Abstract:Although fathers experience emotional stress both during the care period and after discharge, there is limited research focusing exclusively on fathers' experiences after their time in a neonatal intensive care unit. Their experiences are important for tailoring support to fathers based on their individual needs. This paper is part of a longitudinal study in which parents were followed by means of questionnaires and individual interviews during a two-year period after discharge from hospital-based neonatal home care. A qualitative content analysis with an inductive approach was used to elucidate the experiences of fathers six months after their preterm infant's discharge. Twelve fathers participated in the study. The following categories emerged and constituted the result: "Vivid memories from the NICU," "Struggling with new challenges in life" and "Transition to home still in their thoughts." The approach and attitude of health-care professionals may contribute to fathers' involvement during the care period, as fathers tend to set their own feelings aside.

Abstract:The worldwide population of migrant families is on the rise, and there is growing acknowledgement of the significance of supporting parental mental health within these families. However, understanding of the experiences of migrant fathers during the perinatal period remains incomplete. The objective of this review is to provide an overview of existing research on perinatal migrant fathers' experiences in different cultures. Multiple searches were conducted in April 2023 for quantitative, qualitative, and mixed-methods studies across six electronic databases: Medline, CINAHL, Embase, PsycINFO, Web of Science, and Scopus. Fourteen eligible articles were identified, including nine qualitative studies, five quantitative studies, and no mixed-methods studies. The Mixed-methods Appraisal Tool was used to assess the quality of these studies. The quantitative findings were transformed into narrative summaries to be analysed thematically along with the qualitative data. Three themes were identified: (1) Cultural competence (dealing with cultural differences, needs related to original country); (2) Parenthood in a new country (challenges and adaptation to fatherhood, challenging traditional gender norms, lack of extended family and building new support networks, being the main supporter for the family); (3) Needs of the fathers and their personal difficulties. The findings of this review suggest a direction for future research in perinatal psychology. The review also provides insights into the need for social and community support for migrant fathers and how healthcare services can support this group during the perinatal period.

Finding My Way: A Phenomenology of Fathering in the NICU

Abstract:Background: Historically, the relationship between infant and mother in the neonatal intensive care unit (NICU) has been the main focus of parenting research, leaving a gap in the literature regarding the paternal experience. Purpose: The purpose of this study was to gain an understanding of the lived experience of fathering an infant born at less than 28 weeks' gestation admitted to a level III NICU. Methods: Seven fathers of premature infants (25-27 weeks' gestation) participated in a semistructured interview about the experience of becoming a father to a premature infant at least 1 to 2 weeks after the NICU admission. Data were collected in 2015. Findings: The primary themes identified were looking in, persevering, holding, and finding my way. Fathers in this study described feeling like an outsider in the NICU while learning to trust strangers, protect the mother and the child, and continue to work and provide for the family. Holding for the first time is pivotal in this journey, as the moment of solidifying the connection with the child. Implications for practice: The findings from this study bring awareness of the experiences of fathers during the NICU journey of having a premature infant. Nurses should encourage paternal participation and involvement, visitation, and facilitate kangaroo care opportunities early and often. Implications for research: The findings from this study allow nurses to better understand the paternal experience of having a premature infant born at less than 28 weeks. However, future research should continue to investigate the paternal experience with other gestational ages as well as the influence of stress of fathers during this experience.
Former Neonatal Intensive Care Unit Fathers' Involvement 4 Years Later: A Qualitative Study

Abstract:Purpose: To explore the experience of fathering a previous Newborn Intensive Care Unit (NICU) infant four years after hospitalization. Design and methods: Nineteen fathers of former NICU infants were interviewed using a semi-structured schedule. Results: Most of the fathers had children with ongoing health issues (84%). The median age of the fathers was 38 years old. Racial demographics included 12 white fathers (63%), 5 black fathers (26%), and 2 Hispanic fathers (11%). Themes that emerged included adjustments, prepared for anything, closeness, ambivalent normalcy, and vigilance. These fathers of former infants with health issues made 'adjustments' in their lives to be available and better fathers for their children. They familiarized themselves with their children's ongoing health issues so they could be better protectors and be 'prepared for anything', but considered their children 'normal kids' while still maintaining 'vigilance' for future health needs. Fathers described that the experience of having an infant with health issues helped them to develop 'closeness' with the child and boosted their confidence as fathers. Conclusion: This involvement/confidence pattern is critically important to establishing and maintaining fathering, especially for fathers of children with disabilities. Practice implications: Pediatric practitioners can continue to encourage this pattern after the initial hospital stay.

Abstract:Background: Paternal involvement during pregnancy has been shown to have a positive impact on the mother-baby dyad during and after birth. The presence of the father during birth also positively influences father-child attachment, reduces, apprehension, and stress, increases maternal satisfaction, and increases love and respect for the partners. This study explored the experiences of fathers who were present in the delivery room during the delivery process. Methods: This qualitative content analysis study was conducted at a maternity hospital in Kingston, Jamaica from June to July 2022. A total of ten (10) fathers of babies delivered within six weeks were purposefully selected to be interviewed via telephone over four weeks. Data were analysed using a thematic approach. Results: Four sub-themes and one theme were extracted from data analysis. Fathers perceived satisfaction and empowerment in the shadow of positive and negative feelings as the theme. Sub-themes emerged from the data including "clinician's acceptance and support", "close bonding and attachment with mother and baby", "preparedness of fathers to give support", and "inspiring and amazing feeling by fatherhood experiences". Conclusion: Fathers experienced the feeling of empowerment and wished to take up their role during the delivery of their newborns. Their experiences were influenced by the midwives and the resources available to support their accommodation. More policies are needed to strengthen the fathers' involvement during antenatal clinic visits, delivery, and postnatal care.
A qualitative study of the impact of adverse birth experiences on fathers

Abstract:Background: Being present during labour and birth can, for some fathers, result in feelings of fear, uncertainty, anxiety, and helplessness. Witnessing birth complications or adverse events may cause immediate and long-term anxiety and stress. In turn, this experience can impact on men's sense of self and identity as a man and father and can affect his relationship with his infant and partner. The aim of this study was to explore the immediate and longer-term impact of witnessing a complicated or adverse birth experiences on men in heterosexual relationships and their role as a father. Methods: An interpretive qualitative approach informed the design of this study. A total of 17 fathers, one from New Zealand and sixteen from Australia participated through face to face, telephone and email interviews. The ages of the men were between 24 to 48 years, and the time since the adverse birth experience ranged from 4.5 months to 20.5 years. Findings: Thematic analysis revealed three major themes representing men's experiences of witnessing a complicated birth or adverse event; 'Worst experience of my life', 'Negotiating my place: communicating with health professionals' and 'Growing stronger or falling apart'. Men were unprepared and feared for the lives of their infants and partners, they expected and wanted to be involved in the birth and the maternity care journey, instead they were pushed to the side and excluded from the labour and birth during times of emergency. Being excluded from part or all of the birth perpetuated worry and vulnerability as, at times, men were left not knowing anything about what was happening to their partners. Midwives and other health professionals' support was important to the way fathers adjusted and processed the complications of the labour and birth event. This experience impacted on their own mental health and their relationship with their baby and partner. Conclusion: Findings demonstrate that following a complicated or adverse birth experience, men questioned their role as a father, their place in the family and their role at the birth. There is a need to include and inform the expectant father that help is available if they experience negative feelings of hopelessness or despair. Maternity services and care providers need to involve fathers so that they feel part of the maternity care system and journey which may mitigate feelings of helplessness.

Abstract:Objective : Engagement of fathers in family health services confers benefits for the health and wellbeing of the whole family. The childbirth continuum is traditionally considered a feminine event, however, commensurate with the changing paradigm of gender equity in family healthcare worldwide, the role of fathers is in transformation. The aim of the study is to explore father's perceptions and experiences of healthcare engagement during pregnancy and early infant care. Design : Qualitative free-text questions were embedded in a large multi-country, cross-sectional survey, to explored fathers' attendance, participation, and experience of health care during appointments with their pregnant partner and/or baby. Setting and participants : Expectant and new fathers were recruited through Prolific®, an international paid online survey platform. Findings : Qualitative responses (n=889) were provided by fathers from 28 countries, with experiences of a range of contexts and models of care; 46.8% of whose partners were pregnant and 53.2% had given birth since 2020. The findings suggest that although most fathers wanted to attend and participate in maternity and early parenting-related healthcare, multiple barriers were identified at the individual father, organisational context, and societal levels. Fathers reported negative social factors such as gender bias and restrictive gender norms as barriers to their healthcare engagement. In contrast, factors that enabled fathers to overcome barriers included the fathers' feelings of c onfidence in their partner's autonomy and decision-making skills, trusted professional relationships with clinicians, and clinicians with good interpersonal skills. Key conclusions : Multiple barriers restrict the participation of fathers in healthcare for childbearing and early parenting. Knowledge of these barriers can inform healthcare redesign to include more successful engagement strategies for fathers, to benefit fathers, mothers, and infants alike. Implications for practice : Health professionals consulting with the mother, father and infant triad are ideally placed to address the healthcare needs of both parents. Early engagement of fathers in family health care by use of inclusive interpersonal skills and the development of a trusted relationship has potential to improve paternal mental health, and may be associated with benefits for the health, wellbeing and safety of the whole family.

Abstract:Though fatherhood research has recently burgeoned, extant literature primarily focuses on father characteristics and father–child interactions. A critical gap remains in understanding fathers' emotional experiences. Using the Parent Development Interview-Revised, 74 interviews with fathers of toddlers were coded using questions about six positive and negative emotions: joy, pain/difficulty, happiness, pride, annoyance/anger, and guilt. Themes reflected 4 overarching concepts: father–child relationship, child-focused experiences, father-focused experiences, and the fatherhood role or identity. Frequent themes included: watching their children grow, sharing affection and love, children's behavior, and spending time away. Some codes appeared especially salient for some fathers, such as growth in their co-parenting relationship and comparison to their family of origin. This study describes men who are deeply invested in fatherhood, highlighting the importance of research that goes beyond documenting fathers' influence on their children, and instead emphasizes the inherent value of fathers and in understanding fathers' experiences.

Abstract:Objectives To appraise and synthesise qualitative literature identifying: (a) fathers' concerns and challenges during the transition to fatherhood that may be impacting mental health and well-being experiences; and (b) fathers' experiences of antenatal programmes, and psychological and social support, to understand how we can better support fathers in addressing these concerns. Design International, qualitative evidence synthesis, scoping review. Data sources Six electronic databases (Medline, Embase, PsycINFO, CINAHL, Scopus, ASSIA) alongside 'grey' and supplementary searches were conducted March 2024. Eligibility criteria Qualitative studies and qualitative data extracted from mixed methods studies focusing on fathers' mental health within the perinatal period globally were included. Data extraction and synthesis The recommended Joanna Briggs Institute data extraction and critical appraisal tools were used, and an inductive thematic synthesis approach employed. Results 37 qualitative studies were included: UK (n=11), Europe (n=9), Australia (n=7) and Asia (n=6), USA (n=1), Canada (n=1) and the Middle East (n=2). Quality appraisal scores were moderate to high (5–10). Four analytical themes and eight subthemes were generated: (1) diminished partner relationship, (2) provider or protector? Multiplicity of fatherhood identification, (3) forgotten entity within the perinatal experience and (4) "I try to battle it myself," masculine ideals within fatherhood. The findings highlighted that fathers found the transition to be difficult, compounded by insecurity within their role as both partner and father. Fathers found a paucity of paternally focused support and antenatal programmes, not addressing fathers' needs within the transition. Conclusions The many concerns and challenges highlighted in the review demonstrate the importance of understanding the negative impact of the transition on fathers' mental health and well-being. There is a need for greater attention to fathers' experiences of paternal perinatal support and programmes through research and practise to inform future interventional development. Abstract:Background: It is estimated that approximately 13% of expectant fathers experience a pathological and debilitating fear of childbirth. Objective: The aim of this integrative review was to examine and synthesise the current body of research relating to paternal experience of fear of childbirth. Methods: A systematic literature search of five databases—CINAHL, Cochrane Library, MEDLINE, PsycArticles and PsycInfo—identified seventeen papers. Methodological quality of studies was assessed using the Crowe Critical Appraisal Tool. Results: Thematic data analysis identified three themes: the focus of fathers’ childbirth-related fears, the impact of fear of childbirth on health and wellbeing, and fear of childbirth as a private burden. Discussion: Fear of childbirth is a significant and distressing experience for expectant fathers who may benefit from an opportunity to express their childbirth-related fears in an environment where they feel validated and supported. Antenatal education is recommended to enhance fathers’ childbirth-related self-efficacy to reduce fear of childbirth. Conclusions: Fear of childbirth may negatively impact the lives of men and consequently their families. Further investigation into methods and models for identifying and supporting men at risk of or experiencing fear of childbirth is required to improve outcomes for this population of men.

The lived experience of South African men having a premature baby

Abstract:Background: Much has been written about fathers, fatherhood and premature babies. However, in the South African context, studies about the experiences of fathers having a premature baby are lacking. Aim: This study aimed to explore how South African fathers (n = 10) experience having a premature baby using a descriptive phenomenological approach. Setting: This research study was conducted online using various social media platforms such as WhatsApp, Google Meet and through telephonic conversations. Methods: A descriptive phenomenological approach that allowed for the distillation and elucidation of common core experiences among fathers who had a premature baby. Results: The findings demonstrated that the participants experienced intense fears regarding the survival and well-being of their children. They reported experiencing financial difficulties related to hospital bills and experienced being alienated by hospital institutions. Despite these reported barriers, these fathers were adamant in their resolve to support their children and partners during this challenging time. Conclusion: The experiences of fathers were riddled with fear, uncertainty, ambiguity and alienation, which placed them in very precarious situations when trying to navigate their role in a more sensitive and enlightened way. Having a premature infant calls into question the systems that men are positioned within as these systems to a large extent shape these events and how they are experienced. Contribution: This study is original as no other published studies seem to exist in South Africa that speaks to fathers' lived experiences of having a premature baby.
An exploration of masculinity, social support and depression in new and experienced fathers

Abstract:Objective: This study aimed to explore the relationship between masculinity, perceived social support and depression symptomology in the postpartum period in new and experienced fathers. Design: Cross-sectional questionnaire study. Participants: A total of 118 first- and second-time fathers (N = 48) of infants aged under 12-months, currently residing in the United Kingdom. Measurements and findings: Questionnaires consisted of the Edinburgh Postnatal Depression Scale, the Conformity to Masculine Norms Inventory, and the Multidimensional Scale of Perceived Social Support. Data were analysed through inferential statistics. Key conclusions: Masculine norms of self-reliance and primacy of work were positively related to depression symptomology in both father groups. Perceived social support was negatively related to depression symptomology. Further analyses revealed significant effects regarding partner health status and depression symptomology. No significant differences were found between presentation of first- and second-time fathers. Implications for practice: Main findings support partners as a part of the family unit. Findings have implications for midwives in that an increased understanding of these factors in early fatherhood could improve family outcomes.

Abstract:British Journal of Occupational Therapy, Ahead of Print. Introduction:Fatherhood is a significant transition in a man's life, introducing new challenges. Yet men's experiences of this period remain poorly understood. Occupational therapists possess skills and knowledge to support individuals during transition. This study explored the experiences of men during the transition to fatherhood over the first postnatal year.Method:Underpinned by an occupational therapy theoretical perspective, a systematic review was conducted. Following searches of databases: AMED, CINAHL, Medline and PsycINFO, qualitative studies meeting selection criteria were retrieved and quality assessed. Data was extracted and synthesised using an established meta-ethnographic approach.Findings:Seven articles were included in the review. Four interconnected themes: (1) A New Occupational Identity, (2) Relationship Changes, (3) Challenges and Impact, (4) Unmet Occupational Needs, and four subthemes: Roles and Responsibilities, and Adjusting Priorities (theme 1); An Emotional Journey and Exhaustion (theme 3) were identified.Discussion:Fathers' postnatal transitional experiences were reflective of a journey of adjustment and growth. Experiences indicated the requirement for greater support of fathers throughout the antenatal and postnatal transition. Occupational therapists have the skills to work with fathers to balance occupations, manage fatigue and adjust to new roles. Further research should focus on understanding experiences of men in under-represented groups.

Father-Inclusive Perinatal Parent Education Programs: A Systematic Review

Abstract:CONTEXT: Fathers contribute to their children’s health starting at the beginning of life. Few parent education programs include fathers. Among those that do, there is little effort to report program effects on father outcomes. OBJECTIVE: In this systematic review, we examined father-inclusive perinatal parent education programs in the United States as they relate to a range of father outcomes. DATA SOURCES: The databases searched were PubMed, Cumulative Index to Nursing and Allied Health Literature, Embase, Ovid Medline, Cochrane Central Register of Controlled Trials, and PsycINFO. STUDY SELECTION: Studies were included if they included an evaluation of a parent education program and a report of father outcomes measured within 1 year of the child’s birth and were conducted within the United States. DATA EXTRACTION: Of 1353 total articles, 21 met study criteria. RESULTS: The overall state of the father-inclusive perinatal parent education program literature was poor, with few interventions available to fathers. Available programs were associated with increased father involvement, coparenting relationship, partner relationship quality, father’s mental health, and father’s supportive behaviors. Program effects on father-infant interaction, parenting knowledge, and attitudes and parenting self-efficacy were inconclusive. Three programs emerged as best evidence-based interventions. LIMITATIONS: Risk of bias was high for many studies. Outcome variability, small sample size, and publication bias contributed to the weak evidence base. CONCLUSIONS: There is a need for more evidence-based interventions to support fathers. Clinicians play a key role in engaging fathers in early parent education programs and health care settings. Experiences, needs, and perceptions of paternal involvement during the first year after their infants' birth: A meta-synthesis

Abstract:Objectives: Fatherhood has evolved in recent decades from 'a moral teacher' to 'an involved father'. However, fatherhood experiences have not been examined in detail. This meta-synthesis aimed to examine fathers' experiences, needs, and perceptions of their involvement with their infants during the first 12 months of birth. Method: Six electronic databases were systematically searched: PubMed, CINAHL, Embase, Scopus, PsycINFO, ProQuest (grey literature). The search resulted in 13 studies that met the inclusion criteria. Quality appraisal was conducted using the Critical Appraisal Skills Programme Checklist. All 13 studies met the appraisal criteria and were included in the meta-synthesis. The findings of the 13 studies were synthesized using the steps of Sandelowski and Barroso in conducting meta-synthesis. Results: The 13 included studies comprised studies conducted in the West, Africa, and Asia. Fatherhood experiences differed according to different sociocultural contexts. Three themes were identified: (1) trajectory of the father-infant relationship, (2) reinforcements and hindrances to involvement, and (3) change from self-oriented to family-oriented behavior. Changes in a father's relationship with his infant were influenced by relationships with his spouse and family members after the birth of the infant. Reinforcements, hindrances, and needs to fathers' involvement were identified. Fathering responsibility and parenting satisfaction that developed overtime influenced a father's behavior, changing from self-oriented to family-oriented. Significance: This is the first meta-synthesis that examined fathers' experiences, needs, and perceptions of their involvement with their infants during the first 12 months of birth. Multiple factors were found to influence the socially-defined fathering ideology. Cultural practices of fatherhood and fathers' own perceptions shaped their actual fathering behaviors. The findings of this study may guide healthcare professionals as frontline personnel to understand fathers' needs and experiences in order to promote fathers' involvement in the early days after their infants' birth.

Abstract:The perinatal period (from conception to 1 year after birth) is one of biological, psychological, and social transition, requiring adjustment, support, and resources. While the mental health of perinatal women is receiving welcome attention, the absence of clinical guidelines means that the mental health of new fathers (and other non-birthing parents) is likely underassessed. Although around 10% of fathers are reported to experience depression during the perinatal period,1 the true prevalence is likely to be higher given the lack of screening and barriers to disclosure. Paternal perinatal depression may present as difficulties including balancing work and home life, perceived financial strain, and relationship difficulties between fathers and their partners.2 While fathers' partners are their main social support,2 poor marital relationship satisfaction and maternal depression are risk factors for paternal depression.3 Men may be less likely than women to seek help for perinatal depression, normalising their symptoms or not recognising it as something affecting fathers.2 Yet, paternal depression is associated with child emotional and behavioural difficulties,4 as well as postnatal depression in mothers,1 and so attention to the mental health of (expectant) fathers and other non-birthing parents is required. Abstract:BACKGROUND AND AIMS: The importance of fathers' participation for development of the child and the well-being of the family is recognized from earlier research. In Sweden, legislation allows fathers to share the parental leave equally with the mother even so fathers only use a small of their paid leave. The aim of the study was to explore experiences of the first year as a father.METHOD: A phenomenological life world approach was used. Ethical approval was obtained. Ten men, recruited by a purposive sample, were interviewed 12-14 months after the delivery of their first child. The data collection was performed during June and August 2004 in the south-west area of Sweden.FINDINGS: The essence of the experiences of the first year as father was to place the baby in the centre without giving up one's own person. The child provided warmth and happiness in the family and men experienced a deeper relationship to their partner. The contact between father and child was facilitated by engagement and time spent alone with the child. The major constituents identified from the findings were 'To be overwhelmed', 'To master the new situation' and 'To get a new completeness in life'.DISCUSSION AND CONCLUSION: To master fatherhood maintenance of integrity and possibility to develop an independent relationship with the child is important. Fathers are invited to participate in postnatal childbirth education but the activities address women's needs and it is doubtful if the fathers benefit from participation. Though fathers entered the delivery room some decades ago, as a support to the woman, health personnel of today must be aware of fathers' own needs and the impact gender aspects have on their professional support.

Abstract:<p>As a result of societal changes, fathers participate more actively in child care than they used to. In this article, we propose a context‐dependent biobehavioral model of emergent fatherhood in which sociocultural, behavioral, hormonal, and neural factors develop and interact during the first 1,000 days of fatherhood. Sociocultural factors, including different expectations of fathers and varying opportunities for paternal caregiving through paid paternal leave, influence paternal involvement. Levels of hormones (e.g., testosterone, vasopressin, oxytocin, cortisol) predict fathers' parenting behaviors, and involvement in caregiving in turn affects their hormones and brain responses to infant stimuli. The birth of the first child marks the transition to fatherhood and may be a critical period in men's lives, with a smoother transition to fatherhood predicting more optimal involvement by fathers in subsequent years. A focus on prenatal and early postnatal fathering may pave the way for developing interventions that effectively support fathering during pregnancy and in the first years of their children's lives.</p>

Abstract:Around 2,000 sick or premature infants are admitted each year to intensive care units in Northern Ireland--8.1 per cent of the live birth population. Studies show that having a premature or 'high-risk' infant is stressful for both parents, yet most research focuses on the mothers' experiences. A literature review was undertaken in preparation for a study exploring fathers' experiences of caring for an infant admitted to a neonatal intensive care unit. It identified seven papers reporting primary research on the fathers' experiences, and others in which their views were included or assumed. Among the findings were that fathers react and cope differently from mothers, and may be burdened by having to take time off work. In addition, they may not perceive themselves as the primary caregiver to their sick infant, but rather providing a supporting role.
