypochondrium and an altered general well-being, with no other clinical abnormalities.The imaging including abdominal sonography, CT scan, and MRI concluded at a budding lesion formed in the gallbladder wall which measured 65 × 15mm.Cancer of the gallbladder was suspected and the patient was operated. A resection of segment s4b and 5 of the liver was performed with lymphadenectomy. The post-operative clinical course was uneventful.The diagnosis of cystadenocarcinoma of the gallbladder was confirmed on an anatomopathological exam of the specimen. Conclusion: Cystadenocarcinoma is a rare diagnosis.There are no specific symptoms or lesions at the imaging.The confirmation is obtained on an anatomopathological study of the specimen.
A General Approach for Evaluating the Economic Viability of Sustainability of Tropical Cropping Systems

Abstract:This paper presents a methodology for measuring economic viability and agricultural sustainability for new technology evaluation. The approach is based on the concept of interspatial and intertemporal total factor productivity, paying particular attention to the valuation of natural resource stocks and flows. Using a set of data available at the International Institute of Tropical Agriculture, the model is demonstrated by computing the intertemporal and interspatial total factor productivity indices for four cropping systems in southwestern Nigeria. Results show that the sustainability and economic viability measures are sensitive to changes in the stock of nutrients as well as to changes in material input uses and outputs. When common property resource flows are important, the measures provide markedly different results from conventional total factor productivity approaches.

Abstract:Biliary cysts are relatively uncommon and they can be congenital or acquired and can have various presentations such as cholelithiasis, cholangitis, jaundice, and pancreatitis. Biliary cysts are associated with a high risk of biliary cancers and such risk increases with age. Identification of biliary cysts warrants an aggressive approach to lower cancer risk. Surgical management has a high success rate and it lowers morbidity, mortality, and cancer risk. We present a 40-year-old female who had a cholecystectomy in 2016. She presented with obstructive jaundice and was found to have a class I biliary cyst. She underwent endoscopic retrograde cholangiopancreatography with stenting which led to complete resolution of her symptoms. Later, she underwent elective Roux-en-Y hepaticojejunostomy with cyst resection three months later. She underwent a successful recovery.

Abstract:Gastric duplication cysts (GDCs) are rare congenital anomalies, often identified during infancy or childhood. Although typically benign, there have been sporadic reports of malignant transformations, including adenocarcinoma and rare mixed tumors. Herein, we describe a rare case of mixed pancreatobiliary ductal adenocarcinoma and squamous cell carcinoma occurring within a GDC in a 54-year-old Korean woman with a history of melena and hematemesis. Initial gastroscopy and positron emission tomography–computed tomography (PET-CT) revealed a protruding stomach mass. A laparoscopic total gastrectomy was performed, and histological examination confirmed a mixed carcinoma originating from an ectopic pancreas within the duplication cyst. This case is unique as it is the first reported instance in the world of mixed pancreatobiliary ductal adenocarcinoma and squamous cell carcinoma arising from an ectopic pancreas within a GDC. This highlights the importance of considering pancreatobiliary-type adenocarcinoma in the differential diagnosis of malignancies originating from GDCs, which has implications for treatment strategies.

Abstract:The biliary tract's most frequent malignancy is gallbladder cancer (GBC). Due to the growing number of laparoscopic cholecystectomy procedures, the majority of GBC cases in the United States are diagnosed as incidental findings. Roughly 5% of the reported cases of gallbladder adenocarcinomas are of the mucinous type. Mucinous adenocarcinoma tumors contain more than 50% of mucin materials and are clinically characterized by poor prognosis. Initial presentation can mimic acute cholecystitis or cholelithiasis; meanwhile, histological findings post-cholecystectomy are the main diagnostic criteria of mucinous adenocarcinoma of the gallbladder. This case presents a patient who has been diagnosed with cholelithiasis and cholecystitis and subsequently underwent laparoscopic cholecystectomy. Histopathological examination of the gallbladder specimen was consistent with incidental gallbladder adenocarcinoma, the mucinous subtype to be specific.

Abstract:A type of cholangiocarcinoma (CCA) characterized by peculiar histologic patterns and underlying adenofibromatous lesions has been reported in the literature mostly as individual case reports. This study aims to further clarify the defining characteristics of this spectrum of lesions. Clinicopathologic analysis of 8 biliary tumors with tubulocystic architecture arising in the background of adenofibroma-type lesions was performed. Three of these were also investigated with next-generation sequencing with a 174 genes panel. The patients were 5 males and 3 females, with a mean age of 64.6. All tumors were intrahepatic except for one perihilar that protruded into soft tissues. The mean size was 4.4 cm. At histology, all cases showed a peculiar and cytologically bland tubulocystic pattern that closely resembled tubulocystic-type kidney cancers, including back-to-back microcystic units that formed relatively demarcated nodules, and occurring in the background of adenofibromatous lesions. One case showed perineural invasion by otherwise deceptively benign-appearing microcystic structures, one had areas transitioning to intraductal tubulopapillary neoplasm, and 3 cases harbored more conventional small-duct CCA foci. In those 3 cases, both the tubulocystic and conventional CCA components were investigated by next-generation sequencing separately, and they shared the molecular alterations, including recurrent mutations in chromatin remodeling genes, such as ARID1A , BAP1 , and PBRM1 , and the actionable FGFR2-MCU fusion gene. In the limited follow-up, all but one were alive and free of disease after surgical resection. In conclusion, we described a distinct entity of CCA with specific histo-molecular features, for which we propose the designation of tubulocystic carcinoma of bile ducts.

Abstract:In response to their alleged "uterus affinity" the organotropism of various Toxoplasma strains was investigated in the female mouse. Using quantitative histological (immunofluorescence) and serological (Sabin-Feldman-dye test, complement fixation test), we examined the brain, heart, lung, spleen, striated muscle, uterus and ovaries. The following results were obtained: All four of the Toxoplasma strains (ALT, DX, WEISS, GAIL) showed a definite neurotropism. Located in the brain were one hundred times as many cysts as in the sexual organs and ten times as many as in the reamining organs. The uterus and ovary show a low susceptibility, invasion rarely occuring. An "uterus affinity" could not be confirmed. All four Toxoplasma strains showed the same affinity for the various organs. The strain ALT showed the most virulence and formed the most cysts.

Abstract:The use of a Stepped-Frequency Continuous Wave (SFCW) radar is proposed for non-invasive fall and vital signs detection. A fall in principle involves changes both in position and in speed. Measurements have been performed with the radar fixed both on the wall and on the ceiling. In both situations, position and speed of a target have been measured with good accuracy. By combining this information a fall can be properly detected, distinguishing the fall from both walking and sitting movements. The results show the feasibility of this approach. Moreover, the results demonstrate that vital signs can be monitored also.

Abstract:Tumors arising from Meckel's diverticulum (MD) reported in the literature are mainly carcinoid and gastrointestinal stromal tumors. We herein report a rare case of adenocarcinoma arising from intestinal mucosa in an MD with multiple liver metastases at the onset of symptoms. A 57-year-old female complaining of bloody stool for 2 weeks was admitted to our hospital. Colonoscopy revealed massive bloody fluids but did not find any neoplasm. Computed tomography (CT) found a heterogeneous mass at the distal ileum and multiple liver metastases. A segmental ileal resection with local mesentery excision was performed to control the bleeding. During surgery, a tumor arising from a diverticulum in the antimesenteric border of ileum was observed. Histologic examination revealed moderate to poorly differentiated adenocarcinoma. Majority of the MD remain asymptomatic and are diagnosed incidentally during small bowel contrast study, laparoscopy or laparotomy done for unrelated conditions, or until complications arise from the diverticulum. Malignancies are reported to account for only 0.5%-3.2% of the complications. The occurrence of adenocarcinoma in an MD is exceedingly rare. In the few cases described so far, the prognosis of adenocarcinoma within an MD has been poor due to the advanced stage as seen at the time of surgery. Despite the availability of many publications, there is a little consensus concerning the management of an incidental finding of MD. Adenocarcinoma in an MD is extremely sporadic and prognosis has been reported as very poor. However, early diagnosis is challenging. When found incidentally during laparotomy, MD should be carefully examined and best treated with prophylactic resection.
