Abstract:Purpose: To evaluate the efficacy of Equine Assisted Therapy in children with Cerebral Palsy, in terms of gross motor function, performance, and spasticity as well as whether this improvement can be maintained for 2 months after the end of the intervention. Methods: Children with Cerebral Palsy participated in this prospective cohort study. The study lasted for 28 weeks, of which the equine assisted therapy lasted 12 weeks taking place once a week for 30 min. Repeated measures within the subject design were used for the evaluation of each child's physical performance and mental capacity consisting of six measurements: Gross Motor Function Measure-88 (GMFM-88), Gross Motor Performance Measure (GMPM), Gross Motor Function Classification System (GMFCS), Modified Ashworth Scale (MAS) and Wechsler Intelligence Scale for Children (WISC III). Results: Statistically significant improvements were achieved for 31 children in Gross Motor Function Measure and all its subcategories (p < 0.005), also in total Gross Motor Performance Measure and all subcategories (p < 0.005). These Gross Motor Function Measure results remained consistent for 2 months after the last session of the intervention. Regarding spasticity, although an improving trend was seen, this was not found to be statistically significant. Conclusion and implications: Equine Assisted Therapy improves motor ability (qualitatively and quantitatively) in children with Cerebral Palsy, with clinical significance in gross motor function.
Effects of hippotherapy on motor function of children with cerebral palsy: a systematic review study

Abstract:Cerebral palsy includes a spectrum of neurodevelopmental disorders caused by brain damage, leading to impairment of movement, posture, and balance for a lifetime. There are many therapeutic interventions for cerebral palsy. One of them is hippotherapy, an alternative physical therapy. It is a type of equine-assisted activity and therapy where children with cerebral palsy and motor function skills interact with a horse. We aimed to study the effects of hippotherapy, as an alternative therapy, on the motor function of children with cerebral palsy. We performed a review of the latest literature on hippotherapy and cerebral palsy. The criteria we used were specific keywords, publication date, age of the subjects/studied population, and article type. The outcome of our research resulted in ten relevant studies. The findings demonstrate improvements in various aspects of motor function – more specifically in gross motor function skills, balance, coordination, gait parameters, and muscle strength. Two of ten studies used the Horse-Riding Simulator with significant improvements in postural control in sitting, motor function, and hip abduction range of motion. Hippotherapy can improve motor function skills of children between 2 and 14 years of age with cerebral palsy, affecting their well-being and overall quality of life. It can have a positive effect either as a standalone treatment or as a part of other traditional therapies. Extended research is needed to assess whether hippotherapy may present significant long-term changes in motor skills in children with cerebral palsy.

Abstract:Abstract Background Physical therapy treatments incorporating equine movement are recognized as an effective tool to treat functional mobility and balance in children with cerebral palsy (CP). To date, only a few studies examined kinematic outputs of the horses and children when mounted. In this pilot study, to better understand the effectiveness of this type of treatment, we examined the interaction between the horses and children with CP during physical therapy sessions where equine movement was utilized. Methods Four children with CP participated in eight physical therapy sessions incorporating hippotherapy as a treatment intervention. Functional mobility was assessed using the Timed Up Go or the 10 m Walk Test. Inertial measurement unit sensors, attached to children and horses, recorded movements and tracked acceleration, angular velocity, and body orientation. Correlation between vertical accelerations of children and horses were analyzed. In addition, peak frequencies of vertical accelerations of children and horses were compared. Results Functional tests modestly improved over time. The children’s movements, (quantified in frequency and temporal domains) increasingly synchronized to the vertical movement of the horse’s walk, demonstrated by reduced frequency errors and increased correlation. Conclusions The findings suggest that as the sessions progressed, the participants appeared to become more familiar with the horse’s movement. Since the horse’s gait at a walk mimics the human gait this type of treatment may provide individuals with CP, who have abnormal gait patterns, an opportunity for their neuromuscular system to experience a typical gait pattern. The horse’s movement at the walk are consistent, cyclical, rhythmical, reciprocal and multi-dimensional, all of which can facilitate motor learning. The increased synchronization between horse and the mounted participant suggests that physical therapy utilizing equine movement is a viable treatment tool to enhance functional mobility. Abstract:Objective: Traditional Chinese Medicine (TCM) has considerable experience in the treatment of cerebral palsy (CP), but little evidence shows the effect of a combination of TCM and modern rehabilitation therapies on CP. This systematic review aims to evaluate the effect of integrated TCM and modern rehabilitation therapies on motor development in children with CP. Methods: We systematically searched five databases up to June 2022, including PubMed, the Cumulative Index to Nursing and Allied Health, Cochrane Library, Embase, and Web of Science. Gross motor function measure (GMFM) and Peabody Development Motor Scales-II were the primary outcomes to evaluate motor development. Secondary outcomes included the joint range of motion, the Modified Ashworth scale (MAS), the Berg balance scale, and Activities of Daily living (ADL). Weighted mean differences (WMD) and 95% confidence intervals (CIs) were used to determine intergroup differences. Results: A total of 2,211 participants from 22 trials were enrolled in this study. Among these, one study was at a low risk of bias and seven studies showed a high risk of bias. Significant improvements were found in GMFM-66 (WMD 9.33; 95% CI 0.14–18.52, P < 0.05, I 2 = 92.1%), GMFM-88 (WMD 8.24; 95% CI 3.25–13.24, P < 0.01, I 2 = 0.0%), Berg balance scale (WMD 4.42; 95% CI 1.21–7.63, P < 0.01, I 2 = 96.7%), and ADL (WMD 3.78; 95% CI 2.12–5.43, P < 0.01, I 2 = 58.8%). No adverse events were reported during the TCM intervention in the included studies. The quality of evidence was high to low. Conclusion: Integrated TCM and modern rehabilitation therapies may be an effective and safe intervention protocol to improve gross motor function, muscle tone, and the functional independence of children with CP. However, our results should be interpreted carefully because of the heterogeneity between the included studies. Abstract:Autism, Ahead of Print. The purpose of this systematic review was to synthesize the research literature on equine-assisted services for motor outcomes of autistic children. Eligible articles published between June 2012 and June 2023 were identified through electronic database searches, including PubMed, PsycINFO, CINAHL, and Web of Science. The Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines were adopted. Twelve articles met the inclusion criteria, and the extracted data included study characteristics, equine-assisted service features, and major motor outcome findings. Significant improvements were identified in coordination, strength, balance, posture, and overall motor skills, affirming the efficacy of equine-assisted services in enhancing motor outcomes of autistic children. To advance future equine-assisted service studies, prioritization of motor outcomes as primary variables is recommended. This emphasis should be evident in research aims, standardized measures, and terminology used to describe equine-assisted services.Lay abstractEquine-assisted services use various aspects of horses, like their movement, warmth, and size, to enhance individuals' overall well-being. Many studies have underscored the positive impact of equine-assisted services on autistic children, affecting areas such as social interaction, communication, engagement, stereotypical behaviors, and motor functions. Given the growing recognition of motor functions as clinical specifiers in autism, it is imperative to specifically investigate how equine-assisted services influence these outcomes. To address this, we conducted a systematic review to synthesize existing literature on the effects of equine-assisted services on motor outcomes of autistic children. Our review encompassed study characteristics, features of equine-assisted service interventions, and major motor outcomes. We also evaluated the methodological quality of the equine-assisted service studies to offer insights for future research directions. Through thorough database searches and the use of the standardized procedure of conducting a systematic review (The Preferred Reporting Items for Systematic Reviews and Meta-analyses), 12 research studies were identified and included. These studies reported the effects of various equine-assisted service types (such as therapy, horsemanship, or learning) on motor outcomes of autistic children. Significant enhancements in coordination, strength, balance, posture, and overall motor skills indicate the effectiveness of equine-assisted services in improving motor outcomes of autistic children. Building on these findings, we provide recommendations for future research endeavors aimed at enhancing the reliability and validity of using equine-assisted services to improve motor outcomes of autistic children. This work contributes to a deeper understanding of how equine-assisted services can benefit this population and guides the development of more effective equine-assisted service interventions.

Acupuncture and Tuina Treatment for Gross Motor Function in Children with Spastic Cerebral Palsy: A Monocentric Clinical Study

Abstract:Objective: Cerebral palsy (CP) is a condition characterized by abnormal pronunciation, posture, and movement, particularly spastic CP, which involves Gross motor dysfunction due to increased muscle tone and stiffness. This monocentric clinical study aims to evaluate the effectiveness of acupuncture and tuina (AT) in improving gross motor function and alleviating associated symptoms in children diagnosed with spastic CP. Methods: A total of 83 eligible patients received AT treatment, while 85 patients received conventional rehabilitation treatment. Both groups underwent a 12-week treatment period following the research protocol. Pre- and post-treatment assessments included the Modified Ashworth Muscle Tension Scale (MAS), Gross Motor Function Measure (GMFM-D and GMFM-E), 6-min walking distance measurement (6MWD), and Modified Children's Functional Independence Rating Scale (WeeFIM). Results: After 12 weeks of treatment, when compared with baseline, the scores of MAS in both AT group and control group are decreased (p<0.01, p<0.01), the scores of GMFM-D, GMFME, 6MWD, WeeFIM in both group are increased (p<0.01 in all indicators). When compared with control group, AT group had significantly lower MAS scores compared to the control group (p<0.01), indicating reduced muscle tension. Moreover, AT group showed significantly higher scores in GMFM-D, GMFM-E, 6MWD, and WeeFIM compared to the control group (p<0.01 in all indicators), indicating improved gross motor function and functional independence. The study also revealed an inverse correlation between the children's age and treatment efficacy (r= -0.496, p<0.01 in AT group, r=-0.540, p<0.01 in control group), highlighting the importance of early intervention in the management of CP in children. Conclusion: These findings suggest that AT may effectively enhance gross motor function and alleviate associated symptoms in children diagnosed with spastic CP. Moreover, early initiation of treatment is crucial to maximize therapeutic efficacy in children with spastic CP. Trial registration: Chinese Clinical Trial Registry, ChiCTR2200059823. Clinical effects of equine-assisted therapeutic exercises in static and dynamic balance of children with cerebral palsy

Abstract:Objective: To evaluate the efficacy of equine-assisted therapy for the static and dynamic balance in the rehabilitation of children and adolescents with cerebral palsy maintaining obtained improvement 8-weeks after the end of the intervention. Method: The study lasted 28 weeks, of which the intervention lasted 12 weeks. Measurements were taken before, in the middle, after the end of the intervention and follow-up after 12 weeks. Paediatric Balance Scale and dynamic plate were used to evaluate the balance. Wechsler Intelligence Scale for Children (3rd edition) was used to assess mental capacity, and the Gross Motor Function Classification System for the assessment of functional capacity for the participants. Exercises on the horseback were individualised for every participant. Results: The study comprised 27 participants with cerebral palsy. Statistically significant improvements were found for the Paediatric Balance Scale (p<0.001) and the mean dynamic plate pressure for both feet (p<0.05). The Paediatric Balance Scale results remained in the follow-up and were found clinically significant. Conclusions: The results suggest that this type of approach in rehabilitation can be beneficial with clinical significance for improving the motor dysfunctions and quality of life in cerebral palsy.

Abstract:Cerebral palsy (CP) is a nonprogressive brain injury syndrome caused by various reasons in fetuses, infants, or children. [ 1 ] The main manifestations of CP are central motor disorders and postural abnormalities. Moreover, these may be accompanied by varying intellectual disability, sensory impairment, seizures, psychological and behavioral abnormalities, and other abnormalities. CP is not a single-disease, but a group of syndromes with different clinical manifestations. [ 2 , 3 ] During childhood, the brain is constantly developing and maturing. Before the age of 3, it is a crucial stage for brain growth and development. Therefore, the clinical manifestations of children with CP may exhibit different symptoms and signs as they age. For treating children with CP, comprehensive new methods should be adopted, including physical therapy, drug therapy, rehabilitation training, psychological therapy, etc. [ 4 , 5 ] The goal of treatment is to improve the symptoms and signs of the affected children, enhance their quality of life, and enable them to integrate into society as much as possible. Goal-activity-motor enrichment (GAME) therapy is a family-centered rehabilitation approach. GAME develops information and exercise training plans based on parents' issues and requirements and the problems faced by the child. It combines family education with a rich learning environment for children, which can alleviate their motor dysfunction through specific activities and exercise environments. [ 6–8 ] Family posture management is a method of adjusting and controlling posture for special needs children. Parents should maintain the correct posture of their children in their daily care. This can effectively prevent joint contracture deformation, reduce musculoskeletal deformities, and improve gross motor ability, thus enhancing the daily living activities of children. [ 9 , 10 ] In view of this, the study combines home posture control with GAME therapy to explore its impact on children's motor function with severe CP through randomized controlled trials. This paper aims to improve children's motor function having severe CP, enhance their daily living abilities, and lay a solid foundation for their future integration into society.

Abstract:This study aimed to evaluate the effect of physical exercise intervention on gross motor function in children with cerebral palsy (CP).Computer searches of Web of Science, PubMed, Embase (Ovid), Cochrane Library, and Medline databases were conducted on October 6, 2021.Randomized controlled trials (RCTs) or quasi-experimental studies were included.Ten methodological quality trials were identified, and the literature's quality was assessed using the PEDro scale.Data from the Gross Motor Function Assessment Scale GMFM-66, GMFM-88 and A (lying and turning), B (sitting), C (climbing and kneeling), D (standing), and E (walking, running, and jumping) dimension scores were extracted, and Meta-analysis was performed using Revman 5.4.Data were pooled and expressed as mean difference (MD) or standardized difference (SMD) with 95% CI.The results showed that GMFM-66 [SMD=1.90,95%CI(0.55~3.25),Z=2.76, P=0.006], 95%CI (0.83 ~ 2.15), Z=4.42, P<0.00001] and D dimension [SMD=3.13,95%CI (1.23 ~ 5.02), Z=3.23, P=0.001] and E dimension [SMD=3.34,95% CI (2.00 ~ 4.69), Z=4.88, P<0.00001].The study results showed that the differences between the results of the experimental and control groups were statistically significant.Physical exercise could significantly improve gross motor function in children with CP and could be used as a reference for physical therapy in children with CP.These findings should be used with caution based on the limitations of this study.
Feasibility of using a large amplitude movement therapy to improve ambulatory function in children with cerebral palsy

Abstract:Cerebral palsy (CP) is the most common cause of motor disability among children. Limited evidence exists regarding the efficacy of traditional rehabilitation strategies on improving ambulatory function in this population. The purpose of the study was to investigate the feasibility and short-term effects of a novel large amplitude movement therapy on ambulatory functions in children with CP. Temporal-spatial gait characteristics were examined before and after a single intervention session, replicated over five children. Five children with CP (7.0 ± 1.0 years); Gross Motor Function Classification System Levels I-II, participated. Baseline gait parameters were obtained as the participant walked across an instrumented walkway at self-selected and fast speeds. Children then participated in a 20-30 min intervention focused on making body and limb movements as large as possible with gait assessment repeated immediately. All children tolerated testing and therapy with no adverse effects. Outcomes after one intervention included: significantly greater stride velocity; reduced double support time; and greater stride length after training for three of the five participants. Results for this pilot study suggested that the large amplitude movement therapy was feasible for children with CP. There is a need for a larger scale study to determine if the protocol can be effective at an appropriate clinical dose.
The effectiveness and safety of electromyography biofeedback therapy for motor dysfunction of children with cerebral palsy: A protocol for systematic review and meta-analysis.

Abstract:Introduction: To investigate the effectiveness and safety of electromyography (EMG) biofeedback therapy in improving motor dysfunction among children with cerebral palsy (CP). Methods and analysis: The following databases will be searched: PubMed, EMBASE, ScienceDirect, the Cochrane Library, China National Knowledge infrastructure (CNKI), Technology Periodical Database (VIP), WanFang Data and China Biology Medicine (CBM) from inception to June 2019. All relevant randomized controlled trials (RCTs) utilizing EMG biofeedback therapy for CP will be included. The main outcome is the Gross Motor Function Measure (GMFM). Additional outcomes such as the Modified Ashworth Scale (MAS), Integral Electromyogram (iEMG), Composite Spasticity Scale (CSS), passive range of motion (PROM) or other related outcomes will be included, adverse effects of EMG biofeedback therapy and comparators will also be included. Two reviewers will screen studies, extract data and assess quality independently. Review Manager 5.3 will be used to assess the risk of bias, data synthesis, and subgroup analysis. Ethics and dissemination: This systematic review does not require formal ethical approval because all data will be analyzed anonymously. Results will provide a general overview and evidence concerning the effectiveness and safety of EMG biofeedback therapy for children with CP. The findings of this systematic review will be disseminated through peer-reviewed publications or conference presentations.
Goal-Directed Therapy to Improve Gross Motor Function and the Quality of Life of Children with Cerebral Palsy: A Randomized Controlled Trial

Abstract:Background: The multiplicity of interventions for the treatment of cerebral palsy (CP) can cause confusion about which are most suited to certain individuals. Hypothesis is that goal-directed therapy (GDT) can guide integrating therapies to improve clinical outcomes compared with conventional therapy (CT). Method: A prospective, assessor-blinded, randomized controlled trial was done with 23 children with CP (mean age, 4 years 4 months old; SD 1y4mo), who were divided into groups according to their level of gross motor function: GDT and CT. Both groups received 12 physiotherapy (PT) sessions and advice on daily home programs. The GDT group additionally had a team meeting to set a specific goal, and PT programs were shaped toward that goal. Assessments were done at baseline and after treatments, using the Thai-version Gross Motor Function Measure (GMFM-66), CP-Quality of Life (CP-QOL), caregiver burden, and home program compliance. Results: After the treatments, the GDT group showed significant improvements in GMFM-66, CP-QOL, and caregiver burden, while the CT group revealed improvements in caregiver burden and some domains of the GMFM, including sitting and crawling & kneeling. Comparisons between groups found GDT was more effective than CT in improving GMFM-66 and CP-QOL. Home program compliance was higher in the GDT (69%) than the CT group (42%). Conclusion: GDT demonstrated clear gains for children with CP regarding gross motor function and QOL improvements. Team communication toward a customized goal was crucial, empowering the children and their caregivers to comply with home programs to achieve the set goal.

Abstract:Background/Aims Despite its broad application in paediatric care, the effectiveness of therapy based on the Bobath concept has not been systematically researched. The aim of this review was to explore whether there is scientific evidence regarding the validity of the Bobath concept as an effective therapy for children with cerebral palsy. Methods A search was conducted between March and December 2020, of research databases (Cochrane, PEDro, PubMed, Web of Science), selecting randomised controlled trials published since 2015, which compared Bobath therapy with other therapies in terms of their effect on the gross motor functions of children with cerebral palsy. Results Applying the eligibility criteria, four randomised controlled trials were selected, three of which found Bobath therapy to be more effective than other therapies as a therapeutic method, while one found no difference between Bobath therapy and another therapy. No study that met the inclusion criteria found Bobath therapy to be less effective than another therapy. Conclusions The principal limitation of this review has been the small number of randomised controlled trials found and so the results must be interpreted with caution. Nonetheless, studies showed that therapy based on Bobath therapy improves the gross motor function of children with cerebral palsy.
Effects of intensive therapy program on gross motor function of children with spastic cerebral palsy

Abstract:Objective To explore the effects of intensive therapy program on gross motor function of children with spastic cerebral palsy.Methods Thirty 3-15 year old children with spastic cerebral palsy and level Ⅰ-Ⅲ in gross motor function classification system (GMFCS) were randomly divide into 2 groups.The trial group adopted intensive therapy program for treatment,including functional dynamic suit and universal exercise unit combined with functional movement training.The control group adopted core stability training for treatment.Children in two groups took a 1-month training program for 3 hours per day and 5 days per week.The pediatric evaluation of gross motor function measure (GMFM-66) and peabody developmental motor scale (PDMS-2) were administered before and after treatment.Results The scores of GMFM-66 as well as the stationary and locomotion scores of PDMS-2 in both groups showed significant differences within group (P ＜ 0.01) after treatment.For the scores of GMFM and PDMS-2 between two groups,no significant differences were found.Conclusions The intensive therapy program can improve the gross motor function of children with spastic cerebral palsy and gain the same effects as core stability training,which can provide a novel and effective intervention for children with cerebral palsy.
Effects of exercise intervention on balance function in children with cerebral palsy: a systematic review and meta-analysis of randomized controlled trials

Abstract:Objective: To determine the effectiveness of exercise intervention on postural balance, gait parameters, and muscle strength in children with cerebral palsy by quantifying the information from randomized controlled trials (RCTs). Methods: We conducted a systematical search for RCTs from the databases, including PubMed, ISI Web of Science, and Scopus using a between-group design involving children with cerebral palsy and assessing the effect of exercise intervention on postural balance, gait parameters, and muscle strength. The specified inclusion criteria were determined by the PICOS tool. The outcomes of included studies were evaluated by meta-analysis, and subgroup and sensitivity analyses were conducted to analyze the observed heterogeneities using Review Manager 5.4 and Stata version 18.0. The revised Cochrane risk of bias tool for randomized trials (RoB 2) was used to evaluate the risk of bias and quality of the included studies. Results: Twenty-four studies were included in this meta-analysis, with 579 children with cerebral palsy. Exercise intervention showed a statistically significant favorable effect on gross motor function (SMD = 0.32; 95%CI [0.03 to 0.61]; I2 = 16%), anteroposterior stability index (SMD = -0.93; 95%CI [-1.69 to -0.18]; I2 = 80%), and mediolateral stability index (SMD = -0.60; 95%CI [-1.16 to -0.03]; I2 = 73%) compared to control group among children with cerebral palsy. None of the above meta-analyses exhibited publication bias, as indicated by Egger's test with p-values greater than 0.05 for all. Conclusions: Exercise is effective in improving gross motor function and balance in children with cerebral palsy. Due to the lack of studies examining the efficacy of each exercise type, we are unable to provide definitive training recommendations.

Complementary Therapies in Clinical Practice

Abstract:Purpose: To use Traditional Chinese Medicine (TCM) and Western approaches to improve gross motor function and activities of daily living (ADL) in children with spastic cerebral palsy. Methods: Children were randomly divided into a treatment group, which received scalp acupuncture combined with exercise therapy and conventional rehabilitation training, and a control group, which received conventional rehabilitation training alone. Study subjects' gross motor function (gross motor function measure-88 [GMFM-88]) and ADL were evaluated before and after therapy. Results: GMFM-88 and ADL scores were significantly improved in both groups after therapy, but the within group differences in post- and pre-therapy GMFM-88 and ADL scores were significantly higher in the treatment group compared to the control group. Conclusion: Scalp acupuncture combined with exercise therapy and conventional rehabilitation training can significantly improve gross motor function and the ability to perform ADLs in children with spastic cerebral palsy compared to conventional rehabilitation training alone.

Abstract:Physiotherapy plays a key role in the management of cerebral palsy (CP) and comprises of various therapeutic interventions in enhancing the various physiological and functional outcomes. Though physiotherapy is used widely and recommended by all members of the health-care team, the effectiveness of physiotherapy is inconsistent. The objective of this review was to summarize and evaluate the effectiveness of physiotherapy interventions in children with C P. PubMed and Cochrane database were searched from January 2006 to April 2017 using the Medical Subject Heading and general keywords. Only systematic reviews and meta-analysis on PT interventions in children diagnosed with CP were included. Two reviewers independently assessed the methodological quality and retrieved the results. Thirty-four systematic reviews were identified that distinguished 15 different interventions. Moderate evidence of effectiveness was found for constraint-induced movement therapy for upper limb recovery, goal-directed/functional training, and gait training to improve gait speed. Conflicting evidence was found for the role of exercises on strength training and cardiorespiratory training. Intervention such as neurodevelopmental therapy (NDT) was found ineffective. This review suffer from limitations such as including reviews that had small sample size and that had considered heterogeneity of treatment interventions. Hence, the effectiveness of most PT interventions is found to be limited. On the basis of the present evidence, functional goal-oriented approaches are found to be effective and future research is required to determine the best ways to improve functional outcomes in children with CP.

Abstract:Objective To study the effects of motor-development massage therapy on gross motor function in children with cerebral palsy.Methods According to the inclusion and exclusion criterion 142 study participants were included from children with cerebral palsy who had been treated in Rehabilitation Center of Children′s Hospital, Fudan University from Aug. 2000 to Sept.2006, with male 93 ,female 49; spastic 126,athetoid 8,mixed 7 and ataxic 1.The average age is 21.84 months(range from 5 to 95 months)before therapy. The changes of gross motor function in children with cerebral palsy before and after motor-development massage therapy were analyzed by the scoreof gross motor function measure(GMFM). In order to further study the effects of motor-development massage therapy on gross motor function in children with cerebral palsy, two group samples (baseline group with 26 cases and follow-up group with 31 cases) were determined from the total study participants for analysis according to certain inclusion and exclusion criteria. Differences of change of GMFM scores were compared during baseline period, therapy period and follow-up period. Relative validity was adopted to determine the GMFM dimension with the most significant gross motor function change in total study participants. Rasch analysis was used to compare the change of item difficulty of the GMFM dimension with the most significant gross motor function change before and after therapy.Results GMFM scores increased significantly after a therapy period of (3.3±1.4)months in most children of 142 participants(P0.001).The average monthly change value of GMFM 66 and 88 scores in baseline group was significantly higher in therapy period [(2.4±1.0 )months]than in baseline period[(1.9±1.1 )months](P0.05). The average monthly change value of four dimensions of GMFM-88 scores(except dimension A) was significantly higher in therapy period than in baseline period, which was more apparent with dimension D and E(P0.05). The average monthly change value of GMFM scores (except dimension A)in follow-up group was significantly higher in therapy period [(2.7±1.1 )months]than in follow-up period[(2.5±1.2 )months](P0.05). When evaluating the change of gross motor function before and after therapy in total study participants, dimension B of GMFM got the highest relative validity(RV=100%),then GMFM-88 (RV=94%) and GMFM-66(RV=79%).Rasch analysis revealed similar distribution of 20 item difficulty and individual capacity of dimension B before and after therapy in total participants. There was no ststistical difference in 17 of 20 items with paired t test. Also high correlation existed between item difficulty values before and after therapy(r=0.992 1). The results showed motor-development massage therapy hadn't greatly changed the difficulty distribution, difficulty value and difficulty order of 20 items of dimension B of GMFM.Conclusions Motor-development massage therapy could effectively enhance the total gross motor function of children with cerebral palsy in a short period.Moreover, those enhanced gross motor function could maintain for a short period.
Efficacy of cycling interventions to improve function in children and adolescents with cerebral palsy: a systematic review and meta-analysis

Abstract:Objectives: The aim of this study was to determine the efficacy of cycling to improve function and reduce activity limitations in children with cerebral palsy; the optimal training parameters for improved function; and whether improvements in function can be retained. Method: Six databases were searched (until February 2019) and articles were screened in duplicate. Randomized or quasi-randomized controlled trials and pre-post studies were included. Methodological quality was assessed using the Downs and Black scale. Outcomes were reported under the International Classification of Functioning, Disability and Health domains of body functions and activity limitations. Quantitative analyses were completed using RevMan V5.3. Results: A total of 533 articles were identified and 9 studies containing data on 282 participants met full inclusion criteria. Methodological quality ranged from low (14 of 32) to high (28 of 32). Significant improvements were reported for hamstring strength (effect size = 0.77-0.93), cardiorespiratory fitness (effect size = 1.13-1.77), balance (effect size = 1.03-1.29), 3-minute walk test distance (effect size = 1.14) and gross motor function (effect size = 0.91). Meta-analysis suggested that cycling can improve gross motor function (standardized mean difference = 0.35; 95% confidence interval = (-0.01, 0.70); P = 0.05); however, the effect was insignificant when a poor-quality study was omitted. Conclusion: Cycling can improve muscle strength, balance and gross motor function in children with cerebral palsy; however, optimal training doses are yet to be determined. There was insufficient data to determine whether functional improvements can be retained. Conclusions were limited by small sample sizes, inconsistent outcome measures and a lack of follow-up testing.

Abstract:Review question / Objective: Cerebral palsy (CP) is a non-progressive, persistent syndrome occurring in the brain of the fetus or infant[1]. The prevalence of CP is 0.2% worldwide, and the prevalence can increase to 20-30 times in preterm or low birth weight newborns. There are about 6 million children with CP in China, and the number is increasing at a rate of 45,000 per year. Virtual reality (VR) refers to a virtual environment that is generated by a computer and can be interacted with.VR can mobilize the visual, auditory, tactile and kinesthetic organs of CP, so that they can actively participate in the rehabilitation exercise.
