Abstract:To better connect non-emergent 911 callers to appropriate care, Washington, DC, routed low-acuity callers to nurses. Nurses could provide non-emergent transportation to a health centre, recommend self-care or return callers to the traditional 911 system. Over about one year, 6,053 callers were randomized (1:1) to receive a business-as-usual response ( n control = 3,023) or further triage ( n treatment = 3,030). We report on seven of nine outcomes, which were pre-registered (https://osf.io/xderw). The proportion of calls resulting in an ambulance dispatch dropped from 97% to 56% ( β = −1.216 (−1.324, −1.108), P < 0.001), and those resulting in an ambulance transport dropped from 73% to 45% ( β = −3.376 (−3.615, −3.137), P < 0.001). Among those callers who were Medicaid beneficiaries, within 24 hours, the proportion of calls resulting in an emergency department visit for issues classified as non-emergent or primary care physician (PCP) treatable dropped from 29.5% to 25.1% ( β = −0.230 (−0.391, −0.069), P < 0.001), and the proportion resulting in the caller visiting a PCP rose from 2.5% to 8.2% ( β = 1.252 (0.889, 1.615), P < 0.001). Over the longer time span of six months, we failed to detect evidence of impacts on emergency department visits, PCP visits or Medicaid expenditures. From a safety perspective, 13 callers randomized to treatment were eventually diagnosed with a time-sensitive illness, all of whom were quickly triaged to an ambulance response. These short-term effects suggest that nurse-led triage of non-emergent calls can safely connect callers to more appropriate, timely care.

The Effect of Delay Following the Clinical Decision to Perform Tracheostomy in the Critical Care Setting

Abstract:BACKGROUND:Tracheostomy in patients who are critically ill is generally performed due to prolonged mechanical ventilation and expected extubation failure. However, tracheostomy criteria and ideal timing are poorly defined, including equivocal data from randomized controlled trials and median intubation to tracheostomy times that range from 7–21 d. However, a consistent finding is that only ∼50% of late tracheostomy groups actually undergo tracheostomy, with non-performance due to recovery or clinical deterioration. Unlike in many jurisdictions, elective surgical procedures in our institution require a court-appointed guardian, which necessitates an approximately 1-week delay between the decision to perform tracheostomy and surgery. This offers a unique opportunity to observe patients with potential tracheostomy during a delay between the decision and the performance.METHODS:ICU patients who were ventilated were identified for inclusion retrospectively by an application for guardianship relating to tracheostomy, the intention-to-treat point. The main outcomes of tracheostomy, extubation, or death/palliative care after inclusion were noted. Demographics, outcomes, and event timing were compared for the 3 outcome groups.RESULTS:Tracheostomy-related guardianship requests were made for 388 subjects. Of these, 195 (50%) underwent tracheostomy, whereas 127 (33%) were extubated and 66 (17%) either died before tracheostomy (46 [12%]) or were transitioned to palliative care (20 [5%]). The median time (interquartile range) from guardianship request until a defining event was the following: 6.2 (4.0–11) d for tracheostomy, 5.0 (2.9–8.2) d for extubation (P < .001 as compared to tracheostomy group), and 6.5 (2.5–11) d for death/palliative care (P = .55 as compared to tracheostomy). Neurological admissions were more common in the tracheostomy group and less common in the palliative group. Other admission demographics and hospitalization characteristics were similar. Hospital mortality was higher for subjects undergoing tracheostomy (58/195 [30%]) versus extubation (24/127 [19%]) (P = .03).CONCLUSIONS:Delay in performing tracheostomy due to legal requirements was associated with a 50% decrease in the need for tracheostomy. This suggests that decision-making with regard to ideal tracheostomy timing could be improved, saving unnecessary procedures.

Abstract:BACKGROUND Due to staffing changes at scheduled intervals and decreases in essential staff in the evenings, late intensive care unit (ICU) arrivals may be at risk for suboptimal outcomes. Utilizing a regional collaborative, we sought to determine the effect of ICU arrival timing on outcomes in elective isolated coronary artery bypass (CABG). METHODS Adults undergoing elective, isolated CABG from 17 hospitals between 2013-2023 were identified. Patients with missing predicted risk of mortality or missing ICU arrival time were excluded. Late ICU arrival time was defined as between 18:00-06:00. Hierarchical logistic regression with appropriate predicted risk scores was utilized for outcome risk adjustment. RESULTS We identified 11,638 patients, with 972 (8.4%) experiencing late ICU arrival. Late ICU arrival patients had higher predicted risk of morbidity or mortality (8.2% [5.6%, 12.0% vs. 7.7% [5.5%, 11.5%], p=0.048) compared to early ICU arrival patients with longer median cardiopulmonary bypass times (96 minutes [78, 119] vs. 93 [73, 116], p<0.001). Late ICU arrival patients experienced more unadjusted complications including prolonged ventilation (7.7% vs. 4.2%, p<0.001) and operative mortality (2.0% vs. 1.1%, p=0.02), although no difference in failure-to-rescue (11.0% vs. 10.4%%, p=0.84). Logistic regression with risk adjustment demonstrated late ICU arrival as a predictor of prolonged ventilation (OR=1.49 [1.12-1.99], p=0.006). CONCLUSIONS Following adjustment, late ICU arrivals experienced higher rates of prolonged ventilation, although this did not translate to failure-to-rescue.

Abstract:Purpose: This article describes the impact of a hands-on critical care skills day led by clinical nurse specialist (CNS) to enhance junior nurses' clinical skills. Program objectives included providing specialized skills training to meet the needs of high-acuity patient population and reviewing nurse-sensitive indicator–associated care bundles with the goal of increasing compliance. Description of the Project: An 8-hour program of lecture and hands-on skills was developed. Curriculum included nursing-sensitive indicators, sepsis management, complex tubes and drains, airway management, and emergency response. Clinical nurse specialists mentored senior nurses to colead program planning and execution. The program has been offered 9 times. Qualitative and quantitative data were collected on attendees. Outcome: Quantitative data analysis showed that 89% of the assessed components were affected by the program. Medium effect was noted in defibrillator skills, retrieving items from code care, care of suicidal patient, and sepsis screening. Qualitative data based on attendee survey are also presented and analyzed. Decreases in indwelling catheter utilization on select units are discussed. Conclusion: The CNS has the responsibility to ensure nursing practice is reflective of hospital policy and current evidence. This program demonstrates the impact of CNSs in staff education, mentorship, and competency assessment.

Impact of Triage Systems on Time to Diagnosis and Treatment of Traumatic Brain Injuries

Abstract:Background: Intracerebral hemorrhage (ICH) is a potential complication from traumatic brain injury, with a 30-day mortality rate of 35-52%. Rapid diagnosis allows for earlier treatment, which impacts patient outcomes. A trauma activation (TA) is called when injury severity meets institutional criteria. The patient is immediately roomed, and a multispecialty team is present. A trauma evaluation (TE) occurs when injuries are identified after standard triage processes. Objectives: Our aim was to determine whether TA patients with ICH were diagnosed and treated more rapidly than TE patients. Methods: This was a retrospective study of patients presenting to trauma centers within a large hospital system diagnosed with traumatic ICH between January 2018 and December 2018. Patients were categorized as TA or TE patients. The time to diagnosis was compared between groups, and additional times were evaluated, including time to imaging, computed tomography interpretation, and treatment. Results: A total of 294 patients were included. Groups had similar demographic characteristics and medical history; there was no difference in head Abbreviated Injury Score, Injury Severity Score, or anticoagulant use. Time to diagnosis was decreased for TA patients compared with TE patients (p < 0.0001). In addition, TA patients received treatment sooner (median 107 min) than TE patients (184.5 min) (p < 0.0001). Conclusions: Diagnosis and treatment times were significantly faster in TA patients than in TE patients. Given the similarities in injury severity between groups, the increased time to treatment may be detrimental for patients. Trauma activations are a resource-heavy process, but TE delays care. These data suggest that an intermediary process may be beneficial.

Purpose Therefore the purpose of this evaluation was to measure the patient outcomes of introduction of an ACP in triaging patients with NSTE-ACS)patients for coronary angiographic procedures. Methods Quantitative design approach with a specialist cardiology setting using a 2-year observational analysis of the data.The study population included all patients referred for angioplasty or PCI via the Inter-hospital transfer acute coronary syndrome network including all patients referred for PCI following NSTE-ACS. Results : There were a total of n= 4976 patients referred for coronary angiogram +/-proceed procedures between Feb 2018 and Feb 2020. Overall significant positive outcomes were noted across a consistent patient group based on presenting symptoms and patient characteristics when comparing data in the year preceding ACP led triage. A 2-sided p-value <0.05 defined statistical significance and to reject the null hypothesis. There was a significant reduction in waiting time post introduction of ACP led triage (p < 0.0001) equating to one day Comparison from pre ACP to post ACP led triage and clinical cancellation rates was reduced significantly (p = 0.0062). Rates of revascularisation were significantly higher post ACP led triage with a corresponding decrease in those managed medically (47.2% pre vs 43.1% post, p = 0.0037). These higher rates consisted of increased rates of both CABG (12.4% pre vs 13.5% post, p = 0.04) and PCI 40.4% vs 43.4%, p = 0.0216) Conclusion ACP led triage of ACS patients requiring urgent treatment demonstrates improved patient outcomes with economic benefits for healthcare providers, enhancing the service provided and opens up discussions for further quality improvement and implications to practice. Evidence of the benefits of advanced practice and the role of the advanced clinical practitioner is demonstrated within the cardiology setting and for the clinical triage of a patient group.

Abstract:Circulation: Cardiovascular Quality and Outcomes, Ahead of Print. BACKGROUND:Wide interhospital variations exist in cardiovascular intensive care unit (CICU) admission practices and the use of critical care restricted therapies (CCRx), but little is known about the differences in patient acuity, CCRx utilization, and the associated outcomes within tertiary centers.METHODS:The Critical Care Cardiology Trials Network is a multicenter registry of tertiary and academic CICUs in the United States and Canada that captured consecutive admissions in 2-month periods between 2017 and 2022. This analysis included 17 843 admissions across 34 sites and compared interhospital tertiles of CCRx (eg, mechanical ventilation, mechanical circulatory support, continuous renal replacement therapy) utilization and its adjusted association with in-hospital survival using logistic regression. The Pratt index was used to quantify patient-related and institutional factors associated with CCRx variability.RESULTS:The median age of the study population was 66 (56–77) years and 37% were female. CCRx was provided to 62.2% (interhospital range of 21.3%–87.1%) of CICU patients. Admissions to CICUs with the highest tertile of CCRx utilization had a greater burden of comorbidities, had more diagnoses of ST–elevation myocardial infarction, cardiac arrest, or cardiogenic shock, and had higher Sequential Organ Failure Assessment scores. The unadjusted in-hospital mortality (median, 12.7%) was 9.6%, 11.1%, and 18.7% in low, intermediate, and high CCRx tertiles, respectively. No clinically meaningful differences in adjusted mortality were observed across tertiles when admissions were stratified by the provision of CCRx. Baseline patient-level variables and institutional differences accounted for 80% and 5.3% of the observed CCRx variability, respectively.CONCLUSIONS:In a large registry of tertiary and academic CICUs, there was a >4-fold interhospital variation in the provision of CCRx that was primarily driven by differences in patient acuity compared with institutional differences. No differences were observed in adjusted mortality between low, intermediate, and high CCRx utilization sites.

Impact on mortality of prompt admission to critical care for deteriorating ward patients: an instrumental variable analysis using critical care bed strain

Abstract:PurposeTo estimate the effect of prompt admission to critical care on mortality for deteriorating ward patients.MethodsWe performed a prospective cohort study of consecutive ward patients assessed for critical care. Prompt admissions (within 4 h of assessment) were compared to a ‘watchful waiting’ cohort. We used critical care strain (bed occupancy) as a natural randomisation event that would predict prompt transfer to critical care. Strain was classified as low, medium or high (2+, 1 or 0 empty beds). This instrumental variable (IV) analysis was repeated for the subgroup of referrals with a recommendation for critical care once assessed. Risk-adjusted 90-day survival models were also constructed.ResultsA total of 12,380 patients from 48 hospitals were available for analysis. There were 2411 (19%) prompt admissions (median delay 1 h, IQR 1–2) and 9969 (81%) controls; 1990 (20%) controls were admitted later (median delay 11 h, IQR 6–26). Prompt admissions were less frequent (p < 0.0001) as strain increased from low (22%), to medium (15%) to high (9%); the median delay to admission was 3, 4 and 5 h respectively. In the IV analysis, prompt admission reduced 90-day mortality by 7.4% (95% CI 1.7–18.5%, p = 0.117) overall, and 16.2% (95% CI 1.1–31.3%, p = 0.036) for those recommended for critical care. In the risk-adjust survival model, 90-day mortality was similar.ConclusionAfter allowing for unobserved prognostic differences between the groups, we find that prompt admission to critical care leads to lower 90-day mortality for patients assessed and recommended to critical care.

Abstract:Introduction: Despite its importance in emergency medicine, the practice of triage in low- and middle-income countries remains rudimentary, due to dearth of human and material resources, among other reasons. Subjects and methods: In the Children Emergency Room of a teaching hospital, some nonclinical staff were intermittently deployed to coordinate patient traffic and provide brief first- contact triage on arrival. For this, they were internally trained in triage. The present study was aimed at assessing the value and accuracy of first-contact triage provided by nonclinical staff in a children emergency department of a tertiary hospital. Results: Data of 45 patients seen during the period were obtained from the records of the triage officers and included their age, gender, presenting complaints and the findings of the paediatric assessment triangle. There was a statistically significant relationship between the triage ratings of the nonCC –BY 4.0 clinical staff and the qSOFA / L- qSOFA scores obtained by the emergency room doctors, the time spent in CHER and the total duration of admission (R2= 0.36 - 0.44; p = 0.003), (R2= 0.23 - 0.28; p = 0.018) and (R2= 0.33 - 0.40; p = 0.007) respectively. Conclusion: This study shows that to a high degree, the nonclinical staff correctly categorized the severity of illness in the Paediatric Emergency Department of a tertiary level hospital, thus limiting undue tertiary delay in treatment and potential harm from such delays. Hence, the study exposes a potential source of human resources for paediatric emergency triage in LMICs where there is a dearth of professional medical personnel.
Responding to In-hospital Cardiac Arrests During Times of System-wide Strain: A Code Refresher Training

Abstract:Background: Pandemic conditions of system-wide strain are associated with increased rates of in-hospital cardiac arrest (IHCA). During normal times, medical-surgical nurses may forget cardiopulmonary resuscitation (CPR) skills as soon as 3 months after training, leaving them unprepared and anxious about managing cardiac arrests. During pandemic surges, heightened anxiety can also impact concentration and confidence. Method: Clinicians offered a 45-minute mock code training refresher for medical-surgical nurses to improve confidence performing CPR while adhering to pandemic-related safety procedures. In this pre-post clinical education project, nurses' confidence was measured with the Nursing Anxiety and Self-Confidence with Clinical Decision Making© Scale. Results: Although the results were not statistically significant, participants verbally reported increased confidence to initiate resuscitation, collaborate with team members, and use personal protective equipment during the posttraining debrief. Conclusion: A high percentage of RNs do not have adequate confidence and/or competence in performing CPR, particularly during times of system-wide strain, and this brief, inexpensive refresher training warrants further study. Abstract:ABSTRACT Background: Achieving efficient throughput of patients is a challenge faced by many hospital systems. Factors that can impede efficient throughput include increased ED use, high surgical volumes, lack of available beds, and the complexities of coordinating multiple patient transfers in response to changing care needs. Traditionally, many hospital inpatient units operate via a fixed acuity model, relying on multiple intrahospital transfers to move patients along the care continuum. In contrast, the acuity-adaptable model allows care to occur in the same room despite fluctuations in clinical condition, removing the need for transfer. This model has been shown to be a safe and cost-effective approach to improving throughput in populations with predictable courses of hospitalization, but has been minimally evaluated in other populations, such as patients hospitalized for traumatic injury. Purpose: This quality improvement project aimed to evaluate implementation of an acuity-adaptable model on a 20-bed noncritical trauma unit. Specifically, we sought to examine and compare the pre- and postimplementation metrics for throughput efficiency, resource utilization, and nursing quality indicators; and to determine the model's impact on patient transfers for changes in level of care. Methods: This was a retrospective, comparative analysis of 1,371 noncritical trauma patients admitted to a level 1 trauma center before and after the implementation of an acuity-adaptable model. Outcomes of interest included throughput efficiency, resource utilization, and quality of nursing care. Inferential statistics were used to compare patients pre- and postimplementation, and logistic regression analyses were performed to determine the impact of the acuity-adaptable model on patient transfers. Results: Postimplementation, the median ED boarding time was reduced by 6.2 hours, patients more often remained in their assigned room following a change in level of care, more progressive care patient days occurred, fall and hospital-acquired pressure injury index rates decreased respectively by 0.9 and 0.3 occurrences per 1,000 patient days, and patients were more often discharged to home. Logistic regression analyses revealed that under the new model, patients were more than nine times more likely to remain in the same room for care after a change in acuity and 81.6% less likely to change rooms after a change in acuity. An increase of over $11,000 in average daily bed charges occurred postimplementation as a result of increased progressive care–level bed capacity. Conclusions: The implementation of an acuity-adaptable model on a dedicated noncritical trauma unit improved throughput efficiency and resource utilization without sacrificing quality of care. As hospitals continue to face increasing demand for services as well as numerous barriers to meeting such demand, leaders remain challenged to find innovative ways to optimize operational efficiency and resource utilization while ensuring delivery of high-quality care. The findings of this study demonstrate the value of the acuity-adaptable model in achieving these goals in a noncritical trauma care population.

Abstract:Background Patients of adult intensive care units (ICUs) often suffer from a lack of sleep. Reducing anxiety by promoting adaptation to the ICU prior to admission may be an appropriate way to increase sleep quality. Objective The aim of this study was to evaluate the impact on sleep quality of a brief nurse intervention. Methods This was a pilot randomized controlled trial in Spain. Forty patients admitted in hospital for valve cardiac surgery were randomly allocated to (1) control group (n = 20), receiving usual care, and to (2) experimental group (EG, n = 20), receiving a nurse intervention the day before surgery and admission in the ICU. The intervention was based on Roy Adaptation Model. A trained nurse anticipated the stressful stimulus to patients in order to develop functional adaptive behaviors. A set of photographs and videos was used to illustrate the environment and assistance in the ICU. Sleep quality in the ICU was measured with the Richards-Campbell Sleep Questionnaire and usual sleep quality with the Pittsburgh Sleep Quality Index. Results After the intervention, sleep quality was lower in the EG compared with the control group (−4 points in Richards-Campbell Sleep Questionnaire, P = .69). Adjustment for main confounders led this reduction to −1.9 points (P = .87) among patients in EG. Stratified analyses shown a positive impact for people who usually slept well (+5.2 points, P = .77), but negative for those who had previous poor sleep quality (−20.0 points, P = .24). Conclusion A nurse intervention prior to ICU admission did not increase patients’ sleep quality. In addition, the intervention could have incremented anxiety over the patients who used to sleep poorly at their homes.

Abstract:Introduction Emergency Departments (EDs) serve as the first line of healthcare, addressing a range of conditions from minor to life-threatening. With rising patient volumes, many EDs have adopted innovative models like the split-flow system to increase efficiency and manage overcrowding. The split-flow model, designed to improve efficiency, directs lower-acuity patients to a Fast Track area while higher-acuity cases remain in the main ED. However, this model can lead to lower-acuity patients being seen by a provider before more-acute patients, raising concerns about delayed care for more-acute patients. This study aims to investigate whether prioritizing less-acute patients impacts disposition outcomes for more-acute patients, focusing on admission rates and 30- and 90-day return visits. We hypothesized that more-acute patients seen after near-simultaneously arriving less-acute patients will experience higher rates of return visits and increased hospital readmissions compared to those seen prior to near-simultaneously arriving less-acute patients . Materials and Methods This retrospective observational study assessed the impact of triage practices in the Emergency Department (ED) at Long Island Jewish Medical Center (LIJ), a 583-bed tertiary-care hospital. Adult patients (≥ 18 years) presenting to the ED between April 24 and December 13, 2023, were included, with a sample of 126 patient pairs triaged within 10 minutes of each other. Patients were categorized as high-acuity (ESI 1-2) or low-acuity (ESI 3-5). Data was drawn from electronic health records, including the Emergency Severity Index (ESI) level, the ED location to which the patient was triaged (either Fast Track vs. acute care area), return visits at 30 and 90 days, and patient disposition during the 30-day revisit. Results The study comprised 126 patient pairs (252 patients in total). Overall, there were no statistically-significant differences in outcomes for more-acute patients based on the order in which they were seen. However, when more-acute patients were seen first by a physician, the results indicated that 10% returned to the ED within 30 days, with 63% requiring hospital admission. Additionally, 20% of these patients returned within 90 days. Conversely, when less-acute patients were prioritized, 22% of more-acute patients returned to the ED within 30 days, with only 45% requiring admission, and the 90-day return rate increased to 26%. Conclusion Research has demonstrated a connection between timely care and improved patient outcomes. Our previous study revealed that less-acute patients were seen prior to near-simultaneously-arriving more-acute patients approximately 40% of the time. This raises significant ethical concerns, as it contradicts the fundamental principle of emergency medicine, which emphasizes treating patients based on acuity. Although this current study found no significant differences in dispositional outcomes based on the order in which more-acute patients were seen, the trends suggest that seeing more-acute patients later might lead to worse outcomes. When more-acute patients were seen first, 10% returned to the ED within 30 days, with 63% requiring hospital admission, and 20% of indexed patients returned within 90 days. However, when less-acute patients were seen first, the 30-day return rate for more-acute patients increased to 22%, with 45% needing hospital admission, and the 90-day return rate rose to 26%. EDs must develop strategies to balance operational efficiency with the need to prioritize higher-acuity patients to ensure that operational practices do not compromise patient safety.
Tracheotomy Timing and Outcomes in the Critically Ill: Complexity and Opportunities for Progress

Abstract:Objectives/hypothesis: To characterize the effects of tracheotomy timing at our institution on intensive care unit (ICU) length of stay (LOS) and overall hospital LOS. Study design: Retrospective cohort study. Methods: A retrospective study was performed at a tertiary care medical center for patients undergoing tracheotomy over 2.5 years from January 1, 2016 through June 30, 2018. Demographics, survival, duration of endotracheal intubation, timing of tracheotomy, and ICU and overall hospital LOS were assessed. Tracheotomy was considered early (ET) if it was performed by day 7 of mechanical ventilation and late (LT) thereafter. Readmission, mortality, and costs were also tabulated for each aggregate group. Nonparametric statistics were used to compare results. Results: Of the 536 patients included in the analysis, 160 received tracheotomy early and 376 late. Differences between age and sex were not statistically significant. Duration of total ICU stay was shortened by 65% (12.84 ± 17.69 days vs. 38.49 ± 26.61 days; P < .0001), and length of overall hospital course was reduced by 54% (22.71 ± 26.65 days vs. 50.37 ± 34.20 days; P < .0001) in the early tracheotomy group. Observed/expected (O/E) values standardized results to case mix index and revealed LOS of 1.5 for ET and 2.5 for LT, and mortality of 0.76 for ET and 1.25 for LT, and comparable readmissions of both groups. Conclusions: Early tracheotomy in ICU patients is associated with earlier ICU discharge, decreased length of overall hospital stay, and lower mortality when controlling for case mix index. Abstract:Introduction: The 2022 AHA/ASA Guidelines for Intracerebral Hemorrhage (ICH) recommend initiating treatment and lowering blood pressure (BP) within 2 hours of ICH onset and reaching the target systolic BP of 130-150mmHg within one hour of treatment initiation. This study evaluates the effectiveness of nursing education at a Comprehensive Stroke Center (CSC) on improving the timeliness of BP medication administration and BP reduction in ICH patients. Methods: Emergency Department (ED) code stroke patients with a diagnosis of ICH were included in the analysis from 6/2022-12/2022 (pre-intervention) and 1/2023-6/2023 (post-intervention). Guideline based education regarding the importance of timely medication administration was provided in nursing huddles. Nurse to patient ratios were changed to 1:1 until goal BP achieved. BP medications were also placed in the CT scanner pyxis for immediate access. Baseline demographics, NIHSS, ICH score, and door to blood pressure reduction related time metrics were collected. Data were analyzed using R package. Results: 34 patients were analyzed, 12 (35%) pre and 22 (65%) post-intervention. Patient demographics and characteristics were not different between both groups (Table 1). Time from first BP medication order to BP in range improved: 62(46.7-80) vs 43(33-52.3) mins p=0.02. In addition, the time from door to Systolic BP 130-150mmHg improved 81 (68.5-91.3) vs 56 (37.38-73.5) mins p=0.02 (Table 2). Conclusion: Guideline based education is effective in promoting nursing-led interventions to achieve timely BP reduction of ICH patients to the target goal of 130-150 mmHg.

Abstract:Worldwide, we are experiencing an unprecedented, accelerated loss of biodiversity triggered by a bundle of anthropogenic threats such as habitat destruction, environmental pollution and climate change. Despite all efforts of the European biodiversity conservation policy, initiated 20 years ago by the Habitats Directive that provided the legal basis for establishing the Natura 2000 network, the goal to halt the decline of biodiversity in Europe by 2010 has been missed. Hochkirch et al. (2013, Conserv. Lett. 6: 462-467) identified four major shortcomings of the current implementation of the directive concerning prioritization of the annexes, conservation plans, survey systems and financial resources. However they did not account for the intended network character of the Natura 2000 sites, an aspect of highest relevance. This response letter deals with this shortcoming as it is the prerequisite, over any other strategies, ensuring a Natura 2020 network being worth its name.
