Precision treatment of multi-segmental lumbar spinal stenosis in elderly patients with the use of unilateral biportal endoscopy and selective nerve root block: Case report and literature review

Abstract:Background Lumbar spinal stenosis (LSS) is common in the elderly population. In recent years, the unilateral biportal endoscopy (UBE) technique has been used for patients with symptomatic LSS, showing favorable clinical outcomes. However, accomplishing precise decompression in elderly patients with multi-segmental LSS remains difficult. This study was the first to demonstrate successful precise decompression using a combination of UBE and selective nerve root block (SNRB) for an elderly patient with multi-segmental LSS. Case presentation An elderly patient with gradually aggravated radiating leg pain was admitted to our spine center. Preoperative magnetic resonance imaging and computed tomography showed obvious stenosis of the central canal and lateral recess at multiple levels of the lumbar spine. To determine the level responsible, SNRB was performed before decompressive surgery. The responsible level was also confirmed based on substantial self-reported relief of leg pain following SNRB. Precise decompression at the responsible level was performed using the UBE technique under general anesthesia. Leg pain (measured via the visual analog scale) and radiological results were evaluated following surgery and at the 6-month follow-up. Postoperatively, the patient reported complete relief of radiating pain. Three days following the operation, the patient was discharged and followed up for 6 months without symptom recurrence. Sufficient decompression of the responsible level was confirmed using computed tomography. Conclusions The combined use of UBE and SNRB may be considered as an option for precise decompression in elderly patients with radiologically confirmed multi-segmental LSS.
