Dynamic Changes in Bladder Morphology over Time in a Population–Based Female Sample

Abstract:In the treatment of bladder and related cancers, bladder is always changing its position, size and even shape at different filling phases. For this purpose, we quantified the motion of urinary bladder. Daily ultrasound (US) measurements and weekly cone–beam computed tomography (CBCT) scans were sampled from 89 patients in supine position. bladder volume (BV), centroid differences, and three–axes lengths (Lx, Ly, and Lz) were compared across time points in the left–right (LR), anterior–posterior (AP), and superior–inferior (SI) directions. The bladder was a semi–ellipsoid, and BV increased linearly over time, but the mean urinary filling rate vtot varied largely. The vtot was correlated with age (Page, R = −0.28, p = 0.04) and water consumption (Pwat, R = 0.65, p = 0.00), vtot = 3.224(±0.806) - 0.046(±0.014)Page + 0.006(±0.001)Pwat. (p=0.00). The bladder centroid shifted towards superior and anterior with an approximate constant slope (Sp) when BV increased, but the displacement was very small in the LR direction. The Lx, Ly, and Lz increased rapidly with bladder filling for small volume (10–80 ml), especially in the LR direction. For medium volume of 80–320 ml, the SpLR, SpAP, and SpSI decreased by 86.0, 76.2, and 54.8% respectively. As BV increased to 600 ml, the SpLR, SpAP, and SpSI further decreased by 88.7, 33.8, and 45.2% respectively. For very large BV of 600–800 ml, all lengths changed very slowly. Accordingly, the eccentricities decreased except for small BV in coronal plane and larger BV in sagittal plane. The bladder morphology changed regularly in space−time at different bladder filling stages, which was helpful to bladder and related cancers.
