Abstract:Objective . The aim of the study was to investigate the associations among the nine types of body constitution in traditional Chinese medicine (TCM) with the outcomes of overweight, obesity, and underweight. Method . Participants aged 30 to 90 years were recruited from communities in Shanghai and assessed using a self-administered questionnaire pertaining to their demographics, lifestyles, and self-reported medical history. The data of 3748 participants with complete information was available for the analysis. Multinomial logistic regression (MLR) analysis was performed to determine the associations among the TCM constitution variables and the health outcomes. Results . The standards of classification and determination of the constitution in TCM were used to gauge the patients’ constitution type. MLR revealed independent and significant associations among the Qi_Deficient and Yang_Deficient groups with the outcomes of overweight, obesity, and underweight ( P < 0.10 for all). MLR revealed independent and significant associations among the Qi_Deficient and Yang_Deficient groups with the outcomes of overweight, obesity, and underweight ( P < 0.05 for all). Conclusion . Our study revealed significant negative correlations between the Qi_Deficient and Yang_Deficient groups with the outcomes of overweight, obesity, and underweight. On the other hand, positive correlations were found between Phlegm_Dampness and the outcomes of overweight and obesity.

Abstract:Introduction: Humans have individual characteristics, and the TCM constitution reflects their inherent nature. Little is known in the general population about the relationship between TCM constitutions and metabolic syndrome (MetS). The purpose of the present study was to investigate the associations between traditional Chinese medicine (TCM) body constitutions and MetS. Methods: This was a large-scale, community-based, cross-sectional study of 3748 participants. TCM constitution and the diagnosis of MetS were based on a body constitution questionnaire (BCQ) and a self-reported medical history, respectively. Univariate and multivariable logistic regressions (MLR) were applied to assess the strength of the associations. Results: Univariate analysis showed that Yang-deficiency was significantly associated with MetS (P< 0.01). Furthermore, multivariable linear regression showed that Yang-deficiency constitution was significantly and independently associated with MetS, after controlling for potential confounding factors (P< 0.01). As compared with neutral participants (ostensibly normal, with no imbalance), participants with Yang-deficiency constitutions had a higher prevalence of MetS. Conclusion: Our observations suggest that Yang-deficiency constitution is closely associated with MetS and provides evidence for the hypothesis that an unbalanced constitution is closely related to MetS in terms of occurrence, development, diagnosis, treatment, and prognosis.
Lower BMI cutoffs to define overweight and obesity in China.

Abstract:OBJECTIVE:To investigate ethnic difference in the associations of BMI with comorbidity, mortality, and body composition between mainland Chinese and U.S. whites. METHODS:Ethnic-comparison study using data from China Health and Nutrition Survey, U.S. National Health and Nutrition Examination Survey, and data from Zhejiang University (China) and Columbia University (U.S.). RESULTS:Chinese people experienced greater odds of comorbidities than whites for a given BMI after standardizing for age and sex: 43% for diabetes, 30% for dyslipidemia, 28% for hypertension, 38% for metabolic syndrome, and 48% for hyperuricemia. Comparisons of BMI-mortality associations found that the U-shaped BMI-mortality curve shifted 1-2 kg m(-2) to the left in Chinese compared to whites. Compared to whites at BMIs of 25 and 30 kg m(-2), corresponding cutoffs in Chinese were 22.5 and 25.9 kg m(-2) in men, and 22.8 and 26.6 kg m(-2) in women after both fat and fat distribution were taken into account. CONCLUSIONS:Comorbidity, mortality, and body composition data consistently support the use of lower BMI cutoffs in Chinese than those in whites.

Abstract:Objective: The present study investigated the associations of body composition, including skeletal muscle and fat mass, with blood pressure (BP) and hypertension.Methods: Data from 3,130 participants aged 18 to 80 years were analyzed. Body composition and total skeletal muscle (TSM) were measured or calculated based on dual-energy x-ray absorptiometry. Multivariate linear and logistic regression models were used to analyze the associations of TSM, body fat percentage, android to gynoid fat ratio, and leg and arm lean body mass (LBM) with BP and hypertension. The Wald test was used to estimate the differences in the coefficients.Results: TSM indices, body fat percentage, and android to gynoid fat ratio were significantly associated with higher odds ratio for prehypertension and hypertension, except for TSM/weight, after controlling for potential confounders. The standardized beta coefficients of arm LBM indices for systolic and diastolic BP were higher than relevant indices of leg LBM.Conclusions: Different indices of TSM, especially in arm LBM, were all positively associated with elevated BP, prehypertension, and hypertension in Chinese adults, after considering potential confounding factors, including body fat and fat distribution. Future longitudinal studies are warranted to confirm our findings.
Truncal and Leg Fat Associations with Metabolic Risk Factors among Chinese Adults.

Abstract:BACKGROUND AND OBJECTIVES:To examine the associations of regional body fat distribution with metabolic risk factors among Chinese.METHODS AND STUDY DESIGN:Truncal fat (TF) and leg fat (LF) were measured by dualenergy X-ray absorptiometry (DXA) among 947 adults, and abdominal visceral fat (VAT) and subcutaneous fat (SAT), upper leg SAT were measured by magnetic resonance image (MRI) among 103 adults during 2008-2013. Metabolic risk factors included fasting blood glucose, total triglyceride, total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, and metabolic syndrome (MetS).RESULTS:TF showed adverse effect while LF showed beneficial effect on metabolic risk factors, and all these effects were independent of body mass index (BMI) (mostly p<0.01). Individuals with higher TF and lower LF experienced the highest risk of MetS compared to other subgroups of combination of TF and LF (all p<0.05). Abdominal VAT was positively associated with risk of MetS (men: odds ratio (OR)=4.45, 95% confidence interval (CI): 1.18, 16.8; women: OR=6.54, 95% CI: 1.08, 39.6) and serum triglyceride (men: beta (β)=0.379, 95% CI: 0.090, 0.667; women: β=0.700, 95% CI: 0.327, 1.07). Upper leg SAT showed an opposite association with most metabolic factors compared to abdominal SAT and VAT, however, the association was not statistically significant.CONCLUSION:TF and LF showed opposite effects on metabolic risk factors among Chinese adults. Abdominal VAT, but not abdominal SAT, was positively associated with serum triglyceride and risk of MetS. Future studies are warranted to examine the potential mechanism of the opposite effects between TF and LF on metabolic risk factors among Chinese.

Abstract:Limited evidence exists for the association between diet pattern and obesity phenotypes among Chinese adults. In the present study, we analyzed the cross-sectional data from 474,192 adults aged 30-79 years from the China Kadoorie Biobank baseline survey. Food consumption was collected by an interviewer-administered questionnaire. Three dietary patterns were extracted by factor analysis combined with cluster analysis. After being adjusted for potential confounders, individuals following a traditional southern dietary pattern had the lowest body mass index (BMI) and waist circumference (WC); the Western/new affluence dietary pattern had the highest BMI; and the traditional northern dietary pattern had the highest WC. Compared to the traditional southern dietary pattern in multivariable adjusted logistic models, individuals following a Western/new affluence dietary pattern had a significantly increased risk of general obesity (prevalence ratio (PR): 1.06, 95% confidence interval (CI): 1.03-1.08) and central obesity (PR: 1.07, 95% CI: 1.06-1.08). The corresponding risks for the traditional northern dietary pattern were 1.05 (1.02-1.09) and 1.17 (1.25-1.18), respectively. In addition, the associations were modified by lifestyle behaviors, and the combined effects with alcohol drinking, tobacco smoking, and physical activity were analyzed. Further prospective studies are needed to elucidate the diet-obesity relationships.
Central Fat Accumulation Associated with Metabolic Risks Beyond Total Fat in Normal BMI Chinese Adults.

Abstract:OBJECTIVE:To investigate the association of central fat with metabolic risks in normal BMI Chinese people.METHODS:Normal BMI subjects (18.5 ≤ BMI < 24, n = 520) aged 18-82 years were included. Blood pressure, triglycerides, high-density lipoprotein cholesterol and fasting plasma glucose were collected as metabolic risk factors. Dual-energy X-ray absorptiometry was used to measure total and central fat. Central fat was indicated as percent trunk fat (%TF) and android/gynoid fat ratio. Multiple adjusted regression models were applied to investigate the association of total and central fat with metabolic risk factors. The assessment values of these fat measures were further compared by receiver-operating characteristic (ROC) curves.RESULTS:Both total and central fat were correlated with metabolic risk factors. After adjusting for BMI, the significant association of percent body fat (%BF) with metabolic risks diminished, whereas %TF and android/gynoid fat ratio were still correlated. When BMI was replaced with %BF in the models, the results remained unchanged. ROC analysis showed the areas under the curve (AUCs) of total and central fat for at least two metabolic risk factors were comparable in men. However, in women, the AUCs of %TF and android/gynoid fat ratio were significantly greater than those of BMI, %BF and waist circumference.CONCLUSIONS:Central fat accumulation had a stronger association with metabolic risks than total fat in normal BMI Chinese adults, particularly in women. Central fat had more important implications in assessing 'metabolically obese' individuals among normal BMI subjects.

Abstract:Background Adiposity is an established risk factor for diabetes. The different measurements of adiposity for predicting diabetes have been compared in recent studies in Western countries. However, similar researches among Chinese adults are limited. Methods Data were collected from a national survey conducted during September 2014 and May 2015 Among Chinese adults aged 40 years and older across 30 China's provinces. Multilevel model analysis was performed to examine the impacts of different obesity indices [body mass index (BMI), waist circumference (WC), lipid accumulation product index (LAP), visceral adiposity index (VAI), and body adiposity index (BAI)] on the risk of diabetes. Results A total of 162 880 participants were included in this study. Of them, 54.47% were female. With an increase in BMI, WC, LAP, VAI, and BAI, the prevalence of diabetes significantly grew (P < 0.001). The multilevel model analysis showed that WC has the strongest impact on diabetes prevalence, while BAI was the weakest. For one SD increment in BMI, WC, LAP, VAI, and BAI, the prevalence of diabetes increased by 27.0% (Odds Ratio (OR)= 1.270,95% Confidence interval (CI)= 1.251-1.289), 37.4% (OR= 1.374, 95% CI = 1.346-1.401), 28.1% (OR= 1.281,95% CI= 1.266-1.297), 22.0% (OR= 1.220, 95% CI = 1.204-1.236), and 17.4% (OR= 1.174,95% CI= 1.151-1.192), respectively. Conclusion Obesity indicators of BMI, WC, LAP, VAI, and BAI have significant positive relationships with the risk of diabetes. WC has the strongest impact on diabetes, while BAI has the weakest.
Greater abdominal fat accumulation is associated with higher metabolic risk in Chinese than in white people: an ethnicity study.

Abstract:INTRODUCTION:Chinese are reported to have a higher percent body fat (%BF) and a higher percent trunk fat (%TF) than whites for a given body mass index (BMI). However, the associations of these ethnic differences in body composition with metabolic risks remain unknown. METHODS AND PROCEDURES:A total of 1 029 Chinese from Hangzhou, China, and 207 whites from New York, NY, USA, were recruited in the present study. Body composition was measured using dual-energy X-ray absorptiometry (DXA). Analysis of covariance was used to assess the ethnic differences in fat, fat distribution, and metabolic risk factors. RESULTS:After adjusting for BMI, age, and height, Chinese men had an average of 3.9% more %BF and 12.1% more %TF than white men; Chinese women had an average of 2.3% more %BF and 11.8% more %TF than white women. Compared with whites, higher metabolic risks were detected in Chinese for a given BMI after adjusting for age and height. Further adjustment for %BF did not change these ethnic disparities. However, after adjusting for %TF, the ethnic differences decreased and become insignificant in triglyceride, high-density lipoprotein cholesterol, and blood pressure (except for systolic blood pressure in men). For fasting plasma glucose, the ethnic differences persisted after adjustment for %BF, but decreased significantly from 0.910 to 0.686 mmol/L among men, and from 0.629 to 0.355 mmol/L among women, when the analyses were further controlled for %TF. DISCUSSION:Chinese have both higher %BF and %TF than white people for a given BMI. However, only %TF could in part account for the higher metabolic risk observed in Chinese men and women.
Association between obesity indicators and cardiometabolic disease in Chinese adults

Abstract:Background: Obesity is an established risk factor for cardiometabolic disease. Different measurements of obesity with cardiometabolic disease have been compared in recent studies in Western countries. However, obesity-related criteria for the Chinese population differ from the standard World Health Organization guidelines, and similar research in Chinese adults is limited. Measures: Data were obtained from a comprehensive intervention project involving a community population with cardiovascular and cerebrovascular risk factors in Shenzhen in 2015. A total of 4,000 participants (1,605 men and 2,395 women) with a mean age of 56.01±9.78 years were included in this study. Categorical data are reported as percentages, and continuous data are reported as mean ± standard deviation. Logistic regression analyses were conducted to examine the associations of body mass index (BMI), waist circumference (WC), and neck circumference (NC) with hypertension, diabetes, and dyslipidemia among Chinese adults. Results: The participants had a mean BMI of 24.25±3.33 kg/m2, mean NC of 33.59±4.16 cm, and mean WC of 82.44±9.84 cm (men: 85.46±9.10 cm, women: 80.40±9.81 cm). Blood pressure, plasma glucose, and lipid levels in the BMI, WC, and NC groups were statistically significant (p < 0.05). BMI, WC, and NC were positively correlated with systolic blood pressure, diastolic blood pressure, fasting plasma glucose, total cholesterol, and triglyceride and negatively correlated with low-density lipoprotein cholesterol (p < 0.05), while the risk of hypertension, diabetes, and dyslipidemia increased with an increase in BMI, WC, and NC (p < 0.05). One SD of BMI, WC, and NC resulted in an increase of 41%, 22%, and 31% risk of hypertension; 45%, 34%, and 47% risk of diabetes; and 37%, 32%, and 23% risk of dyslipidemia, respectively. Conclusions: Compared to BMI and NC, WC was more strongly associated with cardiometabolic diseases in Chinese adults.
The association between body composition and metabolically unhealthy profile of adults with normal weight in Northwest China

Abstract:Objective Related evidences of metabolically unhealthy profile of adults with normal weight are not well characterized in the Chinese population. This is because they cannot be effectively identified by regular measurements (such as body mass index [BMI]). To overcome this gap in literature, this study aimed at investigating the association between body composition and metabolically unhealthy profile in Chinese adults with normal weight. Methods A total of 5427 individuals with normal-weight were recruited from 15820 people living in Ningxia Hui Autonomous Region in Northwest China. Normal-weight was defined as a BMI of 18.5–23.9 kg/m 2 . Metabolically unhealthy profile was assessed by the National Cholesterol Education Program Adult Treatment Panel III (ATP III). Metabolically unhealthy normal-weight (MUHNW) profile was defined in individuals who had normal weight and at least two cardiometabolic risk factors. Generalized linear model was used to investigate the association between body composition measured by bioelectrical impedance and metabolically unhealthy profile in adults with normal-weight. Results The percentage of metabolically unhealthy profile was 35.86% in adults with normal weight. Different MUHNW distributions were found between males and females depending on age. The percentage of the MUHNW profile significantly increased in women after the age of 55, contrary to men. The association between body composition and MUHNW was affected by age and sex. The increased adiposity indices (fat mass index [FMI], visceral fat level [VFL], waist circumference [WCF]), and reduced skeletal muscle mass ratio [SMR] showed significant differences between MUHNW and metabolically healthy with normal weight (MHNW) (p < 0.05). Conclusion The distribution of MUHNW differed between ages and sexes. FMI, VFL, WCF and SMR could be responsible for the MUHNW adults, providing a new insight into the potential metabolic risks for the adults with normal weight in China. This directs us in the management of the MUHNW for their early prevention.

Abstract:Abstract Most research uses body mass index (BMI) alone to measure obesity. Combined with waist circumference (WC), BMI may better identify obesity-related health risk. And diet is a key component of obesity management. To better understand the relationship between obesity and diet, this study aims to examine two anthropometric measures of obesity and the food consumption among U.S. Chinese older adults. Data were drawn from the PINE study wave III (2015-2017), a prospective cohort study of community-dwelling Chinese older adults (N=3053). We categorized participants into 6 groups: normal BMI (18.5-24.9) with normal WC (women ≤ 88cm and men ≤102cm), normal BMI with high WC (women WC >88cm and men WC >102cm), overweight (BMI=25.0-29.9) with normal WC, overweight with high WC, obese (BMI >30) with normal WC, and obese with high WC. A forty-eight-item food frequency questionnaire was used to measure frequencies of vegetables, fruits, grains, protein foods, dairy, sweets, and alcohol intake. Almost 12% participants had normal BMI but high WC and 10% were overweight with high WC. Participants who were overweight with high WC reported the highest intake of vegetables among groups. Participants with higher WC had significantly higher fruit consumption, compared to those with normal WC, regardless of their BMI. Spearman correlation analysis showed that being overweight with a high WC was correlated with higher frequencies of vegetables and fruits intake and having normal BMI with normal WC was correlated with higher alcohol intake. The findings provide new insights for future research and interventions on obesity/chronic disease management.
Coronary Arterial Disease Correlates with Constitutions of Traditional Chinese Medicine: A Cross-Sectional Study in a Chinese Cohort

Abstract:Introduction: This study aimed to explore the associations between constitutions of traditional Chinese medicine (TCM) and coronary artery disease (CAD).Methods: A large-scale, community-based, cross-sectional study was performed to investigate the associations. A total of 3,748 participants were available for analysis in this study. The assessment of the constitution of TCM was based on recommendations by the Association for Chinese Medicine in China. In this study, a diagnosis of CAD was based on self-reported medical history. The associations were analyzed using univariate and multivariable logistic regression (MLR).Results: Univariate analysis showed Phlegm–dampness, Qi-deficiency, Yang-deficiency and Yin-deficiency constitutions were significantly associated with CAD, respectively ([Formula: see text] for Phlegm–dampness, [Formula: see text] for Qi-deficiency, [Formula: see text] for Yang-deficiency and [Formula: see text] for Yin-deficiency). Furthermore, MLR demonstrated significant associations among the four constitutions and CAD, after controlling for potential confounding factors (Phlegm–dampness: [Formula: see text], [Formula: see text]; Qi-deficient: [Formula: see text], [Formula: see text], Yang-Deficient: [Formula: see text], [Formula: see text]; Yin-deficient: [Formula: see text], [Formula: see text]). As compared with Neutral participants, participants with the four constitutions of TCM had higher prevalence of CAD.Conclusion: Our findings provided evidence that the four constitutions of TCM including Phlegm-dampness, Qi-deficiency, Yan-deficiency and Yin-deficiency were significantly associated with CAD, respectively. Abstract:[Objective] To explore the relationships between abdominal obesity and body-type obesity and TCM constitutional types in order to provide some evidence for adjusting constitutional bias and preventing and treating obesity. [Methods] Through cross-sectional survey,983 data were collected from physical examination centers of two hospitals in Jiangxi and Anhui. The survey of TCM constitution was performed by standardized Constitution in Chinese Medicine Questionnaire(CCMQ). Use discriminatory analysis method to judge the Individual's constitutional types. Logistic regression analysis was used to explore the relationships of abdominal obesity, body-type obesity and TCM constitutional types. [Results] Both logistic regression model one and two showed: Phlegm-wetness type significantly increased the risk of abdominal obesity(OR1=2.92,95%CI=1.76~4.86,P0.01;OR2=2.74, 95%CI=1.63~4.60,P0.01)and body-type obesity(OR1=0.36,95%CI=0.20~0.67,P0.01;OR2=0.38,95%CI=0.21~0.70,P0.01),and Yang-deficiency type significantly decreased the risk of body-type obesity(OR1=0.36,95%CI=0.20~0.67,P0.01;OR2=0.38,95%CI=0.21~0.70,P0.01). [Conclusion] Phlegm-wetness type is the main constitutional types of abdominal obesity and body-type obesity.
The association between abdominal obesity and depressive symptoms among Chinese adults: Evidence from national and regional communities

Abstract:Background The association between body shape and depressive symptoms has been reported in adults. The present study aimed to investigate the association between body shape-specific abdominal obesity and depressive symptoms among multi-regional Asian adults. Methods The 2011–2012 China Health and Retirement Longitudinal Study and 2022–2023 Hangzhou study were used as the discovery and validation datasets, respectively. Body shape was assessed by BMI categories. Abdominal obesity was defined as a body shape index (ABSI) ≥ 75th centile. Depression was measured using 10-item Centre for Epidemiological Studies Depression Scale and Geriatric Depression Scale short 15-item version, respectively. General linear and multinomial logistic models were used to explore the association of ABSI, abdominal obesity with depressive scores and presence, respectively. Results A total of 12,229 and 1210 participants were included in the discovery and validation datasets, respectively. A non-linear reverse L-shaped association was found between ABSI and depressive scores. Participants with abdominal obesity had higher depressive scores (β = 0.05, 95%CI = 0.01–0.09; and β = 0.13, 95%CI = 0.01–0.24; respectively). Stratified analyses showed that abdominal obesity was associated with higher depressive scores (β = 0.09, 95%CI = 0.00–0.17; and β = 0.25, 95%CI = 0.05–0.46; respectively) and presence (OR = 1.46, 95%CI = 1.02–2.10; and OR = 3.95, 95%CI = 1.58–9.84; respectively) in overweight adults. Furthermore, abdominal obesity was associated with depressive symptoms among overweight females, but not among males. Limitation Causal links weren't addressed because of the observational study design. Conclusion Abdominal obesity exhibited a positive association with depressive symptoms among Asian overweight adults, particularly in females. Prevention and early diagnosis of depressive symptoms should focus on overweight females.

Abstract:The World Health Organization states prevention of chronic diseases should be based on good lifestyle behaviors and healthy diets as they can reduce the risk of chronic diseases by 80%. The theory of traditional Chinese medicine constitution (TCMC) emphasizes the congenital differences of TCMC stem from the genes of parents, while acquired differences in TCMC are caused by factors as living environments, lifestyle behaviors, and dietary habits. From the perspective of preventive medicine, this study explores the correlation between dietary habits and lifestyle behaviors as potential risk factors for different types of TCMC—balanced constitution, Yang deficiency, Yin deficiency, and Phlegm stasis. Research data were collected from 2760 subjects aged 30 to 70 years participating in a survey conducted by Taiwan Biobank in 2012 to 2017. The survey included basic demographic characteristics, lifestyle behaviors, and dietary habits along with a Body Constitution Questionnaire. Compared to men, women were 3 to 4 times more likely to develop Yang-deficiency, Yin-deficiency, and Phlegm stasis. Variables that affected TCMC were smoking, midnight snack consumption, consumption of gravy-soaked or lard-soaked rice/noodles, deep-fried soybean products, bread spread, pickled vegetables as side dishes for the main course of a meal, and the dietary habit of vegetables or fruits instead of high-fat desserts. Poor dietary habits and lifestyle behaviors are the cause of unbalanced TCMCs. Understanding the specific TCMC of individual can provide useful information and predictions to maintain physical health and achieve early disease prevention to meet the goal of preventive medicine.

Abstract:Background Existing studies which regarding to the association between individual socioeconomic status (SES) and obesity are still scarce in developing countries. The major aim of this study is to estimate such association in an adult population which was drawn from an economically prosperous province of China. Methods Study population was determined by multilevel randomized sampling. Education and income were chosen as indicators of individual SES, general obesity and abdominal obesity were measured by body mass index (BMI) and waist circumference (WC). Descriptive statistical methods were used to depict overall and factor-specific distributions of general and abdominal obesity among 16,013 respondents. Two-step logistic regression models were fitted on gender basis. Results The age-and-sex adjusted rates of general overweight, general obesity, abdominal overweight and abdominal obesity in study population were 28.9% (95%CI: 27.9%-29.9%), 7.5% (95%CI: 7.0%-8.1%), 32.2% (95%CI: 31.2%-33.3%) and 12.3% (95%CI: 11.6%-13.1%), respectively. Based on model fitting results, a significant inverse association between education and obesity only existed in women, while in men, income rather than education was positively related to obesity. Conclusions The atypical SES-obesity relationship we found reflected the on-going social economy transformation in affluent regions of China. High-income men and poorly-educated women were at higher risk of obesity in Zhejiang province, thus merit intense focuses.

Abstract:BACKGROUND:The purpose of this study is to describe the prevalence of overweight, general obesity, and abdominal obesity and examine their associations with socioeconomic status in a rural Chinese adult population. METHODS:This cross-sectional study was performed on 15,236 participants ≥ 35 years of age (6,313 men [41.4%] and 8,923 women [58.6%]). Each participant's weight, height, waist circumference (WC), and hipline circumference (HC) were measured, and demographic and socioeconomic data were collected using questionnaires. RESULTS:The mean body mass index (BMI) values were 23.31 ± 2.96 and 23.89 ± 3.23 kg m(-2) and the mean WC values were 79.13 ± 8.43 and 79.54 ± 8.27 cm for men and women, respectively. The age-standardized prevalence rates of overweight (BMI ≥ 24.0 kg m(-2)), general obesity (BMI ≥ 28.0 kg m(-2)), and abdominal obesity (WC ≥ 85 cm for men and ≥ 80 cm for women) were 32.0%, 6.7%, and 27.0% for men and 35.1%, 9.7%, and 48.3% for women, respectively. All gender differences were statistically significant (p < 0.001). In addition, the age-specific prevalence rates of general and abdominal obesity slowly decreased among men but sharply increased among women as age increased (p < 0.001). In subsequent logistic regression analysis, educational level was negatively associated with both general obesity and abdominal obesity among women but positively associated with abdominal obesity among men. No significant correlation was found between obesity and income. CONCLUSIONS:These results suggest a high prevalence of obesity which might differ by gender and age, and an inverse association among women and a mixed association among men noted between education and obesity in our locality. Preventive and therapeutic programs are warranted to control this serious public health problem. The gender-specific characteristics of populations at high-risk of developing obesity should be taken into consideration when designing interventional programs.
Association Between Indices of Body Composition and Metabolically Unhealthy Phenotype in China: A Cross-Sectional Study

Abstract:Introduction:Body composition is closely related to metabolic health status. Visceral adipose tissue (VAT) dysfunction contributes to metabolic syndrome. However, results regarding subcutaneous adipose tissue (SAT) and skeletal muscle are controversial. We aimed to determine the association of indices of body composition with abnormal metabolic phenotype in China. Methods:A total of 3, 954 subjects (age 50.2 ± 11.7 years) with body mass index (BMI) more than 18.5 kg/m2 from Pinggu Metabolic Disease Study were analyzed. Quantitative computed tomography (QCT) was performed to measure total adipose tissue (TAT), VAT, SAT area, and lumbar skeletal muscle area (SMA). Participants were divided into six groups on the basis of BMI category (normal weight/overweight/obesity) and metabolic status (healthy/unhealthy), as defined by the presence or absence of components of the metabolic syndrome by Chinese Diabetes Society criteria. Results:63.4%, 39.5%, and 23.3% participants were classified as metabolically healthy phenotype in individuals with normal weight, overweight and obese, respectively. Individuals in the highest TAT, VAT, and VAT/TAT ratio category had higher risk of being metabolically unhealthy than individuals in the lowest group (all p<0.01). While, risk for metabolically unhealthy was reduced significantly in the highest SMA/TAT ratio category when compared with the lowest category in individuals with normal wight and overweight (both p<0.05). Risk for metabolically unhealthy was reduced significantly in the highest SAT category when compared with the lowest category (OR=0.555, 95%CI: 0.360-0.856, p=0.008) in individuals with obese after adjustment for age, sex and BMI. However, skeletal muscle index (SMI) showed no significant association with the metabolically healthy status in different BMI categories (p>0.05). The VAT and VAT/TAT ratio were better diagnostic values of indicators to differentiate metabolically unhealthy subjects from controls compared with other indicators, such as TAT, SAT, SMI, SMA/TAT ratio. Conclusions:Higher visceral adipose tissue was closely associated with metabolically unhealthy phenotype in Chinese adults. Subcutaneous adipose tissue might be a protective factor for metabolic health status only in obese individuals.

Abstract:It is not known which obesity index best explains variations in cardiovascular disease risk across populations. The objective of this study was to differentiate the associations of waist circumference (WC) and body mass index (BMI) with cardiovascular disease risk in a Chinese population. Cardiovascular risk factors, WC, and BMI were measured in 13 817 adults aged more than 18 years in Shanghai. Higher WC tertiles were associated with higher blood pressure and higher cholesterol, triacylglycerol, and glucose concentrations within each tertile of BMI and vice versa. The odds ratios (ORs) of hypertension, dyslipidemia, and metabolic syndrome increased with successive WC (or BMI) tertiles after adjustment for BMI (or WC) and several covariates. However, BMI tertiles were not associated with the ORs of diabetes after adjustment for WC. WC may be better than BMI as an alternative measure of body fatness or fat distribution for predicting diabetic risks in Chinese adults.
