Abstract:Electrogastrography (EGG) is a non-invasive method with high diagnostic potential for the prevention of gastroenterological pathologies in clinical practice. In this study, a review of the measurement systems, procedures, and methods of analysis used in electrogastrography is presented. A critical review of historical and current literature is conducted, focusing on electrode placement, measurement apparatus, measurement procedures, and time-frequency domain methods of filtration and analysis of the non-invasively measured electrical activity of the stomach. As a result, 129 relevant articles with primary aim on experimental diet were reviewed in this study. Scopus, PubMed, and Web of Science databases were used to search for articles in English language, according to the specific query and using the PRISMA method. The research topic of electrogastrography has been continuously growing in popularity since the first measurement by professor Alvarez 100 years ago, and there are many researchers and companies interested in EGG nowadays. Measurement apparatus and procedures are still being developed in both commercial and research settings. There are plenty variable electrode layouts, ranging from minimal numbers of electrodes for ambulatory measurements to very high numbers of electrodes for spatial measurements. Most authors used in their research anatomically approximated layout with two++ active electrodes in bipolar connection and commercial electrogastrograph with sampling rate of 2 or 4 Hz. Test subjects were usually healthy adults and diet was controlled. However, evaluation methods are being developed at a slower pace, and usually the signals are classified only based on dominant frequency. The main review contributions include the overview of spectrum of measurement systems and procedures for electrogastrography developed by many authors, but a firm medical standard has not yet been defined. The Correlation of Gastroparesis and Postprandial Blood Glucose in Elderly Patients with Type 2 Diabetes Mellitus

Abstract:Objective To investigate the relationship between gastroparesis and PBG in elderly patients with T2DM. Methods Eighty patients were devided into 2 groups:non elderly DM group ( n=22 ,<60 years old) and elderly DM group ( n=58 ,≥60 years old).Twenty health participants were used as the control group. Half time gastric emptying of liquid and solid (T1/2 ) ,fasting blood sugar ,1 h ,2 h and 3 h PBG ,HbA1c and 24 h urine protein were evaluated by SPECT technics. Results Half time gastric emptying of solid (T1/2 ) of non elderly DM group and elderly DM group were significantly higher than control group ( t=3.56、9.36 , P<0.05 or 0.01 ). The delay rate of solid phase of elder DM group is significantly higher than non elderly DM group ( t=5.21 , P<0.05 ) .T1/2 ,fasting blood sugar ,2 h and 3 h PBG , HbA1c and 24 h urine protein of elderly DM group is significantly higher than non elderly DM group (t=2.67、3.12、3.43 , P<0.05 or 0.01). The peak time of blood sugar of elder diabetics was in 3 hour after meals. Conclusion Diabetic gas-troparesis is a common chronic complication of T2DM ,the incidence of gastroparesis is high. Doctors should pay attention to the delayed peak time of PBG.
Clinical associations of functional dyspepsia with gastric dysrhythmia on electrogastrography: A comprehensive systematic review and meta‐analysis

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Functional dyspepsia (FD) is a common gastroduodenal disorder, yet its pathophysiology remains poorly understood. Bioelectrical gastric slow‐wave abnormalities are thought to contribute to its multifactorial pathophysiology. Electrogastrography (EGG) has been used to record gastric electrical activity; however, the clinical associations require further evaluation.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Aims</h3><p>This study aimed to systematically assess the clinical associations of EGG in FD.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>MEDLINE, EMBASE, and CENTRAL databases were systematically searched for articles using EGG in adults with FD. Primary outcomes were percentage normal versus abnormal rhythm (bradygastria, normogastria, and tachygastria). Secondary outcomes were dominant power, dominant frequency, percentage coupling, and the meal responses.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>1751 FD patients and 555 controls from 47 studies were included. FD patients spent less time in normogastria while fasted (SMD −0.74; 95%CI −1.22 to −0.25) and postprandially (−0.86; 95%CI −1.35 to −0.37) compared with controls. FD patients also spent more fasted time in bradygastria (0.63; 95%CI 0.33–0.93) and tachygastria (0.45; 95%CI 0.12–0.78%). The power ratio (−0.17; 95%CI −0.83–0.48) and dominant frequency meal‐response ratio (0.06; 95%CI −0.08–0.21) were not significantly different to controls. Correlations between EGG metrics and the presence and timing of FD symptoms were inconsistent. EGG methodologies were diverse and variably applied.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>Abnormal gastric slow‐wave rhythms are a consistent abnormality present in FD, as defined by EGG and, therefore, likely play a role in pathophysiology. The aberrant electrophysiology identified in FD warrants further investigation, including into underlying mechanisms, associated spatial patterns, and symptom correlations.</p></section>

Abstract:The effect of glucose excursions on oxidative stress is an important topic in diabetes research. We investigated this relationship by analyzing markers of oxidative stress and glycemic data from a continuous glucose monitoring system (CGMS) in 30 individuals with normal glucose regulation (NGR), 27 subjects with impaired glucose regulation (IGR), and 27 patients with newly diagnosed type 2 diabetes (T2DM). We compared the mean amplitude of glycemic excursion (MAGE), mean postprandial glucose excursion (MPPGE), and mean postprandial incremental area under the curve (IAUC) with plasma levels of oxidative stress markers 8-iso-PGF2α, 8-OH-dG, and protein carbonyl content in the study subjects. Patients with T2DM or IGR had significantly higher glucose excursions and plasma levels of oxidative stress markers compared to normal controls ( P < 0.01 or 0.05). Multiple linear regression analyses showed significant relationships between MAGE and plasma 8-iso-PGF2α, and between MPPGE and plasma 8-OH-dG in patients with IGR or T2DM ( P < 0.01 or 0.05). Furthermore, 2h-postprandial glucose level and IAUC were related to plasma protein carbonyl content in the study cohort including T2DM and IGR ( P < 0.01). We demonstrate that glucose excursions in subjects with IGR and T2DM trigger the activation of oxidative stress.
Aberrant Brain Spontaneous Activity and Synchronization in Type 2 Diabetes Mellitus Patients: A Resting-State Functional MRI Study

Abstract:The study aimed to investigate the aberration of brain spontaneous activity and synchronization in type 2 diabetes mellitus (T2DM) patients homozygous for the apolipoprotein E (APOE)-ε3 allele. In the APOE-ε3 homozygotes, 37 T2DM patients and 37 well-matched healthy controls (HC) were included to acquire blood sample measurements, neuropsychological tests, and brain functional MRI data. The amplitude of low-frequency fluctuations (ALFF) analysis was conducted to identify the brain areas with abnormal spontaneous activity. Then, the identified brain areas were taken as seeds to compute their functional connectivity (FC) with other brain regions. The two-sample t-test or the Mann–Whitney U test were applied to reveal significant differences in acquired measurements between the two groups. The potential correlations among the three types of measurements were explored using partial correlation analysis in the T2DM group. The T2DM group had elevated glycemic levels and scored lower on the cognitive assessment but higher on the anxiety and depression tests (p < 0.05). The T2DM group exhibited higher ALFF in the left middle occipital gyrus, and the left middle occipital gyrus had lower FC with the left caudate nucleus and the left inferior parietal gyrus (p < 0.05). No significant correlations were observed. T2DM patients homozygous for the APOE-ε3 allele exhibited aberrant brain spontaneous activity and synchronization in brain regions associated with vision-related information processing, executive function, and negative emotions. The findings may update our understanding of the mechanisms of brain dysfunction in T2DM patients in a neuroimaging perspective.

Abstract:Electrogastrography (EGG) is a non-invasive method for the measurement of gastric myoelectrical activity. It was first discovered in 1921 and popularized in 1990s. EGG is attractive because it is non-invasive. However, due to its non-invasive nature, there have also been controversies regarding validity and applications of EGG. The aim of this review is to discuss the methodologies, validation and applications of EGG. Pros and cons of EGG will also be discussed in detail. First, the gastric slow wave and its correlation with gastric motility are presented. The association between gastric dysrhythmia and impaired gastric motility is reviewed. Secondly the method for recording the electrogastrogram is presented in detail and pitfalls in the recording and analysis of EGG are discussed. Thirdly, findings reported in the literature demonstrating the accuracy of EGG in recording gastric slow waves and gastric dysrhythmia are reviewed and discussed. The correlation of the electrogastrogram with gastric contraction is carefully discussed. Finally, applications of EGG in a few major areas are reviewed.
Continuous Glucose Monitoring in Type 2 Diabetes with Gastroparesis

Abstract:OBJECTIVE:To observe the glucose profile in type 2 diabetic gastroparesis.METHODS:31 patients diagnosed with type 2 diabetes were enrolled into this study for measurement of gastric emptying of solids and continuous glucose monitoring to observe blood glucose levels for 72 hours on a balanced diet; the results were compared with 7 subjects with normal glucose regulation.RESULTS:58.1% of 31 type 2 diabetic patients were found to have delayed gastric emptying of solids. The average glucose level was lower after breakfast [(7.82 +/- 1.42) mmol/L vs (9.35 +/- 2.28) mmol/L, P < 0.01] in the patients with gastroparesis than in those without. Maximal blood glucose level after breakfast [(10.21 +/- 2.17) mmol/L vs (12.24 +/- 2.82) mmol/L, P < 0.01] was lower in the patients with gastroparesis but it reached the peak at a similar time. Two hour AUC was also lower after breakfast in the patients with gastroparesis [(877.62 +/- 272.78) min x mmol x L(-1) vs (1028.40 +/- 283.98) min x mmol x L(-1), P < 0.05].CONCLUSION:Post-prandial glucose level in type 2 diabetic patients with gastroparesis tends to be lower than those without.

Defining and Phenotyping Gastric Abnormalities in Long-Term Type 1 Diabetes Using a Novel Body Surface Gastric Mapping Device

Abstract:Background and aims: Diabetic gastroenteropathy is associated with poor glycemic control and morbidity in people with type 1 diabetes (T1D). There is a lack of noninvasive techniques to assess and monitor gastric abnormalities. We aimed to define phenotypes of gastric myoelectrical abnormalities in people with longstanding T1D with and without symptoms using a novel noninvasive body surface gastric mapping (BSGM) device. Methods: BSGM was performed on people with T1D of >10 years duration and matched controls, employing Gastric Alimetry (Alimetry, New Zealand), comprising of a high-resolution 64-channel array, validated symptom-logging App, and wearable reader. Results: Thirty-two people with T1D were recruited (15 with a high symptom burden), and 32 controls. Those with symptoms showed more unstable gastric myoelectrical activity (Gastric Alimetry Rhythm Index 0.39 vs 0.51, P = .017; and lower average spatial covariance 0.48 vs 0.51, P = .009) compared with controls. Symptomatic patients also had a higher prevalence of peripheral neuropathy (67% vs 6%, P = .001), anxiety/depression diagnoses (27% vs 0%, P = .001), and higher mean hemoglobin A1C levels (76 vs 56 mmol/mol, P < .001). BSGM defined distinct phenotypes in T1D participants including those with markedly unstable gastric rhythms (4/32, 12.5%) and abnormally high gastric frequencies (9/32, 28%). Deviation in gastric frequency was positively correlated with symptoms of bloating, upper gut pain, nausea and vomiting, and fullness (R > 0.35, P < .05). Conclusion: Gastric symptoms in people with longstanding T1D correlate with myoelectrical abnormalities on BSGM evaluation, in addition to glycemic control, psychological comorbidities, and peripheral neuropathy. BSGM using Gastric Alimetry identified a range of myoelectrical phenotypes, presenting targets for diagnosis, monitoring, and therapy.
Relationship Between Esophageal Motility Disorders and Autonomic Nervous System in Diabetic Patients: Pilot North African Study

Abstract:Little attention has been given to esophageal disorders in diabetes mellitus. Pathophysiology of esophageal motility disorders (EMD) in patients with diabetes mellitus is multifactorial. The aims of the present study were: (a) to evaluate the prevalence of EMD in patients with Type 2 diabetes mellitus and (b) to determine the relationship between EMD and autonomic neuropathy as assessed by heart rate variability (HRV). All the patients completed a questionnaire about diabetes characteristics and gastrointestinal symptoms. Conventional esophageal manometry was performed in all patients. HRV was measured in three different situations (Lying Position 1, standing position, and Lying Position 2). The temporal and frequency domain parameters were considered for analysis. The prevalence of EMD in our patients was 60.5% (n = 23). Low score physical activity was significantly more frequent in patients with EMD (p = .03). There was an increase in sympathetic activity represented by the low frequency (LF) parameter (p = .027) in the presence of EMD. Whereas parasympathetic modulation of heart rate represented by the high frequency (HF) parameter (p = .027) was declined in patients with EMD compared to those without. The LF/HF ratio was significantly higher (p = .002) in patients with EMD. EMD were prevalent in diabetes mellitus and were associated to autonomic nervous system dysfunction predominantly at the parasympathetic component.
Analyses of types and risk factors of cardiac arrhythmias in hospitalized patients with and without diabetes mellitus

Abstract:Objective To investigate the types and risk factors of cardiac arrhythmias in hospitalized patients with and without diabetes mellitus.Methods The inpatient medical records of patients with cardiac arrhythmias in the People′s Hospital of Peking University from October 1998 to September 2008 were retrospectively analyzed in our present study.All patients were divided into diabetes group(n=501) and non-diabetes group(n=2988).Their clinical characteristics were compared and the risk factors were also investigated.Results Univariate analysis demonstrated that diabetes mellitus was associated with atrial fibrillation(AF) and atrioventricular block(P<0.01).Multivariate analysis demonstrated that cardiac arrhythmias were independently associated with coronary heart disease(CHD)[odds ratio(OR): 1.46;95% confidence interval(CI): 1.08-1.98;P=0.01],hypertension(OR: 2.50;95% CI: 1.96-3.20;P<0.01),hypothyroidism(OR: 3.06;95% CI:1.56-6.02;P<0.01),gastritis/alimentary ulcer(OR:0.60;95% CI:0.37-0.97;P=0.04),immune system disease(OR:2.99;95% CI:1.32-6.77;P<0.01),age(≥65)(OR:1.56;95% CI:0.22-2.00;P<0.01),and hyperglycemia(OR:2.18;95% CI:2.01-2.37;P<0.01).Conclusions Diabetes mellitus is associated with AF and atrioventricular block.The risk factors in non-diabetes group includ CHD,hypertension,hypothyroidism,gastritis/alimentary ulcer,immune system disease,older age.The patients with diabetes mellitus are younger compared with non-diabetes ones,and hyperglycemia and other metabolic disturbance are responsible for their cardiac arrhythmias.
The analysis of hypoglycemic attack-induced abnormal electrocardiogram in 75 cases of senile diabetes mellitus with hypoglycemia episode

Abstract:Objective To evaluate the changes of electrocardiogram(ECG) in the senile diabetes mellitus(DM) with hypoglycemia. Methods The ECG of the 75 cases of senile DM with hypoglycemia from June 1, 2000 to December 30, 2005 were studied retrospectively. Results The changes of ECG were found in 67 cases(89%) of all the patients. The highest morbidity of ECG abnormality was arrhythmia which was found in 54 cases(81%), coronary insufficiency in 39 cases(52%), acute myocardial infarction in 6 cases(8.00%). Conclusions The abnormalities of ECG are commonly seen in senile DM with hypoglycemia.

Abstract:Type 2 diabetes mellitus poses a serious threat to the health of people worldwide. Despite active detection and stringent measures to control and treat diabetes, the condition leads to a variety of complications, including autonomic neuropathy, resulting in autonomic cardiac denervation associated with an increased risk of painless myocardial ischemia and sudden cardiac death. However, digital verification by finding electrocardiographic (ECG) patterns of autonomic dysfunction is possible. The available literature on ECG patterns of myocardial electrical instability, which reflect autonomous myocardial dysfunction, was summarized. A literature review of scientific papers on ECG diagnosis of autonomic myocardial dysfunction in patients with type 2 diabetes mellitus over the past 5 years was performed using PubMed and eLIBRARY data. Among different ECG patterns that reflect autonomic dysfunction, the most common is the investigation of heart rhythm variability, which is based on the mathematical analysis of respiration rate intervals. The simplest variables are used to obtain a series of statistical indicators such as SDANN, RMSSD, NN50, and pNN50. A new and understudied ECG pattern of autonomic dysfunction is heart rate turbulence (HRT), which is a short-term rhythm change that develops in response to a ventricular extrasystole. The extrasystole is followed by a short episode of sinus tachycardia lasting several cardiac cycles, which is followed by a longer episode of bradycardia. These heart rate fluctuations are based on baroreflex compensation of intracardiac hemodynamic changes. Two parameters are estimated to characterize the HRT: turbulence onset and turbulence slope. A specific marker of autonomic dysfunction is the rate of cardiac rhythm recovery after controlled exercise. A recovery rate of 12 beats per first minute was associated with an increased risk of all-cause mortality, whereas a reduction in recovery rate of 40 beats per 2 min was found to be a strong independent risk factor for all-cause mortality. An imbalance in the tone of sympathetic and parasympathetic systems underlies all the described indicators. Therefore, a risk-stratifying significance of autonomic dysfunction markers in a wide range of pathologies, including peptic ulcer disease, gallstone disease, hypertension, chronic heart failure, chronic obstructive pulmonary disease, bronchial asthma, thyreopathies, and anemias apart from type 2 diabetes mellitus, exists. This explains the high sensitivity of tests, while having a low positive predictive value and specificity. The ease of performing tests dictates the need for further study of the features of autonomic dysfunction markers in patients with type 2 diabetes mellitus, since these markers are associated with a high risk of painless myocardial ischemia and sudden cardiac death. Currently, several ECG patterns (heart rate variability, HRT, and heart rate recovery rate) reflect autonomous myocardial dysfunction. The patterns enable early detection of autonomous neuropathy in patients with type 2 diabetes mellitus, which will allow timely diagnosis and adequate therapy. However, further study is required due to their low specificity to identify the peculiarities in specific nosological forms.

Abstract:Introduction: Electroencephalogram (EEG) has the potentials to decipher the neural underpinnings of cognitive processes in clinical and healthy populations. Objective: The current systematic review is intended to examine the functional brain changes underlying cognitive dysfunctions in T2DM patients. Methods: The review was conducted on studies published in the PubMed, WebofScience, Cochrane, PsycInfo database till June 2021. The keywords used were electroencephalogram, T2DM, cognitive impairment/dysfunction. We considered studies using resting-state EEG and ERP. The preferred reporting items for systematic reviews and meta-analysis (PRISMA) guidelines were followed to compile the studies. Results: The search yielded a total of 2384 studies. Finally, 16 independent studies were included. There was a pattern of a shift in EEG power observed from higher to lower frequencies in T2DM patients, though to a lesser degree than Alzheimer's disease patients. P300 latency was increased in T2DM patients mainly over frontal, parietal, and posterior regions. P300 and N100 amplitudes were decreased in T2DM patients than in healthy controls. Conclusion: The results indicate that T2DM has consequences for cognitive functions, and it finds a place in the continuum of healthy cognition to dementia.

Association Analysis of Acyl Ghrelin,des-Acyl Ghrelin and Its Ratio with Insulin Resistance in Patients with Type 2 Diabetes Mellitus

Abstract:Objective To explore the association between the different forms of in vivo ghrelin—Acyl ghrelin( AG) ,Des-acyl ghrelin( DAG) and AG/DAG with insulin resistance( IR) in patients with type 2 diabetes mellitus. Methods From June 2017 to November 2017,eighty-three patients with type 2 diabetes hospitalized in (group T2DM) and 40 healthy subjects (group NC) were hospitalized in Jinling Clinical Medicine were selected. Height body mass,blood pressure,blood lipid,glycated hemoglobin ( HbA1c) and fasting blood glucose (FPG),fasting insulin (FINS),and fasting C peptide (F-C-p) were measured,and all subjects were left with fasting serum,and the concentration of AG and DAG were measured by enzyme linked immunosorbent assay (ELISA). The body mass index (BMI),total gastric starvation (T-ghrelin) level,AG/DAG,insulin resistance index ( HOMA-IR) , insulin sensitivity index ( HOMA-IS ) and islet beta cell function ( HOMA- beta ) were calculated. The differences of the above indexes between the two groups were compared, and the relationship between AG,DAG,T-ghrelin,AG/DAG and FPG,HOMA-IR,HOMA-IS and HOMA- beta in T2DM patients were analyzed. Results ( 1) There were no significant difference in SBP、DBP、TC、LDL-C、AG between group NC and group T2DM(P>0. 05). Compared with NC group,the age、TG、BMI、HbA1c、FPG、FINS、HOMA-IR、HOMA-β、AG/DAG were significantly higher in T2DM group ( t=2. 690,-1. 990, 0. 873, 14. 257, 10. 528, Z=2. 885,-3. 483,-2. 284;P<0. 01,P<0. 05) . The HDL-C,F-C-p,HOMA-IS,HOMA-beta,DAG and T-ghrelin in group T2DM were lower than those of NC group( t or Z=0. 477,-3. 812,-3. 395,-4. 4,-2. 916,-2. 834;P<0. 05 or P<0. 01) . ( 2) The correlation analysis showed that there was a positive correlation between AG and FPG in T2DM group (r=0. 252,P<0. 05),DAG and T-ghrelin were negatively correlated with HOMA-IR (r=-0. 394,-0. 384,P<0. 05),and AG/DAG was positively correlated with HOMA-IR (r=0. 394,0. 384,P<0. 05),but is negatively correlated with HOMA-IS (r=-0. 292,P<0. 05). (3) multivariate linear regression analysis showed that FPG in T2DM patients were the influencing factors of AG ( t=2. 865,P<0. 05) ,while FINS and BMI were the influencing factors of DAG( t=-2. 808、-0. 330,P<0. 05) andT-ghrelin( t=-2. 725、-0. 330, P<0. 05) . HOMA-IR and BMI are the influencing factors of AG/DAG ( t=3. 718,3. 069,P<0. 05) . Conclusion The levels of DAG and T-ghrelin in group T2DM were significantly lower than those in the normal population, and was negatively correlated with the insulin resistance index,and the ratio of AG/DAG was closely related to insulin resistance,and the level of AG was mainly affected by fasting blood glucose.

Abstract:High-resolution electrogastrography (HR-EGG) presents a new paradigm in diagnosing and treating functional gastroduodenal disorders. Unlike traditional electrogastrography, HR-EGG allows for a more precise analysis of the gastric electrical activity, offering improved diagnostic accuracy. Recent studies have revealed the clinical potential of HR-EGG, particularly in detecting abnormal electrical patterns in patients with functional dyspepsia and gastroparesis, supporting the development of novel therapeutic strategies. The non-invasive HR-EGG method has shown promise in identifying new biomarkers. Moreover, further integration of artificial intelligence, is expected to enhance diagnostic efficiency and develop more refined treatment models for functional gastrointestinal disorders.

Abstract:To study the changes of retinal function in type 2 diabetes mellitus(DM) patients without apparently diabetic retinopathy via multifocal electroretinogram. Methods: Thirty-six type 2 DM patients (72 eyes) without visible diabetic retinopathy were selected as the experimental group, and thirty-five healthy subjects (70 eyes) were selected as the control group. All subjects were underwent multifocal electroretinogram (mf- ERG).

Abstract:Gastric per-oral endoscopic myotomy (G-POEM) has emerged as a promising approach for managing refractory gastroparesis. Factors predicting its success remain uncertain. Our study aims to investigate whether features of electrogastrography (EGG) can serve as predictive markers for patient outcomes following G-POEM. By analyzing the relationship between specific gastric myoelectrical activity (GMA) subgroups and patient symptoms post-G-POEM, we seek to understand factors influencing the therapeutic efficacy of G-POEM in treating gastroparesis.
