Abstract:In dry skin (DS), skin-barrier function is easily disturbed and moisturizing factors in the stratum corneum are reduced. Despite being a common condition, DS is often overlooked in patients with advanced age or comorbid diseases. In September 2022, specialists in dermatology and skin care met to discuss unmet needs and management of patients with DS with existing medical conditions or DS induced by ongoing pharmacological treatments. There was consensus about the need to improve the current understanding and management of DS in patients with comorbidities, including type 2 diabetes, chronic kidney disease, radiodermatitis, and photodamaged skin. Clinical guidance related to optimal treatment of DS in patients with advanced age or comorbid diseases is needed. Dexpanthenol-containing emollients have been shown to provide rapid relief from the symptoms and clinical signs of skin inflammation and are well-tolerated and effective in terms of moisturizing and soothing DS and maintaining skin-barrier function. Thus, dexpanthenol-containing emollients may play an important role in future management of DS. Further research is needed to elucidate the efficacy of dexpanthenol across the spectrum of DS, irrespective of comorbidity status or age.

What problem does this paper attempt to address?
The problems that this paper attempts to solve mainly focus on the following aspects: 1. **Unmet Needs in Dry Skin (DS) Management**: Although dry skin is a common condition, it is often overlooked in patients with multiple diseases or the elderly. The paper discusses the unmet needs for dry skin management in these patient groups, especially those dry skin problems caused by existing medical conditions or ongoing drug treatments. 2. **Dry Skin Management in Specific Populations**: - **The Elderly**: Dry skin is very common among the elderly, especially those living in nursing institutions. The paper explores the causes of dry skin in the elderly (including internal factors such as reduced proliferation of basal layer skin cells, reduced epidermal lipid synthesis, etc., and external factors such as air pollution, smoking, malnutrition, and sun exposure), and puts forward prevention and management suggestions. - **Patients with Other Diseases**: The paper also focuses on the dry skin management of patients with type 2 diabetes, chronic kidney disease, radiation - induced dermatitis, etc. For example, patients with type 2 diabetes may have nerve damage and vascular lesions that affect skin health; patients with chronic kidney disease may be accompanied by severe itching; patients receiving radiotherapy may have short - term and long - term skin reactions. 3. **The Role of Products Containing Potential Emollient Additive Ingredients**: The paper evaluates the role of products containing potential emollient additive ingredients in managing dry skin, especially products containing dexpanthenol. Studies have shown that dexpanthenol has the effects of rapidly relieving skin inflammation symptoms, moisturizing, soothing the skin, and maintaining the skin barrier function, and is well - tolerated. 4. **Clinical Guidance and Future Research Directions**: The paper emphasizes the importance of formulating dry skin management guidelines for the elderly and patients with multiple diseases, and points out that further research is needed to clarify the efficacy of dexpanthenol in different dry skin conditions, regardless of whether the patients have comorbidities or their age. In summary, this paper aims to improve the awareness and understanding of dry skin management in the elderly and patients with multiple diseases, and explore the application potential of products containing potential emollient additive ingredients such as dexpanthenol in this field.
