Abstract:INTRODUCTION: Williams syndrome (WS) is a rare neurodevelopmental disorder that is caused by a hemizygous deletion on chromosome 7q11.23. The interest of WS to neurocognitive scientists stems from the uneven profiles of cognitive abilities, with spatial cognition seriously impaired and language and face processing relatively proficient. We know relatively little about the visual search attention and executive function in children with WS. OBJECTIVE: The objective of this study was to examine the nature of visual search attention and executive function in children with WS, compared with children with Down syndrome (DS), healthy chronological age–matched control subjects (CA), and healthy mental age–matched control subjects (MA). METHODS: A total of 142 children were tested: 21 with WS, 25 with DS, 45 CA, and 41 MA. MA were matched to the children with WS and DS using the Peabody Picture Vocabulary Test. All participants were assessed on a set of computerized visual search tasks and Wilding Monster Sorting Test using a touch-screen. RESULTS: The results showed that selective attention, switch, and sustained attention of children with WS all are less developed. Children with WS produced a large number of shape errors, and they also confused shape distractors with targets more than the other groups. Children with WS exhibited poorer executive performance as compared with both groups of typical children. They produced more repetitive errors than did children with DS. CONCLUSIONS: These findings reveal distinct visual search deficits and atypically developing executive function in children with WS.

Abstract:Deaf children have been characterized as being impulsive, distractible, and unable to sustain attention. However, past research has tested deaf children born to hearing parents who are likely to have experienced language delays. The purpose of this study was to determine whether an absence of auditory input modulates attentional problems in deaf children with no delayed exposure to language. Two versions of a continuous performance test were administered to 37 deaf children born to Deaf parents and 60 hearing children, all aged 6-13 years. A vigilance task was used to measure sustained attention over the course of several minutes, and a distractibility test provided a measure of the ability to ignore task irrelevant information - selective attention. Both tasks provided assessments of cognitive control through analysis of commission errors. The deaf and hearing children did not differ on measures of sustained attention. However, younger deaf children were more distracted by task-irrelevant information in their peripheral visual field, and deaf children produced a higher number of commission errors in the selective attention task. It is argued that this is not likely to be an effect of audition on cognitive processing, but may rather reflect difficulty in endogenous control of reallocated visual attention resources stemming from early profound deafness.

Abstract:Background: Enhanced visual attention is one of the major documented effects in auditory deprivation. However, in parallel, it has been shown that the congenitally deaf show deficits in their temporal processing. Aim: In this study, we aimed to study the parameters of visual attention. Materials and Methods: The speed of processing of divided attention (using the symbol digit modality test [SDMT]) and the central attention with attentional blink (AB) using a commercially available App, the BrainBaseline App. We tested these parameters of visual attention in students who are congenitally hearing impaired and those with normal hearing. Results: The measure of visual attention (error scores) using the SDMT did not show any significant differences between the congenitally hearing impaired and the students with normal hearing. However, we report significant differences in the processing speeds of divided attention and the error rates in the AB test between the congenitally hearing impaired and the normal with the hearing impaired having slower speeds of processing but making less errors in AB test. Conclusion: This finding probably indicates the redistribution or allocation of available brain resources as a result of sensory deprivation.

Abstract:OBJECTIVE:To explore the characteristics of visual search attention in children with Williams syndrome (WS).METHODS:Twenty-two children with WS, 13 male and 9 female, aged 12 (6-16), 25 chronological age (CA) and mental age (MA)-matched children with Down's syndrome (DS), 45 biological age-matched children, and 43 mental age-matched children, aged 4 +/- 2 (2-8) underwent Peabody picture vocabulary test (PPVT). Visual search attention including selective, switch, and sustained attention, was tested using a set of computerized visual search tasks via a touch-screen. In the dual target tasks, participants were required to alternate between two different targets. Sustained attention was investigated with vigilance task test.RESULTS:(1) In the single-target searching task with no target similar distractor, the accurate response rate of the WS patients was 71% +/- 25%, significantly lower than that of the CA-matched children (87% +/- 14%, P = 0. 001). The searching time of the WS children was 5 s, significantly longer than that of the CA-matched children (3 s, P = 0.000). The distance for touch of the WS children was 25 relative units, significantly longer than that of the CA-matched children (18 relative units, P = 0.000). The mean reaction time of the WS children was 5 s, significantly longer than that of the children with Down's syndrome (3 s, P = 0.022). The shape error rate of the WS children was 15%, significantly higher than that of the CA-matched children (0, P = 0.000). When non-targets similar to the targets were added, the accurate response rate of the WS group was 39% +/- 20%, significantly lower than those of the CA-matched and MA-matched children (77% +/- 23% and 66% +/- 23% respectively, both P = 0.000); the mean searching time of the WS children was 13 s, significantly longer than those of the CA-matched and MA-matched children (3 s and 5 s respectively, both P = 0. 000); and the distance per touch of the WS children was 41 relative units, significantly longer than those of the CA-matched and MA-matched children (20 and 27 relative units, P = 0.000 and P = 0.004). (2) The results of the dual target tasks showed that the accurate response rate of the WS children was 52% +/- 28%, significantly lower than that of the CA-matched children (78% +/- 22%, P = 0. 000), the mean searching time of the WS children was 11 s, significantly longer than that of the CA-matched children (4 s, P = 0.000); and the distance per touch of the WS children was 54 +/- 27 relative units, significantly longer than that of the CA-matched children (31 +/- 13 relative units, P = 0.000). However, there were not significant differences in the accurate response rate, mean searching time, and distance per touch between the WS and DS children. The switching error rate of the WS children was 13%, significantly higher than those of the CA and MA-matched children (0 and 4% respectively, P = 0.000 and P = 0.004). (3) The vigilance task test showed that the accurate response rate of the WS children was 52% +/- 25%, significantly lower than that of the CA-matched children (80% +/- 21%, P = 0.000); the mean searching time of the WS children was 4 s +/- 1 s, significantly longer than that of the CA-matched children (3 s +/- 1 s, P = 0.000); and the error hit number of the WS children was 8, significantly more than that of the CA-matched children (3, P = 0.000). However, there were not significant differences in the accurate response rate, mean searching time, and error hit number between the WS and DS children. Conclusion Distinct visual search deficits exist in WS children.

Abstract:OBJECTIVE: This study examined whether attention deficits in fragile X syndrome (FXS) can be traced back to abnormalities in basic information processing.METHOD: Sixteen males with FXS and 22 age-matched control participants (mean age 29 years) performed a standard oddball task to examine selective attention in both auditory and visual modalities. Five FXS males were excluded from analysis because they performed below chance level on the auditory task. ERPs were recorded to investigate the N1, P2, N2b, and P3b components.RESULTS: N1 and N2b components were significantly enhanced in FXS males to both auditory and visual stimuli. Interestingly, in FXS males, the P3b to auditory stimuli was significantly reduced relative to visual stimuli. These modality differences in information processing corresponded to behavioral results, showing more errors on the auditory than on the visual task.CONCLUSIONS: The current findings suggest that attentional impairments in FXS at the behavioral level can be traced back to abnormalities in event-related cortical activity. These information processing abnormalities in FXS may hinder the allocation of attentional resources needed for optimal processing at higher-levels.SIGNIFICANCE: These findings demonstrate that auditory information processing in FXS males is critically impaired relative to visual information processing.

Abstract:Difficulty listening in noisy environments is a common complaint of individuals with autism spectrum disorder (ASD). However, the mechanisms underlying such auditory processing challenges are unknown. This preliminary study investigated auditory attention deployment in adults with ASD. Participants were instructed to maintain or switch attention between two simultaneous speech streams in three conditions: location (co-located versus ± 30° separation), voice (same voice versus male–female contrast), and both cues together. Results showed that individuals with ASD can selectively direct attention using location or voice cues, but performance was best when both cues were present. In comparison to neurotypical adults, overall performance was less accurate across all conditions. These findings warrant further investigation into auditory attention deployment differences in individuals with ASD.

Abstract:During typical development, the efficiency with which distinct image attributes, objects such as faces, and emotions are analyzed depends on the development of ocular functioning (sensory level) and brain mechanisms mediating perceptual abilities (neural level). Compared to typically developing individuals, ocular differences are more frequent in persons with Down syndrome (DS) and include an increased incidence of refractive errors, i.e. hyperopia and myopia, and difficulties with accommodation (focusing up close). In most cases, these conditions can be corrected with bifocals, resulting in greatly improved literary skills that underlie reading, particularly for younger persons with DS. In addition, individuals with DS seem to benefit from the spatially structured presentation of visual material when learning, another accommodation that can be implemented during instruction. Finally, although not specific to DS, young children and adults with DS generally identify and recognize emotions less efficiently than do persons without DS. However, among persons with DS, these difficulties seem to be more pronounced for more intense emotions, including fear and anger. In sum, visual profiles based on sensory and perceptual performance can advance our understanding of atypical information processing among persons with DS, as well as provide practical information aimed at improving learning and instruction.
Exploring electrophysiological markers of auditory predictive processes and pathological ageing in adults with Down's syndrome

Abstract:Down's syndrome is associated with pathological ageing and a propensity for early-onset Alzheimer's disease. The early symptoms of dementia in people with Down's syndrome may reflect frontal lobe vulnerability to amyloid deposition. Auditory predictive processes rely on the bilateral auditory cortices with the recruitment of frontal cortices and appear to be impaired in pathologies characterized by compromised frontal lobe. Hence, auditory predictive processes were investigated to assess Down's syndrome pathology and its relationship with pathological ageing. An auditory electroencephalography (EEG) global-local paradigm was presented to the participants, in which oddball stimuli could either violate local or higher level global rules. We characterised predictive processes in individuals with Down's syndrome and their relationship with pathological ageing, with a focus on the EEG event-related potential called Mismatch Negativity (MMN) and the P300. In Down's syndrome, we also evaluated the EEG components as predictor of cognitive decline 1 year later. We found that predictive processes of detection of auditory violations are overall preserved in Down's syndrome but also that the amplitude of the MMN to local deviancies decreases with age. However, the 1-year follow-up of Down's syndrome found that none of the ERPs measures predicted subsequent cognitive decline. The present study provides a novel characterization of electrophysiological markers of local and global predictive processes in Down's syndrome.
Attentional modulation of neural sound tracking in children with and without dyslexia

Abstract:Auditory selective attention forms an important foundation of children's learning by enabling the prioritisation and encoding of relevant stimuli. It may also influence reading development, which relies on metalinguistic skills including the awareness of the sound structure of spoken language. Reports of attentional impairments and speech perception difficulties in noisy environments in dyslexic readers are also suggestive of the putative contribution of auditory attention to reading development. To date, it is unclear whether non-speech selective attention and its underlying neural mechanisms are impaired in children with dyslexia and to which extent these deficits relate to individual reading and speech perception abilities in suboptimal listening conditions. In this EEG study, we assessed non-speech sustained auditory selective attention in 106 7-to-12-year-old children with and without dyslexia. Children attended to one of two tone streams, detecting occasional sequence repeats in the attended stream, and performed a speech-in-speech perception task. Results show that when children directed their attention to one stream, inter-trial-phase-coherence at the attended rate increased in fronto-central sites; this, in turn, was associated with better target detection. Behavioural and neural indices of attention did not systematically differ as a function of dyslexia diagnosis. However, behavioural indices of attention did explain individual differences in reading fluency and speech-in-speech perception abilities: both these skills were impaired in dyslexic readers. Taken together, our results show that children with dyslexia do not show group-level auditory attention deficits but these deficits may represent a risk for developing reading impairments and problems with speech perception in complex acoustic environments. Neurophysiological evidence for selective auditory attention deficits in children with specific language impairment

Abstract:Recent behavioral studies suggest that children with poor language abilities have difficulty with attentional filtering, or noise exclusion. However, as behavioral performance represents the summed activity of multiple stages of processing, the temporal locus of the filtering deficit remains unclear. Here, we used an event-related potential (ERP) paradigm to compare the earliest mechanisms of selective auditory attention in 12 children with specific language impairment (SLI) and 12 matched control children. Participants were cued to attend selectively to one of two simultaneously presented narrative stories. The stories differed in location (left/right speaker), narration voice (male/female), and content. ERPs were recorded to linguistic and nonlinguistic probe stimuli embedded in the attended and unattended story. By 100 ms, typically developing children showed an amplification of the sensorineural response to attended as compared to unattended stimuli. In contrast, children with SLI showed no evidence of sensorineural modulation with attention, despite behavioral performance indicating that they were performing the task as directed. These data are the first to show that SLI children have marked and specific deficits in the neural mechanisms of attention and, further, localize the timing of the attentional deficit to the earliest stages of sensory processing. Deficits in the effects of selective attention on early sensorineural processing may give rise to the diverse set of sensory and linguistic impairments in SLI children.

Abstract:This preliminary study sought to assess biomarkers of attention using electroencephalography (EEG) and eye tracking in two ultra-rare monogenic populations associated with autism spectrum disorder (ASD). Relative to idiopathic ASD (n = 12) and neurotypical comparison (n = 49) groups, divergent attention profiles were observed for the monogenic groups, such that individuals with DYRK1A (n = 9) exhibited diminished auditory attention condition differences during an oddball EEG paradigm whereas individuals with SCN2A (n = 5) exhibited diminished visual attention condition differences noted by eye gaze tracking when viewing social interactions. Findings provide initial support for alignment of auditory and visual attention markers in idiopathic ASD and neurotypical development but not monogenic groups. These results support ongoing efforts to develop translational ASD biomarkers within the attention domain.
Alterations in aperiodic and periodic EEG activity in young children with Down syndrome

Abstract:Down syndrome is the most common cause of intellectual disability, yet little is known about the neurobiological pathways leading to cognitive impairments. Electroencephalographic (EEG) measures are commonly used to study neurodevelopmental disorders, but few studies have focused on young children with DS. Here we assess resting state EEG data collected from toddlers/preschoolers with DS (n=29, age 13-48 months old) and compare their aperiodic and periodic EEG features with both age-matched (n=29) and cognitive-matched (n=58) comparison groups. DS participants exhibited significantly reduced aperiodic slope, increased periodic theta power, and decreased alpha peak amplitude. A majority of DS participants displayed a prominent peak in the theta range, whereas a theta peak was not present in age-matched participants. Overall, similar findings were also observed when comparing DS and cognitive-matched groups, suggesting that EEG differences are not explained by delayed cognitive ability.

Abstract:Persons with single-sided deafness (SSD) typically complain about the impaired ability to locate sounds and to understand speech within background noise. However, the findings from previous studies suggest that paying attention to sounds could mitigate the degraded spatial and speech-in-noise perception. In the present study, we characterize the pattern of cortical activation depending on the side of deafness, and attentional modulation of neural responses to determine if it can assist better sound processing in people with SSD. For the active listening condition, adult subjects with SSD performed sound localization tasks. On the other hand, they watched movies without attending to speech stimuli during passive listening. The sensor-level global field power of N1 and source-level N1 activation were computed to compare the active- and passive-listening conditions and left- and right-sided deafness. The results show that attentional modulation differs depending on the side of deafness: active listening increased the cortical activity in individuals with left-sided deafness but not in those with right-sided deafness. At the source level, the attentional gain was more apparent in left-sided deafness in that paying attention enhanced brain activation in both hemispheres. In addition, SSD participants with larger cortical activities in the right primary auditory cortex had shorter durations of deafness. Our results indicate that the side of deafness can change top-down attentional processing in the auditory cortical pathway in SSD patients.
Evaluation of hearing impairment in patients with Down syndrome

Abstract:Introduction It is estimated that the prevalence of hearing loss in children with Down syndrome (DS) ranges from 38% to 82%. However, the diagnosis of hearing loss in DS is difficult, due to an impaired ability to cooperate for subjective tests. Thus, objective tests such as impedance audiometry, otoacoustic emissions, and auditory brainstem responses (ABRs) may be more appropriate. In this study objective methods without anesthesia were used to determine the amount, type, and prevalence of hearing loss in people with DS. Material and methods The study included 39 subjects with DS, ranging in age from 1 year to 27 years. Hearing tests in DS subjects were performed during physiological sleep or while awake. Otoscopic examination was performed in all DS subjects. If no abnormalities were seen, tympanometry, otoacoustic emissions, and recording of ABRs were attempted. Results Objective hearing tests showed that the DS group had various types of hearing disorders. Based on all objective tests carried out in all ears ( n = 78), the following diagnoses were made: normal hearing, 36 ears (46%); cochlear hearing loss, 27 ears (35%); conductive hearing loss, 8 ears (10%); suspected deafness, 7 ears (9%). Based on wave V thresholds, the following degrees of hearing loss were established: normal hearing, 36 ears (46%); 21–40 dB nHL, 21 ears (27%); 50–60 dB nHL, 8 ears (10%); 70–80 dB nHL, 6 ears (8%); > 80 dB nHL, 7 ears (9%). Conclusions In previous studies of DS subjects by other authors, conductive hearing loss predominated, followed by sensorineural hearing loss. In the present study, more sensorineural than conductive hearing loss was diagnosed. In most cases, the diagnosis of the type of hearing disorder was based on the ABR result, and other tests such as tympanometry and otoacoustic emissions played a supporting role. Due to the often limited cooperation of the patient, the DPOAE test was difficult to perform.

Abstract:The auditory cortex has been shown to participate in visual processing in individuals with complete auditory deprivation. However, it remains unclear whether partial hearing deprivation like single-sided deafness (SSD) leads to similar cross-modal plasticity. To investigate this, we enrolled individuals with long-term SSD, into functional MRI scans under resting-state and a visuo-spatial working memory task. Contrary to previous findings in bilateral deafness, our study revealed decreased activation in the auditory cortex in both left (LSSD) and right (RSSD) single-sided deafness compared to normal hearing controls, with statistical significance in RSSD. The degree of involvement was correlated with residual hearing ability in RSSD. These observations suggest that SSD can lead to a downward cross-modal plasticity: the more hearing ability lost, the fewer brain resources in the auditory cortex can be applied to visual tasks. In addition, the fronto-parietal cortex was observed to be less activated during the visual task in RSSD while the resting-state fMRI revealed increased functional connectivity between the fronto-parietal cortex and the auditory cortex, suggesting fronto-parietal resources may be recruited less by vision but more by hearing. The LSSD showed a similar alteration trend with RSSD, but without statistical significance. Together these findings may indicate that when hearing is partially deprived in SSD, there may be redistribution for brain resources between hearing and vision, and vision tends to allocate less resources. Our findings in this pilot study of unilateral auditory-deprived individuals enrich the understanding of cross-modal plasticity in the brain.

Abstract:Adolescents with Attention-deficit/hyperactivity disorder (ADHD) have difficulty processing speech with background noise due to reduced inhibitory control and working memory capacity (WMC). This paper presents a pilot study of an audiovisual Speech-In-Noise (SIN) task for young adults with ADHD compared to age-matched controls using eye-tracking measures. The audiovisual SIN task consists of varying six levels of background babble, accompanied by visual cues. A significant difference between ADHD and neurotypical (NT) groups was observed at 15 dB signal-to-noise ratio (SNR). These results contribute to the literature of young adults with ADHD.
