Abstract:The incidence of invasive aspergillosis (IA) , which is commonly caused by Aspergillus fumigatus, has increased recently in immunocompromised patients and has become the common cause of death in these patients. Antifungal resistance is one of the reasons for treatment failure. Since the first itraconazole-resistant A. fumigatus was reported in 1997, the reports on clinical strains of triazole-resistant A. fumigatus have increased, as well as studies on the resistant mechanisms. In this paper, the known molecular mechanisms of antifungal resistance in Aspergillus, especially in A. fumigatus, are reviewed.

Abstract:The growing use of immunosuppressive therapies has resulted in a dramatic increased incidence of invasive fungal infections (IFIs) caused by Aspergillus fumigatus , a common pathogen, and is also associated with a high mortality rate. Azoles are the primary guideline-recommended therapy agents for first-line treatment and prevention of IFIs. However, increased azole usage in medicinal and agricultural settings has caused azole-resistant isolates to repeatedly emerge in the environment, resulting in a significant threat to human health. In this review, we present and summarize current research on the resistance mechanisms of azoles in A. fumigatus as well as efficient susceptibility testing methods. Moreover, we analyze and discuss the putative clinical (bedside) indication of these findings from bench work.
Azole resistance in Aspergillus fumigatus: a growing public health concern

Abstract:Purpose of review: Reports from the end of the 2000s forced the medical community to take azole resistance in Aspergillus fumigatus into account. Not only patients with chronic aspergillus disease, who develop resistance during long-term azole treatment, but also azole-naive patients are at risk, owing to the presence of resistant strains in the environment. The purpose of this review is to overview the latest findings concerning the origin, evolution, and implications of azole resistance in A. fumigatus. Recent findings: TR34/L98H is the predominant resistance mechanism of environmental origin in A. fumigatus. Recent epidemiological data show that this mechanism is an expanding problem, with reports from China, Iran, and India. However, the TR34/L98H strains from the Middle East are genotypically different from the European isolates; their emergence is, therefore, not due to simple geographical spread of the 'European' isolates. A new environmental resistance mechanism, TR46/Y121F/T289A, was detected in the Netherlands, conferring voriconazole resistance. In patients chronically treated with triazoles, the spectrum of resistance has become more diverse, with the emergence of non-CYP51A-mediated mechanisms. Central registration of treatment and outcome data of patients with resistant aspergillus disease are needed. Summary: Azole resistance in A. fumigatus is evolving to a global health problem.

Abstract:Agricultural industry uses pesticides to optimize food production for the growing human population. A major issue for crops is fungal phytopathogens, which are treated mainly with azole fungicides. Azoles are also the main medical treatment in the management of Aspergillus diseases caused by ubiquitous fungi, such as Aspergillus fumigatus. However, epidemiological research demonstrated an increasing prevalence of azole-resistant strains in A. fumigatus. The main resistance mechanism is a combination of alterations in the gene cyp51A (TR34/L98H). Surprisingly, this mutation is not only found in patients receiving long-term azole therapy for chronic aspergillosis but also in azole naïve patients. This suggests an environmental route of resistance through the exposure of azole fungicides in agriculture. In this review, we report data from several studies that strongly suggest that agricultural azoles are responsible for medical treatment failure in azole-naïve patients in clinical settings.

Abstract:Aspergillus fumigatus, a ubiquitous pathogen, causes aspergillosis in humans, especially in immunodeficient patients. Azoles are frontline antifungal drugs for treating aspergillosis. The recent global emergence of azole resistance in A. fumigatus has become a serious problem worldwide. It has arisen through two routes: long-term azole medical therapy, called the patient route, and the use of azole fungicides in its habitats especially for agricultural activities, called the environmental route. Resistant strains developed through the latter route show cross-resistance to medical azoles because of the identical molecular target Cyp51A between azole compounds used for medical treatment and agricultural disease control. In azole-resistant strains arising through the environmental route, A. fumigatus is observed frequently to possess mutations in the cyp51A gene linked to tandem repeats in the promoter region such as TR34 /L98H and TR46 /Y121F/T289A. The results of microsatellite genotyping analyses of resistant A. fumigatus strains have suggested a transboundary spread of this microorganism in many countries. Diverse actors are involved in the global highway of transmission. Therefore, the matter must be addressed as a "One Health" issue. This review presents a background of azole resistance in A. fumigatus and introduces newly discovered difficulties generated as this pathogen spreads worldwide.

Abstract:Triazole-resistance in Aspergillus fumigatus is widespread. We evaluated whether triazole-resistance in A. fumigatus and its related cyp51A mutations, induced by a combination of the triazole fungicides difenoconazole and propiconazole, differs from resistance induced by the individual fungicides. Both difenoconazole and propiconazole can induce triazole-resistance in A. fumigatus. Resistance is much easier induced by formulated fungicides or a combination of these two fungicides compared with standard fungicides or individual fungicides, respectively. Six different mutations (G138S, G138D, H147Y, I246M, M263I and D430N) were identified in the induced resistant strains. The H147Y, I246M and M263I mutations were associated with triazole-resistance. This implies that the application of a combination of difenoconazole and propiconazole may result in higher triazole-resistance in A. fumigatus and more mutations in the cyp51A gene.

Abstract:Purpose of the Review Azole-resistant Aspergillus fumigatus is an emerging clinical problem. Resistance can develop both with clinical exposure and with environmental exposure to azole and azole-like fungicides used against phytopathogens. Here, our current understanding of the epidemiology and prevalence of this emerging problem is reviewed. Recent Findings Azole-resistant A. fumigatus is now a worldwide problem, as it has been documented in clinical and environmental samples on all but one continent. Although Europe has been the center of this issue, hotspots are now emerging in Asia. However, there are several limitations to our current understanding of the extent of this problem, including issues with surveillance strategies and our reliance on phenotypic methods for detecting resistance. Summary Azole-resistant A. fumigatus is an emerging issue both clinically and in the environment that requires greater attention in order to preserve this very useful class of antifungal agents for the treatment of aspergillosis.

Abstract:<h3 class="a-plus-plus">Purpose of Review</h3>Azole resistance in Aspergillus spp. is becoming a public health problem worldwide. However, data about this subject is lacking in Latin American countries. This review focuses in the epidemiology and molecular mechanisms of azole resistance in Aspergillus spp. emphasizing in Latin America. Data on Aspergillus fumigatus stands out because it is the most prevalent Aspergillus spp. pathogen.<h3 class="a-plus-plus">Recent Findings</h3>Azole resistance in Aspergillus spp. emergence was linked with intensive use of these antifungals both in the clinical setting and in the environment (as pesticides). Reports on azole-resistant A. fumigatus strains are being constantly published in different countries. Molecular mechanisms of resistance mainly involve substitution in the azole target (CYP51A) and/or overexpression of this gene. However, several other non-CYP51A-related mechanisms were described. Moreover, intrinsically resistant cryptic Aspergillus species are starting to be reported as human pathogens.<h3 class="a-plus-plus">Summary</h3>After a comprehensive literature review, it is clear that azole resistance in Aspergillus spp. is emerging in Latin America and perhaps it is underestimated. All the main molecular mechanisms of azole resistance were described in patients and/or environmental samples. Moreover, one of the molecular mechanisms was described only in South America. Cryptic intrinsic azole-resistant species are also described.

Abstract:The airborne fungus Aspergillus fumigatus poses a serious health threat to humans by causing numerous invasive infections and a notable mortality in humans, especially in immunocompromised patients. Mould-active azoles are the frontline therapeutics employed to treat aspergillosis. The global emergence of azole-resistant A. fumigatus isolates in clinic and environment, however, notoriously limits the therapeutic options of mould-active antifungals and potentially can be attributed to a mortality rate reaching up to 100 %. Although specific mutations in CYP 51A are the main cause of azole resistance, there is a new wave of azole-resistant isolates with wild-type CYP 51A genotype challenging the efficacy of the current diagnostic tools. Therefore, applications of whole-genome sequencing are increasingly gaining popularity to overcome such challenges. Prominent echinocandin tolerance, as well as liver and kidney toxicity posed by amphotericin B, necessitate a continuous quest for novel antifungal drugs to combat emerging azole-resistant A. fumigatus isolates. Animal models and the tools used for genetic engineering require further refinement to facilitate a better understanding about the resistance mechanisms, virulence, and immune reactions orchestrated against A. fumigatus. This review paper comprehensively discusses the current clinical challenges caused by A. fumigatus and provides insights on how to address them.

Abstract:Abstract Purpose of Review Azole resistance in Aspergillus fumigatus is an emerging public health issue with global distribution and has been linked to use in agricultural and horticultural settings. In 2022, the World Health Organization (WHO) created a fungal pathogen priority list, and A. fumigatus was listed as a critical pathogen. Currently, Africa lacks effective surveillance systems for this emerging threat, mostly due to lack of capacity and diagnostics to determine azole resistance in routine clinical settings. This review aims to address and improve on the current diagnostic tools and future perspective strategies in tackling clinical and environmental antifungal-resistant (AFR) A. fumigatus in Africa. We emphasized on the importance of early diagnosis and misdiagnosis associated with aspergillosis caused by Aspergillus sp., cross talk between clinical and environmental, mode of action and resistance mechanism, collaborative one health approach, and future perspectives for AFR A. fumigatus management strategies. Recent Findings Early diagnosis and effective management of invasive aspergillosis are critical. On the continent, very few laboratories routinely conduct antifungal susceptibility testing on Aspergillus species. Where this occurs, it is culture-based in vitro antifungal susceptibility testing. Drug repurposing and the need for a non-culture-based molecular method (PCR) are critical. Summary Enhancing promising future perspectives of non-cultured approaches such as whole-genome sequencing, CRISPR/Cas9, and RNAi-mediated technologies to complement the culture-based approach as important strategies to mitigate and overcome emerging issues of AFR A. fumigatus in Africa.

Abstract:Drug resistance poses a serious threat to human health and agricultural production. Azole drugs are the largest group of 14-α sterol demethylation inhibitor fungicides that are used both in agriculture and in clinical practice. As plant-pathogenic molds share their natural environment with fungi that cause opportunistic infections in humans, both are exposed to a strong and persistent pressure of demethylase inhibitor (DMI) fungicides, including imidazole and triazole drugs. As a result, a loss of efficacy has occurred for this drug class in several species. In the clinical setting, Aspergillus fumigatus azole resistance is a growing public health problem, and finding the source of this resistance has gained much attention. It is urgent to determine if there is a direct link between the agricultural use of azole compounds and the different A. fumigatus resistance mechanisms described for clinical triazoles. In this study, we performed A. fumigatus susceptibility testing against clinical triazoles and crop protection DMIs using a collection of azole-susceptible and -resistant strains which harbor most of the described azole resistance mechanisms. Various DMI susceptibility profiles have been found in the different A. fumigatus population groups based on their azole resistance mechanism and previous whole-genome sequencing (WGS) analysis, which suggests that the different resistance mechanisms have different origins and are specifically associated with the local use of a particular DMI. IMPORTANCE Due to the worldwide emergence of A. fumigatus azole resistance, this opportunistic pathogen poses a serious health threat, and therefore, it has been included in the watch list in the CDC publication Antibiotic Resistance Threats in the United States, 2019 (CDC, 2019). Azoles play a critical role in the control and management of fungal diseases, not only in the clinical setting but also in agriculture. Thus, azole resistance leads to a limited therapeutic arsenal which reduces the treatment options for aspergillosis patients, increasing their mortality risk. Evidence is needed to understand whether A. fumigatus azole resistance is emerging from an agricultural source due to the extended use of demethylase inhibitors as fungicides or whether it is coming from somewhere else, such as the clinical setting. If the environmental route is demonstrated, the current use and management of azole antifungal compounds might be forced to change in the coming years.

Abstract:Aspergillus fumigatus is an opportunistic human pathogen that causes aspergillosis, a spectrum of environmentally acquired respiratory illnesses. It has a cosmopolitan distribution and exists in the environment as a saprotroph on decaying plant matter. Azoles, which target Cyp51A in the ergosterol synthesis pathway, are the primary class of drugs used to treat aspergillosis. Azoles are also used to combat plant pathogenic fungi. Recently, an increasing number of azole-naive patients have presented with pan-azole–resistant strains of A . fumigatus . The TR 34 /L98H and TR 46 /Y121F/T289A alleles in the cyp51A gene are the most common ones conferring pan-azole resistance. There is evidence that these mutations arose in agricultural settings; therefore, numerous studies have been conducted to identify azole resistance in environmental A . fumigatus and to determine where resistance is developing in the environment. Here, we summarize the global occurrence of azole-resistant A . fumigatus in the environment based on available literature. Additionally, we have created an interactive world map showing where resistant isolates have been detected and include information on the specific alleles identified, environmental settings, and azole fungicide use. Azole-resistant A . fumigatus has been found on every continent, except for Antarctica, with the highest number of reports from Europe. Developed environments, specifically hospitals and gardens, were the most common settings where azole-resistant A . fumigatus was detected, followed by soils sampled from agricultural settings. The TR 34 /L98H resistance allele was the most common in all regions except South America where the TR 46 /Y121F/T289A allele was the most common. A major consideration in interpreting this survey of the literature is sampling bias; regions and environments that have been extensively sampled are more likely to show greater azole resistance even though resistance could be more prevalent in areas that are under-sampled or not sampled at all. Increased surveillance to pinpoint reservoirs, as well as antifungal stewardship, is needed to preserve this class of antifungals for crop protection and human health.

Surveillance of Amphotericin B and Azole Resistance in Aspergillus Isolated from Patients in a Tertiary Teaching Hospital

Abstract:The genus Aspergillus harbors human infection-causing pathogens and is involved in the complex one-health challenge of antifungal resistance. Here, a 6-year retrospective study was conducted with Aspergillus spp. isolated from patients with invasive, chronic, and clinically suspected aspergillosis in a tertiary teaching hospital. A total of 64 Aspergillus spp. clinical isolates were investigated regarding molecular identification, biofilm, virulence in Galleria mellonella, antifungal susceptibility, and resistance to amphotericin B and azoles. Aspergillus section Fumigati (A. fumigatus sensu stricto, 62.5%) and section Flavi (A. flavus, 20.3%; A. parasiticus, 14%; and A. tamarii, 3.1%) have been identified. Aspergillus section Flavi clinical isolates were more virulent than section Fumigati clinical isolates. Furthermore, scant evidence supports a link between biofilm formation and virulence. The susceptibility of the Aspergillus spp. clinical isolates to itraconazole, posaconazole, voriconazole, and amphotericin B was evaluated. Most Aspergillus spp. clinical isolates (67.2%) had an AMB MIC value equal to or above 2 µg/mL, warning of a higher probability of therapeutic failure in the region under study. In general, the triazoles presented MIC values above the epidemiological cutoff value. The high triazole MIC values of A. fumigatus s.s. clinical isolates were investigated by sequencing the promoter region and cyp51A locus. The Cyp51A amino acid substitutions F46Y, M172V, N248T, N248K, D255E, and E427K were globally detected in 47.5% of A. fumigatus s.s. clinical isolates, and most of them are associated with high triazole MICs. Even so, the findings support voriconazole or itraconazole as the first therapeutic choice for treating Aspergillus infections. This study emphasizes the significance of continued surveillance of Aspergillus spp. infections to help overcome the gap in knowledge of the global fungal burden of infections and antifungal resistance, supporting public health initiatives.

Abstract:Aspergillus fumigatus is a ubiquitous pathogenic mold and causes several diseases, including mycotoxicosis, allergic reactions, and systemic diseases (invasive aspergillosis), with high mortality rates. In its ecological niche, the fungus has evolved and mastered many reply strategies to resist and survive against negative threats, including harsh environmental stress and deficiency of essential nutrients from natural environments, immunity responses and drug treatments in host, and competition from symbiotic microorganisms. Hence, treating A. fumigatus infection is a growing challenge. In this review, we summarized A. fumigatus reply strategies and escape mechanisms and clarified the main competitive or symbiotic relationships between A. fumigatus , viruses, bacteria, or fungi in host microecology. Additionally, we discussed the contemporary drug repertoire used to treat A. fumigatus and the latest evidence of potential resistance mechanisms. This review provides valuable knowledge which will stimulate further investigations and clinical applications for treating and preventing A. fumigatus infections.

Abstract:The use of antifungal agents, especially the azole class, has increased in parallel with a higher incidence of fungal infections, particularly in immunocompromised patients. This situation has favored the appearance of Candida species, prominent among them C. albicans and C. globrata, with acquired resistance to these agents. This review focuses on the latest developments in investigations of molecular mechanisms contributing to azole resistance. Multiple resistance mechanisms have been described that can coexist in resistant clinical isolates. Understanding resistance mechanisms is of value not only for the design of new antifungal agents but also the development of strategies of overcome or delay the emergence of resistance.

Abstract:Azole resistance in Aspergillus fumigatus has emerged as a worldwide public health problem. We sought here to demonstrate the occurrence and characteristics of azole resistance in A. fumigatus from different parts of China. A total of 317 clinical and 144 environmental A. fumigatus isolates from 12 provinces were collected and subjected to screening for azole resistance. Antifungal susceptibility, cyp51A gene sequencing, and genotyping were carried out for all suspected azole-resistant isolates and a subset of azole-susceptible isolates. As a result, 8 (2.5%) clinical and 2 (1.4%) environmental A. fumigatus isolates were identified as azole resistant. Five azole-resistant strains exhibit the TR34/L98H mutation, whereas four carry the TR34/L98H/S297T/F495I mutation in the cyp51A gene. Genetic typing and phylogenetic analysis showed that there was a worldwide clonal expansion of the TR34/L98H isolates, while the TR34/L98H/S297T/F495I isolates from China harbored a distinct genetic background with resistant isolates from other countries. High polymorphisms existed in the cyp51A gene that produced amino acid changes among azole-susceptible A. fumigatus isolates, with N248K being the most common mutation. These data suggest that the wide distribution of azole-resistant A. fumigatus might be attributed to the environmental resistance mechanisms in China.

Abstract:Objectives: Invasive mold infections associated with Aspergillus species are a significant cause of mortality in immunocompromised patients. The most frequently occurring aetiological pathogens are members of the Aspergillus section Fumigati followed by members of the section Terrei. The frequency of Aspergillus terreus and related (cryptic) species in clinical specimens, as well as the percentage of azole-resistant strains remains to be studied. Methods: A global set (n = 498) of A. terreus and phenotypically related isolates was molecularly identified (beta-tubulin), tested for antifungal susceptibility against posaconazole, voriconazole, and itraconazole, and resistant phenotypes were correlated with point mutations in the cyp51A gene. Results: The majority of isolates was identified as A. terreus (86.8%), followed by A. citrinoterreus (8.4%), A. hortai (2.6%), A. alabamensis (1.6%), A. neoafricanus (0.2%), and A. floccosus (0.2%). One isolate failed to match a known Aspergillus sp., but was found most closely related to A. alabamensis. According to EUCAST clinical breakpoints azole resistance was detected in 5.4% of all tested isolates, 6.2% of A. terreus sensu stricto (s.s.) were posaconazole-resistant. Posaconazole resistance differed geographically and ranged from 0% in the Czech Republic, Greece, and Turkey to 13.7% in Germany. Abstract:Aspergillus fumigatus can cause different clinical manifestations including chronic pulmonary infections, as well as invasive aspergillosis which is highly mortal in the immunocompromised host. Azole antifungal drugs, voriconazole in particular, are the first-line recommended therapeutic regimen. Azoles inhibit 14-α demethylase enzyme encoded by the cyp51A gene. In recent years, increased azole resistance is observed among environmental and clinical A.fumigatus isolates. Two different mechanisms have been proposed for the development of resistance. The first one is the triggering of resistance as a result of long-term clinical azole use. Point mutations in cyp51A gene are generally responsible for this type of azole resistance. The second mechanism is incidental environmental azole exposure due to the use of azoles as agricultural fungicides. Contact with azoles for extended periods and at varying concentrations causes selective pressure and mutations on sporulating A.fumigatus. Since the resistant strains may persist in nature, susceptible individuals may be infected by acquisition of these strains from the environment. When genotypically examined, the cyp51A gene of the resistant isolates of environmental origin specifically presents with a tandem repeat in the promoter region in addition to the point mutation in codon 98 (TR34/L98H). The aim of this study was to investigate azole resistance rates in A.fumigatus strains isolated from clinical specimens and landscaping areas around Hacettepe University Faculty of Medicine Hospital by phenotypical and genotypical methods. Agar screening test was used as the initial test to detect azole resistance in isolates identified as A.fumigatus sensu stricto according to thermotolerance test results. For all strains that grew on any of the azole containing plates in agar screening test, minimum inhibitory concentration (MIC) values were determined by "European Committee on Antimicrobial Susceptibilitiy Testing" reference microdilution method for the confirmation of the resistance. In addition, cyp51A gene sequence was investigated in selected isolates and mutation analysis was performed. A total of 483 clinical and 65 environmental A.fumigatus sensu stricto isolates were included in the study. The first group of clinical isolates consisted of 215 strains isolated in 1997-2015, revived from stock and tested. The second group consisted of 268 strains belonging to the time period of 2016-2018, during which routine azole agar screening tests were performed for A.fumigatus isolates. When all isolates (n= 483) were evaluated together, 11 isolates (1 before 2015 and 10 between 2016-2018), were found to be resistant to itraconazole (2.3%). None of the mutations previously reported to be associated with azole resistance in Aspergillus strains that were detected in cyp51A sequence analysis, However, polymorphisms which are not (yet) fully elucidated in relation to the resistance (Y46F, G89G, V172M, T248N, E255D, L358L, K427E, C454C, Y431D and Q141H in one strain) were shown to exist in resistant isolates. These results have shown that the rate of azole resistance among clinical A.fumigatus isolates was low (2.3%) in our center. Further studies are required to demonstrate the possible role of the detected polymorphisms on azole resistance and to clarify other mechanisms related with high azole MIC values. In addition, since high azole resistance has been reported from one region in our country, it has been concluded that multicenter studies are required to determine the azole resistance status and the range for the azole resistance ratio in different regions and to reveal resistance mutations that may be specific to our country.

Abstract:Aspergilli may cause various pulmonary diseases in humans, including allergic bronchopulmonary aspergillosis (ABPA), chronic pulmonary aspergillosis (CPA), and acute invasive pulmonary aspergillosis (IPA). In addition, chronic colonization may occur in cystic fibrosis (CF). Aspergillus fumigatus represents the main pathogen, which may employ different morphotypes, for example, conidia, hyphal growth, and asexual sporulation, in the various Aspergillus diseases. These morphotypes determine the ease by which A. fumigatus can adapt to stress by antifungal drug exposure, usually resulting in one or more resistance mutations. Key factors that enable the emergence of resistance include genetic variation and selection. The ability to create genetic variation depends on the reproduction mode, including, sexual, parasexual, and asexual, and the population size. These reproduction cycles may take place in the host and/or in the environment, usually when specific conditions are present. Environmental resistance is commonly characterized by tandem repeat (TR)-mediated mutations, while in-host resistance selection results in single-resistance mutations. Reported cases from the literature indicate that environmental resistance mutations are almost exclusively present in patients with IA indicating that the risk for in-host resistance selection is very low. In aspergilloma, single-point mutations are the dominant resistance genotype, while in other chronic Aspergillus diseases, for example, ABPA, CPA, and CF, both TR-mediated and single-resistance mutations are reported. Insights into the pathogenesis of resistance selection in various Aspergillus diseases may help to improve diagnostic and therapeutic strategies. Article published online: 09 January 2024 © 2024.
