Abstract:OBJECTIVES: The aim of the current study was to develop sex and age-adjusted criterion-referenced fitness standards to predict independent physical functioning into later life.DESIGN: Cross-sectional.SETTING: National Survey of Physical Fitness and Physical Activity, Portugal.PARTICIPANTS: A national representative sample of 3074 non-institutionalized older adults (65-84years).MEASUREMENTS: Projected capacity for physical independence was assessed by the Composite Physical Function (CPF) scale, and physical fitness was assessed using the Senior Fitness Test battery. ROC analyses were used to evaluate the classification accuracy of physical fitness components, and determine the best cutoff values of functional fitness to predict loss of independence. Stepwise multivariate logistic regression was used to identify sex and age category fitness components that were predictors of independence with follow-up ROC analysis to verify the model's discriminative capability.RESULTS: ROC curve analysis showed that the fitness tests were good in diagnosing physical independence (AUC>0.7). Logistic regression models demonstrated that different fitness components should be targeted according to age and sex, but with an emphasis on aerobic endurance and agility/dynamic balance. Overall, models provided higher AUC (males: 0.79-0.86; females: 0.73-0.85) when comparing the best fitness tests for each sex and age category. The sensitivity of newly developed models ranged from 80.5 to 87.7 in males and from 68.2 to 86.3 in females, while the specificity ranged from 62.1 to 73.8 in males and between 58.2 and 82.9 in females.CONCLUSION: The Senior Fitness Test provides a good field tool that allows the identification of fitness levels that older adults need to achieve to be physically independent later in life, and permits the accurate planning and implementation of exercise-based interventions for older adults.

Abstract:Objective: This study aimed to establish normative data for older adults in Hong Kong and explore age and sex differences in functional fitness.Methods: A sample of 944 independent community-dwellers, aged 65-74 years, was evaluated using the Senior Fitness Test battery in addition to hand grip and single leg stance tests. Normative data were reported for the 10th, 25th, 50th, 75th, and 90th percentiles in 5-year age groups.Results: Except for upper extremity muscle strength in women and body mass index (BMI) in both sexes, ageing-associated degradation was observed in all testing parameters especially in flexibility, balance, and agility. Significant sex differences were found in all testing parameters with the exception of BMI and static balance with eyes open. Moreover, men demonstrated higher capacities for muscle strength, agility, balance, and aerobic endurance, whereas women showed superior flexibility.Conclusion: The normative values enable the evaluation of individual performance regarding the fitness status of older adults in Hong Kong. Abstract:Introduction The use of normative values and/or standards of functional fitness in adults is relevant to overall health and well-being. The objectives of the study were: to identify the physical tests of the senior fitness test (SFT) that have been applied since its proposal and to describe the proposed percentiles according to age, sex and country. Methods A systematic review study was conducted in the Pubmed and Scopus databases. As eligibility criteria, we considered the period from 1999 to 2022 that presented data on SFT test used in the population over 60 years of age and that described normative values through percentiles. MeSH were used as: (1) Physical fitness, Exercise test, Senior Fitness Test, Functional fitness, Cardiorespiratory fitness, (2) older adult, aged, (3) Reference standards, standards, standards of care. Boolean operators “AND” and “OR” were included. Data extracted from the selected studies included: year of publication, country, sample age, sample size, sample sex, fitness component. Results and discussion Seven studies were identified in five countries (03 in China, 01 in Poland, 01 in Portugal, 01 in Spain and 01 in United States). The age range ranged from 60 to 103 years. The studies were conducted in both sexes. The study with the smallest sample size was by Chung et al. (China) with 944 participants and the largest number of participants was the study by Rikli and Jones in the United States with 7,183 participants. In general, no study was able to complete 100% (8 components) of the tests proposed in the SFT. Normative values were presented through percentile distribution (p10, p50 and p90) organized by age ranges. Males presented better performance in FPF tests than females in all tests. Since the first publication of the SFT until 2022, seven articles have been published in countries such as United States, China (three regional studies), Poland, Portugal and Spain. No study has published the complete battery with its eight components. The percentiles of functional fitness reflect decline with advancing age. Functional Fitness Norms and Trends of Community-Dwelling Older Adults in Urban China

Abstract:This study aimed to (1) establish age- and sex-specific functional fitness (FF) norms in Chinese older adults living in urban communities and (2) explore ageing-related degradations and sex differences in each FF dimension. A pool of 2398 individuals (1128 men; 1270 women) aged 60–98 years were recruited from urban communities of Nanjing, China. FF was measured using the Senior Fitness Test battery. FF norms were established for men and women in 5-year age intervals using five percentiles (10th, 25th, 50th, 75th, 90th). Significant degradations in FF were correlated with increases in age. Around half of test items in 75–79 years group (3 for men; 4 for women) and over half in 80–84 years group (5 for men; 4 for women) exhibited significant decreases in FF compared with the adjacent younger group, indicating that ages of 75 and 80 years are two potential turning points in FF declines. Significant differences existed between the overall FF of men and women; women outperformed men in flexibility and men outperformed women in upper body strength, agility/dynamic balance, and aerobic endurance. Significant age and sex effects occurred in each fitness dimension, which helps individualised program design and promotes an active lifestyle in older adults.
Aging Metrics Incorporating Cognitive and Physical Function Capture Mortality Risk: Results from Three Prospective Cohort Studies

Abstract:Abstract Background: The aims of this study were to: 1) describe the proportions of vulnerable persons identified by three existing aging metrics that incorporate cognitive and physical function; 2) examine the associations of the three metrics with mortality; and 3) develop and validate a new simple functional score for mortality prediction. Methods: The three aging metrics were the combined presence of cognitive impairment and physical frailty (CI-PF), the frailty index (FI), and the motoric cognitive risk syndrome (MCR). We operationalized them with data from two large cohort studies: the China Health and Retirement Longitudinal Study (CHARLS) and the US National Health and Nutrition Examination Survey (NHANES). Logistic regression models or Cox proportional hazard regression models, and receiver operating characteristic curves were used to examine the associations of the three metrics with mortality. A new functional score was developed and validated in the Rugao Ageing Study (RAS), an independent dataset. Results: In CHARLS, the proportions of vulnerable persons identified by CI-PF, FI, and MCR were 2.2%, 16.6%, and 19.6%, respectively. Each metric predicted mortality after adjustment for age and sex, with some variations in the strength of the associations (CI-PF, odds ratio (OR)=2.87, 95% confidence interval (CI)=1.74, 4.74; FI, OR=1.94, 95% CI=1.50, 2.50; MCR, OR=1.27, 95% CI=1.00, 1.62). CI-PF and FI had additional predictive utility beyond age and sex, as demonstrated by integrated discrimination improvement, and continuous net reclassification improvement (all P <0.001). These results were replicated in NHANES. Furthermore, we developed a new functional score by selecting six self-reported items from CI-PF and FI in CHARLS, and demonstrated that it predicted mortality risk. This functional score was further validated in RAS. To facilitate the quick screening of persons with deteriorations in cognitive and physical function, we introduced a publicly available online tool designed for this new functional score. Conclusions: Despite the inherent differences in the aging metrics incorporating cognitive and physical function, they consistently capture mortality risk. The findings support the incorporation of cognitive and physical function for risk stratification in both Chinese and US persons, but call for caution when applying them in specific study settings.

Abstract:OBJECTIVES: To investigate age-associated changes in functional fitness (FF) among Chinese older women and to identify any urban-rural disparity.METHODS: Nine hundred and sixty-five participants aged 60-84 yrs were assessed for their functional fitness. They were stratified into five groups by age and two groups by residential areas. Two-way ANOVA was used to observe the group differences in functional fitness, and linear regression analysis was conducted to identify each of the functional fitness measures as a function of age and residential area.RESULTS: All functional fitness tests showed a significant decline across age groups. Age-related decline in muscle strength occurred relatively earlier (age 65-69) compared to the decline in aerobic endurance (age 75-79). Linear regression analysis indicated that age has notable effects on each of the functional fitness measures and predicts balance control better than other factors. Urban participants outperformed their rural counterparts in all FF tests except for upper body flexibility and BMI. Notably, the greatest disparity occurred between 70 and 79 years of age.CONCLUSIONS: Enhanced physical fitness monitoring and targeted interventions that focus on strength training and increasing moderate- to high-intensity leisure-time physical activity are necessary to improve the functional fitness of Chinese older women, especially those in rural areas.

Age and task differences in functional fitness in older women: comparisons with Senior Fitness Test normative and criterion-referenced data

Abstract:Age-related changes in physical abilities, such as strength and flexibility, contribute to functional losses. However, older individuals may be unaware of what specific physical abilities compromise independent functioning. Three groups of women, aged 60 to 69, 70 to 79, and 80 to 92 years, were administered the Senior Fitness Test (SFT) to determine age differences in physical abilities and risk for functional losses. The oldest group showed significant differences in lower body strength, aerobic endurance, and agility and dynamic balance when compared with the other groups who performed similarly. Across all groups, a faster rate of decline was found for lower body strength (50.6%) and dynamic balance and agility (45.7%) than upper body strength (21.3%) and aerobic endurance (33.6%). Criterion-referenced (CR) fitness standards suggested that 45% of the individuals were at risk for loss of independent functioning. This study highlights age-related differences in physical abilities and the risk for the loss of independence in later life.

Abstract:This study was designed to construct normative physical fitness scores for older, functionally independent adults living in the community. The Physical Activities Readiness Questionnaire and Barthel Index were initialized to screen those who have heart illness, arthritis, and functional dependence. After providing informed consent, each participant was instructed to perform seven tests in five categories, including body mass index, muscle strength/endurance (grasp test and 30-s chair stand test), balance (open-eye stand on right foot), flexibility (chair sit-and-reach test), and aerobic endurance (2- and 3-min step tests with preset cadence). Twenty-two assessors were recruited and trained by a physical fitness instructor to ensure acceptable interrater reliability. The valid sample size was 1,104. Test performances were significantly different for male and female participants for all test categories, with the exception of aerobic endurance. Mean scores of all tests correlated negatively with age. The authors constructed the percentile distributions for the seven fitness tests for both genders. Results are expected to be helpful in assessing physical fitness and evaluating physical activity in older adults.

Abstract:BackgroundThe aim of this study was to develop and validate a functional ability index (FAI) that incorporates aspects of intrinsic capacity and environmental factors of older individuals based on the World Health Organization framework of healthy ageing.MethodsData of 7016 participants ≥60 years participating in the baseline survey of China Health and Retirement Longitudinal Study was used for the development and internal validation of the FAI. External validation was performed in a separate cohort of 1295 older individuals aged ≥60 years. Functional independency was considered the primary outcome and additional proxies of healthy ageing were considered as secondary outcomes. Cluster dendrogram was used to identify the distinct hierarchical clusters of all included variables for inclusion in the FAI. Backward elimination logistic regression model was implemented to identify the most significant variables associated with independency to be included in the FAI.ResultsThe FAI score ranged from 0 to 19 and individuals having FAI ≥ 12 were more likely to be independent and at lower risk of negative outcomes. For each unit increase in the FAI the risk of having independency increased by 30%–58% cross-sectionally in the two cohorts, whilst the 2-year risk of independency increased by 20%. The FAI demonstrated a C-statistic of 0.73 (95% confidence interval, 0.72 and 0.75) for the primary outcome.ConclusionsThe FAI we developed effectively measured the functional ability status of community dwelling older individuals. FAI could serve as a tool for evaluating older individual's functional ability in routine health assessment.

Abstract:This study assesses the functional fitness of a convenient sample of older adults (>70 years), to examine correlations between functional fitness and several other health-related variables and to compare with criterion performance data as established by Rikli and Jones (2001). One hundred and seven community-dwelling older adults with an average age of 78.36 +/- 5.60 years performed the Senior Fitness Test (SFT) and responded to several health-related questionnaires. The SFT scores were similar to the scores in the low-active group data published by Rikli and Jones (1999b). There was a strong correlation between the 30-second arm curl and the 2-minute step-in-place (r = .54, p < .01). More than one-half of the participants performed in the normal range or above normal range, according to the criterion performance data. This demonstrates a high level of functional fitness.

Abstract: This study was performed to establish the normative values and integrated score of the functional fitness on the basis of the senior fitness test (SFT) among Chinese community-dwelling older adults in Suzhou. In this cross-sectional descriptive study, 1,122 community-dwelling older adults aged 60 years old and above were recruited at Suzhou, China, by using a multistage stratified sampling method and accepted the SFT measurements. Sex- and age-specific normative values of each index of the SFT were established by using the percentile method. The SFT integrated score was established using factor analysis according to the data of 70% of the participants (construction group) and verified using the error rate from the data of the remaining 30% of the participants (verification group). Normative-referenced percentile values at the 5th, 10th, 25th, 35th, 50th, 65th, 75th, 90th, and 95th percentiles for each index of SFT were established for the men and women among the different age groups. Five indices of the SFT, namely, 2-min step test, 30-s arm curl, 30-s chair stand, chair sit-and-reach, and 8-ft up-and-go (TUGT), gradually declined with age in both sexes ( < .05). The SFT integrated score was calculated as follows: F = 3.8 × 2-min step test + 3.8 × 30-s arm curl + 3.8 × 30-s chair stand + 2.2 × back starch + 2.6 × chair sit-and-reach + 4 × TUGT - .04 × BMI. The formula was verified using the error rate. The error rates of the verification group compared with the construction group in each grade score of SFT were lower than 5%. Based on the data from the community-dwelling older adults in Suzhou, China, the functional fitness normative values for each index of the SFT and the integrated score of SFT were established. The SFT integrated score formula was verified to be reasonable and effective.
Exercise, disability and physical functioning in older Chinese adults during their last six years of life: a cohort study

Abstract:Abstract Background Little is known about the potential effects of exercise behaviour on disability and physical functioning (PF) in late-life. This study aimed to investigate whether exercise behaviour with four different long term exercise status influences disability and PF in older Chinese adults during their last six years of life. Methods Secondary data analyses were performed using participants (≥ 65 years) from the Chinese Longitudinal Healthy Longevity Study. Exercise behaviour was collected every three years during their last six years of life. 4265 participants were enrolled and divided into four groups, including long-term non-exercising (NN), non-exercising to exercise (NY), exercise to non-exercising (YN), and long-term exercising (YY). Disability was measured based on the Katz Index of Activities of Daily Living scale and divided into three disability levels. PF was included as chair-rise, pick-up book and turn-around. Risks burden in disability and PF among four groups were compared using logistic regression analyses. Results Compared with NN, the risk of developing severe disability was attenuated in YY (OR 0.11, 95%CI 0.06 to 0.21) and attenuated in NY (OR 0.27, 95%CI 0.17 to 0.44) and increased in YN (OR 1.36, 95%CI 1.08 to 1.72). Moderate disability risk was increased in YN (OR 1.85, 95%CI 1.26 to 2.71) only. The risk of developing physical dysfunction ( chair-rise, pick-up book, and turning around 360°) was decreased in YY and NY, and did not have significant difference in YN. Conclusions EP were associated with disability and PF in older Chinese adults during their late-life. Late-life exercise behaviour is still beneficial. Our findings suggest that it is meaning to encourage older people doing EP at late-life in China.
Role of physical performance measures for identifying functional disability among Chinese older adults: Data from the China Health and Retirement Longitudinal Study

Abstract:Background: Functional disability is a common health burden in older adults and follows a hierarchical pattern. Physical performance measures are useful for the objective estimation of functional disability. This study primarily aimed to compare the validity of handgrip strength and gait speed, alone and in combination, for recognizing the functional disability among Chinese older adults. This study also aimed to stratify the functional disability according to the criterion-referenced values of handgrip strength and gait speed. Methods: We selected 6127 respondents from the 2011 wave of the China Health and Retirement Longitudinal Study. Here, we defined functional disability as needing any help in any items of activities of daily living (ADL) and instrumental activities of daily living (IADL). To assess the validity of physical performance measures alone and in combination for the recognition of functional disability, we conducted the receiver operating characteristic analysis. Results: Compared with handgrip strength, the gait speed could better discriminate ADL disability and showed a satisfactory discriminant validity (area under the curve ≥ 0.7) in men. However, this finding was not found in the recognition of IADL disability. When combining these two measures, the parallel test showed a high sensitivity with a poor specificity, whereas the serial test showed a perfect specificity with a poor sensitivity. Conclusion: We developed the hierarchical cut-off values of handgrip strength and gait speed for identifying and stratifying the functional disability among Chinese adults over 60 years old. The speed test was superior to handgrip strength in identifying ADL disability. The parallel tests of those with high sensitivity perhaps could help identify the functional disability. Further work on cost-utility analysis is warranted.
Decline of Functional Fitness in Free Living Japanese Older Adults

Abstract:Purpose: Functional fitness declines with advancing age even in healthy adults. Considering the importance of functional fitness in old age, as well as the magnitude of the rapidly growing older population in Japan, the aim of this study was to compare the functional fitness in apparently healthy Japanese older adults of different ages, also with that of people of young age when fitness variables are reported to be at their peak level. Methods: Five hundred seventy-seven participants (396 older adults, 71 ± 6 yr, and 181 younger adult, 19 ± 2 yr) volunteered to participate in this cross sectional study. Older adults were selected by public posting from a metropolitan city, and younger adults were students of a university. Arm curls, chair stands, up and go, back scratch, sit and reach, and 12-min walk tests were performed to evaluate functional fitness. Results: Among older participants, lower body muscular endurance, lower and upper body flexibility and agility in both men and women were significantly higher in 6 th decade compared to 7 th or 8 th decade. No significant difference in men and women was noted in 12-min walk distance performance between 6 th and 7 th decade indicating that aerobic capacity declined slowly in these people until advanced age. Conclusions: Our findings suggest that aerobic capacity is well preserved until advanced age in many Japanese people. However, to prevent or control age-associated decline in muscular endurance, agility, and flexibility, these people should perform physical activities that incorporate aerobic, resistance, and flexibility exercises.
Further understanding of the Senior Fitness Test: Evidence from community-dwelling high function older adults in Hong Kong

Abstract:Objectives: This study examined the factor structure of the Senior Fitness Test and examined its measurement and structural invariance across participants with normal body weight and participants who were overweight. Methods: Eight hundred and forty-five community-dwelling older adults in Hong Kong participated in this study. Their functional fitness was assessed using the Senior Fitness Test. Confirmatory factor analysis was conducted to examine and compare two competing models: a one-factor model and a second-order model. Multiple-group confirmatory factor analysis was conducted to evaluate the measurement and structural invariance of the SFT measurement model. Multiple fit indices were used to evaluate the adequacy of the model fit to the data. Results: The second-order model outperformed the one-factor model. For men participants, the second-order 4-factor model was indicated, while for women participants, the second-order 5-factor model was evidenced. The measurement and structural invariance of the SFT measurement model across normal weight and overweight participants were also supported. Conclusion: This study provided initial evidence for the construct validity of the SFT. However, there were some differences in the factor structure of the SFT for men and women participants. Body composition did not necessarily reflect the functional fitness of men participants. All of the older adults responded to the testing items in a similar way regardless of body weight status in both men and women samples. The significance of each component for functional fitness was also similar for participants with different body weight statuses.

Abstract:Background: Self-efficacy has been shown to be both an antecedent and determinant of behaviors such as physical activity and may explain variance in the performance of functional tasks among older adults.Purpose: The objectives of the current study were: first, to identify potential latent factors of functional fitness (i.e., the ability to perform activities of daily living) among older adults; and second, to determine the extent to which self-efficacy contributed to the variance in functional fitness over and above other known correlates.Methods: Older adults (n = 190, M age = 69.4 years) completed a functional fitness test battery, maximal graded exercise test, and demographics and self-efficacy questionnaires.Results: Structural equation modeling supported two latent factors of functional fitness representing “Flexibility” and “Physical Power.” Further analyses indicated sex as the sole significant correlate of Flexibility. Greater Physical Power was associated with being male, younger, and having higher self-efficacy.Conclusions: These results support the role of self-efficacy as a correlate of performance on functional tasks. Targeting flexibility and physical power to improve functional fitness among older men and women, respectively, warrants examination.

Abstract:Background: It's really important to measure the actual functional physical fitness of elderly independent living persons from different environments to know the level and rate of decrease may inform about the threat of loss of functional independence, hence the need to monitor and assess the senior's motor realm and adapt to it the appropriate programs and treatments in the care for the elderly person. Methods: The study involved 5367 people (mean age 69,63 ± 7,06), including 4164 women (mean age 69,55 ± 7,11) and 1203 men (mean age 69,91 ± 6,86) aged 60 to 93 years old. We have measured basic anthropometric features and physical fitness (by using Senior Fitness Test). Results: The average values in individual SFT tests significantly decrease along with age. After age of 80 and 85 there were no sex differences in SFT. The largest deficits concern the dynamic balance and the decrease reaches 69% in men and 62% in women A significantly higher rate of decline in aerobic capacity concerns men (43%) than women (36.9%). A clearly lower rate of loss occurs in the muscular strength of the lower and upper body and does not exceed 30%. Conclusions: The results are of great clinical importance for the development of effective prevention and gerontological education programs in terms of promoting active lifestyle and successful ageing, at the same time limiting the economic consequences of treatment and hospitalisation.

Abstract:Background: Physical activity (PA) and functional fitness (FF) are crucial for promoting independent living and healthy aging in older adults. However, there is a lack of normative values for the Senior Fitness Test (SFT) among older adults (aged 55–90) living in southern rural Taiwan, particularly in the Chiayi region, which has been relatively underserved in terms of health-related resources compared to northern Taiwan. Purpose: This study aimed to determine the age- and gender-specific normative fitness scores for a large representative population of community-dwelling older adults in southern rural Taiwan. Methods: A cross-sectional descriptive study was conducted to obtain normative FF scores for 3332 community-dwelling elderly people (1057 men and 2275 women) in Chiayi, through the implementation of functional fitness tests across 72 Chiayi communities. The developed normative data served as a reference for exercise prescription for the elderly in southern Taiwan. Results: The study showed that the average normative values of all functional fitness tests declined significantly with increasing age (p < 0.05). Sex differences were also found in all measures of FF tests, with elderly women scoring significantly better than men on flexibility (scratch back: −1.34 ± 9.03 vs. −6.54 ± 11.07; chair sit and reach: 6.56 ± 9.44 vs. 0.56 ± 10.40) (p < 0.05), while men scored significantly higher than women on strength (grip strength: 30.83 ± 8.06 vs. 21.82 ± 5.32; bicep curl: 19.25 ± 5.44 vs. 17.64 ± 4.98) (p < 0.05). Furthermore, four FF normative scores in southern rural Taiwan were found to be significantly higher than their counterparts living in northern Taiwan. Conclusion: These findings contribute positively to the evaluation of fitness levels among older adults in southern rural Taiwan and provide a concrete reference for developing sound PA programs for this population. The results suggest that strategies aimed at promoting increased participation in PA among older adults need to consider age-, sex-, and region-specific factors.

Abstract:The problems related to an aging population have been exacerbated and are becoming more serious in China.The elder commonly suffer from chronic diseases which result in not only negative effects on their health conditions and the quality of life,but also heavy financial burdens to both family and society.Physical fitness assessment plays an important role in early identification of factors influencing health.Although considerable improvement has been made,indexes and indicators of physical assessment for the elderly still need to be elucidated.In this review,we discussed different methods used for the measurement of physical conditions among China's aged population.Our object is to provide scientific basis for predicting,preventing chronic diseases among the aged population and guiding actions on ageing in China.
