Abstract:Objective To investigate the variation of anti-coagulation and fibrinolysis function in orthotopic liver transplantation(OLT).Methods The anti-coagulation and fibrinolytic function were assessed systematically in 14 cases of OLT.Results The anti-coagulation function increased at first,then reduced gradually and recovered at last.The fibrinolytic function increased gradually during operation,reached the highest level in the reinfusion and 1 hour after reinfusion.Conclusions The detections of anti-coagulation and fibrinolysis function in OLT and the effective measures to rectify anti-coagulation and fibrinolysis function by monitoring the changes timely and accurately in OLT.
Predictors of renal function recovery among patients undergoing renal replacement therapy following orthotopic liver transplantation

Abstract:Renal dysfunction frequently occurs during the periods preceding and following orthotopic liver transplantation (OLT), and in many cases, renal replacement therapy (RRT) is required. Information regarding the duration of RRT and the rate of kidney function recovery after OLT is crucial for transplant program management. We evaluated a sample of 155 stable patients undergoing post-intensive care hemodialysis (HD) from a patient population of 908 adults who underwent OLT. We investigated the average time to renal function recovery (duration of RRT required) and determined the risk factors for remaining on dialysis > 90 days after OLT. Log-rank tests were used for univariate analysis, and Cox proportional hazards models were used to identify factors associated with the risk of remaining on HD. The results of our analysis showed that of the 155 patients, 28% had pre-OLT diabetes mellitus, 21% had pre-OLT hypertension, and 40% had viral hepatitis. Among the patients, the median MELD (Model for End-Stage Liver Disease) score was 27 (interquartile range [IQR] 22-35). When they were listed for liver transplantation, 32% of the patients had serum creatinine (Scr) levels > 1.5 mg/dL or were on HD, and 50% had serum creatinine (Scr) levels > 1.5 mg/dL or were on HD at the time of OLT. Of the transplanted patients, 25% underwent pre-OLT intermittent HD, and 14% and 41% underwent continuous renal replacement therapy (CRRT) pre-OLT and post-OLT, respectively. At 90 days post-OLT, 118 (76%) patients had been taken off dialysis, and 16 (10%) patients had died while undergoing HD. The median recovery time of these post-OLT patients was 33 (IQR 27-39) days. In the multivariate analysis, fulminant hepatic failure as the cause of liver disease (p<0.001), the absence of pre-OLT hypertension (p = 0.016), a lower intraoperative fresh-frozen plasma (FFP) transfusion volume (p = 0.019) and not undergoing pre-OLT intermittent HD (p = 0.032) were associated with performing RRT for less than 90 days. Therefore, a high proportion of OLT patients showed improved renal function after OLT, and those who were diagnosed with fulminant hepatic failure, had no pre-OLT hypertension, received a lower transfused volume of intraoperative FFP and did not undergo pre-OLT intermittent HD had a higher probability of recovery.
Effects of Prostaglandin E1 and Low Dose Dopamine on Renal Function in Patients Undergoing Orthotopic Liver Transplantation

Abstract:Objective To evaluate the effects of prostaglandin E1 (PGE1) and low-dose dopamine on renal function during orthotopic liver transplantation (OLT) .Methods Eighteen ASA Ⅲ-Ⅳ patients with end-stage liver diseases undergoing OLT were randomly divided into two groups of 9 patients each : PGE1 group (group P) and low-dose dopamine group (group D). Anesthesia was induced with midazolam 0.1-0.2 mg·kg-1 , fentanyl 5 μg·kg-1 and pipecuronium 0.1 mg·kg1 and maintained with isoflurane inhalation and intermittent i.v. boluses of fentanyi and pipecuronium. The patients were mechanically ventilated after tracheal intubation. PGE1 was infused at 0.4-0.8μg· kg-1 · h-1 in group P and dopamine at 1 -3 μg · kg-1· min in group D after induction until the end of operation. Swan-Ganz catheter was inserted via right internal jugular vein or subclavian vein and radial artery was cannulated. MAP, ECG, CVP, SvO2 , cardiac output ( CO), SpO2 , PET CO2 and core temperature were continuously monitored during operation. Venous blood samples were taken and urine was collected before induction of anesthesia (T1 .baseline), during preanhepatic (T2) anhepatic (T3) and neohepatic phases (T4) and at the end of operation (T5) for determination of serum creatinine (Cr) concentration and serum and urine concentration of β2 -microglobulin (β2-MG). Creatinine clearance ratio (CCr) was calculated. Total urine output during operation and urine output and the amount of furosemide given during anhepatic phase were recorded. Core body temperature was maintained above 35.5℃ during operation. Veno-venous bypass (VVB) was performed during anhepatic phase.Results In group P, compared to baseline there were no significant changes in MAP, Cr and CCr duringoperation, while serum β2-MG decreased significantly at T5 and urine β2-MG increased significantly at T3-5 . In group D serum β2-MG was significantly decreased while urine β2 -MG significantly increased at T2.5 compared to baseline. There was significantly more urine output during anhepatic phase and the whole operation and less furosemide was given in group D than in group P. Conclusion Low dose dopamine is more effective in protecting renal function during OLT than PGE1 .
[Prophylaxis and management of early acute renal failure after orthotopic liver transplantation]

Abstract:Objective: To summarize the experiences of prophylaxis and management of early period acute renal failure (ARF) after orthotopic liver transplantation (OLTx). Methods: The clinical data of 516 patients from January 2004 to December 2006 at our hospital undergoing allograft orthotopic liver transplantation, 418 males and 98 females, aged 43 (8-76) years old, were collected to analyze the incidence of early ARF. Results: Of these patients, 106 (20.5%)experienced early ARF, including 21 mild, 40 moderate and 45 severe cases. The integrated control treatment including retrieving prerenal factors, adjusting immunosuppressive schedule, drug therapy and blood purification were taken to cure ARF and 21 patients of severe ARF needed continuous renal replacement therapy (CRRT). Of 106 patients, 37 (34.9%) died within 1 month post-OLT. The one month post-OLT survival rate of patients with MELD (model for end-stage liver disease) score less than 20 fared better than those whose MELD score between 20 to 30 and MELD score over 30. Conclusion: The prophylaxis and management of early post-OLT ARF is a systemic procedure from preoperative assessment to postoperative management. The significant steps of preventing and curing ARF within perioperative procedure of OLTx include preoperative evaluation of patient renal function, various intra-operative measures for protecting renal function, reducing renal function impairment due to drug toxicity after OLT and CRRT treatment if needed.

Abstract:One‐hundred‐fifteen patients with no preexisting renal disease underwent orthotopic liver transplantation between December 1984 and February 1988. Immunosuppression was based on cyclosporine and steroids with azathioprine added after treatment for rejection. One‐ and 2‐year patient survival was 88 and 82%, respectively. Glomerular filtration rate averaged 98.4±5.1 cc/min preoperatively and dropped to 54.8±4.2 cc/min 1 month postoperatively. Renal function was stable at 1 and 2 yr postoperatively. A drop in average whole blood cyclosporine levels resulted in better preservation of renal function with no increase in rejection rate or decrease in patient survival. An investigation of factors responsible for the decrease in renal function found no difference in cyclosporine levels, nephrotoxic drugs or events in patients with large or small drops in glomerular filtration rate post‐op. Patients who developed early renal dysfunction (serum creatinine > 2 mg/dl in the first 3 weeks posttransplant) had a significantly lower GFR 1 month and 1 yr post‐op compared with those who did not. There is a predictable decrease in renal function post‐liver transplant and the level of renal function post‐operatively can be predicted by the level of renal function preoperatively. The renal dysfunction appears to be stable at 1 and 2 yr postoperatively.

Perioperative use of continuous renal replacement therapy for orthotopic liver transplantation

Abstract:Purpose: We present a retrospective study describing the perioperative use of continuous renal replacement therapy (CRRT) for orthotopic liver transplantation (OLT). Materials and methods: We retrospectively reviewed the clinical course of patients who underwent OLT with the perioperative use of CRRT. The following variables were recorded: Gender, age, indication for transplantation, time when CRRT was initiated, postoperative need for CRRT, and the patient and organ (liver, kidneys) outcome up to 1 year after transplantation. Results: Among 105 patients who underwent OLT from 2006 to 2010; we used CRRT in 12 cases (11.4%) perioperatively, including 9 (8.3%) patients intraoperatively. Perioperative CRRT was employed for volume, electrolyte, and/or pH management. All patients who underwent CRRT perioperatively were alive at 1 month, 10 (83.3%), at 3 and 6 months and 9 (75%) at 1 year after OLT. Only 1 surviving patient (8.3%) required renal replacement therapy at 1 month after surgery. Renal replacement therapy was not required in any surviving patient up to 12 months posttransplantation. Conclusion: Perioperative and especially intraoperative use of CRRT therapy can potentially improve the outcomes of patients undergoing OLT.
Continuous Renal Replacement Therapy after Liver Transplantation: Peri-Operative Associated Factors and Impact on Survival

Abstract:Continuous renal replacement therapy (CRRT) following orthotopic liver transplantation (OLT) is usually started for multifactorial reasons, with variable incidence among series. This paper presents a single-center retrospective observational study on the early use (within one week) of CRRT after consecutive cadaveric OLT from January 2008 to December 2016. Preoperative patient characteristics and intraoperative data were collected, and patients were divided into two groups (CRRT and no CRRT) to explore the factors associated with the use of CRRT. Repeated measurements of postoperative creatinine were analyzed with generalized estimating equation (GEE) models. Among 528 OLT patients, 75 (14.2%) were treated with CRRT at least once in the first week. Patients treated with CRRT showed lower survival in a Kaplan−Meier curve (log-rank p value < 0.01). Patients treated with CRRT had a more severe preoperative profile, with a significantly higher age, MELD, BUN, creatinine, and total bilirubin, as well as a longer surgery time and a higher number of transfusions of red blood cells, plasma, and platelets (all p values < 0.05). In a stepwise multiple analysis, the following characteristics remained independently associated with the use of CRRT: the MELD score OR 1.12 (95% CL: 1.07−1.16), p value < 0.001, and the preoperative value for blood urea nitrogen OR 1.016 (95% CL: 1.010−1.023), p value < 0.001. The early use of CRRT after OLT occurred at a low rate in this large cohort; however, it was associated with worse outcomes. Apart from the preoperative severity, repeated intraoperative hypotension episodes, which were likely modifiable or preventable, were associated with the increased use of CRRT and higher postoperative creatinine.
Intraoperative Continuous Renal Replacement Therapy During Liver Transplantation: A Meta-Analysis

Abstract:Continuous renal replacement therapy (CRRT) is frequently used to treat recipients with renal failure before or after liver transplantation (LT), though evidence supporting its use during surgery remains unclear. Therefore, we conducted a quantitative meta-analysis to evaluate the effect of intraoperative continuous renal replacement therapy (IORRT) in recipients with pretransplant severe renal dysfunction. We searched PubMed, Embase, and the Cochrane database for trials focusing on LT recipients supported with or without IORRT. Outcomes assessed were mortality, preoperative characteristics, intraoperative data, and predefined postoperative outcomes. Seven trials with 1051 recipients were eligible. Preoperatively, the IORRT group recipients had higher Model for End-Stage Liver Disease scores (weighted mean difference [WMD], 6.19; 95% confidence interval [CI], 2.51-9.87), Charlson scores (WMD, 0.45; 95% CI, 0.09-0.80), acute liver failure (odds ratio [OR], 1.82; 95% CI, 1.27-2.61), serum creatinine (WMD, 71.33 μmol/L; 95% CI, 1.98-140.69 μmol/L), total bilirubin level (WMD, 5.05 μmol/L; 95% CI, 1.75-8.35 μmol/L), intensive care unit admission (OR, 3.53; 95% CI, 1.23-10.13), vasoactive therapy (OR, 3.80; 95% CI, 2.64-5.46), ventilator care (OR, 2.52; 95% CI, 1.18-5.35), and renal replacement therapy (RRT) (OR, 29.37; 95% CI, 7.66-112.54) compared with control patients. IORRT patients also required more intraoperative blood product transfusion and had more post-LT RRT (OR, 25.67; 95% CI, 4.92-133.85). However, there were no significant differences in short-term mortality (OR, 2.12; 95% CI, 0.82-5.44) between the groups. In addition, worse longterm mortality was seen in the IORRT group. In conclusion, IORRT is feasible and safe and may help sicker recipients tolerate the LT procedure to achieve short-term clinical outcomes comparable with less ill patients without IORRT. More high-quality evidence is needed to verify our conclusion in the future.
Functional Changes of the Heart and Lung in Perioperative Period of Orthotopic Liver Transplantation.

Abstract:OBJECTIVETo investigate the variation of functions of the heart and lung during orthotopic liver transplantation (OLT).METHODSPulmonary artery cannula and right radial artery cannula were indewelled before anaesthesia and the parameters of hemodynamics in different periods were monitored. Analysis of variance was used to reveal the variation among the groups. T hypothesis test in paired data was used to compare the preoperative parameters with those in each period during operation and after operation respectively, and to compare the parameters immediately after operation with those in each period after operation respectively.RESULTSDuring the operation, heart rate increased, but blood pressure decreased significantly at the beginning of no-liver period, increased again in a short period and then increased progressively 12 h after operation. Pulmonary artery pressure (PAP) increased from before the no-liver period to 60 h after operation. Pulmonary wedge pressure changed in accordance with the variation of PAP. Cardiac output was maintained at a high-output level from before operation to 60 h after operation. Systemic vascular resistance (SVR) was within the normal limits before operation, whereas pulmonary vascular resistance (PVR) was lower than normal. In the no-liver period during the operation, SVR decreased significantly. Both SVR and PVR increased progressively and returned to normal postoperatively.CONCLUSIONSThe patients undergoing OLT have a high cardiac output and low resistance obstacle before and during the operation, and will recover gradually after operation. Monitoring hemodynamics during the peri-operative period is of significance in the prevention and treatment of pneumonedema and cardiac functional insufficiency.
Anesthetic support in synchronous liver and kidney transplantation

Abstract:Background/aims: We performed a retrospective evaluation of 11 patients in the final stages of hepatic disease with chronic kidney failure, in whom simultaneous double liver-kidney transplantation was performed. Methodology: In the immediate pre-, intra- and postoperative periods, we assessed metabolic, hemodynamic and coagulation parameters; bicarbonate, calcium and inotropic drug requirements; the incidences during reperfusion of the graft; the surgical technique employed; the need for hemodialysis and/or ultrafiltration; and the survival rate of the patients. Results: Of the 11 cases studied, four patients needed hemodialysis, while only one patient needed ultrafiltration; three patients required both techniques, and no dialysis or ultrafiltration was performed in three patients. The following surgical techniques were employed: Total clamping of the inferior vena cava using an external venovenous bypass in two cases; total clamping of the inferior vena cava without an external venovenous bypass in three cases; and partial clamping of the inferior vena cava with preservation of the retrohepatic cava in six cases. The results showed one death in the first postoperative month and two deaths in the course of subsequent follow-up. The survival rate was 72.7%. Conclusions: The use of conventional intraoperative hemodialysis and/or ultrafiltration is feasible, useful and achieves good results in patients undergoing double liver-kidney transplantation. Partial clamping of the inferior vena cava at the anhepatic stage appears to reduce the need for ultrafiltration. There is no increase in perioperative mortality in patients who underwent liver transplantation while conserving their renal function.
Assessment of the Combined Effect of Plasma Exchange and Plasma Perfusion on Patients with Severe Hepatitis Awaiting Orthotopic Liver Transplantation

Abstract:To determine if plasma exchange combined with plasma perfusion is a reliable and effective temporary liver support treatment for patients on the waiting list for OLT, we tested this method in 5 patients with end-stage and 3 patients with middle-stage severe hepatitis. Four patients were successfully controlled until a donor liver was available 4 to 13 days later. In contrast, the remaining 4 patients were not adequately controlled by this treatment and experienced aggravated disease progression, dying 3 to 8 days after treatment while still awaiting OLT. Of those 4 patients who received OLT, 2 patients died from multi-organ failure caused by hepatic failure, while the other 2 survived. These findings show that plasma exchange combined with plasma perfusion provides temporary support for some patients on the waiting list for OLT. The ability of patients to successfully bridge to OLT is closely associated with the degree of liver failure, complications, multi-organ failure, and the length of the waiting period for a donor liver.

Abstract:Objective To study the risk factors of acute renal failure (ARF) complicated to orthotopic liver transplantation (OLT).Methods The clinical data of 62 patients undergoing OLT were analyzed, which were divided into two groups: patients with ARF (23 cases) and patients without ARF (39 cases). Twenty-nine variables, including clinical and experimental variables, were analyzed by t test for continuous variables and χ~2 test for discrete variables. The variables with significance (P(0.05)) were then analyzed with stepwise logistic regression.Results Five continuous variables,(including) preoperative blood total bilirubin concentration, volume of preoperative ascites, volume of (intraoperative) blood-loss, volume of intraoperative blood transfusion and duration of anhepatic phase, had significant difference between two groups. Four discrete variables, including preoperative ICU care, preoperative hepatic encephalopathy, intraoperative low blood pressure and postoperative infection, had significant difference between two groups. The stepwise logistic regression analysis for 9 variables demonstrated that volume of intraoperative blood-loss and postoperative infection were the (independent) risk factors of ARF complicated to OLT.Conclusion The reason for ARF complicated (to OLT) is multiple. Postoperative infection and volume of intraoperative blood-loss are the (independent) risk factors of ARF complicated to OLT.
Changes in Regional Cerebral Oxygen Saturation ( Rso2 ) During Orthotopic Liver Transplantation Without Veno-Venous Bypass

Abstract:Objective To evaluate the changes in regional cerebral oxygen saturation (rSO2) monitored by near infrared spectroscopy during orthotopic liver transplantation (OLT) without veno-venous bypass. Methods Twenty ASA Ⅲ-Ⅳ patients of both sexes (12 males, 8 females) with end-stage liver diseases aged 2O-58 yrs undergoing OLT without veno-venous bypass were studied. Radial artery was cannulated and Swan-Ganz catheter was placed via right internal jugular vein. ECG, MAP, HR, CVP, MPAP, cardiac output (CO) , SpO2, PETCO2 and T were monitored during operation. rSO2 was monitored by near-infrared spectroscopy (INVOS 31OO) . The probes were placed on the forehead on both sides. rSO2 was recorded at the skin incision (T1 ), 5 min before and at the beginning of anhepatic stage (T2, T3 ) , at 30 min after the start of anhepatic stage (T4 ) and 5 min (T5), 15 min (T6 ) , 30 min (T7) , 90 min (T8 ) and 3 h (T9 ) after the start of neohepatic stage. Anesthesia was induced with midazolam 0.1 mg·kg-1 , fentanyl 5-10μg·kg-1 , scopolamine 0.6 mg and vecuronium 0.1 mg·kg-1 , and maintained with isoflurane 0.5%-1.0% and propofol infusion (1-2 mg·kg-1·h-1 ) in combination with intermittent i.v. boluses of midazolam, fentanyl and vecuronium. The patients was mechanically ventilated after tracheal intubation, PaCO2 was maintained between 35-45 mm Hg, SpO2 was maintained at 100% and Hct≥30% .Results MAP, CVP, MPAP and CO decreased significandy during anhepatic stage (T3, T4 ) compared with the baseline values at T1 (P 0.05) and returned to the baseline levels during the neohepatic stage (T5-9 ) . The changes in bilateral rSO2 were similar. rSO2 decreased significandy during the anhepatic stage (T3,T4) compared with the baseline values at T1 ( P0.05) and then returned to the baseline levels during the neohepatic stage. PaCO2 increased significantly at the start of neohepatic stage (T5 ) compared with the baseline value at T1 and then gradually decreased during neohepatic stage (T6-9) but still significantly higher than that at T4 (P0.05) Conclusion There is a tendency toward imbalance between cerebral O2 delivery and consumption during anhepatic and early neohepatic stage during OLT without veno-venous bypass.

Abstract:Objective To evaluate the changes of pulmonary gas exchange and intrapulmonary shunt during orthotopic liver transplantation (OLT) Methods Eleven ASA Ⅲ Ⅳ patients (male 8, female 3) with end stage liver diseases were studied Age ranged from 17 67 yr Anesthesia was induced with midazolam 0 1 mg·kg -1 , fentanyl 5μg·kg -1 and pipecuronium 0 1 mg·kg -1 and maintained with isoflurane inhalation and intermittent iv boluses of fentanyl and pipecuronium combined with epidural anesthesia (T 8 9 ) The patients were mechanically ventilated with 100% O 2 during operation After induction of anesthesia Swan Ganz catheter was inserted via right internal jugular or subclavian vein Cardiac output (CO),mixed venous oxygen saturation and core venous temperature were continuously monitored with continuous cardiac output monitor (Baxter,Vigilance) ECG, CVP, SpO 2 and P ET CO 2 were also continuously monitored during operation Radial artery was cannulated for continuous direct blood pressure monitoring Arterial and mixed venous blood samples were taken at the beginning of operation, 120 min during preanhepatic stage, 30 and 60 min during anhepatic stage, 5 and 60 min during neohepatic stage and at the end of operation Alveolar arterial oxygen partial pressure difference (A aDO 2) , intrapulmonary shunt (Qs/Qt) , oxygen delivery index (DO 2I) and oxygen consumption (VO 2 I ) were calculated During operation body temperature was maintained above 35 5℃ Veno venous bypass (VVB) was performed during anhepatic stage Results At the beginning of operation, when FiO 2 was 100%, PaO 2 was only (376±141) mmHg, A aDO 2 and Qs/Qt were all higher than normal values There were no significant changes in all parameters 120 min during preanhepatic stage as compared with those at the beginning of operation PaO 2 increased and A aDO 2 and VO 2I decreased significantly during anhepatic stage Qs/Qt, A aDO 2 and PaCO 2 increased and PaO 2 decreased significantly, but there were no significant changes in cardiac index, DO 2I and VO 2I during early neohepatic stage Cardiac index, DO 2I and VO 2I increased at 60 min during neohepatic stage and at the end of operation Conclusions There are severe changes of pulmonary gas exchange during OLT
Intraoperative Anesthetic Strategies to Mitigate Early Allograft Dysfunction After Orthotopic Liver Transplantation: A Narrative Review

Abstract:Orthotopic liver transplantation (OLT) is the most effective treatment for patients with end-stage liver disease (ESLD). Hepatic insufficiency within a week of OLT, termed early allograft dysfunction (EAD), occurs in 20% to 25% of deceased donor OLT recipients and is associated with morbidity and mortality. Primary nonfunction (PNF), the most severe form of EAD, leads to death or retransplantation within 7 days. The etiology of EAD is multifactorial, including donor, recipient, and surgery-related factors, and largely driven by ischemia-reperfusion injury (IRI). IRI is an immunologic phenomenon characterized by dysregulation of cellular oxygen homeostasis and innate immune defenses in the allograft after temporary cessation (ischemia) and later restoration (reperfusion) of oxygen-rich blood flow. The rising global demand for OLT may lead to the use of marginal allografts, which are more susceptible to IRI, and thus lead to an increased incidence of EAD. It is thus imperative the anesthesiologist is knowledgeable about EAD, namely its pathophysiology and intraoperative strategies to mitigate its impact. Intraoperative strategies can be classified by 3 phases, specifically donor allograft procurement, storage, and recipient reperfusion. During procurement, the anesthesiologist can use pharmacologic preconditioning with volatile anesthetics, consider preharvest hyperoxemia, and attenuate the use of norepinephrine as able. The anesthesiologist can advocate for normothermic regional perfusion (NRP) and machine perfusion during allograft storage at their institution. During recipient reperfusion, the anesthesiologist can optimize oxygen exposure, consider adjunct anesthetics with antioxidant-like properties, and administer supplemental magnesium. Unfortunately, there is either mixed, little, or no data to support the routine use of many free radical scavengers. Given the sparse, limited, or at times conflicting evidence supporting some of these strategies, there are ample opportunities for more research to find intraoperative anesthetic strategies to mitigate the impact of EAD and improve postoperative outcomes in OLT recipients.

Abstract:Liver transplantation (LT) is an effective and lifesaving treatment for patients with end-stage liver disease and hepatocellular carcinoma. Significant improvement in intermediate and long-term survival has been possible due to advancements in immunosuppressive therapy, perioperative care, and surgical techniques. Despite these advances, metabolic complications, including diabetes mellitus, cardiovascular diseases, malignancies, and renal dysfunction, are challenging issues after LT. Acute kidney injury (AKI) and chronic kidney disease (CKD) after LT are common and result in significant morbidity and mortality. Early diagnosis of kidney injury after LT is challenging, and no technique has yet proven effective in prediction of renal dysfunction. The methods for assessing renal function range from formulas that predict glomerular filtration rate to non-invasive biomarkers. The universal adoption of the model for end-stage liver disease has a direct impact on the incidence of peri-transplant AKI and development of CKD in the long-term. Post-LT renal dysfunction is multifactorial and is usually a result of pre-transplantation comorbidities, occurrence of renal dysfunction on the waiting list, perioperative events, and post-transplant nephrotoxic immunosuppressive medication use. Early identification of patients at risk for renal dysfunction and adoption of preventive measures are crucial in the pre-transplant period. No data are currently available to suggest a surgical technique that reliably demonstrates renal protection. Nephroprotective strategies during LT follow accepted surgical practice guidelines, such as maintenance of intravascular volume and mean arterial pressure. The management of kidney disease following LT is challenging, as by the time the serum creatinine is significantly elevated, few interventions impact the course of progression. Early nephroprotective measures are strongly advised and they mostly center on delaying the administration of calcineurin inhibitors (CNIs) during the initial postoperative period, lowering CNI dosage and combining CNI with mycophenolate mofetil and everolimus. The reasons for renal failure following LT, the techniques used to diagnose it, and the therapies designed to preserve renal function both immediately and late after LT are all critically evaluated in this review.
Effects of Intraoperative Continuous Renal Replacement Therapy on Outcomes in Liver Transplantation

Abstract:Background: Intraoperative continuous renal replacement therapy (CRRT) may need to be indicated for liver transplant recipients who show renal dysfunction. We aimed to investigate effects of intraoperative CRRT on outcomes after liver transplant. Material and methods: This study included all adult patients who underwent liver transplant between January 2005 and May 2017 and were found to have renal dysfunction, which was defined as glomerular filtration rate < 30 mL/min at transplant. The patients were divided into 3 groups for outcome analysis: elective CRRT group (renal replacement therapy was already introduced pretransplant, group 1, n = 70), urgent CRRT (intraoperative CRRT was indicated because of unexpected renal dysfunction, group 2, n = 15), and no CRRT group (no intraoperative CRRT even with renal dysfunction, group 3, n = 57). Post-transplant outcomes were analyzed to determine effects of CRRT. Results: Postoperative complication rates were similar in the 3 groups (P = .056). Group 1 showed the highest rate of postoperative renal replacement therapy (86.4% in group 1 vs 66.7% and 10.7% in groups 2 and 3, P < .001). Long-term renal function (at 3, 6, and 12 months post transplant) was similar among the 3 groups (P = .50, .77, and .52, respectively). Group 1 showed a higher risk of 1-year graft loss (hazard ratio, 2.55; P = .03) and mortality (hazard ratio, 2.71; P = .03) than group 3, whereas groups 2 and 3 were similar. Conclusion: CRRT used in the urgent setting did not show obvious benefit. Hence, its application should be carefully considered in those who unexpectedly present with acute kidney injury at the time of transplant.

Instead, the computing power and economy of sale of minicomputers, the successful application of dedicated systems to a number of hospital information management functions, and the oversize investment and uncertain benefit of large scale systems efforts in the past have together caused many hospitals to make various commitments to information systems functional in particular areas in which the need has been felt to be strongest. It now appears unlikely that any single system will issue forth which has the combined capabilities of the best financial system, clinical laboratory information system, pharmacy inventory control system, and medical record management system currently available, to cite a few examples of functional areas which have met with relatively successful application.
Effects of Different Vasopressors on Hemodynamics in Patients Undergoing Orthotopic Liver Transplantation.

Abstract:Background The hyperdynamic circulatory state in end-stage liver disease is similar to the hemodynamic state in endotoxic shock. Recent research indicated that proper use of norepinephrine ( NE) in patients with endotoxic shock could improve the perfusion of visceral organs and raise the survival rate. In this study, dopamine (DA) or NE combined with DA was infused during the orthotopic liver transplantation (OLT) to observe and compare their effects on hemodynamics, oxygenation, and renal function during different stages of the operation.Methods Thirty American Society of Anesthesiology (ASA) III-IV patients undergoing OLT were randomly divided into group DA and group NE with 15 patients in each group. Vasopressors were infused after induction of anesthesia. DA was infused in group DA; DA and NE in group NE. Data of hemodynamics, oxygenation and renal function were collected after induction, I hour in preanhepatic, anhepatic, neohepatic phase and at the end of operation.Results Heart rate(HR) and mean arterial pressure (MAP) of the two groups were stable. In anhepatic phase, central venous pressure(CVP), mean pulmonary arterial pressure( MPAP), pulmonary arterial wedge pressure (PAWP cardiac output (CO), and cardiac index ( Cl) decreased, whereas systemic vascular resistance (SVR and systemic vascular resistance index (SVRI) increased significantly (P<0.05). The hemodynamic variables of group NE were more stable than that of group DA. Pulmonary vascular resistance (PVR), pulmonary vascular resistance index (PVRI), power of hydrogen ( pH), and mixed venous oxygen saturation (SvO(2)) had no significant changes. Oxygen delivery (DO2) and oxygen consumption (VO2), decreased during anhepatic phase (P<0.05), but lactic acid ( LAC) increased since anhepatic phase. Blood urea nitrogen ( BUN) maintained relatively stable during different phases. Group NE had more urine output (F=4.733, P=0.039).Conclusions During OLT, both DA and NE combined with DA can maintain hemodynamics stable, whereas the latter may create better condition. NE has positive effects on maintenance of renal function.
