Abstract:Secondary hemorrhage occurs as a complication of central nervous system (CNS) following acupuncture has been reported in few cases. The present study reports a case of a 70-year-old woman who presents medulla oblongata hemorrhage after acupuncture. The patient suffered severe headache, neck pain, vomiting, and weakness after the neck acupuncture. The purpose of presenting this case report is to raise awareness among clinicians of fateful CNS complications during acupuncture. More importantly, qualified training of acupuncturists is anticipated and standardized processes of clinical acupuncture operations needs to be developed, in order to avoid the occurrence of acupuncture accidents.

Abstract:At present, there are many disputes and divergences in the diagnosis , treatment and evaluation for facial paralysis by acupuncture and moxibustion .This article reviews several points on following aspects: the time when acupuncture should be involved;which acupuncture points and methods should be chosen , including enhancing and reducing manipulation;whether steroids or antiviral medicine should be given;whether auxiliary facial muscle training is needed .The paper proposes that acupuncture should be involved in the acute stage ;the choice of acupuncture points , the method of manipulation and the amount of stimulation should be con-firmed; syndrome classification needs to be standardized; steroids but not antiviral medicine should be given;auxiliary facial muscle training should be taken as well as acupuncture .

Abstract:Background: Isolated ischemic oculomotor nerve palsy as a type of ophthalmic disease is rarely observed in clinical practice. Quality of life is frequently impacted by isolated ischemic oculomotor nerve palsy due to its lack of treatment options and long-term visual impairment. We describe an acupuncture-treated instance of isolated ischemic oculomotor paralysis. Methods: Acupoints including Jingming (BL 1), Chengqi (ST 1), Cuanzhu (BL 2), and Sizhukong (TE 23) on the right side, and bilateral Fengchi (GB 20), Waiguan (TE 5), Hegu (LI 4), and Zulinqi (GB 41) were selected for needling. Each treatment lasted for 30 minutes, once every other day. Acupuncture treatment was administered for a total of 11 times. Results: Acupuncture is a promising treatment option for isolated ischemic oculomotor nerve palsy. Conclusions: Ischemic oculomotor nerve paralysis can affect the quality of life of patients. Acupuncture intervention can promote the recovery of the disease is a very effective treatment measure.

Abstract:'s experience is introduced in the treatment of cortical blindness with the comprehensive acupuncture therapy for benefiting and resolving stasis. The comprehensive therapy includes acupuncture at Sishencong (EX-HN 1), Xinming No.1 (Extra), Shang Jianming, Chengqi (ST 1), Xinming No.2 (Extra), optic area and visual area (scalp acupuncture); acupoint injection applied alternatively at Qiuhou (EX-HN 7) and Taiyang (EX-HN 5); the auricular point paste stimulation at Brain Point, Shenmen (TF), Gan (CO), Shen (CO), Yan (LO), Eye No.1 and Eye No.2; as well as the tapping therapy with skin needle at Zhengguang No.1 and Zhengguang No.2. Additionally, 3 typical cases of cortical blindness are introduced. The coordination of the different acupuncture techniques is analyzed so as to explain its actions as benefiting , resolving stasis, promoting blood circulation and nourishment of the eyes.

Abstract:Oculomotor nerve paralysis (ONP) is a common disease discovered in the ophthalmology department, mainly manifested as ptosis with the same side, eyeball in the fixed external exhibition space, the movement of inward, upward, and downward are limited, which can lead to some complications such as exotropia, ptosis, diplopia, and others. [ 1 , 2 ] The etiology of ONP is complex and diverse, and microcirculation disorder is considered to be the leading cause. [ 3 , 4 ] In various publications, some researchers have established the fact that acupuncture therapy is a safe and feasible method for ONP. [ 5 , 6 ] However, the effect of electroacupuncture (EA) for breast cancer patients with ONP after chemotherapy was rarely reported. Here, we described a case of a 56-year-old breast cancer patient with ONP, who was successfully treated by using EA therapy as choosing the acupoints of Jingming (BL1), Cuanzhu (BL2), Yuyao (EX-HN4), Sizhukong (TE23), Tongziliao (GB1), Taiyang (EX-HN5), Chengqi (ST1) on the lesion side.

Abstract:Purpose To provide a new classification system for diplopia and evaluate the etiology and clinical features of diplopia subtypes in south China. Methods In this retrospective study, all patients presenting with diplopia over the period from 2012 to 2014 in south China were reviewed. Patients were categorized into 3 groups according to their extraocular muscle (EOM) dysfunction: single EOM (sEOM), multiple EOMs (mEOMs), and a comitant strabismus group. Clinical data evaluated included age, sex, medical history, etiology and duration of diplopia, ocular alignment, and ocular motility. Results A total of 303 patients were enrolled. The most common type of EOM dysfunction was sEOM (158 cases, 52.1%), followed by mEOMs ( n = 119, 39.3%), and finally the comitant strabismus group ( n = 26, 8.6%). Overall, the most common cause of diplopia involved orbital diseases. Within the sEOM group, microangiopathy ( n = 42, 26.6%) and trauma ( n = 41, 25.9%) were the major etiologies, with the lateral rectus (LR) ( n = 86, 54.4%) being the most frequently involved. There were 12 (4.0%) patients who were considered as nasopharyngeal carcinoma (NPC)-associated diplopia (10 caused by radiation neuropathy following radiation therapy). Thyroid associated ophthalmopathy (TAO, 56 cases, 47.1%) was the predominant etiology found in the mEOMs group. Acute acquired comitant esotropia (AACE, 14 cases, 53.9%) was the most common etiology in the comitant strabismus group. Conclusions This new classification system for assessing diplopia as based on EOM dysfunction represents an easy-to-follow approach that can be readily adapted for the clinical use. While microangiopathy and trauma represent common etiologies of diplopia, both orbital diseases and NPC-associated diplopia also warrant special attention when assessing diplopia within patients in south China.

Abstract:Introduction: Research has demonstrated that electroacupuncture (EA) stimulation of paralyzed muscles significantly improves nerve regeneration and functional recovery. Description: An 81-year-old man with no history of diabetes mellitus or hypertension presented with a history of brainstem infarction. Initially, the patient had medial rectus palsy in the left eye and diplopia to the right in both eyes, which almost returned to normal after six sessions of EA. Methods: The CARE guidelines informed the case study report. The patient was diagnosed with oculomotor nerve palsy (ONP) and photographed to document ONP recovery after treatment. The selected acupuncture points and surgical methods are listed in the table. Discussion: Pharmacological treatment of oculomotor palsy is not ideal, and its long-term use has side effects. Although acupuncture is a promising treatment for ONP, existing treatments involve many acupuncture points and long cycles, resulting in poor patient compliance. We chose an innovative modality, electrical stimulation of paralyzed muscles, which may be an effective and safe complementary alternative therapy for ONP.

Abstract:[Objective] To investigate the efficacy of acupuncture in combination with point injection for treatment of apoplec-tichemiplegia.[Method] Ninety-eight patients with apoplectichemiplegia were randomly divided into two groups, treatment Group A of 60 cases(acupuncture plus point injection) and control group B of 38 cases(acupuncture).The curative effects and nervous function deficit were evaluated.[Results] The total effective rate was 96.7% in the treatment group and 84.2% in the control group.There was significant difference between the two groups(P<0.05).There was a significant difference in nervous function deficits core between pretreatment and post-treatment(P<0.01) and between the groups(P<0.05).[Conclusion] Acupuncture in combination with point injection has an exact effect on apoplectichemiplegia.

Abstract:This article presents an overview of the most important points a neurologist must remember when dealing with a patient complaining of diplopia. Patients with monocular diplopia and those with full ocular motility and comitant misalignment should be referred to an ophthalmologist and do not require further testing. Patients with recent onset of binocular diplopia who have associated "brainstem" symptoms should have an urgent brain MRI. All patients with 3rd cranial nerve palsy require urgent brain CTA to rule out compressive aneurysmal lesion. Management of patients over 50 years of age with microvascular risk factors with new onset of 6th nerve palsy is controversial: some image these patients at presentation while others choose a short period of observation as most of these patients would have a microvascular etiology for the 6th nerve palsy which should improve spontaneous in 2-3 months. All others with 6th nerve palsy require brain MRI with contrast. Patients with 4th palsy with hyperdeviation that worsens in downgaze should have an MRI with contrast and all others referred to an ophthalmologist. If there is more than one cranial nerve palsy, urgent neuroimaging should be performed with attention to cavernous sinus and superior orbital fissure. Ocular myasthenia should be suspected in patients with eye misalignment that does not fit a pattern for any cranial nerve palsy. Orbital pathology (most commonly thyroid eye disease) can result in restriction of ocular motility and has specific clinical signs associated with it.

Abstract:Rationale: This case report aims to provide clinical evidence on the effectiveness of integrating chiropractic and moxibustion techniques for treating pseudomyopia accompanied by elevated intraocular pressure resulting from cervical spine issues because the application of complementary medicine modalities for managing such vision disorders currently lacks adequate investigations. Patient concerns: A 6-year-old patient presented with blurred vision, intermittent ocular discomfort, and upper cervical discomfort. Diagnoses: Spine-related increased intraocular pressure and pseudomyopia. Interventions: The patient received integrative treatment of chiropractic and walnut-shell moxibustion 3 times a week for a total of 10 treatment sessions. Outcomes: The patient exhibited progressive improvements in visual acuity and reductions in intraocular pressure over the treatment period, with unaided vision exceeding 2 lines of improvement in visual acuity charts and normalized intraocular pressure after 10 treatment sessions. These therapeutic effects were sustained at 3-month follow-up. Lessons: The integrative use of chiropractic and walnut-shell moxibustion demonstrates considerable potential in alleviating symptoms of pseudomyopia, reducing intraocular pressure, and restoring visual function in spine-related pseudomyopia cases.

Effect of acupuncture treatment on nonketotic hyperglycemic hemichorea-hemiballismus: a case report

Abstract:Nonketotic hyperglycemic hemichorea-hemiballismus is a rare syndrome in the clinic, and treatment is often delayed. Hypoglycemic therapy is the most widely used and effective treatment, but some patients experience a slower improvement. Other symptomatic treatment medicines have some degree of side effects. Acupuncture treatment is beneficial for hemichorea-hemiballismus. A male patient, aged 59 years, first visited our hospital outpatient department due to motor agitation with involuntary movements of the right limb. He had a history of type 2 diabetes mellitus and poor blood glucose control. His serum glucose was 26.5 mmol/L (normal: 4.4-6.1 mmol/L), and magnetic resonance imaging demonstrated an irregular area of high signal intensity in T1-weighted imaging, low signal intensity on T2-weighted imaging, and high signal intensity in the left corpus striatum in T2-FLAIR imaging. Hospitalization was recommended for the patient. After ruling out other possibilities, he was eventually diagnosed with nonketotic hyperglycemic hemichorea-hemiballismus. Intensive glycemic control was immediately started with subcutaneous injection and acupuncture treatment at "governor vessel 13 acupoints", and the involuntary movements completely disappeared on the ninth day of hospitalization. The pathophysiology of nonketotic hyperglycemic hemichorea-hemiballismus is unclear. Different patient histories lead to different brain tissue conditions, and relapses and uncontrolled blood glucose add difficulties to treatment. According to Traditional Chinese Medicine theory, insufficient kidney essence leads to brain dystrophy and causes the symptoms of hemichorea-hemiballismus. Research evidence has shown that acupuncture at "governor vessel 13 acupoints" has a beneficial treatment effect on nonketotic hyperglycemic hemichorea-hemiballismus.

Abstract:A 74-year-old woman presented with acute-onset right ptosis and binocular diplopia. CT scan showed low-density lesions in the bilateral basal ganglia and adjacent to lateral ventricles. Intracranial aneurysm was not detected. This case highlights the importance of neurologic localization of ophthalmoplegia based on physical examination and the microanatomy of the oculomotor nerve.
Curative effect of needling in 3 acupoints around eye and Fengchi (GB20) on optic atrophy

Abstract:Objective To study the curative effect of needling in 3 acupoints around eye[Jingming(BL1),Chengqi(ST1) and Shangming]and Fengchi(GB20) on optic atrophy.Methods The patients(n = 40) were randomly divided into treatment group and control group(each n = 20) by applying prospective randomized controlled method.Control group was given Chinese medicinal decoction and support therapy of nerve nutrition,and treatment group was given acupuncture in Jingming(BL1),Chengqi(ST1),Shangming and Fengchi(GB20) once a day besides of the same treatment as control group.After 4 w,the changes of vision,visual field and visual evoked potential(VEP) were observed in two groups.Results The total effective rate was 81%in treatment group and 52%in control group.After treatment and in treatment group,vision was significantly improved,mean light sensitivity(MS)increased significantly,mean defect(MD) decreased significantly and evoked potential amplitude increased significantly(P 0.05).Conclusion Needling in 3 acupoints around eye and Fengchi(GB20) has good curative effect on optic atrophy.

Abstract:Objective: Acupuncture has certain effects to improve myopia visual function, but its neural mechanism is unclear. In this study, we acupunctured at the right Taiyang acupoint of myopic patients to analyze the effects of acupuncture on visual function and electroencephalographic activity and to investigate the correlation between improvements in visual function and changes in the brain. Methods: In this study, a total of 21 myopic patients were recruited. The contrast sensitivity (CS) of the subjects was examined before and after acupuncture, and electroencephalography (EEG) data of the entire acupuncture process were recorded. Results: The study found that compared with before acupuncture, the CS of both eyes in myopic patients at each spatial frequency was increased after acupuncture; compared with the resting state, the contribution of microstate C was decreased during the post-acupuncture state, and the transition probability between microstate A and microstate C was reduced; in addition, the contribution of microstate C was negatively correlated with CS at both 12 and 18 cpd. Conclusion: The contrast sensitivity of myopic patients was improved after acupuncture at the Taiyang acupoint (20 min), which may be related to microstate C.

Abstract:BACKGROUND Cervical dystonia (CD) is a type of muscle tone disorder that usually occurs in the neck muscles. Due to the intermittent or continuous involuntary contraction of the neck muscles, the head and neck are twisted and skewed and some postural abnormalities occur. Long-term abnormal posture or pain can cause negative emotions in patients, which can affect their quality of life. CASE SUMMARY This case report included a 37-year-old woman who was diagnosed with CD associated with anxiety and depression; the accompanying symptoms were head and neck tilt of approximately 90° to the right and mental abnormality. After two courses of acupuncture treatment, the patient’s head and neck can be maintained in a normal position, and the negative emotions can be relieved. CONCLUSION This case indicates that acupuncture can effectively improve CD and the emotional state and quality of life of patients, making it an effective alternative treatment for the condition.

[Efficacy observation on electroacupuncture in the treatment of oculomotor impairment caused by ophthalmic nerve injury]

Abstract:Objective: To observe the difference in the clinical efficacy on oculomotor impairment between electroacupuncture and acupuncture and explore the best therapeutic method in the treatment of this disease. Methods: Sixty cases of oculomotor impairment were randomized into an electroacupuncture group and an acupuncture group, 30 cases in each one. In the electroacupuncture group, the points were selected on extraocular muscles, the internal needling technique in the eye was used in combination of electroacupuncture therapy. In the acupuncture group, the points and needling technique were same as the electroacupuncture group, but without electric stimulation applied. The treatment was given 5 times a week, 15 treatments made one session. After 3 sessions of treatment, the clinical efficacy, palpebral fissure size, pupil size, oculomotor range and the recovery in diplopia were compared before and after treatment in the two groups. Results: In the electroacupuncture group, the palpebral fissure size was (9.79+/-2.65)mm and the eyeball shifting distance was (18.12+/-1. 30)mm, which were hig-her than (8.23+/-2.74)mm and (16.71+/-1. 44)mm respectively in the acupuncture group. In the electroacupuncture group, the pupil diameter was (0. 44 +/-0. 42)mm, which was less than (0. 72 +/- 0. 53)mm in the acupuncture group, indicating the significant difference (all P<0. 05). The cured rate was 63. 33% (19/30) and the total effective rate was 93.33% (28/30) in the electroacupuncture group, which was better than 36.67% (11/30) and 83. 333 (25/30) in the acupuncture group separately, indicating the significant difference (all P<0. 05). Conclusion: Electroacupuncture presents the obvious advantages in the treatment of oculomotor impairment, characterized as quick and high effect, short duration of treatment and remarkable improvements in clinical symptoms, there are important significance for the improvement of survival quality of patients.
