Abstract:Circulating tumor cells (CTCs) have been considered as a potential biomarker for evaluation of cancer metastasis and prognosis, especially in hepatocellular carcinoma (HCC). However, the isolation and detection of rare CTCs in HCC patients face enormous challenges due to omittance and nonspecific binding. We previously designed a small molecular NIR fluoresent agent, named MLP, which had high affinity with a tumor cell-overexpressed enzyme, aminopeptidase N (APN). Based on that, in this work we introduced a novel strategy via coassembling the antiepithelial cell adhesion molecule (EpCAM) antibody and MLPinto theFe3O4 magnetic nanobeads (MB-MLP-EpCAM) to isolate and identify HCC-CTCs coinstantaneously. MB-MLP-EpCAM significantly improved the CTC-capture efficiency (&gt;85%) without sacrificing cell viability (&gt;90%). Most importantly, the advantages of precise dual-targetability, high resolution of fluorescence imaging, and prominent selectivity make our nanoplatform have great potential to achieve <i>in vivo</i> real-time identification and monitoring of CTCs clinically. The Supporting Information is available free of charge at <a class="ext-link" href="/doi/10.1021/acs.nanolett.0c04180?goto=supporting-info">https://pubs.acs.org/doi/10.1021/acs.nanolett.0c04180</a>.Synthetic route of MLP, MLP-PEG, TZ-COOH, and TZ-PEG, 1H NMR and HRMS spectra of MLP and TZ-COOH, the standard curve of Fe(acac)3, MRI images, magnetic response and capture efficiency of MB-MLP-EpCAM on HepG2 cells in PBS (<a class="ext-link" href="/doi/suppl/10.1021/acs.nanolett.0c04180/suppl_file/nl0c04180_si_001.pdf">PDF</a>)The nude mice were injected with GFP-HepG2 cells and MB-MLP-EpCAM. What problem does this paper attempt to address?
The main problem that this paper attempts to solve is to improve the detection efficiency and specificity of circulating tumor cells (CTCs) in hepatocellular carcinoma (HCC). Specifically, the researchers face the following challenges: 1. **Rarity of CTCs**: The number of CTCs in the blood is very small, which makes their isolation and detection extremely difficult. 2. **Nonspecific binding**: Existing CTCs detection methods (such as the magnetic bead method based on EpCAM antibody) are prone to generate false - positive and false - negative signals, affecting the accuracy of detection. 3. **Cell heterogeneity**: CTCs are highly heterogeneous, and CTCs from different patients may express different markers, which increases the complexity of detection. To overcome these challenges, the researchers designed a new type of dual - targeted magnetic fluorescent nanobeads (MB - MLP - EpCAM), which combines anti - epithelial cell adhesion molecule (EpCAM) antibody and a high - affinity small - molecule fluorophore MLP (targeting aminopeptidase N, APN). This dual - targeted strategy aims to: - **Improve capture efficiency**: By simultaneously targeting EpCAM and APN, the capture rate of CTCs is significantly increased (> 85%). - **Maintain cell viability**: While achieving high - efficiency capture, it also ensures the cell survival rate (> 90%). - **Reduce false - positive signals**: Through the dual - targeted design, false - positive signals caused by a single target are reduced, and the specificity of detection is improved. - **Enable real - time monitoring**: These nanobeads can identify and monitor CTCs in vivo in real - time, providing new possibilities for clinical applications. In summary, this study aims to develop an efficient and specific CTCs detection method to improve the early diagnosis, recurrence prediction, and prognosis evaluation of hepatocellular carcinoma.
